OBJETIVO: O trauma é um problema de saúde pública de enormes proporções. Constitui-se na principal causa de óbitos na população jovem. O Major Trauma Outcome Study (MTOS) é um estudo descritivo e retrospectivo da gravidade das lesões e evolução dos pacientes, considerado como o maior arquivo contemporâneo de informações descritivas de traumatizados. O objetivo do presente estudo é comparar o cálculo retrospectivo do New Injury Severity Score (NISS) com o Injury Severity Score (ISS) já calculado prospectivamente, utilizando o Trauma and Injury Severity Score (TRISS) e uma simples modificação deste índice, denominado de NTRISS (New Trauma and Injury Severity Score), e também comparar esta população submetida à laparotomia com os pacientes do MTOS. MÉTODO: Foram estudados 1.380 pacientes adultos traumatizados e submetidos à laparotomia na Disciplina de Cirurgia do Trauma da Unicamp, em Campinas, durante um período de oito anos. Os dados avaliados foram: demográficos, causa do trauma (fechado ou penetrante, ferimento por projétil de arma de fogo ou arma branca), estado fisiológico na admissão (RTS), diagnóstico anatômico de lesões (ATI, ISS e NISS), probabilidade de sobrevida utilizando o TRISS e o NTRISS, e a evolução do paciente (sobrevivência ou óbito). Foram utilizadas as estatísticas Z e W, inicialmente descritas por Flora, a fim de comparar a predição de óbitos ou sobreviventes com o estudo controle (MTOS). RESULTADOS: A maioria dos pacientes (88,3%) era do sexo masculino e jovem (média de idade de 30,4 anos). O ferimento por projétil de arma de fogo foi o mecanismo de trauma mais freqüente, com 641 casos (46,4%). Quatrocentos e trinta pacientes (31,2%) sofreram trauma fechado. As médias do ATI, ISS e NISS foram, respectivamente, de 12,3, 17,6 e 22,1. A taxa global de mortalidade foi de 16,8% e os pacientes com trauma contuso tiveram a maior mortalidade (29,3%). O NISS identificou melhor os sobreviventes e óbitos se comparado ao ISS, obtendo-se uma maior especificidade com o NTRISS. Foi observado um número significativamente menor de sobreviventes do que o esperado pelo estudo basal, com Z -16,24 com o TRISS e Z -9,40 se aplicado o NTRISS. Variações no valor da estatística W para cada paciente mostraram uma diferença no número de óbitos equivalente a 7,89 mais casos de óbito do que o esperado pelo MTOS, por 100 pacientes tratados, ao se empregar o TRISS, enquanto que estes valores foram reduzidos para 5,14 utilizando-se o NTRISS. CONCLUSÕES: Os métodos utilizados para cálculo da probabilidade de sobrevivência apresentaram limitações, particularmente nesta população com predomínio dos traumas penetrantes. O NISS, com o seu derivado NTRISS, foi o escore que obteve uma melhor predição de sobrevivência se comparado com o ISS. Os resultados obtidos com o TRISS e NTRISS foram estatisticamente piores do que os do MTOS, porém este processo de monitorização destes pacientes traumatizados tem sido importante para assegurar uma condição continuada de controle de qualidade.

Atresia de coanas é uma patologia rara cuja incidência varia de 1:5.000-8.000 nascidos vivos, sendo mais comum no sexo feminino e associada a outras malformações. FORMA DE ESTUDO: clínico retrospectivo. Quando unilateral, tem seu diagnóstico retardado pela carência de sintomas, porém, quando bilateral, necessita de tratamento imediato devido à insuficiência respiratória e à incapacidade de o recém nascido realizar respiração oral. Neste artigo relatamos a incidência e a experiência do HRAC-USP.

Num grupo de 263 crianças de 0 a 24 meses de idade, atendidas de outubro de 1963 a setembro de 1964 no Centro de Saúde da Lapa, na Capital de São Paulo, foram feitos exames de fezes para a pesquisa de Escherichia coli, do grupo da gastroenterite infantil, Shigella, Salmonella, poliovírus, vírus Coxsackie e vírus ECHO. O agente mais freqüentemente isolado foi o vírus da poliomielite (15,97% de positividade), seguindo-se-lhe a Escherichia coli G.E.I. (10,65%), Shigella (9,51%), vírus Coxsackie (7,22%), Salmonella em 3,04% dos casos e vírus ECHO no mesmo percentual. Em 16,22% das 111 que apresentaram resultado positivo, o exame revelou a presença de dois ou mais agentes. Os agentes acima referidos foram isolados de 42,21% da totalidade das crianças examinadas. Dentre as 167 que apresentavam diarréia, a porcentagem de positividade chegou a 47,90%, e nas 96 que nao a referiam, desceu a 32,29%. Sòmente em relação às shigelas foi possível evidenciar associação entre isolamento de microrganismos e quadro diarréico. Para a E. coli 0111, salmonelas e vírus Coxsackie A, os resultados foram apenas sugestivos desta associação, e para os demais agentes não houve evidência alguma favorável à mesma.

Com o objetivo de determinar os níveis de pressão arterial e a prevalência de hipertensão arterial em uma população jovem, foram realizados pesquisas em dois anos sucessivos em 1.288 e 736 estudantes de Botucatu, SP (Brasil) tendo sido comparados os resultados obtidos. As médias das pressões sistólicas da população estudada e dos dois grupos etários desta população (16 a 20 anos e 21 a 25 anos) foram idênticas em ambos os anos, tendo as médias das pressões diastólicas diferido de no máximo 2 mmHg; as médias, tanto sistólicas quanto diastólicas, dos dois sexos e da parcela branca da população estudada quanto à idade e sexo também diferiram de no máximo 2 mmHg. As médias da população estudada e sua parcela branca, em ambos os anos, foram superiores no sexo masculino e no grupo etário de 21 a 25 anos. Na população negra e amarela houve disparidade de resultados entre 1975 e 1976, indicando influência da exiguidade do tamanho dos contigentes negro e amarelo desta população. A prevalência de hipertensão arterial (pressão sistólica igual ou maior que 140 mmHg e diastólica igual ou maior que 90 mmHg) foi de 5,04% em 1975 e 6,22% em 1976, tendo sido em ambos os anos maior no sexo masculino do que no feminino e no grupo de 21 a 25 do que no de 16 a 20 anos.

São apresentados resultados em relação a infecção expertmental de Zygodontomys lasiurus (Rodentia-Cricetidae) com duas cepas de T. cruzi isoladas de casos humanos, Y e Berenice, e uma isolada de um triatomíneo silvestre, chamada costalimai. Foram realizados estudos em relação a evolução da parasitemia, duração da patência e prepatência da infecção. Com o objetivo de verificar a agressividade e tropismo tissular das cepas de T. cruzi nesta espécie de roedor, foram também realizados estudos histopatológicos. Os resultados obtidos mostraram que os níveis de parasitemias foram baixos para as três cepas estudadas. A patência da infecção variou de 14 a 16 dias nos animais inoculados com a cepa Y, 26 a 29 dias com a Berenice e 9 a 13 dias com a costalimai. O período prepatente variou de 3 a 5 dias nos animais inoculados com a cepa Y, de 2 a 6 dias com a cepa Berenice e de 6 a 8 dias com a costalimai. As três cepas apresentaram em Z. lasiurus, comprometimento nitidamente muscular, provocando reações leves, moderadas e intensas.

OBJETIVO: Analisar os fatores associados à mortalidade em pacientes para tratamento da fratura proximal do fêmur e as diferentes formas de ajuste de risco. MÉTODOS: Estudo de coorte não-concorrente com 390 pacientes com 50 anos ou mais. Os pacientes foram identificados no Sistema de Informações Hospitalares do Sistema Único de Saúde, internados para correção cirúrgica da fratura proximal do fêmur em um hospital universitário do município do Rio de Janeiro, entre 1995 e 2000. Foram levantados dados complementares nos prontuários desses pacientes e elaborados modelos de regressão logística para a chance de óbito em 90 dias após a data da admissão, segundo perfil dos pacientes e do tratamento. Foram calculados índices de classificação da gravidade do paciente. RESULTADOS: A taxa de mortalidade foi de 7,4% e os fatores que tiveram efeito sobre o óbito foram: idade (OR=1,06; IC 95%: 1,02;1,11), quantidade de comorbidades (OR=1,44; IC 95%: 1,12;1,69), índice de comorbidade de Charlson acima de 2 (OR=6,67; IC 95%: 2,98;22,16) e intervalo de tempo para a realização da cirurgia (OR=1,04; IC 95%:1,02;1,07). CONCLUSÕES: A quantidade de comorbidades e o índice de comorbidade de Charlson auxiliaram na predição da mortalidade.

It has been well demonstrated the relationship between the infection with high-risk human papillomavirus (HPVs) genotypes and cervical cancer. In Northeastern Argentina a high incidence of this pathology has been described and therefore a high prevalence of HPV infection is expected. In order to identify HPV genotypes associated with malignant and pre-malignant cervical lesions present in the area, 53 ecto-endo cervical cell specimens obtained from women with cytohistological alterations were studied by a PCR-RFLP technique. Out of 53 patients, 34 (64.2%) were positive for HPV infection, being HPV-16 (32.3%) the most frequently found genotype, followed by HPV-58 (14.7%), -6, -18 and -45 (5.9%), -33, -52, -53, -54, -56, -66, -MM4 and -LVX100 (2.9%). Also 5 cases of infection caused by multiple genotypes were found, which corresponded to 14.7% of the positive cases. Results indicate that besides HPV-16 and -18, the most prevalent high-risk HPV genotypes worldwide, others like -45 and -58 as well as co-infection cases are frequent between women of Northeastern Argentina, and a particular attention should be paid to this circumstance because it could be an epidemiological feature of regional importance and a useful information for a future vaccination program.

We described a case of salmonellosis in a 33-year old HIV-infected patient. The patient presented oral and esophageal candidiasis, intense epigastric and retrosternal pain. During the physical examination he was hypochloraemic, acyanotic, hypohydrated, anicteric and afebrile. Admittance laboratorial tests indicated: red cells 3.6 millions/mm³; hemoglobin, 10.1 g/dL; leukocyte count, 3,000/mm³, with 1% of eosinophils, 14% of non-segmented and 53% of segmented neutrophils and 31% of lymphocytes. The blood culture was positive for Salmonella enterica subsp houtenae serogroup O:16. This is probably the first human report of bacteremia due to Salmonella enterica subsp houtenae in Brazil associated to HIV-infected patient.

Q fever has been considered non-existing in Brazil where reports of clinical cases still cannot be found. This case-series of 16 patients is a result of a systematic search for such illness by means of clinical and serologic criteria. Serologic testing was performed by the indirect microimmunofluorescence technique using phase I/II C. burnetii antigens. Influenza-like syndrome was the most frequent clinical form (eight cases - 50%), followed by pneumonia, FUO (fever of unknown origin), mono-like syndrome (two cases - 12.5% each), lymphadenitis (one case - 6.3%) and spondylodiscitis associated with osteomyelitis (one case - 6.3%). The ages varied from four to 67 years old with a median of 43.5. All but one patient had positive serologic tests for phase II IgG whether or not associated with IgM positivity compatible with acute infection. One patient had both phase I and phase II IgG antibodies compatible with chronic Q fever. Seroconvertion was detected in 10 patients. Despite the known limitations of serologic diagnosis, the cases here reported should encourage Brazilian doctors to include Q fever as an indigenous cause of febrile illness.

This article reports the HPV status and cervical cytological abnormalities in patients attended at public and private gynecological services from Rio de Janeiro State. It also comments the performance of each HPV DNA tests used. A set of 454 women from private health clinics was tested by routine Capture Hybrid II HPV DNA assay. Among these, 58.4% presented HPV and nearly 90% of them were infected by high risk HPV types. However, this group presented few premalignant cervical lesions and no invasive cervical cancer was registered. We also studied 220 women from low income class attended at public health system. They were HPV tested by polymerase chain reaction using My09/11 primers followed by HPV typing with E6 specific primers. The overall HPV prevalence was 77.3%. They also showed a high percentage of high squamous intraepithelial lesion-HSIL (26.3%), and invasive cervical carcinoma (16.3%). HPV infection was found in 93.1% and 94.4% of them, respectively. The mean ages in both groups were 31.5 and 38 years, respectively. In series 1, HPV prevalence declined with age, data consistent with viral transient infection. In series 2, HPV prevalence did not decline, independent of age interval, supporting not only the idea of viral persistence into this group, but also regional epidemiological variations in the same geographic area. Significant cytological differences were seen between both groups. Normal and benign cases were the most prevalent cytological findings in series 1 while pre-malignant lesions were the most common diagnosis in the series 2. HPV prevalence in normal cases were statistically higher than those from series 1 (p < 0.001), indicating a higher exposure to HPV infection. Women from both samples were referred for previous abnormal cytology. However, socio-demographic evidence shows that women from series 1 have access to treatment more easily and faster than women from series 2 before the development of pre-malignant lesions. These data provides baseline support for the role of social inequalities linked to high risk HPV infection leading to cervical cancer. Broadly screening programs and the development of safe and effective vaccines against HPV would diminish the toll of this disease that affect mainly poor women.

PURPOSE: The aim of this study was to investigate the frequency of HPV infection and the types 16 and 18 in cervical samples from patients attended at two public health services of the city of Belo Horizonte, MG. METHODS: Cervical samples from 174 patients were collected for cytopathological and molecular tests. HPV infection was searched by PCR utilizing MY09 and MY11 primers or HPV 16 and HPV 18 specific primers. RESULTS: Amongst the 174 samples analyzed, 20.7% presented squamous intraepithelial and/or invasive lesions detected on cytopathological analysis and of those, 94.4% were infected by HPV. HPV 16 was found in 20% of the cases of low-grade squamous intraepithelial lesions and in 40% and 50% of high-grade squamous intraepithelial lesion and squamous invasive carcinoma, respectively. HPV 18 was detected in 6.7% of the low-grade lesion samples and in two HPV16 co-infected samples. In 50% of the cases of high-grade lesion, the HPV type was not determined. CONCLUSION: The HPV 16 was the virus type more frequently detected. However, more than 50% of the positive samples at the cytopathological analysis were negative for HPV 16 and 18, indicating that possibly other virus types are present in relative high frequencies in the studied population.

Introduction: Maternal HIV infection and related co-morbidities may have two outstanding consequences to fetal health: mother-to-child transmission (MTCT) and adverse perinatal outcomes. After Brazilian success in reducing MTCT, the attention must now be diverted to the potentially increased risk for preterm birth (PTB) and intrauterine fetal growth restriction (IUGR). Objective: To determine the prevalence of PTB and IUGR in low income, antiretroviral users, publicly assisted, HIV-infected women and to verify its relation to the HIV infection stage. Patients and Methods: Out of 250 deliveries from HIV-infected mothers that delivered at a tertiary public university hospital in the city of Vitória, state of Espírito Santo, Southeastern Brazil, from November 2001 to May 2012, 74 single pregnancies were selected for study, with ultrasound validated gestational age (GA) and data on birth dimensions: fetal weight (FW), birth length (BL), head and abdominal circumferences (HC, AC). The data were extracted from clinical and pathological records, and the outcomes summarized as proportions of preterm birth (PTB, < 37 weeks), low birth weight (LBW, < 2500g) and small (SGA), adequate (AGA) and large (LGA) for GA, defined as having a value below, between or beyond the ±1.28 z/GA score, the usual clinical cut-off to demarcate the 10th and 90th percentiles. Results: PTB was observed in 17.5%, LBW in 20.2% and SGA FW, BL, HC and AC in 16.2%, 19.1%, 13.8%, and 17.4% respectively. The proportions in HIV-only and AIDS cases were: PTB: 5.9 versus 27.5%, LBW: 14.7% versus 25.0%, SGA BW: 17.6% versus 15.0%, BL: 6.0% versus 30.0%, HC: 9.0% versus 17.9%, and AC: 13.3% versus 21.2%; only SGA BL attained a significant difference. Out of 15 cases of LBW, eight (53.3%) were preterm only, four (26.7%) were SGA only, and three (20.0%) were both PTB and SGA cases. A concomitant presence of, at least, two SGA dimensions in the same fetus was frequent. Conclusions: The proportions of preterm birth and low birth weight were higher than the local and Brazilian prevalence and a trend was observed for higher proportions of SGA fetal dimensions than the expected population distribution in this small casuistry of newborn from the HIV-infected, low income, antiretroviral users, and publicly assisted pregnant women. A trend for higher prevalence of PTB, LBW and SGA fetal dimensions was also observed in infants born to mothers with AIDS compared to HIV-infected mothers without AIDS.

Compound 16.842 was tried with three different groups of patients in order to evaluate its tolerancy, and efficacy as well, with a view of using the drug in mass campaigns against hookworm. Group I, used for a preliminary trial, consisted of 38 patients attending an out-patient clinic, and living either in the out-skirts or in the various urban areas of the city of Rio de Janeiro, including some inmates of an orphanage. Group II, a field trial, was carried out in two farms, where the drug was administered both to the positive cases (124) and to the rest of the population (nearly 90%). Group III, a field trial was also carried out in a small town where nearly 40% of the total population was treated with the Compound. Tolerability was considered rather satisfactory, mainly among the patients receiving two single doses (50-150mg), according to the age, 4 - 6 weeks apart). These results suggest that 2-4 courses of therapy within a shorter span of time should be the ideal for a mass treatment campaign. Efficacy varied from 26.6% to 76.2% parasitological cure in the various groups, with a wide range of variation also in the percent of mean reduction of eggs for hookworm. The drug showed also some effect against Ascaris lumbricoides giving cure rates between 10,5% and 35.7% in the various groups, with a percentage reduction in mean egg counts of 27% to 83.3% according to the various groups. It was concluded that Compound 16.842 possesses a marked effect on hookworm and a mild effect on A. lumbricoides. The findings indicate the need for more accurate studies to determine the most efficient schedules of treatment and the real value of the drug, as compared to other antihelminthics against the two parasites under study.
