Nonstructural protein 4 (NSP4), encoded by group A rotavirus genome segment 10, is a multifunctional protein and the first recognized virus-encoded enterotoxin. The NSP4 gene has been sequenced, and five distinct genetic groups have been described: genotypes A-E. NSP4 genotypes A, B, and C have been detected in humans. In this study, the NSP4-encoding gene of human rotavirus strains of different G and P genotypes collected from children between 1987 and 2003 in three cities of West Central region of Brazil was characterized. NSP4 gene of 153 rotavirus-positive fecal samples was amplified by reverse transcriptase-polymerase chain reaction and then sequenced. For phylogenetic analysis, NSP4 nucleotide sequences of these samples were compared to nucleotide sequences of reference strains available in GenBank. Two distinct NSP4 genotypes could be identified: 141 (92.2%) sequences clustered with NSP4 genotype B, and 12 sequences (7.8%) clustered with NSP4 genotype A. These results reinforce that further investigations are needed to assess the validity of NSP4 as a suitable target for epidemiologic surveillance of rotavirus infections and vaccine development.

The dispersal and survival of the phlebotomines Nyssomyia intermedia and Nyssomyia neivai (both implicated as vectors of the cutaneous leishmaniasis agent) in an endemic area was investigated using a capture-mark-release technique in five experiments from August-December 2003 in municipality of Iporanga, state of São Paulo, Brazil. A total of 1,749 males and 1,262 females of Ny. intermedia and 915 males and 411 females of Ny. neivai were marked and released during the five experiments. Recapture attempts were made using automatic light traps, aspiration in natural resting places and domestic animal shelters and Shannon traps. A total of 153 specimens (3.48%) were recaptured: 2.59% (78/3,011) for Ny. intermedia and 5.35% (71/1,326) for Ny. neivai. Both species were recaptured up to 144 h post-release, with the larger part of them recaptured within 48 h. The median dispersion distances for Ny. intermedia and Ny. neivai, respectively, were 109 m and 100 m. The greatest dispersal range of Ny. intermedia was 180 m, while for Ny. neivai one female was recaptured in a pasture at 250 m and another in a pigsty at 520 m, showing a tendency to disperse to more open areas. The daily survival rates calculated based on regressions of the numbers of marked insects recaptured on the six successive days after release were 0.746 for males and 0.575 for females of Ny. intermedia and 0.649 for both sexes of Ny. neivai. The size of the populations in the five months ranged from 8,332-725,085 for Ny. intermedia males, 2,193-104,490 for Ny. intermedia females, 1,687-350,122 for Ny. neivai males and 254-49,705 for Ny. neivai females.

The goal of treatment of chronic hepatitis C is to achieve a sustained virological response, which is defined as exhibiting undetectable hepatitis C virus (HCV) RNA levels in serum following therapy for at least six months. However, the current treatment is only effective in 50% of patients infected with HCV genotype 1, the most prevalent genotype in Brazil. Inhibitors of the serine protease non-structural protein 3 (NS3) have therefore been developed to improve the responses of HCV-infected patients. However, the emergence of drug-resistant variants has been the major obstacle to therapeutic success. The goal of this study was to evaluate the presence of resistance mutations and genetic polymorphisms in the NS3 genomic region of HCV from 37 patients infected with HCV genotype 1 had not been treated with protease inhibitors. Plasma viral RNA was used to amplify and sequence the HCV NS3 gene. The results indicate that the catalytic triad is conserved. A large number of substitutions were observed in codons 153, 40 and 91; the resistant variants T54A, T54S, V55A, R155K and A156T were also detected. This study shows that resistance mutations and genetic polymorphisms are present in the NS3 region of HCV in patients who have not been treated with protease inhibitors, data that are important in determining the efficiency of this new class of drugs in Brazil.

The aim of the present study was to assess the prevalence of Haemophilus influenzaetype b (Hib) nasopharyngeal (NP) colonisation among healthy children where Hib vaccination using a 3p+0 dosing schedule has been routinely administered for 10 years with sustained coverage (> 90%). NP swabs were collected from 2,558 children who had received the Hib vaccine, of whom 1,379 were 12-< 24 months (m) old and 1,179 were 48-< 60 m old. Hi strains were identified by molecular methods. Hi carriage prevalence was 45.1% (1,153/2,558) and the prevalence in the 12-< 24 m and 48-< 60 m age groups were 37.5% (517/1,379) and 53.9% (636/1,179), respectively. Hib was identified in 0.6% (16/2,558) of all children in the study, being 0.8% (11/1,379) and 0.4% (5/1,179) among the 12-< 24 m and 48-< 60 m age groups, respectively. The nonencapsulate Hi colonisation was 43% (n = 1,099) and was significantly more frequent at 48-< 60 m of age (51.6%, n = 608) compared with that at 12-< 24 m of age (35.6%, n = 491). The overall resistance rates to ampicillin and chloramphenicol were 16.5% and 3.7%, respectively; the co-resistance was detected in 2.6%. Our findings showed that the Hib carrier rate in healthy children under five years was very low after 10 years of the introduction of the Hib vaccine.

Urban occurrence of human and canine visceral leishmaniasis (VL) is linked to households with characteristics conducive to the presence of sand flies. This study proposes an ad hoc classification of households according to the environmental characteristics of receptivity to phlebotominae and an entomological study to validate the proposal. Here we describe the phlebotominae population found in intra- and peridomiciliary environments and analyse the spatiotemporal distribution of the VL vector Lutzomyia longipalpis of households receptive to VL. In the region, 153 households were classified into levels of receptivity to VL followed by entomological surveys in 40 of those properties. Kruskal-Wallis verified the relationship between the households’ classification and sand fly abundance and Kernel analysis evaluated L. longipalpis spatial distribution: of the 740 sand flies were captured, 91% were L. longipalpis; 82% were found peridomiciliary whilst the remaining 18% were found intradomiciliary. No statistically significant association was found between sandflies and households levels. L. longipalpis counts were concentrated in areas of high vulnerability and some specific households were responsible for the persistence of the infestation. L. longipalpis prevails over other sand fly species for urban VL transmission. The entomological study may help target the surveillance and vector control strategies to domiciles initiating and/or maintaining VL outbreaks.

Na Atenção Primária à Saúde (APS) o quadro de trabalhadores de enfermagem é planejado de forma empírica, gerando distorção entre a alocação e a real necessidade das unidades de saúde. O objetivo deste trabalho foi identificar as intervenções de enfermagem na APS para subsidiar o dimensionamento dos trabalhadores. Foram utilizadas as seguintes fontes: revisão bibliográfica em bases de dados no período de 1999-2009; observação em campo em Unidade de Saúde da Família; levantamento em prontuários de famílias; mapeamento das atividades em intervenções de enfermagem segundo a taxonomia Nursing Interventions Classification e validação dessas intervenções. Identificaram-se 169 atividades: 11 atividades associadas; 5 pessoais; e 153 de cuidados diretos e indiretos que foram mapeadas e validadas em 7 domínios, 15 classes e 46 intervenções da NIC. O estudo possibilitou o reconhecimento das práticas de enfermagem na APS por meio de uma linguagem padronizada, subsidiando a sua aplicação na construção de instrumentos para a identificação da carga de trabalho.

Este estudo estimou a validade de constructo pelo teste de grupos conhecidos do Instrumento para Mensuração do Impacto da Doença no Cotidiano do Valvopata (IDCV) quanto a sinais e sintomas, função ventricular sistólica, fração de ejeção do ventrículo esquerdo (FEVE) e qualidade de vida relacionada à saúde (QVRS) em 153 coronariopatas em seguimento ambulatorial. Os dados foram obtidos pela aplicação do IDCV e das versões brasileiras do The Medical Study 36-item Short Form Health Survey – SF-36 e MacNew Heart Disease Health-related Quality of Life Questionnair e. Foi utilizado o teste de Mann-Whitney para verificar a capacidade do IDCV em discriminar o impacto quanto a sinais e sintomas, FEVE e disfunção sistólica ventricular, bem como o teste de Kruskal-Wallis para verificar seu poder de discriminação em relação à QVRS. Constatou-se que o IDCV discriminou o impacto entre aqueles que pontuaram nos quartis (≤Q1, Q1-Q3, ≥Q3) de QVRS. Os achados deste estudo contribuem para o refinamento do IDCV na mensuração do impacto da doença entre coronariopatas.

AbstractOBJECTIVEAnalyzing beliefs and actions of nurses in exercising patient advocacy in a hospital context.METHODA quantitative cross-sectional exploratory and descriptive study, conducted with 153 nurses from two hospitals in southern Brazil, one public and one philanthropic, by applying Protective Nursing Advocacy Scale - Brazilian version. Data were analyzed using descriptive statistics and analysis of variance.RESULTSNurses believe they are advocating for patients in their workplaces, and agree that they should advocate, especially when vulnerable patients need their protection. Personal values and professional skills have been identified as major sources of support for the practice of advocacy.CONCLUSIONNurses do not disagree nor agree that advocating for patients in their working environments can bring them negative consequences. It is necessary to recognize how the characteristics of public and private institutions have helped or not helped in exercising patient advocacy by nurses.

The present work aims at knowing the faunal composition of drosophilids in forest areas of southern Brazil. Besides, estimation of species richness for this fauna is briefly discussed. The sampling were carried out in three well-preserved areas of the Atlantic Rain Forest in the State of Santa Catarina. In this study, 136,931 specimens were captured and 96.6% of them were identified in the specific level. The observed species richness (153 species) is the largest that has been registered in faunal inventories conducted in Brazil. Sixty-three of the captured species did not fit to the available descriptions, and we believe that most of them are non-described species. The incidence-based estimators tended to give rise to the largest richness estimates while the abundance based give rise to the smallest ones. Such estimators suggest the presence from 172.28 to 220.65 species in the studied area. Based on these values, from 69.35 to 88.81% of the expected species richness were sampled. We suggest that the large richness recorded in this study is a consequence of the large sampling effort, the capture method, recent advances in the taxonomy of drosophilids, the high preservation level and the large extension of the sampled fragment and the high complexity of the Atlantic Rain forest. Finally, our data set suggest that the employment of estimators of richness for drosophilid assemblages is useful but it requires caution.

Os efeitos adversos dos metais pesados para as diversas formas de vida dificultam a recuperação de solos contaminados por estes elementos. Neste trabalho, avaliaram-se os efeitos da inoculação com fungos micorrízicos arbusculares no crescimento e absorção de metais de mudas de cinco espécies arbóreas, transplantadas para misturas que continham diferentes proporções de um solo contaminado (PSC). Mudas de Senna multijuga (L.C. Rich.) Irwin et Barneby (cássia verrugosa), Luehea grandiflora Mart. et Zucc. (açoita-cavalo), Enterolobium contortisiliquum (Vell.) Morong (tamboril), Albizia lebbeck (L.) Benth. (albizia) e Senna macranthera (Collard.) Irwin et Barneby (fedegoso), inoculadas e sem inoculação, foram transplantadas para as misturas de solos e desenvolvidas por 180 dias, no período de abril a novembro de 1996, em vasos, em casa de vegetação do Departamento de Ciência do Solo da UFLA, Lavras (MG). Verificou-se que a elevação na PSC na mistura reduziu o desenvolvimento das mudas e a colonização micorrízica (CM), sendo isto causado pela elevada absorção de metais pelas plantas, especiamente, de Cd e Zn. A inoculação favoreceu o crescimento das mudas após transplantio, sendo esse efeito mais evidente nas misturas de solo com baixa PSC. A CM foi reduzida de 70 a 90% no solo não contaminado para valores próximos de zero na mistura com alta PSC. Os níveis críticos de toxidez (redução de 10% na matéria seca das plantas inoculadas) dos metais no solo foram, em mg dm-3, de 83, 57, 153, 256 e 16, para o Zn, e de 1,3; 0,9; 0,8; 4,0 e 1,6, para Cd, para açoita-cavalo, cássia verrugosa, fedegoso, tamboril e albizia, respectivamente. Observando esses níveis críticos, as plantas não inoculadas apresentaram produção de matéria seca relativa, média para todas as espécies, de apenas 39%, evidenciando os benefícios da inoculação para o crescimento pós-transplantio das mudas. Esses benefícios relacionaram-se com menores teores de metais na parte aérea. Mesmo desconhecendo os mecanismos envolvidos nestas respostas, os resultados deste trabalho evidenciaram a importância das micorrizas arbusculares para o crescimento de mudas de árvores e para a recuperação de áreas tropicais contaminadas com metais pesados.

A possibilidade de utilização de normas obtidas em determinado local, para diferentes condições de clima e solo, pode conferir aos métodos de diagnose do estado nutricional um caráter de universalidade. Neste sentido, por meio da aplicação dos métodos: Sistema Integrado de Diagnose e Recomendação (DRIS), DRIS modificado (M-DRIS) e Diagnose da Composição Nutricional (CND) em plantios florestais de Eucalyptus grandis, objetivou-se comparar a diagnose, aplicando-se os métodos, mediante normas específicas ou gerais, em localidades da região Centro-Leste do Estado de Minas Gerais, utilizando diferentes critérios de avaliação. Foram utilizados dados de produtividade e de teores de N, P, K, Ca e Mg nas folhas, referentes a 993 talhões de Eucalyptus grandis plantados no espaçamento 3 x 2 m, com idades variando de 72 a 153 meses, e abrangendo seis localidades da região estudada. A universalidade das normas foi avaliada, comparando-se a freqüência de diagnoses concordantes (FDC) do potencial de resposta à adubação (PRA), obtida mediante normas específicas ou gerais, considerando os seguintes critérios de avaliação: os nutrientes separadamente; desde todos (5) até nenhum nutriente (0); e apenas para o limitante primário por deficiência (p) ou por excesso (n). O grau de universalidade das normas variou de acordo com o critério adotado para sua avaliação. Em virtude da ausência de universalidade das normas, é preferível a utilização de normas específicas em vez de normas gerais.

DRIS, an Diagnosis and Recommendation Integrated System, is a tool to evaluate the nutritional status of plants. Different DRIS formulas have been proposed to improve the efficiency of the crop nutrition diagnoses. The objective of this study was to compare the nutritional diagnosis of the formulas of Beaufils (1973), of Jones (1981) and of Elwali and Gascho (1984), based on the degree of agreement in commercial orchards of Theobrama grandiflorum trees. Leaf samples of 5 to 18 year-old cupuaçu trees were collected from 153 commercial orchards in agroforestry and monoculture systems in the state of Rondonia, Brazil. Bivariate relationships between nutrition concentrations in healthy trees were used to calculate DRIS norms. DRIS indices were calculated based on the different formulas and interpreted by the Potential Fertilizer Response method, in five categories. The DRIS norms, DRIS index calculations and their interpretations were developed using the DRIS Cupuaçu computer program (www.dris.com.br). The different DRIS formulas resulted in similar diagnoses with a degree of agreement of > 90% for the nutrients N, P, K, Ca, and Mg.

A produtividade dos pomares de cupuaçu na Amazônia tem experimentado lento declínio, provavelmente devido à diminuição da fertilidade do solo, requerendo adubações. Na ausência de recomendações técnicas para quantificar a demanda por nutrientes por essa cultura, o uso do Sistema Integrado de Diagnose e Recomendação (DRIS) pode representar uma alternativa para suprir essa necessidade. O objetivo deste trabalho foi avaliar os diagnósticos produzidos por diferentes fórmulas e critérios de interpretação dos índices DRIS em pomares de cupuaçu. Amostras foliares de 153 pomares foram analisadas para os nutrientes N, P, K, Ca, Mg, Zn, Mn, Cu e Fe e diagnosticadas por três fórmulas DRIS: Beaufils, Jones e Wadt e colaboradores. Na interpretação dos índices DRIS, utilizaram-se: o método DRIS Matéria Seca (M-DRIS), que separa os nutrientes em limitantes ou não limitantes, e o método do Potencial de Resposta à Adubação, o qual separa os nutrientes no estado nutricional de insuficientes, equilibrados ou em excesso. Foram também adotados níveis críticos para os teores dos nutrientes foliares em cupuaçueiros, oriundos da literatura. Os diagnósticos produzidos pelos diferentes critérios de interpretação dos índices DRIS não foram consistentes entre si ou em comparação com o método do nível crítico. A exceção foi a fórmula de Wadt e colaboradores que mostrou maior consistência entre os diagnósticos, independentemente do critério utilizado para sua interpretação. Os valores sugeridos para avaliação do estado nutricional pelo método do nível crítico não foram satisfatórios.

Soil information is needed for managing the agricultural environment. The aim of this study was to apply artificial neural networks (ANNs) for the prediction of soil classes using orbital remote sensing products, terrain attributes derived from a digital elevation model and local geology information as data sources. This approach to digital soil mapping was evaluated in an area with a high degree of lithologic diversity in the Serra do Mar. The neural network simulator used in this study was JavaNNS and the backpropagation learning algorithm. For soil class prediction, different combinations of the selected discriminant variables were tested: elevation, declivity, aspect, curvature, curvature plan, curvature profile, topographic index, solar radiation, LS topographic factor, local geology information, and clay mineral indices, iron oxides and the normalized difference vegetation index (NDVI) derived from an image of a Landsat-7 Enhanced Thematic Mapper Plus (ETM+) sensor. With the tested sets, best results were obtained when all discriminant variables were associated with geological information (overall accuracy 93.2 - 95.6 %, Kappa index 0.924 - 0.951, for set 13). Excluding the variable profile curvature (set 12), overall accuracy ranged from 93.9 to 95.4 % and the Kappa index from 0.932 to 0.948. The maps based on the neural network classifier were consistent and similar to conventional soil maps drawn for the study area, although with more spatial details. The results show the potential of ANNs for soil class prediction in mountainous areas with lithological diversity.
