Multi-resistant gram-negative rods are important pathogens in intensive care units (ICU), cause high rates of mortality, and need infection control measures to avoid spread to another patients. This study was undertaken prospectively with all of the patients hospitalized at ICU, Anesthesiology of the Hospital São Paulo, using the ICU component of the National Nosocomial Infection Surveillance System (NNIS) methodology, between March 1, 1997 and June 30, 1998. Hospital infections occurring during the first three months after the establishment of prevention and control measures (3/1/97 to 5/31/97) were compared to those of the last three months (3/1/98 to 5/31/98). In this period, 933 NNIS patients were studied, with 139 during the first period and 211 in the second period. The overall rates of infection by multi-resistant microorganisms in the first and second periods were, respectively, urinary tract infection: 3.28/1000 patients/day; 2.5/1000 patients/day; pneumonia: 2.10/1000 patients/day; 5.0/1000 patients/day; bloodstream infection: 1.09/1000 patients/day; 2.5/1000 patients/day. A comparison between overall infection rates of both periods (Wilcoxon test) showed no statistical significance (p = 0.067). The use of intervention measures effectively decreased the hospital bloodstream infection rate (p < 0.001), which shows that control measures in ICU can contribute to preventing hospital infections.

The antimicrobial activity of three different extracts (hexanic, ethyl acetate, methanol) obtained from Brazilian Drosera species (D. communis, D. montana var. montana, D. brevifolia, D. villosa var. graomogolensis, D. villosa var. villosa, Drosera sp. 1, and Drosera sp. 2 ) were tested against Staphylococcus aureus (ATCC 25923), Enterococcus faecium (ATCC23212), Pseudomonas aeruginosa (ATCC27853), Escherichia coli (ATCC11229), Salmonella choleraesuis (ATCC10708), Klebsiella pneumoniae (ATCC13883), and Candida albicans (a human isolate). Better antimicrobial activity was observed with D. communis and D. montana var. montana ethyl acetate extracts. Phytochemical analyses from D. communis, D. montana var. montana and D. brevifolia yielded 5-hydroxy-2-methyl-1,4-naphthoquinone (plumbagin); long chain aliphatic hydrocarbons were isolated from D. communis and from D. villosa var. villosa, a mixture of long chain aliphatic alcohols and carboxylic acids, was isolated from D. communis and 3b-O-acetylaleuritolic acid from D. villosa var. villosa.

A prospective study of fungal and bacterial flora of burn wounds was carried out from February 2004 to February 2005 at the Burns Unit of Hospital Regional da Asa Norte, Brasília, Brazil. During the period of the study, 203 patients were treated at the Burns Unit. Wound swab cultures were assessed at weekly intervals for four weeks. Three hundred and fifty four sampling procedures (surface swabs) were performed from the burn wounds. The study revealed that bacterial colonization reached 86.6% within the first week. Although the gram-negative organisms, as a group, were more predominant, Staphylococcus aureus (28.4%) was the most prevalent organism in the first week. It was however surpassed by Pseudomonas aeruginosa form third week onwards. For S. aureus and P. aeruginosa vancomycin and polymyxin were found to be the most effective drugs. Most of the isolates showed high level resistance to antimicrobial agents. Fungi were found to colonize the burn wound late during the second week postburn, with a peak incidence during the third and fourth weeks. Species identification of fungi revealed that Candida tropicalis was the most predominant, followed by Candida parapsilosis. It is crucial for every burn institution to determine the specific pattern of burn wound microbial colonization, the time-related changes in the dominant flora, and the antimicrobial sensitivity profiles. This would enable early treatment of imminent septic episodes with proper empirical systemic antibiotics, without waiting for culture results, thus improving the overall infection-related morbidity and mortality.

Complex physalin metabolites present in the capsules of the fruit of Physalis angulata L. have been isolated and submitted to a series of assays of antimicrobial activity against Pseudomonas aeruginosa ATCC 27853, Staphylococcus aureus ATCC 29213, S. aureus ATCC 25923, S. aureus ATCC 6538P, Neisseria gonorrhoeae ATCC 49226, Escherichia coli ATCC 8739; E. coli ATCC 25922, Candida albicans ATCC 10231 applying different methodologies such as: bioautography, dilution broth, dilution agar, and agar diffusion techniques. A mixture of physalins (pool) containing physalins B, D, F, G inhibit S. aureus ATCC 29213, S. aureus ATCC 25923, S. aureus ATCC 6538P, and N. gonorrhoeae ATCC 49226 at a concentration of 200 mg/µl, using agar dilution assays. The mixture was inactive against P. aeruginosa ATCC27853, E. coli ATCC 8739; E. coli ATCC 25922, C. albicans ATCC 10231 when applying bioautography assays. Physalin B (200 µg/ml) by the agar diffusion assay inhibited S. aureus ATCC 6538P by ± 85%; and may be considered responsible for the antimicrobial activity.

Twelve extracts obtained from nine plants belonging to six different genera of Clusiaceae were analyzed against Gram-negative (Escherichia coli and Pseudomonas aeruginosa) and Gram-positive (Staphylococcus aureus and Enterococcus faecalis) bacteria using the microdilution broth assay. Tovomita aff. longifolia, T. brasiliensis, Clusia columnaris, Garcinia madruno, Haploclathra paniculata, and Caraipa grandifolia extracts showed significant results against the bacteria. The organic extract obtained from the leaves of T. aff. longifolia showed minimal inhibitory concentration (MIC) = 70 µg/ml and minimal bactericidal concentration (MBC) = 90 µg/ml against E. faecalis and the organic extract made with the stem of C. columnaris showed MIC = 180 µg/ml and MBC = 270 µg/ml against P. aeruginosa. None of the antibacterial extracts showed lethal activity against brine shrimp nauplii. On the other hand, both aqueous and organic extracts obtained from the aerial organs of Vismia guianensis that were cytotoxic to brine shrimp nauplii did not show a significant antibacterial activity in the assay.

The sensitivity of two Gram positive (Staphylococcus aureus and Bacillus subtilis) and two Gram negative (Escherichia coli and Pseudomonas aeruginosa) pathogenic multi-drug resistant bacteria was tested against the crude extracts (cold aqueous, hot aqueous, and methanol extracts) of leaves and seeds of Argemone mexicana L. (Papaveraceae) by agar well diffusion method. Though all the extracts were found effective, yet the methanol extract showed maximum inhibition against the test microorganisms followed by hot aqueous extract and cold aqueous extract.

Emerging resistance phenotypes and antimicrobial resistance rates among pathogens recovered from community-acquired urinary tract infections (CA-UTI) is an increasing problem in specific regions, limiting therapeutic options. As part of the SENTRY Antimicrobial Surveillance Program, a total of 611 isolates were collected in 2003 from patients with CA-UTI presenting at Latin American medical centers. Each strain was tested in a central laboratory using Clinical Laboratory Standard Institute (CLSI) broth microdilution methods with appropriate controls. Escherichia coli was the leading pathogen (66%), followed by Klebsiella spp. (7%), Proteus mirabilis (6.4%), Enterococcus spp. (5.6%), and Pseudomonas aeruginosa (4.6%). Surprisingly high resistance rates were recorded for E. coli against first-line orally administered agents for CA-UTI, such as ampicillin (53.6%), TMP/SMX (40.4%), ciprofloxacin (21.6%), and gatifloxacin (17.1%). Decreased susceptibility rates to TMP/SMX and ciprofloxacin were also documented for Klebsiella spp. (79.1 and 81.4%, respectively), and P. mirabilis (71.8 and 84.6%, respectively). For Enterococcus spp., susceptibility rates to ampicillin, chloramphenicol, ciprofloxacin, and vancomycin were 88.2, 85.3, 55.9, and 97.1%, respectively. High-level resistance to gentamicin was detected in 24% of Enterococcus spp. Bacteria isolated from patients with CA-UTI in Latin America showed limited susceptibility to orally administered antimicrobials, especially for TMP/SMX and fluoroquinolones. Our results highlight the need for developing specific CA-UTI guidelines in geographic regions where elevated resistance to new and old compounds may influence prescribing decisions.

Seven medicinal plant extracts traditionally used in Kenya, mainly for management of infectious conditions, were chosen and screened for their antibacterial activity against Gram-negative (Pseudomonas aeruginosa and Escherichia coli) and Gram-positive (Bacillus cereus and Staphylococcus aureus) bacteria. Antibacterial activity was tested using the broth dilution method. Harrisonia abyssinica and Terminalia kilimandscharica extracts showed significant activity against Gram+ and Gram- bacteria. The methanolic extracts of T. kilimandscharica bark and H. abyssinica bark and leaves showed minimum inhibitory activity against all tested bacteria, with minimal inhibitory concentrations ranging from 25-150 mg/mL. Ajuga remota and Amaranthus hybridus, which are lethal to brine shrimp nauplii, showed significantly lower antibacterial activity than those that were relatively non-toxic.

A atividade antimicrobiana das Pastilhas de Paraformaldeído, reproduzindo as condições de uso das Instituições de Saúde do Brasil (sem o aquecimento, sem o acréscimo da umidade relativa, a 5%, por um período longo de exposição de 12 horas), foi avaliada "in vitro" por meio do monitoramento microbiológico, segundo a metodologia da AOAC, adotada oficialmente pelo Ministério da Saúde do Brasil para o registro dessa categoria de produtos. Os resultados dos experimentos refutaram a ação esterilizante das Pastilhas de Paraformaldeído nestas condições. Diante destes resultados, realizou-se, então, os testes para avaliação desinfetante do produto utilizando-se o Método de Diluição de Uso, preconizada pela AOAC, adaptado para produtos gasosos, contra os microrganismos teste padronizados Staphylococcus aureus (ATCC nº. 6538), Samonella choleraesuis (ATCC nº. 10708) e Pseudomonas aeruginosa (ATCC nº. 15442). Nestes experimentos, os resultados das culturas mostraram-se 100% negativos contra todas as bactérias testadas inferindo-se indícios de atividade desinfetante de alto nível.

O estudo realizou a análise bacteriológica de sabões líquidos utilizados para lavagem das mãos dos profissionais de saúde. Trata-se de estudo exploratório transversal, desenvolvido nas unidades de internação de hospital de médio porte em Fortaleza/CE. Os dados foram colhidos no período de maio a julho de 2007. Do total de 59 frascos com sabão líquido, 33 continham os seguintes microorganismos: Burkholderia cepacia (n=14), Pseudomonas putidas (9), Pseudomonas aeruginosa (3), Klebsiella pneumoniae (3), Enterobacter cloacae (2), Pseudomonas luteola (2). As unidades com maior número de amostras contaminadas foram a clínica cirúrgica (n=7) e a clínica dermatológica (n=4). A contaminação também foi verificada em frasco original do mesmo lote de sabão líquido usado para abastecer as saboneteiras. Podemos concluir ser necessário disciplinar e controlar a qualidade desses produtos nas linhas de produção tanto quanto nas fases de uso nos serviços de saúde, sobretudo porque sua utilidade se presta à prevenção de infecção hospitalar.

Estudo experimental laboratorial que comparou a ação de cinco métodos de desinfecção na remoção de biofilme em endoscópios gastrintestinais. Foram utilizados como corpos de prova tubos novos transparentes de politetrafluoretileno (Teflon®) simulando os canais flexíveis dos endoscópios. Após limpeza prévia os tubos foram contaminados intencionalmente com Pseudomonas aeruginosa para formação de biofilme e submetidos à desinfecção. Como resultado, nenhum deles removeu 100% dos biofilmes. O que mais removeu fisicamente o biofilme foi o glutaraldeído 2% em processadora automática, provavelmente justificado pela dupla limpeza, já que o equipamento conta com essa fase no início do seu ciclo. O método que se mostrou menos eficiente para remoção de biofilme e outros resíduos foi água eletrolítica ácida. Esses resultados sugerem que a limpeza é mais impactante na remoção de biofilmes do que a desinfecção consecutiva, uma vez que o glutaraldeído, desinfetante da máquina que se mostrou mais eficiente, é um fixador de resíduos orgânicos.

OBJECTIVE To evaluate the in vitro antimicrobial activity of the Eucalyptus globulus essential oil, and of the xylitol and papain substances against the following microorganisms: Pseudomonas aeruginosa; Samonella sp.; Staphylococus aureus; Proteus vulgaris; Escherichia coli and Candida albicans. METHOD The in vitro antimicrobial evaluation was used by means of the agar diffusion test and evaluation of the inhibition zone diameter of the tested substances. Chlorhexidine 0.5% was used as control. RESULTS The Eucalyptus globulus oil showed higher inhibition than chlorhexidine when applied to Staphylococcus aureus, and equal inhibition when applied to the following microorganisms: Escherichia coli, Proteus vulgaris and Candida albicans. Papain 10% showed lower antimicrobial effect than chlorhexidine in relation to Candida albicans. Xylitol showed no inhibition of the tested microorganisms. CONCLUSION The Eucalyptus globulus oil has antimicrobial activity against different microorganisms and appears to be a viable alternative as germicidal agent hence, further investigation is recommended.

O trabalho teve o objetivo avaliar o crescimento de mudas de mamoeiro do grupo Solo, sob diferentes doses dos fertilizantes naturais bokashi e pó de algas marinhas (Lithothamnium sp). O substrato utilizado foi a mistura de terra, areia e composto orgânico (3:2:1, v/v). Os tratamentos consistiram de quatro doses do fertilizante bokashi (0; 3; 6; 10%, v/v) e quatro doses do fertilizante lithothamnium (0; 3; 6; 10 g L-1), adicionados ao substrato, antes do enchimento das sacolas. Utilizou-se o delineamento experimental em blocos inteiramente casualizados, em esquema fatorial 4x4, com quatro repetições e cinco plantas por parcela experimental. Foram realizadas avaliações aos 15; 30; 60 e 100 dias após a semeadura, sendo elas: emergência (%), número de folhas, comprimento da parte aérea e da raiz (cm), massa seca da parte aérea, da raiz e total (mg). Houve interação significativa dos fatores testados para o comprimento da parte aérea, aos sessenta e cem dias após a semeadura; efeito isolado do bokashi para todas as variáveis analisadas, exceto na emergência, comprimento da parte aérea e número de folhas aos trinta dias após a semeadura. O uso conjugado dos fertilizantes mostrou efeito positivo na precocidade e altura da planta, podendo ser recomendado na formulação de substratos para a produção de mudas de mamoeiro do grupo Solo.

210Po concentrations have been determined in one green alga and in five freshwater plants grown in a pond of the Carapebus restinga (State of Rio de Janeiro). The alga Chara sp showed elevated concentration of 210Po, similar to that observed for marine algae. All the other plants had the lowest concentration of 210Po in the stems and the highest in the roots. Intermediate values were observed in the leaves. The unexpected high concentration of 210Po in the roots, even superior to reported values for roots of plants from high radioactive background areas, must be due to the elevated levels of this radionuclide in associated soils that are known to be rich in humic organic material. There seem to have been no translocation of this radionuclide from the roots to the other parts of the plants.
