O objetivo desta pesquisa foi avaliar os fatores preditores e o grau de adesão aos anti-retrovirais por meio de auto-relato e data de retirada da medicação. Trata-se de um estudo transversal em que foram entrevistados 67 pacientes. Foram considerados aderentes aqueles pacientes que utilizaram mais de 90% das doses. Os resultados de adesão foram: referida (72,7%); calculada por dose esquecida no último dia (70%); por três dias (76,1%); por sete dias (80,5%); e por quinze dias (80,5%); calculada pela data de retirada da medicação no período de três meses (53,7%); e em seis meses (47,8%). As variáveis associadas significativamente com a adesão foram: escolaridade, fato de viver com a família, referir boa adesão, avaliação positiva da terapia anti-retroviral, diagnóstico por doença oportunista, menor contagem de linfócitos CD4 maior que 200 células/mm³ e o fato de estar no primeiro tratamento. A partir da identificação dos fatores que comprometem a adesão, devem ser elaboradas estratégias individuais e coletivas para a promoção da adesão.

Averiguou-se a soroprevalência para HIV, sífilis, toxoplasmose, citomegalovirose e rubéola em gestantes de Sergipe, nordeste do Brasil, verificando-se a associação com idade e procedência. Selecionaram-se 9.550 gestantes (2.112 da capital e 7.438 do interior) testadas consecutivamente durante o primeiro atendimento pré-natal em 2007. Foram encontradas as seguintes frequências de soropositividade: sífilis (0,9%; IC95% 0,7%-1,6%), HIV (0,14%; IC95% 0,08%-0,2%), toxoplasmose (IgG 69,3%; IC95% 68,3%-70,2%; IgM 0,4%, IC95% 0,3%-0,6%), citomegalovirose (IgG 76,6%, IC95% 75,7%-77,5%; IgM 0,2%, IC95% 0,09%-0,3%) e rubéola (IgG 71,6%, IC95% 70,7%-72,6%; IgM 0,1%, IC95% 0,04%-0,2%). A soropositividade para toxoplasmose incrementou com a idade. A prevalência de anticorpos IgG para toxoplasmose, CMV e rubéola foi maior na capital, Aracaju, que nos demais municípios do Estado de Sergipe. Foi encontrada associação entre a idade e soropositividade para a toxoplasmose. Os achados revelam grande proporção de gestantes susceptíveis a toxoplasmose, rubéola e citomegalovirose, principalmente no interior do estado, com risco para seus filhos.

This study aimed to identify the main comorbidities in elderly chagasic patients treated in a reference service and identify possible associations between the clinical form of Chagas' disease and chronic diseases. Ninety patients aged 60 years-old or over were interviewed and their clinical diagnoses recorded. The study population profile was: women (55.6%); median age (67 years); married (51.1%); retired (73.3%); up to four years' education (64.4%); and earning less than two minimum wages (67.8%). The predominant forms of Chagas' disease were the cardiac (46.7%) and mixed forms (30%). There was a greater proportion of mild cardiac dysfunction (84.1%), frequently in association with megaesophagus. The mean number of concurrent diseases was 2.856 ± 1.845, and 33% of the patients had four or more comorbidities. The most frequent were systemic arterial hypertension (56.7%), osteoporosis (23.3%), osteoarthritis (21.2%) and dyslipidemia (20%). Positive correlations were verified between sex and comorbidities and between age group and comorbidities.

O objetivo deste estudo foi determinar as fontes de alimento sanguíneo de fêmeas de Lutzomyia whitmani, espécie de flebotomíneo incriminada no Maranhão como principal vetor da leishmaniose cutânea americana. Para isso, 70 fêmeas desta espécie coletadas no município de Axixá, área com um dos maiores números de casos de leishmaniose cutânea americana em humanos no Maranhão, foram analisadas utilizando a técnica da precipitina. Dos indivíduos analisados, 90% apresentaram reação a algum tipo de antissoro e dentre estes, 73% apresentaram reações do tipo simples com predominância para sangue de galinha (22,2%), roedor (14,3) e humano (12,7%). Nas reações duplas predominaram as combinações galinha/humano (6,3%), galinha/gambá (4,8%), boi/humano e gambá/humano (3,2%). Assim, concluímos que seres humanos, animais domésticos e sinantrópicos constituem fonte alimentar sanguínea para Lutzomyia whitmani podendo desempenhar um papel importante no ciclo de transmissão da leishmaniose cutânea americana explicando os casos da doença em Axixá.

O estudo objetivou avaliar a atividade in vivo do itraconazol e terbinafina no tratamento da esporotricose cutânea experimental. Foram utilizados 80 ratos Wistar divididos em quatro grupos (TERB20, TERB30, ITRA e CONT) inoculados no coxim plantar esquerdo com 0,2ml de solução contendo 2x103 células de Sporothrix schenckii/ml e tratados com terbinafina (20 e 30mg/kg), itraconazol (10mg/kg) e placebo durante 13 semanas. As lesões do sítio de inoculação foram avaliadas e mensuradas semanalmente, assim como a disseminação das mesmas. Após foi realizada análise micológica e histopatológica. Os resultados demonstraram que os animais do grupo ITRA diferiram estatisticamente em todos os parâmetros avaliados em relação ao CONT. Em relação à terbinafina, não houve diferenças estatísticas entre os grupos tratados e controle. Pode-se confirmar a boa atividade do itraconazol no tratamento da esporotricose e a pouca eficácia da terbinafina nas doses utilizadas, sendo necessários mais estudos com este antifúngico.

INTRODUÇÃO: O Cryptococcus neoformans é uma levedura capsulada, agente etiológico da criptococose em humanos e animais, encontrado em fontes ambientais, incluindo excretas de pombos, é uma importante causa de mortalidade em indivíduos imunodeprimidos em todo o mundo. MÉTODOS: Com o objetivo de verificar a ocorrência do Cryptococcus neoformans, em excretas de pombos, na Cidade de Pelotas, pesquisou-se 70 ambientes, incluindo prédios, praças e locais ao ar livre, da Cidade de Pelotas, RS. Após a coleta, os excrementos foram adicionados de salina com cloranfenicol, homogeneizados, semeados em ágar Sabouraud com cloranfenicol e ágar Níger e incubados a 32ºC. Para identificação, realizou-se exame direto, prova da fenoloxidase, urease, assimilação de carboidratos e cultura em meio CGB. RESULTADOS: Dos locais estudados (nº =70), em 26 (37,1%) havia excretas de pombos. Estes lugares foram representados por prédios históricos (nº =8), torre de igreja (nº =1), engenhos e armazéns de arroz (nº =7), praça (nº =1) e locais ao ar livre (nº =9), o isolamento de Cryptococcus neoformans ocorreu em 26,9% (nº =7/26), destes locais. CONCLUSÕES: O estudo chama a atenção, para o isolamento do fungo em áreas urbanas, que apresentavam grande acúmulo de excretas, indicando um risco para a saúde pública, especialmente para indivíduos imunocomprometidos.

INTRODUCTION: Isoenzymatic analyses were performed involving species of the Nyssorhynchus and Anopheles subgenera in order to estimate the intra and interspecies genetic variability. METHODS: Mosquitoes were caught at different localities in the Amazon region. The collection and rearing of mosquitoes in the laboratory followed specific protocols. For the genetic variability analyses, the technique of horizontal electrophoresis on starch and starch-agarose gel with appropriate buffer systems was used. The alloenzyme variation was estimated using the Biosys-1 software. RESULTS: Out of the 13 loci, eight were polymorphic. Anopheles nuneztovari presented the largest number of alleles per locus, while the smallest number was detected in Anopheles marajoara from Macapá. The largest number of polymorphic loci was found for Anopheles marajoara from Maruanum and the smallest for Anopheles benarrochi (Guayará Mirim). Anopheles darlingi (Macapá) presented the greatest heterozygosity (Ho = 0.167 ± 0.071), while the lowest heterozygosity (Ho = 0.045 ± 0.019) was observed in Anopheles intermedius (Pacoval) of the subgenus Anopheles. Wright's F coefficient revealed considerable genetic structuring between the populations of Anopheles darlingi (Fst = 0.110) and between the populations of Anopheles marajoara (Fst = 0.082). CONCLUSIONS: Considering all the species studied, the genetic distance ranged from 0.008 to 1.114. The greatest distance was between Anopheles mattogrossensis and Anopheles oswaldoi, while the smallest was between the Anopheles benarrochi populations.

INTRODUCÃO: Estudo retrospectivo com o objetivo de avaliar a prevalência e fatores associados à soropositividade para o HTLV-1/2, no período de 1995 a 2008, no Hemocentro Regional de Uberaba e descrever os doadores soropositivos quanto ao gênero, idade, estado civil, cor de pele e procedência. MÉTODOS: Foram realizados análise estatística descritiva, testes qui-quadrado e odds ratio para comparação de proporções e gráfico de dispersão com coeficiente de correlação linear. RESULTADOS: Dentre x doadores testados, foi encontrada a prevalência de sorologia positiva para o HTLV de 0,02% e indeterminada de 0,09%. Houve uma redução significativa da sorologia positiva para HTLV, no período de 2002 a 2008, em comparação ao período de 1995 a 2001. Dentre os soropositivos, observou predomínio significante no gênero feminino. CONCLUSÕES: Imputamos a queda gradativa de soropositividade no período à exclusão permanente dos doadores de repetição soropositivos e ao aprimoramento dos métodos de triagem clínica e dos testes sorológicos ao longo dos anos com reflexos positivos na segurança transfusional.

INTRODUÇÃO: O presente estudo buscou identificar os efeitos adversos e o efeito protetor da vacina contra influenza, em idosos acima de 60 anos, aplicada pela rede pública no município de Tubarão, Santa Catarina. MÉTODOS: Foi realizada uma coorte prospectiva de pacientes durante o período de maio a setembro de 2008. Foram recrutados 341 idosos, sendo 289 vacinados na rede pública (VSUS) e 52 não vacinados (NV). A incidência do efeito protetor foi verificada através da comparação dos grupos VSUS com os NV. RESULTADOS: Um percentual de 22,5% de pacientes apresentou pelo menos um efeito adverso. Comparando o grupo VSUS e NV quanto à incidência de sintomas gripais durante o inverno, a presença de pelo menos um sintoma foi verificada em 47% e 28,8% (RR = 1,11; 1,02-1,22; p = 0,0156), respectivamente. CONCLUSÕES: Os resultados apontam uma baixa incidência de efeitos adversos. A maior incidência de sintomas gripais no grupo VSUS comparado ao NV pode estar relacionado às características da população que costuma fazer a vacina contra influenza.

INTRODUCTION: This study was developed to evaluate the situation of leprosy in the general population of the municipality of Buriticupu, State of Maranhão, Brazil. METHODS: We used the method of active search to identify new cases from 2008 to 2010. Bacilloscopy of intradermal scrapings was performed in all patients with skin lesions compatible with leprosy, and histopathological examination in those who had doubts on the definition of the clinical form. RESULTS: The study included 19,104 individuals, with 42 patients diagnosed with leprosy after clinical examination, representing a detection rate of 219.84 per 100,000 inhabitants. The predominant clinical presentation was tuberculoid with 24 (57.1%) cases, followed by borderline with 11, indeterminate with four, and lepromatous with three cases. The study also allowed the identification of 81 patients with a history of leprosy and other skin diseases, such as pityriasis versicolor, dermatophytosis, scabies, vitiligo, and skin carcinoma. The binomial test showed that the proportion of cases in the headquarters was significantly higher than that in the villages (p = 0.04), and the generalized exact test showed that there was no association between age and clinical form (p = 0.438) and between age and gender (p = 0.083). CONCLUSIONS: The elevated detection rate defines the city as hyperendemic for leprosy; the active search for cases, as well as the organization of health services, is an important method for disease control.

INTRODUCTION: Some viruses of the Herpesviridae family are frequently the etiologic agents of oral lesions associated with HIV. The aim of this study was to identify the presence of herpes simplex virus types 1 and 2 (HSV-1, HSV-2), Varicella Zoster virus (VZV), Epstein-Barr virus (EBV), human cytomegalovirus (HCMV), human herpesvirus type 6, type 7 and type 8 (HHV-6, HHV-7 and HHV-8) in the oral cavity of HIV-infected children/adolescents and verify the association between viral subtypes and clinical factors. METHODS: The cells of oral mucosa were collected from 50 HIV infected children/adolescents, 3-13 years old (mean age 8.66). The majority (66%) of selected were girls, and they were all outpatients at the pediatric AIDS clinic of a public hospital in Rio de Janeiro. Nested-PCR was used to identify the viral types. RESULTS: Absence of immunosuppression was observed in 66% of the children. Highly active antiretroviral therapy (HAART) was used by 72.1% of selected and moderate viral load was observed in 56% of the children/adolescents. Viral types were found in 86% of the children and the subtypes were: HSV-1 (4%), HSV-2 (2%), VZV (4%), EBV (0%), HCMV (24%), HHV6 (18%), HHV-7 (68%), HHV8 (0%). CONCLUSIONS: The use of HAART has helped to reduce oral lesions, especially with herpes virus infections. The health professionals who work with these patients should be aware of such lesions because of their predictive value and the herpes virus can be found circulating in the oral cavity without causing lesions.

INTRODUCTION: Sequential antibiotic therapy (SAT) is safe and economical. However, the unnecessary use of intravenous (IV) administration usually occurs. The objective of this work was to get to know the effectiveness of an intervention to implement the SAT in a teaching hospital in Brazil. METHODS: This was a prospective and interventional study, historically controlled, and was conducted in the Hospital de Clínicas, Universidade Federal de Uberlândia, State of Minas Gerais, Brazil, a high complexity teaching hospital having 503 beds. In each of the periods, from 04/04/05 to 07/20/05 (pre-intervention) and from 09/24/07 to 12/20/07 (intervention), 117 patients were evaluated. After the pre-intervention period, guidelines were developed which were implemented during the intervention period along with educational measures and a reminder system added to the patients' prescription. RESULTS: In the pre-intervention and intervention periods, the IV antibiotics were used as treatment for a average time of 14.8 and 11.8 days, respectively. Ceftriaxone was the antibiotic most prescribed in both periods (23.4% and 21.6% respectively). Starting from the first prescription of antibiotics, the average length of hospitalization time was 21.8 and 17.5 days, respectively. The SAT occurred only in 4 and 5 courses of treatment, respectively, and 12.8% and 18.8% of the patients died in the respective periods. CONCLUSIONS: Under the presented conditions, the evaluated intervention strategy is ineffective in promoting the exchange of the antibiotic administration from IV to oral treatment (SAT).

Introduction We analyzed the vertical and monthly distributions of culicid species in the gallery forest of Brasília National Park, with an emphasis on the potential vectors of yellow fever (YF). Methods Between September 2010 and August 2011, mosquitoes were captured on the ground and in the canopy of the forest for five consecutive days per month, from nine to 15 hours. The mosquitoes were examined to verify natural infection with flaviviruses by isolation in Aedes albopictus Skuse, 1864 cells followed by indirect immunofluorescence. Results We identified 2,677 culicids distributed in 29 species. Most of the mosquitoes were captured at ground level (69%) during the rainy season (86%). The most abundant species were Sabethes (Sabethes) albiprivus Theobald, 1903; Limatus durhamii Theobald, 1901; Haemagogus (Conopostegus) leucocelaenus Dyar & Shannon, 1924; Haemagogus (Haemagogus) janthinomys Dyar, 1921; Aedes (Ochlerotatus) scapularis Rondani, 1848; Psorophora (Janthinosoma) ferox Von Humboldt, 1819; and Aedes (Ochlerotatus) serratus Theobald, 1901. Limatus durhamii, Limatus durhamii, Psorophora ferox, Aedes scapularis and Aedes serratus showed significant differences (p<0.05) in their habitat use. Limatus durhamii was found more often in the canopy, unlike the other species. During the rainy season, the most abundant species were Sa. albiprivus, Haemagogus leucocelaenus and Limatus durhamii. During the dry season, the potential YF vectors exhibited a very low frequency and abundance, except Aedes scapularis and Aedes serratus. No flavivirus was detected in the 2,677 examined mosquitoes. Conclusions We recommend continued and systematic entomological monitoring in areas vulnerable to the transmission of YF in the Federal District of Brazil.

Introduction The incidence of canine leptospirosis in Brazil needs to be assessed Methods The same dogs in southern Brazil were sampled over two years to determine the prevalence, incidence and association of canine leptospirosis with various risk factors. Results In 2009, the prevalence was 33 (14.4%) of 228 dogs, with a predominance of serovar Canicola (33.4%). In 2010, 90 dogs were re-evaluated (the remaining dogs were lost to deaths, address changes and donations), and the prevalence was found to be 35 (38.9%) of 90, with the predominant serovar being Icterohaemorrhagiae (51.4%). Moreover, the incidence was 26 of 90 (28.9%), and the disease was statistically associated with age (2009) and street access (2010). Conclusions Our findings revealed instability in the dog population and age to be relevant risk factors for canine leptospirosis.

Introduction In addition to the common alterations and diseases inherent in the aging process, elderly persons with a history of leprosy are particularly vulnerable to dependence because of disease-related impairments. Objective determine whether physical impairment from leprosy is associated with dependence among the elderly. Methods An analytical cross-sectional study of elderly individuals with a history of leprosy and no signs of cognitive impairment was conducted using a database from a former leprosy colony-hospital. The patients were evaluated for dependence in the basic activities of daily living (BADL) and instrumental activities of daily living (IADL), respectively) and subjected to standard leprosy physical disability grading. Subsequently, descriptive and univariate analyses were conducted, the latter using Pearson's chi-squared test. Results A total of 186 elderly persons were included in the study. Of these individuals, 53.8% were women, 49.5% were older than 75 years of age, 93% had four or less years of formal education, 24.2% lived in an institution for the long-term care of the elderly (ILTC), and 18.3% had lower limb amputations. Among those evaluated, 79.8% had visible physical impairments from leprosy (grade 2), 83.3% were independent in BADL, and 10.2% were independent in IADL. There was a higher impairment grade among those patients who were IADL dependent (p=0.038). Conclusion s: The leprosy physical impairment grade is associated with dependence for IADL, creating the need for greater social support and systematic monitoring by a multidisciplinary team. The results highlight the importance of early diagnosis and treatment of leprosy to prevent physical impairment and dependence in later years.
