A pair matched case/control study was conducted from January 1991 to 30 June 1992 in order to define clinical and laboratory findings associated with DMAC infection in AIDS patients. Since DMAC infection is usually associated with advanced immunodeficiency, and therefore also with other opportunistic illnesses, in addition to the number of CD4+ lymphocytes, cases and controls were matched using the following criteria: date of AIDS diagnosis and antiretroviral therapy, number and severity of associated opportunistic infections and, whenever possible, type of Pneumocystis carinii prophylaxis, age and gender, in this order of relevance. Cases (defined as patients presenting at least one positive culture for MAC at a normally sterile site) and controls presented CD4+ lymphocyte counts below 50 cel/mm3. A significantly higher prevalence of general, digestive and respiratory signs, increased LDH levels, low hemoglobin levels and CD4+ cell counts were recorded for cases when compared to controls. Increases in gGT and alkaline phosphatase levels seen in cases were also recorded for controls. In conclusion, the strategy we used for selecting controls allowed us to detect laboratory findings associated to DMAC infection not found in other advanced immunossupressed AIDS patients without DMAC.

We have searched for Mycobacterium leprae DNA for 36kDa protein in urine using a M. leprae specific PCR technique. A limited number of 16 patients (of which 11 belonged to lepromatous leprosy and five to tuberculoid leprosy) and eight healthy individuals were included for the present study. The number of urine samples positive by PCR were 36.4% (4/11) in lepromatous patients and 40% (2/5) in tuberculoid patients. None of the samples from healthy individuals was positive. To our knowledge, the results indicate, for the first time, the presence of M. leprae DNA in urine from leprosy patients. Another important finding obtained out of the study is that amongst treated patients 66.6% (4/6) were positive whereas amongst untreated only 20% (2/10) were positive. From the present indicative data it appears that treatment improves the PCR results with urine as a sample. Thus, the approach could prove to be useful for monitoring the treatment response of individual patients and needs to be further evaluated with a large number of patients.

The authors observed an injury caused by the sting of a false tocandira ant in the hand of an amateur fisherman and they describe the clinical findings and the evolution of the envenoming, which presented an acute and violent pain, cold sweating, nausea, a vomiting episode, malaise, tachycardia and left axillary's lymphadenopathy. About three hours after the accident, still feeling intense pain in the place of the sting, he presented an episode of great amount of blood in the feces with no history of digestive, hematological or vascular problems. The intense pain decreased after eight hours, but the place stayed moderately painful for about 24 hours. In that moment, he presented small grade of local edema and erythema. The authors still present the folkloric, pharmacological and clinical aspects related to the tocandiras stings, a very interesting family of ants, which presents the largest and more venomous ants of the world.

Immune reconstitution inflammatory syndrome (IRIS) is an atypical and unexpected reaction related to highly active antiretroviral therapy (HAART) in human immunodeficiency virus (HIV) infected patients. IRIS includes an atypical response to an opportunistic pathogen (generally Mycobacterium tuberculosis, Mycobacterium avium complex, cytomegalovirus and herpes varicella-zoster), in patients responding to HAART with a reduction of plasma viral load and evidence of immune restoration based on increase of CD4+ T-cell count. We reported a case of a patient with AIDS which, after a first failure of HAART, developed a subcutaneous abscess and supraclavicular lymphadenitis as an expression of IRIS due to Mycobacterium avium complex after starting a second scheme of HAART.

Development and standardization of reliable methods for detection of Mycobacterium tuberculosis in clinical samples is an important goal in laboratories throughout the world. In this work, lung and spleen fragments from a patient who died with the diagnosis of miliary tuberculosis were used to evaluate the influence of the type of fixative as well as the fixation and paraffin inclusion protocols on PCR performance in paraffin embedded specimens. Tissue fragments were fixed for four h to 48 h, using either 10% non-buffered or 10% buffered formalin, and embedded in pure paraffin or paraffin mixed with bee wax. Specimens were submitted to PCR for amplification of the human beta-actin gene and separately for amplification of the insertion sequence IS6110, specific from the M. tuberculosis complex. Amplification of the beta-actin gene was positive in all samples. No amplicons were generated by PCR-IS6110 when lung tissue fragments were fixed using 10% non-buffered formalin and were embedded in paraffin containing bee wax. In conclusion, combined inhibitory factors interfere in the detection of M. tuberculosis in stored material. It is important to control these inhibitory factors in order to implement molecular diagnosis in pathology laboratories.

The effects of both garlic (Allium sativum) and onion (Allium cepa) on some biochemical parameters in Schistosoma mansoni infected mice individually and mixed either with or without the currently used drug, praziquantel (PZQ) were investigated. These involved some immunological parameters, namely IgM, IgG, interleukins 2 and 6 (IL-2 and 6) and tumor necrosis factor (TNF-α), some antioxidant enzymes [catalase, superoxide dismutase (SOD) and glutathione peroxidase (GPX)]. In addition, parasitological and histopathological investigations were performed. No changes were observed in the normal control mice treated with dry extract of onion or garlic, individually or mixed, with or without PZQ, compared to the normal healthy control group. Infection with S. mansoni showed an increase in IgG, IgM, IL-2, IL-6, TNF-α and catalase enzyme, accompanied with a decrease in GPX and SOD antioxidant enzyme activities. Remarkable amelioration was noticed in the levels of all the measured parameters in S. mansoni infected mice after administration of the studied extracts. Moreover a significant reduction in worm burden, hepatic and intestinal eggs and oogram count was noticed which was reflected in normalization of liver architecture.

Leprosy is a disease caused by Mycobacterium leprae that carries a high risk of disability, making early diagnosis mandatory. This study aimed to determine the applicability of anti-PGL-1 IgM antibody detection, using the ML FLOW technique, as an assistant tool for the detection of leprosy infection in asymptomatic household contacts (AHHC) of multibacillary leprosy index cases from Midwest Brazil. Serological changes induced by the prophylaxis of these household contacts with Bacillus Calmette-Guérin (BCG) were also verified. A total of 91 AHHC were assessed, among which, 18.68% (n = 17) presented both positive bacilloscopy and positive anti-PGL-1 IgM serology. Positivity concordance between these two laboratorial exams (Kappa Index = 1; p < 0.001) was indicated, however, one case did not demonstrate concordance between the semiquantitative assessment of anti-PGL-1 IgM and the bacilloscopy index (Kappa Index = 0.96; p < 0.001). Among the 17 AHHC with positive bacilloscopy, eight were reassessed after prophylaxis with BCG and two of them presented negative anti-PGL-1 IgM serology, being these patients who had presented a bacilloscopy index of < 2[+] in the initial assessment. This study shows that anti-PGL-1 IgM detection may be used as a tool to determine the bacillary load in AHHC and to detect immune changes related to prophylaxis by nonspecific vaccination.

New methodologies were developed for the identification of Nocardia but the initial diagnosis still requires a fast and accurate method, mainly due to the similarity to Mycobacterium, both clinical and bacteriologically. Growth on Löwenstein-Jensen (LJ) medium, presence of acid-fast bacilli through Ziehl-Neelsen staining, and colony morphology can be confusing aspects between Nocardia and Mycobacterium. This study describes the occurrence of Nocardia spp. in a mycobacterial-reference laboratory, observing the main difficulties in differentiating Nocardia spp. from Mycobacterium spp., and correlating isolates with nocardiosis cases. Laboratory records for the period between 2008 and 2012 were analyzed, and the isolates identified as Nocardia sp. or as non-acid-fast filamentous bacilli were selected. Epidemiological and bacteriological data were analyzed as well. Thirty-three isolates identified as Nocardia sp. and 22 as non-acid-fast bacilli were selected for this study, and represented 0.12% of isolates during the study period. The presumptive identification was based on macroscopic and microscopic morphology, resistance to lysozyme and restriction profiles using the PRA-hsp65 method. Nocardia spp. can grow on media for mycobacteria isolation (LJ and BBL MGIT™) and microscopy and colony morphology are very similar to some mycobacteria species. Seventeen patients (54.8%) were reported and treated for tuberculosis, but presented signs and symptoms of nocardiosis. It was concluded that the occurrence of Nocardia sp. during the study period was 0.12%. Isolates with characteristics of filamentous bacilli, forming aerial hyphae, with colonies that may be pigmented, rough and without the BstEII digestion pattern in PRA-hsp65 method are suggestive of Nocardia spp. For a mycobacterial routine laboratory, a flow for the presumptive identification of Nocardia is essential, allowing the use of more accurate techniques for the correct identification, proper treatment and better quality of life for patients.

Foi isolada uma cepa de Schistosoma mansoni proveniente de dois pacientes tratados com hycanthone, por duas vezes, na dose de 2,5mg/kg, i.m., em janeiro e em abril de 1970, e com niridazole (25mg/kg dia x 5 oral), em abril de 1971. O número de ovos por grama de fezes nestes pacientes antes do tratamento era de 2675 e 1025, respectivamente e, após o terceiro tratamento, em torno de 100 ovos/g. Miracídios obtidos das fezes destes pacientes, infectaram caramujos (Biomphalaria glabrata), que passaram a eliminar cercárias (cepa WW). Estas foram utilizadas para infecção experimental de camundongos albinos. Os animais infectados foram tratados com esquemas múltiplos de hycanthone, niridazole e oxamniquine. Estudos comparativos das cepas WW e LE (esta última mantida rotineiramente em nossos laboratórios) mostraram diferenças acentuadas quando à sensibilidade aos esquistossomicidas usados. De fato, com hycanthone, na dose de 80mg/kg, i.m. houve 100% de alteração do oograma nos camundongos infectados com a cepa L.E. e de 0,0% nos infectados com a cepa WW. Com a oxamniquine e niridazole as diferenças foram menores, mas, ainda assim, suficientes para indicar maior resistência da cepa WW a estes esquistossomicidas. Esta é a primeira vez na literatura, que se demonstra resistência em cepas de S. mansoni provenientes de pacientes tratados.

Efetuaram os autores teste tuberculínico, com PPD (RT 23), 10 UT, em 3.6S4 crianças sadias, que receberam, pela via oral, em três oportunidades separadas por intervalos de um mês, vacina BCG líquida ou liofilizada e placebo representado por preparação sem bacilos. Dois grupos foram basicamente estabelecidos, tendo os limites etários correspondido a noventa dias em um deles e a essa idade e quinze anos no outro. Considerando os módulos com tamanhos superiores a cinco milímetros, observaram taxas de positividades de 37,6% e 21% relativamente aos indviduos separados da maneira citada e em avaliações levadas a efeito no máximo nove meses depois, mas as cifras pertinentes ao produto isento de bacilos álcool-ácido-resistentes e ao submetido à liofilização mostraram-se expressivamente menores. Valorizada somente a alergização, as percentagens indicadas e não desprezíveis atestaram a ocorrência de destacada absorção, sobretudo ao ser levado em conta o sucedido quanto às pessoas de menores idades.

Exemplares de Biomphalaria tenagophila, originários de Londrina, mostraram-se resistentes à infecção por cepa simpátrica de Schistosoma mansoni e por cepa originária de Belo Horizonte (MG). Variando-se o diâmetro e a idade dos caramujos submetidos à infecção e aumentando-se o número de miracidios a que cada caramujo foi exposto, não se conseguiu obter cercárias de S. mansoni, 70 dias após a tentativa de infecção.

Sete macacos (Cebus apella sp) foram inoculados por via subcutânea peto Trypanosoma cruzi com um número de formas tripomastigotas que variou de 25 x 103 a 5 x 1O6. Um outro primata, também da mesma espécie, foi inoculado por via palpebral com fezes de Triatoma infestans contendo formas de T. cruzi provenientes da mesma cepa "Y". A presença do T. cruzi foi assinalada na maioria dos animais, pela primeira vez, no 8º dia após a infecção. Os picos máximos da parasitemia foram observados entre o 9º e o 12º dia da infecção. O número de tripanosomas caiu acentuadamente do 19º ao 25º dia. Todos os animais infectados sobreviveram. Estudos estão sendo conduzidos com o intuito de se observar o comportamento destes animais na fase crônica da doença.

No presente estudo camondongos "Swiss" foram submetidos a tratamento com a droga Ro 7-1051 (N-Benzil-2-Nitro-imidazolacetamida) antes de sua inoculação com a cepa Colombiana do T. cruzi e comparados com animais inocuiados com a mesma cepa e tratados em fase inicial de infecção. Com avaliação feita por parasitemia periférica, xenodiagnõstico, sub-inoculação em camondongos recém-nascidos e exame histopatológico foram obtidos melhores resultados com os animais submetidos a tratamento prévio, tendo-se verificado que 55,5% dos animais permaneceram negativados durante o curso da experiência, enquanto os tratados precocemente tiveram um índice de cura de 28,5%.
