OBJETIVO Analisar a estrutura física, as condições de trabalho dos profissionais da saúde e o delineamento de processos estabelecidos em unidades prisionais. MÉTODOS Foram analisados 34 centros de detenção provisória e 69 unidades prisionais masculinas e seis femininas do Estado de São Paulo, Brasil, em 2009. Foi desenvolvido instrumento autoaplicativo para coleta de dados quantitativos sobre as características de estrutura, equipamento e pessoal para atendimento à saúde nas unidades prisionais. A análise de variância (ANOVA) ou equivalente não paramétrico e os testes de Qui-quadrado ou Fisher foram utilizados para comparação de variáveis contínuas ou categóricas, respectivamente, entre os grupos estudados. RESULTADOS Os principais problemas foram o retardo nos resultados de exames laboratoriais e de imagem. Quanto às equipes, grande maioria apresentou condições próximas da proposta pela Comissão InterGestores Bipartite 2013, mas sem que isso se refletisse em melhoria dos indicadores. Com relação ao processo, observou-se que mais de 60,0% das unidades prisionais estão localizadas em cidades pequenas, sem condições estruturais de saúde para garantir o atendimento secundário ou terciário para continuidade do processo de tratamento. CONCLUSÕES O perfil apresentado das unidades prisionais do País poderá ser utilizado para planejamento e acompanhamento de ações futuras para melhoria contínua das condições estruturais de saúde.

OBJETIVO Analisar os fatores associados à persistência à hormonioterapia para câncer de mama visando à melhoria da qualidade do cuidado prestado. MÉTODOS Estudo longitudinal a partir de dados secundários. Foi analisada uma coorte de 5.861 mulheres com câncer de mama registradas em diferentes bancos de dados do Instituto Nacional de Câncer e do Sistema Único de Saúde. Todas as pacientes foram tratadas nesse hospital, que dispensa a medicação gratuitamente, e o período de seguimento foi de janeiro de 2004 a outubro de 2010. Variáveis sociodemográficas, comportamentais, clínicas, de estilos de vida e de aspectos do serviço de saúde integraram-se à análise para testar associação com a persistência ao tratamento hormonal, pelo método de Kaplan-Meier e Riscos Proporcionais de Cox. RESULTADOS A persistência geral à hormonioterapia foi de 79,0% ao final do primeiro ano e 31,0% em cinco anos de tratamento. O risco de descontinuidade à hormonioterapia mostrou-se maior entre mulheres com idade inferior a 35 anos, com estadiamento mais grave (III e IV), usuárias de álcool, que realizaram quimioterapia, e para cada hospitalização, cada exame e cada mês, entre o diagnóstico e o início do tratamento adicional. Na direção oposta, o risco de descontinuidade mostrou-se menor entre as mulheres com nível médio e superior de escolaridade, com companheiro, com história familiar de câncer, submetidas à cirurgia e que tiveram consultas com mastologista e com oncologista clínico. CONCLUSÕES Das mulheres com câncer de mama, 69,0% não persistiram ao término de cinco anos do tratamento hormonal, aumentando o risco de uma resposta clínica inadequada. Os resultados mostram aspectos do cuidado que podem conduzir a melhores respostas ao tratamento.

A simple method of rubella antigen production by treatment with sodium desoxycholate for use in enzyme immunoassay (IMT-ELISA) is presented. When this assay was compared with a commercial test (Enzygnost-Rubella, Behring), in the study of 108 sera and 118 filter paper blood samples, 96.9% (219/226) overall agreement and correlation coefficient of 0.90 between absorbances were observed. Seven samples showed discordant results, negative by the commercial kit and positive by our test. Four of those 7 samples were available, being 3 positive by HI.

Between 1992 and 1997, 790 blood donors with anti-HCV EIA-2 strongly reagent (relationship between the sample optical density/cut-off > 3) detected at the blood bank serological screening, were evaluated in ambulatory environment. They were all negative for Chagas disease, syphilis, hepatitis B (HBsAg) and AIDS. Blood samples were collected at the first ambulatorial evaluation, for hemogram, biochemical tests and new serological tests for HCV (anti-HCV EIA-2). In blood samples of 226 repeatedly reagent anti-HCV EIA-2 blood donors, supplementary "immunoblot" test for HCV (RIBA-2) was used. In 209 donors, the presence of HCV-RNA was investigated by the PCR test. The abdominal ultrasonography was realized in 366 donors. In 269 patients liver biopsy was performed for the histopathological study. The follow-up of blood donors showed that 95.6% were repeatedly EIA-2 reagent, 94% were symptomless and denied any hepatitis history, with only 2% mentioning previous jaundice. In 47% of this population at least one risk factor has been detected for the HCV transmission, the use of intravenous drugs being the main one (27.8%). Blood transfusion was the second factor for HCV transmission (27.2%). Hepatomegaly was detected in 54% of the cases. Splenomegaly and signs of portal hypertension have seldom been found in the physical examination, indicating a low degree of hepatic compromising in HCV. Abdominal ultrasound showed alterations in 65% of the subjects, being the steatosis the most frequent (50%). In 83.5% of the donors submitted to the liver biopsy, the histopathological exam showed the presence of chronic hepatitis, usually classified as active (89%) with mild or moderate grade in most of the cases (99.5%). The histopathological exam of the liver was normal in 1.5% of blood donors. The RIBA-2 test and the HCV-RNA investigation by PCR were positive in respectively 91.6 and 75% of the anti-HCV EIA-2 reagent donors. The HCV-RNA research was positive in 82% of the RIBA-2 positive subjects, in 37.5% of the indeterminate RIBA-2 donors and in 9% of the negative RIBA-2 donors. Chronic hepatitis has also been observed in 50% of the histopathological exams of the anti-HCV EIA-2 reagent donors which were indeterminate RIBA-2. Among 18 blood donors with minimal changes histopathological exam 11 (61%) were HCV-RNA positive. Our blood donors anti-HCV reagent generally had clinical, laboratorial and histopathological features observed in patients with chronic HCV hepatitis and a high proportion could be identified in interviews and medical evaluation realized in blood blanks. Generally, these HCV infected donors are identified and discharged only by the serological tests results.

The present study intended to analyze the seroprevalence of Helicobacter pylori, IgG, and its relation to dyspepsia in a population from the western Amazon region. During the "Projeto Bandeira Científica", a University of São Paulo Medical School program, in Monte Negro's rural areas, state of Rondônia, 266 blood samples were collected from volunteers. The material was tested for IgG antibodies anti-Helicobacter pylori by ELISA method and the participants were also interviewed on dyspepsia, hygiene and social aspects. Participants aged between five and 81 years old (34 years on average), 149 (56%) were female and 117 (44%) male. We found 210 (78.9%) positive, 50 (18.8%) negative and six (2.3%) undetermined samples. Dyspeptic complaints were found in 226 cases (85.2%). There was no statistical association between dyspepsia and positive serology for H. pylori. We concluded that the seroprevalence in all age categories is similar to results found in other studies conducted in developing countries, including those from Brazil. On the other hand, the seroprevalence found in Monte Negro was higher than that reported in developed countries. As expected, there was a progressive increase in the positivity for H. pylori in older age groups.

We analyzed the sandflies around houses and domestic animal shelters located in residences close to forests in localities on the banks of the Araguari River, Uberlândia, MG, from February 2003 to November 2004. The phlebotomines were captured in the peridomiciliary area, where Shannon traps were utilized in the peridomicile and CDC traps in animal shelters. 2,783 specimens of sandflies were captured, 2,140 females (76.9%) and 643 males (23.1%), distributed between 17 species. The most abundant species was Nyssomyia neivai (88.1%), followed by Nyssomyia whitmani (3.1%). The presence of Lutzomyia longipalpis was also confirmed, it is the main vector of Leishmania (L.) infantum chagasi which causes visceral leishmaniasis. The presence of species involved in the transmission of leishmaniases in the municipality of Uberlândia is cause for concern. The presence of L. longipalpis indicates that its urbanization may not have been aleatory and instead occurred through the destruction of wild ecotopes. More studies of their occupation in anthropic environments need to be made.

Cryptosporidium spp. and Cystoisospora belli are monoxenic protozoa that have been recognized as the causative agents of chronic diarrhea in immunocompromised individuals, especially HIV-infected subjects. The objective of this study was to evaluate the frequency of these intestinal protozoa in HIV-positive patients in the Triângulo Mineiro region of Brazil and to correlate the presence of these infections with clinical, epidemiological and laboratory data of the patients. Oocysts were detected in stool samples of 10 (16.9%) of the 59 patients studied, while Cryptosporidium spp. were present in 10.1% (6/59) and C. belli in 6.7% (4/59). The frequency of these parasites was higher among patients with diarrheic syndrome and CD4+ T lymphocyte counts < 200 cells/mm 3 , demonstrating the opportunistic characteristic of these infections. A significant association was observed between the lack of adherence to antiretroviral therapy and the presence of Cryptosporidium spp. and/or C. belli. Parasitism with Cryptosporidium spp. was more frequent in February and April, the months following the period of high rainfall. The same was not observed for C. belli. Genetic characterization of two isolates led to the identification of Cryptosporidium parvum, one of the main species associated with the zoonotic transmission of cryptosporidiosis.

We conducted a serological survey to determine the presence of hantavirus infection in rodents in Uberlândia, Minas Gerais as well as to identify and characterize associated factors. Rodents were captured using Sherman live-capture traps set in rural and peri-urban environments. A total of 611 rodents were captured. There was a higher trap success in peri-urban areas (26.3%) and a higher prevalence of antibodies among rodents captured in rural areas (2.9%). Necromys lasiurus was the most common species (42.2%) and the more frequently infected (4.6%). One Calomys tener (1/141; 0.7%) and one Calomys sp. (1/14; 7.1%) were also positive for the hantavirus infection. In N. lasiurus, antibody prevalence correlated with population density (p < 0.01), age class (p = 0.003) and presence of scars (p = 0.02). The data confirm that horizontal transmission is the main mechanism that maintains the virus in nature. The higher seropositivity in N. lasiurus is consistent with genetic studies that associate this species with an Araraquara virus reservoir; the seropositivity of C. tener and Calomys sp. may indicate the occurrence of spillover infection or the presence of other circulating hantaviruses.

SUMMARY Little is known about the frequency of intestinal parasites in the elderly due to a lack of attention given to the occurrence of these infections among older adults. This study compares the frequency of Strongyloides stercoralis and other enteroparasites between elderly living in nursing homes (n = 100) and those noninstitutionalized (n = 100) from Uberlândia, state of Minas Gerais, southeastern Brazil, associated with data of epidemiological and socio-demographic conditions. Through coproparasitological examination of both groups, enteroparasites were detected in 15 of 200 individuals examined (7.5%; CI: 5.1- 9.9). S. stercoralis was the most frequent parasite 10/200 (5%; CI: 4.2-5.8), being significantly higher in males and in individuals with autonomy for daily living activities. There were no statistical differences in the prevalence of parasites between the two groups compared. In conclusion, S. stercoralis infection was highly prevalent in elderly patients and it does not depend on whether the individual was institutionalized or not.

Um grupo de 226 universitários do sexo masculino, cursando os 3 primeiros anos da escola, foram interrogados com relação às moléstias venéreas adquiridas durante êste período. A enfermidade mais freqüente foi a gonorréia, com uma incidência anual média de 119 por mil para os indivíduos que moravam com suas famílias, e de 153 por mil para aqueles que residiam em "repúblicas" de estudantes. Embora se tratasse de um estudo retrospectivo, os dados colhidos parecem suficientemente exatos para permitir a exclusão de uretrites de outras origens. No mesmo grupo foram observados 5 casos de sífilis, e respectivamente 9, 7 e 10 casos de condylomata acuminata, cancro mole e uretrite não específica. Comenta-se a terapêutica medicamentosa, totalmente ortodoxa, empregada bem como as medidas profiláticas, também inadequadas. A atitude dos universitários quanto ao problema preventivo revelou-se suficientemente madura, e medidas de controle das enfermidades venéreas parecem inteiramente viáveis nêste meio.

Revendo a literatura não encontramos estudos anatômicos dos gânglios intrapancreáticos na forma crônica da doença de Chagas; lesões dos mesmos poderiam explicar, ao menos em parte, os distúrbios funcionais do pâncreas exócrino e endócrino descritos nesta forma da doença. Decidimos então analisar morfologicamente tais gânglios. Para isso, estudamos segmentos transversais da cabeça, corpo e cauda do pâncreas de doze chagásicos crônicos, com idade média de 46,5 ± 9,1 anos, e quatorze controles, com idade média de 41,2 ± 11,0 anos. Os segmentos foram processados histologicamente e seccionados de forma seriada até o esgotamento, analisando-se os cortes múltiplos de sete. Para análise estatística, usamos o teste não-paramétrico de Mann-Whitney. Na cabeça do pâncreas, a contagem de neurônios teve média de 57,3 ± 50,8 para o grupo chagásico e 117,5 ± 99,0 para o grupo controle (p < 0,05); no corpo, 25,9 ± 19,4 para o grupo chagásico e 54,7 ± 47,8 para o controle (p < 0,05); na cauda, 23,4 ± 16,3 para o chagásico e 54,1 ± 29,2 para o controle (p < 0,01), sendo a contagem total de 106,6 ± 71,1 para o chagásico e 226,3 ± 156,5 para o controle (p < 0,01). Nossos achados nos permitiram concluir que: a) ocorreu despopulação neuronal estatisticamente significante no grupo chagásico em relação ao controle, em cada segmento pancreático analisado, bem como no órgão como um todo; b) 50% dos chagásicos tiveram número total de neurônios inferior ao menor número dos controles (80); c) 75% e 91,6% dos chagásicos tiveram número de neurônios inferior, respectivamente, à mediana (171) e à média (226) do grupo controle; d) assim, a despopulação neuronal pancreática foi frequente, porém não constante; e) o fator idade não pareceu ter sido o responsável pela despopulação neuronal dos chagásicos.

Com objetivo de estudar a situação da filariose linfática em Alagoas, foi realizado um inquérito hemoscópico na população geral de áreas urbanas das três diferentes regiões fisiográficas do estado. Dos 101 municípios, foram pesquisados aleatoriamente 10, sendo a bancroftose detectada somente na capital, Maceió. Em um estudo seccional feito com a população geral de 4 bairros desta cidade foram examinados 10.973 indivíduos sendo detectados 226 microfilarêmicos, com prevalências nos bairros variando de 0 a 5,4%. Tanto a prevalência de microfilarêmicos como a microfilaremia média foram significativamente maiores em indivíduos do sexo masculino. Entre os examinados não nascidos em Maceió, o tempo de residência na área endêmica foi significativamente maior entre microfilarêmicos que entre amicrofilarêmicos. Baseado nestes dados, medidas de controle já foram implementadas visando a eliminação da filariose linfática na região.

A population-based cross-sectional study was set up in Sabará country, Southeastern Brazil, to identify asymptomatic human visceral leishmaniasis in an urban area of low disease prevalence. Blood was collected on filter paper (n=1,604 inhabitants) and examined by indirect immunofluorescent test, enzyme-linked immunosorbent assay and immunochromatographic strip test. The prevalence rates of infection ranged from 2.4 to 5.6% depending on the test used. One year later, venous blood was collected in a subset of 226 participants (102 seropositive and 124 seronegative). The tests performed were IFAT, ELISA, rk39-ELISA, polymerase chain reaction and hybridization with Leishmania donovani complex probe. No clinical signs or symptoms of leishmaniasis were observed. Using hybridization as a reference test, the sensitivity and specificity of serology were respectively: 24.8 and 71% (ELISA); 26.3 and 76.3% (rk-39); 30.1 and 63.4% (IFAT). Due to disagreements, different criteria were tested to define the infection and hybridization should be considered in epidemiological studies.

INTRODUCTION: Hantavirus is a genus of ribonucleic acid (RNA) viruses included in the family Bunyaviridae. Hantaviruses are rodent-borne zoonoses that, in the last 18 years, became an emergent public health problem in the Americas, causing a severe cardiopulmonary syndrome. This disease has no specific treatment and has a high case fatality. The transmission of hantavirus to man occurs by inhaling aerosols of rodent excreta. The aim of this study was to determine the prevalence of antibodies to hantavirus in the population of the rural settlement of Tupã in the county of Marcelândia, state of Mato Grosso, Brazil. METHODS: The participants of the serologic survey were visited at their homes and selected randomly among the settlement population. Blood samples of the participants were collected by venopuncture. The serum samples were tested by an IgG-ELISA using an N recombinant protein of Araraquara hantavirus as antigen, using the protocol previously established by Figueiredo et al. RESULTS: IgG antibodies to hantavirus were detected in 7 (13%) of the 54 participants. The positivity was higher among men. It was observed that there was an association of seropositivity to hantavirus within the participants born in the south of Brazil. CONCLUSIONS: The results suggest that, in this rural area, everyone is exposed to the same risk of becoming infected with hantavirus, and, therefore, there is a need to intensify surveillance activities and education of the local people to prevent this viral infection.

INTRODUCTION: Sequential antibiotic therapy (SAT) is safe and economical. However, the unnecessary use of intravenous (IV) administration usually occurs. The objective of this work was to get to know the effectiveness of an intervention to implement the SAT in a teaching hospital in Brazil. METHODS: This was a prospective and interventional study, historically controlled, and was conducted in the Hospital de Clínicas, Universidade Federal de Uberlândia, State of Minas Gerais, Brazil, a high complexity teaching hospital having 503 beds. In each of the periods, from 04/04/05 to 07/20/05 (pre-intervention) and from 09/24/07 to 12/20/07 (intervention), 117 patients were evaluated. After the pre-intervention period, guidelines were developed which were implemented during the intervention period along with educational measures and a reminder system added to the patients' prescription. RESULTS: In the pre-intervention and intervention periods, the IV antibiotics were used as treatment for a average time of 14.8 and 11.8 days, respectively. Ceftriaxone was the antibiotic most prescribed in both periods (23.4% and 21.6% respectively). Starting from the first prescription of antibiotics, the average length of hospitalization time was 21.8 and 17.5 days, respectively. The SAT occurred only in 4 and 5 courses of treatment, respectively, and 12.8% and 18.8% of the patients died in the respective periods. CONCLUSIONS: Under the presented conditions, the evaluated intervention strategy is ineffective in promoting the exchange of the antibiotic administration from IV to oral treatment (SAT).
