A serological survey of Toxoplasma gondii infection in population groups in Fortaleza, Brazil, was conducted, in order to identify seroprevalence rates according to age and risk factors associated with acquired infection. A Toxoplasma IgG-antibody enzyme immunoassay (Sanofi Pasteur Diagnostics, Marnes la Coquette, France) was employed to assess the immunity. Public day-care centers and schools were randomly selected, while three large antenatal clinics and maternity units were choosen by its importance. Population groups and results of 997 blood tests were: 227 children (mean age 3.8 years), 22.8% seropositives; 584 students (mean 11.4 years), 58.4%, and pregnant and postpartum women (mean 24 years), 71.5% seropositives (p < 0.001). Of 256 participants reporting close contact with cats, 59.8% were seropositive, in contrast with 51% seropositives without contact (relative risk 1.17; 95% confidence interval 1.04-1.33; p = 0.01). Having three or more siblings at home was related to higher seroprevalence in children and students (relative risk 1.39; 95% confidence interval 1.21-1.60; p < 0.01). In conclusion, toxoplasmosis seroprevalence showed a rapid increase during the first ten years of life, in association with close contact with cats and larger households, probably related to inappropriate hygiene and child-care practices.

Burkholderia pseudomallei has rarely been isolated from environmental and clinical specimens in South America, particularly, in Brazil. This report describes a case of melioidosis with fulminant sepsis in a 10 year old boy, from rural area, in Tejuçuoca, State of Ceará, Brazil. Blood samples were positive and, through the analysis of results from biochemical tests and of drugs susceptibility profile, identified this gram-negative bacillus as B. pseudomallei. The contamination source remains obscure in this case, as soil and water tanks samples submitted to microbiological analyses did not indicate the presence of B. pseudomallei.

Oysters are edible organisms that are often ingested partially cooked or even raw, presenting therefore a very high risk to the consumers' health, especially in tropical regions. The presence of Vibrio cholerae and Vibrio parahaemolyticus in oysters sampled at an estuary in the Brazilian northeastern region was studied, with 300 oysters tested through an 8-months period. The salinity of the water at the sampling point varied between 3% and 27‰. V. cholerae was the most frequently detected species (33.3% of the samples), and of the 22 V. cholerae isolates, 20 were identified as non-O1/non-O139, with two of the colonies presenting a rough surface and most of remaining ones belonging to the Heiberg II fermentation group. V. parahaemolyticus was isolated from just one of the samples. Other bacteria such as Providencia spp., Klebsiella spp. and Morganella morganii were also isolated.

Airbone fungi are considered important causes of allergic rhinitis and allergic asthma. The knowledge of these fungi in a city or region is important for the ecological diagnosis and specific treatment of allergic manifestations induced by inhalation of fungal allergens. The airborne fungi of Fortaleza, State of Ceará, Brazil, were studied during a one year period. Five hundred and twenty Petri dishes with Sabouraud dextrose agar medium were exposed at ten different locations in the city. The dishes exposed yielded one thousand and five hundred and twenty one colonies of twenty four genera. The most predominants were: Aspergillus (44.7%), Penicillium (13.3%), Curvularia (9.8%), Cladosporium (6.8%), Mycelia sterilia (6.0%), Fusarium (3.5%), Rhizopus (3.1%), Drechslera (2.6%), Alternaria (2.4%) and Absidia (2.2%). The results shown that Aspergillus, Penicillium, Mycelia sterilia, Fusarium and Alternaria were found during all months in the year. Absidia was more frequent during the dry season. Anemophilous fungi and the high concentration of spores in the air are important because may result in an increased number of people with allergic respiratory disease.

Between April and October 2002, thirty fortnightly collections of oysters (Crassostrea rhizophorea) from a natural oyster bed at the Cocó River estuary in the Sabiaguaba region (Fortaleza, Ceará, Brazil) were carried out, aiming to isolate Aeromonas spp. strains. Oyster samples were submitted to the direct plating (DP) and the presence/absence (P/A) methods. Aeromonas were identified in 15 (50%) samples analyzed by the DP method and in 13 (43%) analyzed by the P/A method. A. caviae, A. eucrenophila, A. media, A. sobria, A. trota, A. veronii bv. sobria, A. veronii bv. veronii and Aeromonas sp. were isolated. The predominant species was A. veronii (both biovars), which was identified in 13 (43%) samples, followed by A. media in 11 (37%) and A. caviae in seven (23%). From the 59 strains identified, 28 (48%) presented resistance to at least one of the eight antibiotics tested.

Trichosporon spp. are yeasts capable of causing invasive disease, which mainly affect immunocompromised patients. A clinical strain of T. asahii was isolated from the blood cultures of patients admitted to the General Hospital of Fortaleza. Susceptibility tests were conducted by disk diffusion and broth microdilution. The isolated strain of T. asahii was resistant to fluconazole. The patient used amphotericin B and caspofungin in order to facilitate the microbiological cure. It was the first isolation and identification of T. asahii in blood culture in Ceará, Brazil.

By controlling the transmission of Chagas disease, the challenge of providing assistance to millions of infected patients that reach old age arises. In this study, the socioeconomic, demographic and comorbidity records of all elderly chagasic patients followed at the Pharmaceutical Care Service of the Chagas Disease Research Laboratory were assessed. The information related to the clinical form of the disease was obtained from medical records provided by the Walter Cantídio University Hospital. The profile of the studied population was: women (50.5%); mean age of 67 years; retired (54.6%); married (51.6 %); high illiteracy rate (40.2%); and family income equal to the minimum wage (51.5%). The predominant clinical forms of Chagas disease were cardiac (65.3%) and indeterminate (14.7%). The main electrocardiographic changes were the right bundle branch block (41.0%), associated or not with the anterosuperior left bundle branch block (27.4%). The average number of comorbidities per patient was 2.23 ± 1.54, with systemic arterial hypertension being the main one found (67.0%). It was found that the elderly comprise a vulnerable group of patients that associate aging with cardiac and/or digestive disorders resulting from the evolution of Chagas disease and other comorbidities, which requires special attention from health services to ensure more appropriate medical and social care.

This report is a retrospective study of the epidemiology of scorpion sting cases recorded from 2007 to 2013 in the State of Ceará, Northeastern Brazil. Data were collected from the Injury Notification Information System database of the Health Department of Ceará. A total of 11,134 cases were studied and distributed across all the months of the studied period and they occurred mainly in urban areas. Victims were predominantly 20-29 years-old women. Most victims were bitten on the hand; and received medical assistance within 1-3 hours after being bitten. Cases were mostly classified as mild and progressed to cure. Scorpion envenomation in Ceará is an environmental public health problem that needs to be monitored and controlled throughout the year.

In the American continent, honeybee envenomation is a public health problem due to the high incidence and severity of the cases. Despite its medical importance, there is a lack of epidemiological studies on this topic in Brazil, especially referring to the Northeastern states. The present study has aimed to describe the epidemiological features of honeybee envenomation cases in the state of the Ceará, Northeastern Brazil, from 2007 to 2013. Data were collected from the Injury Notification Information System database of the Health Department of Ceará. A total of 1,307 cases were analyzed. Cases were shown to be distributed in all the months of the studied years, reaching higher frequencies in August. The majority of cases occurred in urban areas and involved men aged between 20 and 29 years. Victims were mainly stung on the head and torso, and they received medical assistance predominantly within 3 hours after being stung. Local manifestations were more frequent than systemic ones. Most cases were classified as mild and progressed to cure. The high number of honeybee sting cases shows that Ceará may be an important risk area for such injuries. Moreover, the current study provides data for the development of strategies to promote control and prevention of bee stings in this area.

Os autores demonstram através de testes em camundongos que variando o meio de cultivo a avirulência da cepa Y, cultivada, do Trypanosoma cruzi se conserva inalterada. Diante da possibilidade de que se trate de um mutante da mesma linhagem Y virulenta, mantida em animais, sugerem a designação de PF (Pedreira de Freitas) para essa nova cepa.

São apresentadas as principais informações de ordem geográfica do Estado, que é dividido em 23 micro-regiões e 141 municípios. Desses, em 129 foram encontrados triatomíneos, de acordo com informações obtidas em diversos organismos de pesquisa, de 1957 a 1974. A espécie principal no Estado é o Triatoma brasiliensis, semi-doméstico, encontrado nas casas de 91,5% dos municípios (129), suas taxas de infecção variam de 1,0 a 40,4% (média de 8,2%), o P. megistus é a terceira espécie e é encontrado em 61,7% dos municípios (87), com uma taxa de infecção muito variável, com média de 3,9% e máxima de 25,9%. Em áreas restritas como a do Cariri essa taxa subirá para 24,5%. A segunda espécie, menos doméstica que as duas primeiras é o T. pseudomaculata, encontrado em 68,8% dos municípios e taxas de infecção variando de 0,3% a 7,1% e uma média de 4,2%. A quarta espécie é o Rhodnius nasutus, encontrado em 17,7% dos municípios e infectado em 1,0%. Penetrando na casa (não foram encontradas ninfas) há uma quinta espécie, o Panstrongylus lutzi capturado em algumas casas de 26 municípios (18,4%) e com elevadas taxas de infecção: 17,9%. Com taxas globais de triatomíneos infectados por T. cruzi acima de 10% estão 12 municípios, dos quais mais da metade constitui as 4 micro-regiões centrais do Estado: Sertão de Quixeramobim, Sertão de Senador Pompeu, Sertão dos Inhamuns e Médio Jaguaribe.

Do Ceará, estado com problema de Doença de Chagas, Russas tem recebido a maior soma de investigações, verificando-se 11,6% de infecção humana, em 1959. Recentemente foram examinados cães, gatos e outros animais encontrando-se altas taxas de infecção. Selecionou-se, na presente pesquisa, uma micro-área (Fazenda Boa Vista) na região, onde haviam sido antes examinados 52 cães e 21 gatos, com 11 cães e 10 gatos positivos. Foram examinados mais 12 cães e 10 gatos, com 1 e 5 positivos, respectivamente, elevando-se a taxa de positividade global para 29,5%. Nessa micro-área foram examinados 125 pessoas pela RFC e pelo xenodiagnóstico, encontrando-se uma positividade de 7,2%. Desses positivos, 55% residem em casas com animal positivo, sendo que duas casas apresentaram duas pessoas positivas cada. Do total de xenos somente dois foram positivos. Na micro-área foram capturados 405 triatomíneos, com 27,7% positivos, dentro e fora das casas (galinheiros, currais, paredes externas, telheiros e alpendres). A espécie prevalente é o Triatoma brasiliensis (93,5%), encontrada dentro das casas (52,8%) com taxa de infecção de 30,2% e fora das casas (47,2%) com 24,6% de infecção. No total da região examinada na pesquisa anterior, as taxas de infecção foram 19,6% dentro das casas e 21,8% fora. A segunda espécie é o T. pseudomaculata (5,4%), encontrado mais fora das casas (72,8%) e infectado 25%. Foram capturados 8 exemplares de P. megistus sempre fora das casas, sendo 4 na micro-área; 5 exemplares estavam infectados. Na micro-área, tal como em 2 outras áreas da região, foram encontrados roedores domésticos e marsupiais com T. cruzi: 4 em 10 Rattus rattus e 1 em 3 Didelphis azarae, tal como já havia sido verificado por um dos AA. em outras áreas do Estado. Conclui-se pela existência de um dinâmico ciclo doméstico e paradoméstico de circulação do T. cruzi na micro-área, com abundantes oportunidades de infecção do homem e dos animais domésticos.

Foram isoladas duas cepas de Trypanosoma cruzi de marsupiais de área não endêmica e três cepas de área endêmica: uma de homem, uma de marsupial e uma de cão. Foram utilizados para uma prova de suscetibilidade frente a estas cepas 20 ninfas de 5º estádio das seguintes espécies de triatomíneos e testada previamente a capacidade de sugar camundongos brancos em gramas de sangue: Triatoma brasiliensis sugou uma média de 0,09 g, Triatoma infestans sugou 0,13 g, Panstrongylus megistus sugou 0,13 g e Rhodnius prolixus sugou 0,09 g. O T. infestans infectou-se com as taxas mais elevadas com todas as cepas; o T. brasiliensis foi a espécie que menos infectou-se, sendo seguida de perto pelo R. prolixus o P. megistus foi testado somente com as cepas de área não endêmica, apresentando taxas de infecção intermediárias entre o T. infestans e o T. brasiliensis. As cepas que mais infectam são as de marsupiais e a que menos infecta é a proveniente de cão.

Foram examinados 1.368 mamíferos em ecótopos naturais ou artificiais: 443 cães com 2,5% positivos; 605 gatos com 0,7%, 65 roedores domésticos com 4,6%, 6 coelhos e 10 cobaias negativos. Dentre os animais silvestres, foram examinados 115 preás (Galea s. spixii) com 2,3% positivos, 12 "cassacos" (Didelphis azarae) dos quais 9 positivos; 5 quirópteros, 5 punáres (Cercomys c. laurentius) e 3 pebas (Dasypus sexcinctus) negativos. A amostra apresentada é suficiente para considerada a existência de ciclos doméstico e silvestre de T. cruzi na área estudada e considerada endêmica da Doença de Chagas.

Os autores apresentaram informações sobre 6.556 triatomíneos capturados na área pela SUCAM, de 1964 a 1974, os quais apresentavam T. cruzi em 4,7%. O estudo atual revelou 1.290 exemplares em 69 localidades trabalhadas, 63,8% com triatomíneos, das quais 52,3% apresentavam triatomíneos infectados, o que significa 33,3% da área. Foram pesquisados ecótopos artificiais distante até 200 metros das casas. Dos 37 ecótopos naturais pesquisados, 73,6% eram habitados por morcegos, 23,7% por preás e 5,2% por punarés, espécies que apresentam variáveis taxas de infecção por T. cruzi. O T. brasiliensis corresponde a 65,5% dos triatomíneos capturados e 942% dos triatomíneos infectados são desta espécie, considerada a principal transmissora de Doença de Chagas na região. Em ecótopos naturais é a única espécie encontrada (10 vezes em 55, os triatomíneos estavam infectados). Dentro das habitações a espécie é menos abundante e menos infectada (2,7%). A segunda espécie é o T. pseudomaculata, (34,3%) considerada sem importância na transmissão da Doença de Chagas; a terceira espécie é o P. megistus, da qual foram capturados apenas 2 exemplares (0,2%).
