OBJETIVO: avaliar a frequência de testes anti-HIV realizados no pré-natal e de testes rápidos solicitados para estantes internadas para o parto. MÉTODO: trata-se de um estudo de corte transversal com 711 gestantes atendidas no momento do parto no período de janeiro a julho de 2010. Excluíram-se do estudo aquelas admitidas para controle clínico e as que não permitiram que seus dados fossem incluídos na pesquisa. Utilizou-se o teste do χ² ou o teste de Fisher para comparação de proporções na análise univariada. Foram incluídas no modelo de regressão logística todas as variáveis com valor p<0,25, chamado de modelo inicial. Utilizou-se o pacote estatístico SPSS e adotou-se o nível de significância estatística de 5%. RESULTADOS: a idade média das pacientes foi de 25,77±6,7 anos, sendo a idade máxima e mínima de 44 e 12 anos, respectivamente. A média da idade gestacional no momento do atendimento foi de 38,41±6,7 semanas. Destas pacientes, 96,3% (n=685) tinham acompanhamento pré-natal, sendo que 11,1% (n=79) fizeram pré-natal na Maternidade Therezinha de Jesus, da Faculdade de Medicina da Universidade Federal de Juiz de Fora (UFJF), em Juiz de Fora, Minas Gerais. A média de consultas no pré-natal foi de 6,85±2,88, mas 28,1% tiveram menos de 6 consultas. Identificaram-se 10 gestantes soropositivas para o HIV (1,4%), sendo 2 pacientes sabidamente soropositivas. As demais (n=8) foram rastreadas no momento do parto e, por isso, não receberam a profilaxia ARV no pré-natal. Três pacientes foram admitidas em período expulsivo e também não receberam a profilaxia intraparto. Entretanto, todos os recém-nascidos foram avaliados e foi realizada a supressão da lactação e iniciada a formulação láctea. CONCLUSÕES: apesar das medidas estabelecidas pelo Ministério da Saúde, ainda existem falhas na abordagem destas pacientes. Somente com o envolvimento dos gestores e a capacitação dos profissionais envolvidos no atendimento será possível o correto direcionamento de ações que possibilitem a prevenção efetiva da transmissão vertical do HIV.

OBJETIVO: Investigar a presença de sintomas depressivos em mulheres com dor pélvica crônica. MÉTODOS: Foi realizado um estudo de corte transversal descritivo, no qual foram incluídas mulheres com diagnóstico de dor pélvica crônica, com idade maior ou igual a 18 anos, sem gravidez no último ano e sem câncer. A amostra foi definida com base no cálculo de amostragem representativa que estimou o quantitativo de 50 mulheres, que estavam em seguimento no ambulatório de Ginecologia, tendo sido encaminhadas pela rede básica do Sistema Único de Saúde. A coleta dos dados ocorreu no período de outubro de 2009 a maio de 2010. Foram analisadas características sociodemográficas, econômicas e clínicas. A intensidade da dor foi avaliada pela escala analógica visual. Os sintomas depressivos foram investigados pelo Inventário de Depressão de Beck.Para análise estatística, foram utilizadas medidas de posição (média, mediana), dispersão (desvio padrão) e teste do χ2. O nível de significância estatística adotado foi de p≤0,05. RESULTADOS: A média de idade das participantes foi de 41,6±9,4 anos. Predominaram mulheres com nível médio de escolaridade, cor parda, religião católica, que viviam com o companheiro fixo. A maioria (98%) estava economicamente ativa, tendo como ocupação serviços domésticos. Quanto à percepção subjetiva da dor, evidenciou-se que 52% afirmaram dor intensa e 48%, moderada. A maioria das mulheres (52%) convivia com a dor há menos de cinco anos, e 30%, há mais de 11 anos. O escore médio do BDI foi de 17,4 (±9,4). Foi observado que 58% das mulheres tiveram sintomas depressivos de grau leve, moderado e grave avaliados pelo BDI. Os sintomas depressivos de frequências mais elevadas foram fatigabilidade, perda da libido, irritabilidade, dificuldade de trabalhar, preocupações somáticas, choro, insatisfação, tristeza e insônia. CONCLUSÃO: Os sintomas depressivos foram frequentes nessas mulheres com dor pélvica crônica.

OBJETIVO: Comparar os sintomas de depressão e o comportamento sexual de gestantes com histórico de aborto espontâneo de repetição (AER) com gestantes que não vivenciaram AER.MÉTODOS: Trata-se de um estudo prospectivo caso-controle. O primeiro grupo é formado por mulheres que apresentaram AER e o segundo, por primigestas. Foram utilizados o Beck Depression Inventory (BDI), o Female Sexual Function Index (FSFI) e mais um questionário desenvolvido pelos autores sobre aspectos emocionais decorrentes da relação sexual na gravidez. Para comparar as variáveis quantitativas, com distribuição normal, utilizou-se o teste t de Student, e as variáveis categóricas foram comparadas com base no teste do χ2 ou no teste exato de Fisher. A significância estatística foi considerada para valores de p<0,05.RESULTADOS: O BDI indicou frequência aproximadamente duas vezes maior de depressão no Grupo AER (19,9 versus 10,0%, respectivamente). Quanto à função sexual, os escores médios do FSFI foram de 21,1 e 16,4 (p<0,05), respectivamente nos Grupos AER e Controle, embora apenas no âmbito do desejo (média: 3,4±1,3 para o Grupo AER e 3,7±1,1 para o Grupo Controle) não houvesse diferença significante (p=0,1). Observamos que, independentemente de a gestante ter ou não histórico de AER, quanto maior o escore de depressão, menor o escore de sexualidade (r=-0,3).CONCLUSÕES: Gestantes do Grupo AER apresentam depressão com frequência duas vezes mais elevada e função sexual mais comprometida. Há associação inversa entre depressão e função sexual.

PURPOSE: The objective of this study was to investigate the relationship between overactive bladder syndrome and anxiety in older women.METHODS: Of the 198 older women who were invited, 29 were excluded and 166 were then divided into two groups according to the Advanced Questionnaire of Overactive Bladder (OAB-V8): one group with overactive bladder symptoms (OAB-V8≥8) and the other without the symptoms of an overactive bladder (OAB-V8<8). The purpose was to conduct a frequency analysis and to investigate the relation of the social demographic data and anxiety in the two groups. The Beck Anxiety Inventory (BAI) was used to evaluate the level of anxiety. The Kolmogorov-Smirnov test was used to determine the distribution of the data. The differences between the two groups for the continuous variables were analyzed by the Mann-Whitney U test, the differences for the categorical variables were analyzed by the Chi-Square test and the association between the continuous variables was analyzed by the Spearman Correlation test. The tests were two-tailed with a confidence level of 5%.RESULTS: Overall, the frequency of an overactive bladder was present in 117 (70.5%) of the participants. The body mass index (BMI) of the group with overactive bladder symptoms was significantly higher than the BMI of those without these symptoms (p=0.001). A higher prevalence of mild, moderate and severe anxiety was observed among older women with overactive bladder symptoms. In addition, the overactive bladder symptoms group presented a positive low correlation with anxiety symptoms (r=0.345) and with BMI (r=0.281). There was a small correlation between BMI and anxiety symptoms (r=0.164).CONCLUSIONS: Overactive bladder syndrome was prevalent among older women and the existence of these symptoms was linked to the presence of mild, moderate and/or severe anxiety symptoms.

In the last few years, the use of laparoscopy in veterinary medicine has expanded and consequently so was the need for studies that establish the advantages, disadvantages and possible complications of each procedure. The purpose of the current study was to describe a laparoscopic splenectomy technique and the alterations due to this access, and compare it to the open procedure in dogs. A total of 15 healthy female mongrel dogs were used, with mean weight of 17.4±2.5kg. The animals were distributed into three groups: Group IA of open splenectomy (laparotomy) using double ligation of the vessels of the splenic hilum with poliglicolic acid, Group IB of open splenectomy (laparotomy) with bipolar electrocoagulation of the splenic hilum, and Group II of laparoscopic access with bipolar electrocoagulation of the splenic hilum. Operative time, blood loss, size of incisions, complications during and after surgery were evaluated. Other parameters included pain scores, white blood cell (WBC) counts and postoperative serum concentrations of alanine aminotransferase (ALT), alkaline phosphatase (ALP), creatine kinase (CK), C-reactive protein (CRP), glucose and cortisol. No differences were found in the evaluation of parameters between both open splenectomy techniques employed. Laparoscopic access presented significant differences (p<0,05) when compared with open surgery: Longer operative time, smaller abdominal access, decrease in blood loss, lower concentrations of CRP, higher levels of CK and ALP, and lower scores in the pain scale. Laparoscopic surgery showed fewer complications of the surgical wound. No significant differences were observed between groups in the postoperative temperature, WBC, ALT, cortisol and glucose concentrations. In conclusion, the laparoscopic technique is useful for splenectomy in dogs, being advantageous in terms of blood loss, surgical stress and surgical wounds. However, it expends more operative time and causes transitory increase in hepatic and muscular enzymes.

Com o objetivo auxiliar profissionais médico-veterinários no reconhecimento das lesões de bovinos encontradas na linha de inspeção de carnes em matadouros frigoríficos, três condições granulomatosas de bovinos foram pesquisadas e suas semelhanças e diferenças avaliadas. Essas três condições granulomatosas foram actinobacilose (causada por Actinobacillus lignieresii), actinomicose (causada por Actinomyces bovis) e mastite estafilocócica (causada por Staphylococcus aureus). Em 505 lesões encontradas em bovinos abatidos para consumo humano, 40 eram uma dessas três lesões granulomatosas: 24 eram actinobacilose, 10 eram actinomicose e seis eram mastite estafilocócica. De um modo geral, os aspectos macro e microscópicos dessas três lesões eram bastante semelhantes, mas suas localizações ajudavam a presumir sua etiologia. A. lignieresii afetou tecidos moles, principalmente língua e linfonodos da cabeça; A. bovis afetou o tecido ósseo, principalmente o da mandíbula; e S. aureus teve a glândula mamária como o tecido alvo. Histologicamente, os granulomas resultantes da infecção por qualquer um desses três agentes continham uma estrutura amorfa, eosinofílica, com clavas irradiadas, localizada centralmente; essa estrutura era rodeada por neutrófilos íntegros e degenerados, que, por sua vez, eram cercados por um manto de macrófagos epitelioides e ocasionais células gigantes multinucleadas. Esses mantos de macrófagos eram irregularmente infiltrados por linfócitos e plasmócitos que tendiam a se acumular na periferia da lesão, que era cercada por uma cápsula de tecido conjuntivo. Dependendo da aplicação do método de coloração adequado, o agente etiológico podia ser visto em cada um dos três tipos de lesão granulomatosa. No caso da mastite estafilocócica, cocos intralesionais foram observados tanto nas colorações por HE como nas de Gram, nessa última como cocos gram-positivos. O agente da actinobacilose (bacilos gram-negativos) e da actinomicose (bacilos gram-positivos) pôde ser observado somente nas colorações de Gram. Os diagnósticos diferenciais para essas três condições são discutidos.

Graxains-do-campo habitam o centro-leste da América do Sul, desde o sudeste do Brasil e leste da Bolívia. A displasia renal compreende um espectro de anomalias e é frequentemente relatada em cães de diversas raças, sendo a maioria dos relatos achados de necropsia. A maior parte das doenças renais congênitas nos cães apresenta caráter progressivo, portanto o tratamento é limitado e direcionado a fim de retardar a progressão da doença renal estabelecida. No presente relato é descrita a realização de nefrectomia laparoscópica em graxaim-do-campo (Pseudalopex gymnocercus) com displasia renal. Suspeitou-se de doença no rim direito por meio de ultrassonografia e urografia excretora. Para o procedimento cirúrgico foram utilizados três portais (10, 10, 5mm), endoscópio rígido de 10mm/0(0) e clipes para hemostasia dos vasos renais. A técnica cirúrgica utilizada neste canídeo selvagem foi semelhante à usada em cão doméstico, devido à escassez de relatos em cirurgias em graxaim e a inexistência de descrições de nefrectomia videolaparoscópica em graxaim-do-campo. O animal foi acompanhado por um período de 30 dias pós-cirurgia sem a ocorrência de complicações. O presente relato demonstra que a técnica proposta foi adequada para a realização de nefrectomia videolaparoscópica em Graxaim-do-campo.

A survey was undertaken aiming to obtain an overview of ocular and periocular lesions diagnosed in domestic mammals over a period of 50 years in a veterinary pathology diagnostic laboratory in the Central Region of the State of Rio Grande do Sul, Brazil. In this lab, 33,075 histophatological exams had been performed over the period surveyed, of which 540 (1.6%) concerned ocular and periocular lesions. For various reasons ninety specimens were excluded from the study and the remaining 450 consisted of samples from dogs (53.5%), cattle (28.2%), cats (11.1%), horses (5.1%) sheep (1.3%), rabbits (0.4%), and pig (0.2%). The eyelids were the most prevalent (248/450) site of lesions in each of the species studied, followed by third eyelid (73/450), and conjunctiva (27/450). In dogs (241 samples) lesions in sebaceous glands (including Meibomian glands) were the most common findings (75/241), followed by melanocytic tumors (52/241) and nonspecific conjunctivitis (13/241). Squamous cell neoplasms, both benign and malignant, were relatively common. In cattle, anatomical sites affected by ocular and periocular lesions, in decreasing order of frequency, were eyelid, cornea and third eyelid. Squamous cell carcinoma (SCC) alone accounted for 80.3% of all diagnoses, while all neoplastic lesions made up for 85.0% of the lesions diagnosed in cattle. Neoplasia accounted for most of the lesions diagnosed in cats (39/50 cases); all of these were malignant, and SCC, hemangiosarcoma and fibrosarcoma were the most common types diagnosed. In horses, 19 out of 23 submissions were neoplasms and most were sarcoid (8/23) and SCC (8/23). There were six submissions from sheep with unpigmented skin, all of which represented SCC of the eyelids (5) and third eyelid (1).

Hyphema (hemorrhage within the anterior chamber of the eye) can be caused by several mechanisms and can easily be detected in routine ophthalmic or necroscopic examination as discolored red eye(s). The purpose of this study is to report the cause of hyphema diagnosed as a postmortem finding in dogs and cats. Twenty cases, 14 dogs and six cats of several ages and breeds and of both sexes were included in the study. Hyphema presented as a unilateral (14 cases out of 20) or bilateral (6/20) disorder in dogs and cats and extension of hemorrhage varied from minimal to diffuse. Hyphema was secondary to systemic disease (15/20) or occurred as a primary ocular lesion (5/20) in four dogs and one cat. Primary hyphema was always unilateral. In four of these cases, the cause of hyphema was trauma and remaining case was caused by phacoclastic uveitis in a dog with bilateral hypermature cataract. Various causes of bleeding disorders were found related to secondary hyphema: in decreasing order of frequency, they included vasculitis (8/15), systemic hypertension (5/15), and acquired coagulopathies (2/15). Vasculitis due to feline infectious peritonitis accounted for half of the cases (n=3) of systemic hyphema in cats. The various pathological aspects and pathogenesis of hyphema in dogs and cats are described and discussed.

Echinolaena inflexa (Poir.) Chase is a native C3 grass species with high biomass production and abundance in the cerrado comparable to Melinis minutiflora Beauv., a C4 forage grass of African origin, widespread in the cerrado, displacing some native herbaceous species. These species accumulate mainly starch, differently of Lolium multiflorum Lam. ssp. italicum Beck cv. Lema (ryegrass), a C3 temperate forage grass that accumulates fructans as the major storage carbohydrate in the vegetative tissues. In the present study, we analyzed diurnal variations in the content and composition of non-structural carbohydrates (NSC) in the aerial vegetative parts of the three species. Results showed similarity in water-soluble carbohydrates (WSC) and starch contents between Melinis minutiflora and Echinolaena inflexa, with the exotic grass accumulating higher amounts of NSC. However, the levels of sucrose, glucose and fructose were 10 fold higher in Lolium multiflorum, that accumulates fructan as the main storage carbohydrate. Although the contents of WSC and starch were similar in the tropical species E. inflexa and M. minutiflora, their diurnal variations were different and could be related to starch metabolism, especially in M. minutiflora.

Sleep-related complaints have become a highlight for physicians as well as public health administrators. Studies of sleep patterns and sleep-related complaints of shift workers have been useful in minimizing reduction in the quality of life due to the warping of the sleep-wake cycle. The objective of the present study was to assess patterns of sleep, sleep-related complaints as well as physical activity and scoring rates for depression and anxiety in interstate bus drivers. Data were obtained with a sleep questionnaire, with the Beck inventory for depression, and the State-Trait Anxiety Inventory (STAI). A total of 400 interstate bus drivers from the northern, southern, central-western and south-eastern regions of Brazil were interviewed. Sixty percent of the subjects interviewed presented at least one sleep-related complaint, 16% admitted to have dozed at the wheel while on duty, and 41% stated that they exercised on a regular basis. Other sleep disturbance complaints reported were: sleep latency 29'17"; physical fatigue, 59.8%; mental fatigue, 45.4%; sleepiness, 25.8%; irritability, 20.6%; insomnia, 37.5%, respiratory disturbances, 19.25% and snoring, 20.75%. Scores for anxiety and depression were not in the pathological range. The present data reinforce the view that bus drivers are generally discontent with shift work and its effects on sleep. Consequently, it is very important to establish an appropriate work schedule for drivers, besides implementing photo-therapy and physical activities in order to minimize sleepiness when driving.

We investigated the effect of age and sex on the serum activity of hexosaminidase (HEX) and ß-glucuronidase (BGLU) in 275 normal term infants aged 12 h to 12 months. Up to six weeks of life, HEX was significantly higher in boys (P<=0.023). During the age period of 1-26 weeks, BGLU was also higher in boys, but differences were significant only at 2-6 and 7-15 weeks (P<=0.016). The developmental pattern of HEX and BGLU was sex dependent. HEX activity increased in both sexes from 4-7 days of life, reaching a maximum of 1.4-fold the birth value at 2-6 weeks of age in boys (P<0.001) and a maximum of 1.6-fold at 7-15 weeks in girls (P<0.001). HEX activity gradually decreased thereafter, reaching significantly lower levels at 27-53 weeks than during the first three days of life in boys (P = 0.002) and the same level of this age interval in girls. BGLU increased in both sexes from 4-7 days of age, showing a maximum increase at 7-15 weeks (3.3-fold in boys and 2.9-fold in girls, both P<0.001). Then BGLU decreased in boys to a value similar to that observed at 4-7 days of age. In girls, BGLU remained elevated until the end of the first year of life. These results indicate a variation of HEX and BGLU activities during the first year of life and a sex influence on their developmental pattern. This observation should be considered in the diagnosis of GM2 gangliosidosis and mucopolysaccharidosis type VII.

Much effort has been devoted to the identification of immunologically important antigens of Mycobacterium tuberculosis and to the combination of target antigens to which antibodies from serum of tuberculous patients could react specifically. We searched for IgG antibodies specific for antigens of 45 to 6 kDa obtained after sonication of the well-characterized wild M. tuberculosis strain in order to detect differences in the antibody response to low molecular weight antigens from M. tuberculosis between patients with pulmonary tuberculosis and contacts. Specific IgG antibodies for these antigens were detected by Western blot analysis of 153 serum samples collected from 51 patients with confirmed pulmonary tuberculosis. Three samples were collected from each patient: before therapy, and after 2 and 6 months of treatment. We also analyzed 25 samples obtained from contacts, as well as 30 samples from healthy individuals with known tuberculin status, 50 samples from patients with other lung diseases and 200 samples from healthy blood donors. The positive predictive value for associated IgG reactivity against the 6-kDa and 16-kDa antigens, 6 and 38 kDa, and 16 and 38 kDa was 100% since simultaneous reactivity for these antigens was absent in healthy individuals and individuals with other lung diseases. This association was observed in 67% of the patients, but in only 8% of the contacts. The humoral response against antigens of 16 and 6 kDa seems to be important for the detection of latent tuberculosis since the associated reactivity to these antigens is mainly present in individuals with active disease.

The International Classification of Diseases, 10th edition (ICD-10) defines atypical bulimia nervosa (ABN) as an eating disorder that encompasses several different syndromes, including the DSM-IV binge eating disorder (BED). We investigated whether patients with BED can be differentiated clinically from patients with ABN who do not meet criteria for BED. Fifty-three obese patients were examined using the Structured Clinical Interview for DSM-IV and the ICD-10 criteria for eating disorders. All volunteers completed the Binge Eating Scale (BES), the Beck Depression Inventory, and the Symptom Checklist-90 (SCL-90). Individuals fulfilling criteria for both ABN and BED (N = 18), ABN without BED (N = 16), and obese controls (N = 19) were compared and contrasted. Patients with ABN and BED and patients with ABN without BED displayed similar levels of binge eating severity according to the BES (31.05 ± 7.7 and 30.05 ± 5.5, respectively), which were significantly higher than those found in the obese controls (18.32 ± 8.7; P < 0.001 and P < 0.001, respectively). When compared to patients with ABN and BED, patients with ABN without BED showed increased lifetime rates of agoraphobia (P = 0.02) and increased scores in the somatization (1.97 ± 0.85 vs 1.02 ± 0.68; P = 0.001), obsessive-compulsive (2.10 ± 1.03 vs 1.22 ± 0.88; P = 0.01), anxiety (1.70 ± 0.82 vs 1.02 ± 0.72; P = 0.02), anger (1.41 ± 1.03 vs 0.59 ± 0.54; P = 0.005) and psychoticism (1.49 ± 0.93 vs 0.75 ± 0.55; P = 0.01) dimensions of the SCL-90. The BED construct may represent a subgroup of ABN with less comorbities and associated symptoms.

The objective of the present study was to establish the frequency of psychiatric comorbidity in a sample of diabetic patients with symmetric distal polyneuropathy (SDPN). Sixty-five patients with type 2 diabetes mellitus were selected consecutively to participate in the study at Instituto Estadual de Diabetes e Endocrinologia. All patients were submitted to a complete clinical and psychiatric evaluation, including the Portuguese version of the structured clinical interview for DSM-IV, the Beck Depression Inventory, the Neuropathy Symptom Score, and Neuropathy Disability Score. SDPN was identified in 22 subjects (33.8%). Patients with and without SDPN did not differ significantly regarding sociodemographic characteristics. However, a trend toward a worse glycemic control was found in patients with SDPN in comparison to patients without SDPN (HbA1c = 8.43 ± 1.97 vs 7.48 ± 1.95; P = 0.08). Patients with SDPN exhibited axis I psychiatric disorders significantly more often than those without SDPN (especially anxiety disorders, in general (81.8 vs 60.0%; P = 0.01), and major depression - current episode, in particular (18.2 vs 7.7%; P = 0.04)). The severity of the depressive symptoms correlated positively with the severity of SDPN symptoms (r = 0.38; P = 0.006), but not with the severity of SDPN signs (r = 0.07; P = 0.56). In conclusion, the presence of SDPN seems to be associated with a trend toward glycemic control. The diagnosis of SDPN in diabetic subjects seems also to be associated with relevant psychiatric comorbidity, including anxiety and current mood disorders.
