Response to treatment with antimonial drugs varies considerably depending on the parasite strain involved, immune status of the patient and clinical form of the disease. Therapeutic regimens with this first line drug have been frequently modified both, in dose and duration of therapy. A regimen of 20 mg/kg/day of pentavalent antimony (Sb5+) during four weeks without an upper limit on the daily dose is currently recommended for mucosal disease ("espundia"). Side-effects with this dose are more marked in elderly patients, more commonly affected by this form of leishmaniasis. According to our experience, leishmaniasis in Rio de Janeiro responds well to antimony and, in cutaneous disease, high cure rates are obtained with 5 mg/kg/day of Sb5+ during 30 to 45-days. In this study a high rate of cure (91.4%) employing this dose was achieved in 36 patients with mild disease in this same geographic region. Side-effects were reduced and no antimony refractoriness was noted with subsequent use of larger dose in patients that failed to respond to initial schedule.

SUMMARYThis study evaluated the applicability of kDNA-PCR as a prospective routine diagnosis method for American tegumentary leishmaniasis (ATL) in patients from the Instituto de Infectologia Emílio Ribas (IIER), a reference center for infectious diseases in São Paulo - SP, Brazil. The kDNA-PCR method detected Leishmania DNA in 87.5% (112/128) of the clinically suspected ATL patients, while the traditional methods demonstrated the following percentages of positivity: 62.8% (49/78) for the Montenegro skin test, 61.8% (47/76) for direct investigation, and 19.3% (22/114) for in vitro culture. The molecular method was able to confirm the disease in samples considered negative or inconclusive by traditional laboratory methods, contributing to the final clinical diagnosis and therapy of ATL in this hospital. Thus, we strongly recommend the inclusion of kDNA-PCR amplification as an alternative diagnostic method for ATL, suggesting a new algorithm routine to be followed to help the diagnosis and treatment of ATL in IIER.

Over a complete two-year period, phlebotomine specimens were caught in an area of cutaneous leishmaniasis occurrence in the municipality of Angra dos Reis. A manual suction tube was used to catch phlebotomines on house walls, and also light traps in domestic and peridomestic settings and in the forest. This yielded 14,170 specimens of 13 species: two in the genus Brumptomyia and eleven in the genus Lutzomyia. L. intermedia predominantly in domestic and peridomestic settings, with little presence in the forest, with the same trend being found in relation to L. migonei, thus proving that these species have adapted to the human environment. L. fischeri appeared to be eclectic regarding location, but was seen to be proportionally more endophilic. L. intermedia and L. migonei were more numerous in peridomestic settings, throughout the year, while L. fischeri was more numerous in domestic settings except in March, April, May and September. From the prevalence of L. intermedia, its proven anthropophily and findings of this species naturally infected with Leishmania(Viannia) braziliensis, it can be incriminated as the main vector for this agent of cutaneous leishmaniasis in the study area, especially in the peridomestic environment. L. fischeri may be a coadjuvant in carrying the parasite.

SUMMARY In this study, Leishmaniaspecies were identified by Polymerase Chain Reaction (PCR). The epidemiology of patients suspected of having American Cutaneous Leishmaniasis in the municipality of Assis Brasil, Acre State, located in the Brazil/Peru/Bolivia triborder was also investigated. By PCR, the DNA of Leishmaniawas detected in 100% of the cases (37 samples) and a PCR-Restriction Fragment Length Polymorphism (RFLP) of the hsp 70gene identified the species in 32 samples: Leishmania (Viannia) braziliensis (65.6%) , L. (V.) shawi (28.1%) , L. (V.) guyanensis (3.1%) and mixed infection L. (V.) guyanensis and L. (Leishmania) amazonensis (3.1%)This is the first report of L. (V.) shawiand L. (L.) amazonensis in Acre. The two predominant species were found in patients living in urban and rural areas. Most cases were found in males living in rural areas for at least three years and involved in rural work. This suggests, in most cases, a possible transmission of the disease from a rural/forest source, although some patients had not engaged in activities associated with permanence in forestall areas, which indicate a possible sandflies adaptation to the periurban setting.

A medida dos diâmetros longitudinal e transverso de uma lesão ulcerada na leishmaniose tegumentar não reflete a profundidade, a qual também tem importância na avaliação da cura. A proposta deste estudo foi avaliar o volume da úlcera leishmaniõtica, através da moldagem seriada, antes e após o tratamento. A moldagem da cavidade, apôs assepsia local com água e sabão, foi feita com material plástico gelatinoso ã base de sílica, alginato de potássio, sulfato de cálcio e óxido de magnésio, comercializado sob o nome de jeltrate® (Dentsply Laboratory). Dissolveu-se 9,5g de jeltrate® em 20ml de água e aplicou-se o gel sobre a úlcera, o qual se solidificou em cinco minutos. Este molde foi então preenchido com acrílico autopolimerizante e o seu volume mensurado através do peso, usando-se balança analítica (Técnica 1) ou através do deslocamento da água, aplicando o princípio de Arquimedes (Técnica 2). Mostramos os dados de um estudo i-ealizado numa área de transmissão de Leishmania (Viannia) braziliensis onde 20 pacientes foram tratados com três diferentes esquemas: sete pacientes receberam isotianato depentamidina, sete receberam sulfato de aminosidine e seis antimoniato de meglumina. Os resultados mostram em todos os pacientes redução uniforme do volume das úlceras melhor avaliada por qualquer uvta das duas técnicas utilizadas. Em relação aos esquemas terapêuticos houve diferença estatística significante entre os três esquemas usando o teste T de student, o qual mostrou que o sulfato de aminosidine produziu maior redução de volume que os outros compostos. Moldagens seriadas são registros permanentes e refletem melhor a cura clínica. Concluímos que a medida da lesão ulcerada é útil na avaliação terapêutica como procedimento prático, barato e que pode ser utilizado em estudos de campo.

Tratamos com ambisome (2 a 5g totais de dose) seis pacientes com leishmaniose mucosa sem resposta a tratamento com glucantime (20mg SbV/kg/dia). A dose diária usada foi 2 a 3mg/kg/dia, aplicada por um mínimo de 20 dias. Após 26 a 38 meses de acompanhamento, cinco pacientes estão clinicamente curados. Um recidivou aos 6 meses. Não foram observados efeitos colaterais além de cefaléia, após a injeção. O ambisome constitue uma opção terapêutica para os pacientes com leishmaniose mucosa sem resposta aos antimoniais.

Em 1996 foram avaliados clinicamente 20 dos 21 pacientes com leishmaniose mucosa, tratados em 1994 com sulfato de aminosidine 16mg do sal/kg/dia, intramuscular, por 20 dias. Um paciente foi a óbito por causas não relacionadas com a leishmaniose mucosa. Dos 14 pacientes (66,7% N = 21) que inicialmente alcançaram a remissão completa dos sinais e sintomas durante os três primeiros meses de seguimento, sete (50% N = 14) permaneceram livres de doença por 24 meses e sete pacientes apresentaram recidiva neste período. O acompanhamento sorológico mostrou pobre correlação com a avaliação clínica.

A eficácia e segurança do antimoniato de meglumina e do estibogluconato de sódio BP 88R foram comparadas no tratamento da leishmaniose cutânea em Corte de Pedra, Bahia, área endêmica de leishmaniose causada por Leishmania (Viannia) braziliensis. Realizou-se um estudo quase-experimental que incluiu 127 pacientes cujo diagnóstico baseou-se na observação clínica e a intradermorreação de Montenegro. Cinqüenta e oito pacientes receberam antimoniato de meglumina e 69 estibogluconato de sódio. Utilizou-se a dose de 20 mg/Sb v/kg/dia por 20 dias, em ambos os grupos. Os pacientes foram acompanhados a cada dez dias durante o tratamento e mensalmente por três meses. Observou-se a cura em 62% dos pacientes tratados com antimoniato de meglumina e em 55% daqueles tratados com estibogluconato de sódio (p = 0,42). A cefaléia foi mais freqüente na primeira metade do tratamento no grupo tratado com estibogluconato de sódio (p = 0,026). Na segunda metade do tratamento, os pacientes tratados com estibogluconato de sódio apresentaram maior freqüencia de mialgia/artralgia (p = 0,004) e dor abdominal/anorexia (p = 0,004). Três pacientes tratados com o estibogluconato de sódio apresentaram efeitos colaterais graves.

Casos de leishmaniose tegumentar no norte do estado do Paraná têm sido assinalados desde os anos 50, mas no final dos anos 80, houve um surto epidêmico. Para conhecer o perfil epidemiológico da leishmaniose cutânea na região Norte do Paraná foi feito um levantamento de casos, entre 1993 e 1998. Foram notificados 316 casos da doença em 35 municípios paranaenses. Dezesseis casos eram importados de outros estados e do Paraguai. Os indivíduos do sexo masculino (61,2%), na faixa etária de 15 a 49 anos (70,8%) representaram a maioria dos casos de leishmaniose tegumentar. Sessenta e sete porcento dos pacientes apresentaram lesões únicas, 31% lesões múltiplas e 2% lesões de mucosas. Num total de 367 lesões analisadas 47,7% estavam localizadas nos membros inferiores, 26,7% nos membros superiores e 16% na face. Trinta e duas cepas de Leishmania isoladas foram identificadas, com base em 13 isoenzimas, como pertencentes à espécie Leishmania (Viannia) braziliensis.

Setenta e nove pacientes com leishmaniose tegumentar americana, forma cutânea, foram divididos em dois grupos: o grupo experimental (I), formado por 38 pessoas que receberam isotionato de pentamidina, 4mg/kg/dia, três aplicações, IM, durante uma semana, e o grupo controle (II), formado por 41 doentes tratados com N-metilglucamina, 20mgSbV/kg/dia por 20 dias, EV. Foram identificados, por técnica de anticorpos monoclonais, 21 isolados com predominância de Leishmania (Viannia) braziliensis. Encontrou-se cura clínica em 71,05% do grupo experimental e 73,17% do grupo controle (p=0,47). Alterações de ECG foram mais freqüentes utilizando antimonial pentavalente, com significância (p<0,05). O tratamento com a pentamidina mostrou eficácia semelhante quando comparado ao antimonial e apresenta vantagens como duração reduzida de tratamento e baixa toxicidade cardiológica.

We retrospectively analyzed a series of 151 cases of cutaneous leishmaniasis treated between 1967 and 1982. One-hundred-and-thirty-nine (92%) patients presented with active lesions and were treated with daily doses of meglumine antimoniate: 81 adults received a 5-ml vial IM and 58 children received 1 to 5ml. Forty-five (32.4%) patients underwent continuous treatment with meglumine antimoniate for 25 to 116 days without rest intervals, and 94 (67.6%) intermittent treatment with 2 to 5 series of meglumine antimoniate. Intermittent series could include schedules of daily IM applications for 10 to 25 days each and intervals varying from 10 to 60 days. Antimony dose was calculated for 66 (47.5%) patients and ranged from 3.9 to 28.7 Sb5+/kg/day. Of these, 35 patients received >10mg and 31 patients <10mg Sb5+/kg/day. Median time of healing was longer for lesions on the legs and feet - 67.5 days versus 48.7 days (p < 0.001) for other sites. However, there were no significant differences in the median time of healing between adults and children, intermittent and continuous regimens or high and low antimony doses. Fifty-one patients were reassessed 5 to 14 years after treatment and showed no evidence of disease. These results support further investigation (clinical trials) on treatment using low doses of antimony.

The diagnosis for American cutaneous leishmaniasis is based on an association of clinical, epidemiological and laboratory characteristics. The present study identified the circulating species of Leishmania in the State of Pernambuco, described its clinical-epidemiological characteristics and diagnosed the disease. Nineteen patients presenting active lesions who had been diagnosed through clinical evaluation and laboratory tests were selected. The tests included direct investigation, in vitro culturing, Montenegro skin test, indirect immunofluorescence and polymerase chain reaction. The Montenegro Skin Test showed positive results in 89% of the patients; indirect immunofluorescence, in 79%; direct investigation, in 58%; and polymerase chain reaction in 75%. Seven Leishmania (Viannia) braziliensis samples were isolated from these patients and were characterized by means of specific monoclonal antibodies. These data confirm that a combination of different diagnosis techniques is needed in order to obtain efficient results and that, so far, Leishmania (Viannia) braziliensis is the only species responsible for American cutaneous leishmaniasis infection in Pernambuco. Thus, it is essential to identify the parasite species involved in cases of human disease in an endemic area in order to determine the clinical and epidemiological characteristics, especially with regard to diagnosis, therapy development and disease prognosis.

Relata-se novo surto de LTA em militares com 71 casos confirmados pelos critérios clínico, epidemiológico e laboratorial. Obteve-se o isolamento de sete amostras, identificadas como Leishmania (Viannia) braziliensis. A ocorrência de surtos nesta região confirma o caráter endêmico, cuja magnitude parece estar relacionada a não adoção de medidas de proteção individual.

American cutaneous leishmaniasis (ACL) is a complex disease with clinical and epidemiological features that may vary from region to region. In fact, at least seven different Leishmania species, including Leishmania (Viannia) braziliensis, Leishmania (Viannia) guyanensis, Leishmania (Viannia) lainsoni, Leishmania (Viannia) naiffi, Leishmania (Viannia) shawi, Leishmania (Viannia) lindenbergi, and Leishmania (Leishmania) amazonensis, have been implicated in the etiology of ACL in Brazil, and numerous phlebotomine sandfly species of the genus Lutzomyia have been regarded as putative or proven vectors. Because ACL is a focal disease, understanding the disease dynamics at the local level is essential for the implementation of more effective control measures. The present paper is a narrative review about the ACL epidemiology in Pernambuco, northeastern Brazil. Furthermore, the need for more effective diagnosis, treatment, control and prevention strategies for the affected populations is highlighted. This paper will provide researchers with a critical appraisal of ACL in Pernambuco. Hopefully, it will also be helpful for public health authorities to improve current control strategies against ACL at the state and country levels.

Introduction During a diagnostic evaluation of canine visceral leishmaniasis (VL), two of seventeen dogs were found to be co-infected by Leishmania (Viannia) braziliensis and Leishmania (Leishmania) chagasi. Methods Specific polymerase chain reaction (PCR) and restriction fragment length polymorphism-PCR (RFLP-PCR) assays were performed. Results PCR assays for Leishmania subgenus identification followed by RFLP-PCR analysis in biopsies from cutaneous lesions and the spleen confirmed the presence of Leishmania (Viannia) braziliensis and Leishmania (Leishmania) chagasi in those fragments. Conclusions This report reinforces the importance of using serological and molecular techniques in the epidemiological surveillance of canine populations in endemic areas in which both diseases are known to co-exist. In such cases, a reassessment of the control measures is required.
