It has been previously shown that dextran sulfate administered to diabetic rats accumulates in the liver and kidney, and this could be due to a malfunction of the lysosomal digestive pathway. The aim of the present study was to evaluate the expression and activities of lysosomal enzymes that act upon proteins and sulfated polysaccharides in the livers of diabetic rats. Diabetes mellitus was induced by streptozotocin in 26 male Wistar rats (12 weeks old), while 26 age-matched controls received only vehicle. The livers were removed on either the 10th or the 30th day of the disease, weighed, and used to evaluate the activity, expression, and localization of lysosomal enzymes. A 50-60% decrease in the specific activities of cysteine proteases, especially cathepsin B, was observed in streptozotocin-induced diabetes mellitus. Expression (mRNA) of cathepsins B and L was also decreased on the 10th, but not on the 30th day. Sulfatase decreased 30% on the 30th day, while glycosidases did not vary (or presented a transitory and slight decrease). There were no apparent changes in liver morphology, and immunohistochemistry revealed the presence of cathepsin B in hepatocyte granules. The decrease in sulfatase could be responsible for the dextran sulfate build-up in the diabetic liver, since the action of sulfatase precedes glycosidases in the digestive pathway of sulfated polysaccharides. Our findings suggest that the decreased activities of cathepsins resulted from decreased expression of their genes, and not from general lysosomal failure, because the levels of glycosidases were normal in the diabetic liver.

The present study aimed to investigate visceral adipose tissue-specific serpin (vaspin) concentrations in serum and term placentas and relate these values to insulin resistance and lipid parameters in women with gestational diabetes mellitus (GDM). A total of 30 GDM subjects and 27 age-matched pregnant women with normal glucose tolerance (NGT, control) were included. Serum glucose, glycated hemoglobin (HbA1c), lipid profile, insulin, and vaspin were measured at the end of pregnancy, and homeostasis model of assessment-insulin resistance (HOMA-IR) values were calculated. Vaspin mRNA and protein levels in placentas were measured by real-time fluorescence quantitative reverse transcription polymerase chain reaction (RT-qPCR) and Western blotting, respectively. Serum vaspin levels were significantly lower in the GDM group than in controls (0.49±0.24 vs 0.83±0.27 ng/mL, respectively; P<0.01). Three days after delivery, serum vaspin levels were significantly decreased in subjects with GDM (0.36±0.13 vs0.49±0.24 ng/mL, P<0.01). However, in the GDM group, serum vaspin levels were not correlated with the parameters evaluated. In contrast, in the control group, serum vaspin levels were positively correlated with triglycerides (TG; r=0.45, P=0.02) and very low-density lipoprotein cholesterol (VLDL-C; r=0.42, P=0.03). Placental mRNA vaspin (0.60±0.32 vs0.68±0.32, P=0.46) and protein (0.30±0.08 vs0.39±0.26; P=0.33) levels in the GDM group did not differ significantly from those in the control group, but were negatively correlated with neonatal birth weight in the GDM group (r=-0.48, P=0.03; r=-0.88; P<0.01). Our findings indicated that vaspin may be an important adipokine involved in carbohydrate and lipid metabolism and may also play a role in fetal development.

The production of oxygen free radicals in type 2 diabetes mellitus contributes to the development of complications, especially the cardiovascular-related ones. Peroxiredoxins (PRDXs) are antioxidant enzymes that combat oxidative stress. The aim of this study was to investigate the associations between the levels of PRDX isoforms (1, 2, 4, and 6) and cardiovascular risk factors in type 2 diabetes mellitus. Fifty-three patients with type 2 diabetes mellitus (28F/25M) and 25 healthy control subjects (7F/18M) were enrolled. We measured the plasma levels of each PRDX isoform and analyzed their correlations with cardiovascular risk factors. The plasma PRDX1, -2, -4, and -6 levels were higher in the diabetic patients than in the healthy control subjects. PRDX2 and -6 levels were negatively correlated with diastolic blood pressure, fasting blood sugar, and hemoglobin A1c. In contrast, PRDX1 levels were positively correlated with low-density lipoprotein and C-reactive protein levels. PRDX4 levels were negatively correlated with triglycerides. In conclusion, PRDX1, -2, -4, and -6 showed differential correlations with a variety of traditional cardiovascular risk factors. These results should encourage further research into the crosstalk between PRDX isoforms and cardiovascular risk factors.

Transforming growth factor beta 1 (TGF-β1) and bone morphogenetic protein-2 (BMP-2) are important regulators of bone repair and regeneration. In this study, we examined whether TGF-β1 and BMP-2 expressions were delayed during bone healing in type 1 diabetes mellitus. Tibial fractures were created in 95 diabetic and 95 control adult male Wistar rats of 10 weeks of age. At 1, 2, 3, 4, and 5 weeks after fracture induction, five rats were sacrificed from each group. The expressions of TGF-β1 and BMP2 in the fractured tibias were measured by immunohistochemistry and quantitative reverse-transcription polymerase chain reaction, weekly for the first 5 weeks post-fracture. Mechanical parameters (bending rigidity, torsional rigidity, destruction torque) of the healing bones were also assessed at 3, 4, and 5 weeks post-fracture, after the rats were sacrificed. The bending rigidity, torsional rigidity and destruction torque of the two groups increased continuously during the healing process. The diabetes group had lower mean values for bending rigidity, torsional rigidity and destruction torque compared with the control group (P<0.05). TGF-β1 and BMP-2 expression were significantly lower (P<0.05) in the control group than in the diabetes group at postoperative weeks 1, 2, and 3. Peak levels of TGF-β1 and BMP-2 expression were delayed by 1 week in the diabetes group compared with the control group. Our results demonstrate that there was a delayed recovery in the biomechanical function of the fractured bones in diabetic rats. This delay may be associated with a delayed expression of the growth factors TGF-β1 and BMP-2.

INTRODUÇÃO: A causa mais frequente de insuficiência renal crônica (IRC) nos pacientes iniciando tratamento dialítico nos países desenvolvidos é o diabetes mellitus (DM), com índices crescentes e que hoje se aproximam de 50%. No Brasil, os dados disponíveis indicam que essa prevalência é inferior, em torno de 27%, embora venha aumentando. OBJETIVOS: Estimar a prevalência de DM na população adulta de pacientes em tratamento hemodialítico por IRC em Porto Alegre, Brasil. MÉTODOS: Estudo transversal descritivo, quantitativo e analítico, com inquérito sobre a prevalência de DM entre os 1.288 pacientes em tratamento por hemodiálise (HD) crônica nas 15 clínicas do município nos meses de junho e julho de 2009. Resultados: 488 pacientes diabéticos foram identificados, uma prevalência de 37,9%, variando de 21 - 75% nas diferentes clínicas da cidade. CONCLUSÕES: A prevalência de DM entre pacientes em HD crônica em Porto Alegre é muito superior ao que tem sido atribuído como causa de IRC no país, o que indica a possibilidade de que essa etiologia de IRC possa estar sendo subestimada.

INTRODUÇÃO: O controle intensivo da glicemia reduz significativamente o risco de desenvolvimento de complicações microvasculares, incluindo a nefropatia. OBJETIVOS: Foi avaliado o impacto do controle glicêmico, por meio do cálculo da glicemia média semanal (GMS) e variabilidade glicêmica (VG), sobre a pressão arterial (PA) nas 24 horas (MAPA), excreção urinária de albumina (EUA) e taxa de filtração glomerular (TFG). MÉTODOS: 53 pacientes com diabetes mellitus tipo 2 (DM2), dividida aleatoriamente em dois grupos para receber tratamento convencional ou intensivo. Esse último incluía visitas semanais para ajustes da medicação e aplicação de um plano educacional durante seis semanas. RESULTADOS: Observou-se controle glicêmico (GMS < 150 mg/dL e VG < 50) em 75% (n = 21) dos pacientes do grupo intervenção (GI) (n = 28) e em 24% (n = 6) do grupo convencional (GC) (n = 25) (p < 0,001). Dos 27 pacientes dos dois grupos que obtiveram controle glicêmico, 14 apresentavam inicialmente média da PA sistólica (PAS) > 120 mmHg e que se reduziu de 138,4 ± 10,1 para 127,8 ± 11,6 mmHg (p = 0,023) ao final das seis semanas. Foram observadas reduções da PAS e PA diastólica (PAD) na vigília e durante o sono, que não ocorreram no grupo (n = 17) sem controle glicêmico e PAS >120 mmHg. Inicialmente, 15 pacientes apresentavam TFG >120 mL/min, sendo que após seis semanas, apenas o subgrupo que alcançou controle glicêmico (n = 7) mostrou redução de 137,2 ± 16 para 122,2 ± 25,2 mL/min (p = 0,02). No inicio do estudo, outros quinze pacientes apresentavam microalbuminúria. Após seis semanas, independente de terem alcançado o controle glicêmico preconizado, observou-se redução da EUA de 63,0 ± 43,1 para 24,8 ± 19,5 mg/g de creatinina (p = 0,02). CONCLUSÃO: Assim, o controle glicêmico obtido em curto prazo resultou na redução da PA, da TFG e da EUA nos pacientes com DM2 que apresentavam alterações desses parâmetros, alterações benéficas no que se refere à proteção renal.
