OBJETIVO: avaliar as diferenças entre o resultado materno e perinatal de gestações complicadas pela pré-eclâmpsia, segundo classificação em sua forma grave/leve e de início precoce/tardio. MÉTODOS: estudo retrospectivo envolvendo 211 gestações complicadas pela pré-eclâmpsia, avaliadas em centro universitário de referência, no período de 2000 a 2010. O diagnóstico e a gravidade da doença foram baseados nos valores da pressão arterial, proteinúria e nos achados clínicos e laboratoriais. A idade da gestante, cor da pele, paridade, pressão arterial, valores de proteinúria semiquantitativa, presença de incisura bilateral em artérias uterinas à doplervelocimetria e as condições de nascimento foram comparados entre os casos de forma leve/grave, assim como entre aqueles de surgimento precoce/tardio. A doença foi considerada precoce quando diagnosticada antes da 34ª semana. RESULTADOS: a maioria das gestantes apresentava a forma grave da pré-eclâmpsia (82,8%) e 50,7%, de início precoce. Os valores da pressão arterial (133,6±14,8 versus 115,4 mmHg, p=0,0004, e 132,2±16,5 versus 125,7 mmHg, p=0,0004) e proteinúria semiquantitativa (p=0,0003 e p=0,0005) foram mais elevados nas formas grave e precoce em relação às formas leve e tardia. O peso ao nascimento (1.435,4±521,6 versus 2.710±605,0 g, 1.923,7±807,9 versus 2.415,0±925,0 g, p<0,0001 para ambos) e o índice de Apgar (p=0,01 para ambos) foram menores nas formas grave e precoce da pré-eclâmpsia, em relação às formas leve e tardia. Por outro lado, a presença de incisura bilateral em artérias uterinas se associou às formas de início precoce (69,2 versus 47,9%, p=0,02), enquanto a restrição de crescimento fetal foi mais frequente nas formas graves da pré-eclâmpsia (30 versus 4,4%, p=0,008). CONCLUSÃO: a classificação da pré-eclâmpsia baseada em parâmetros clínicos maternos refletiu melhor as condições de nutrição fetal, enquanto o seu surgimento precoce se associou melhor à vasculopatia placentária detectada à doplervelocimetria.

OBJETIVOS: Analisar o desfecho perinatal e pediátrico de fetos que apresentaram translucência nucal (TN) acima do percentil 95 (P95) e cariótipo normal a fim de obter dados que permitam um melhor aconselhamento materno pré-natal. METÓDOS: Análise de fetos em um serviço terciário de obstetrícia apresentaram TN acima do P95 e cariótipo normal entre os anos 2005 e 2011. Analisamos o seguimento ultrassonográfico gestacional, ecocardiografia (ECO) fetal e pós-natal, peso, comprimento e escore de Apgar ao nascimento, além do desenvolvimento neuropsicomotor por meio do Ages and Stages Questionnaire (ASQ) até julho de 2012. RESULTADOS: Durante esse período, foram 116 casos de translucência nucal acima do P95, sendo que em 79 (68%) foi realizado cariótipo fetal. Das análises, 43 foram normais (54,4%) e 36, alteradas (45,6%). Nos fetos com cariótipo normal, houve um abortamento na 15ª semana gestacional com pentalogia de Cantrel, um óbito na 24ª semana com diversas anomalias estruturais, um óbito neonatal sem causa definida e dois casos de comunicação intraventricular (CIV) detectados no ECO fetal. Na avaliação ecocardiográfica pós-natal, persistiu um caso de CIV e foi diagnosticado um caso de comunicação interatrial (CIA) e persistência do canal arterial (PCA). Entre os 40 casos sobreviventes, apenas 1 criança apresentou atraso no desenvolvimento da fala e outra apresentou quadro de autismo. Os demais casos resultaram em desenvolvimento neuropsicomotor normal. CONCLUSÃO: No acompanhamento dos fetos com TN aumentada e cariótipo normal, os pais podem ser mais bem aconselhados de que, frente a um exame morfológico-ecocardiográfico do 2º trimestre sem alterações, a probabilidade de a criança nascer viva e bem é alta (93,5%).

PURPOSE: To evaluate the obstetric and perinatal outcomes evolution of triplet pregnancies.METHODS: A prospective observational study was conducted in triplet pregnancies delivered over 16 years in a tertiary obstetric center with differentiated perinatal support. Evaluation of demographic factors, obstetric complications, gestational age at delivery, mode of delivery, birth weight and immediate newborn outcome were done over a 16 years period. A global characterization of the sample was performed considering the listed parameters. Variables were categorized in three groups according to year of occurrence: 1996-2000, 2001-2006, 2007-2011, and all parameters were compared.RESULTS: Of the 33 triplets included, 72.7% resulted from induced pregnancies. All except one patient received prenatal corticosteroids and five received tocolytics. All women delivered prenatally and no significant differences were seen in the mean gestational age at delivery or birth weight towards time. There were three intrauterine fetal deaths. Neonatal immediate outcomes were not significantly different over the years.CONCLUSION: Despite remarkable progresses in perinatal and neonatal cares, no noticeable impact in triplet gestations' outcomes was seen, sustaining that triplets should be avoided due to their great risk of prematurity and neonatal morbidities, either by limiting the numbers of embryos transferred or by fetal reduction.

OBJETIVO: Identificar os fatores obstétricos e perinatais associados à morbimortalidade perinatal em gestações que cursaram com amniorrexe prematura.MÉTODOS: Estudo transversal de base hospitalar, com dados secundários de prontuários de pacientes (n=87) que evoluíram com quadro de amniorrexe prematura com idade gestacional entre 24 e 42 semanas, definida pela ultrassonografia, e internadas no período de janeiro a abril de 2013 em uma maternidade pública no estado do Acre, região Norte do Brasil. Os dados foram submetidos à análise bivariada para seleção de variáveis que compuseram o modelo múltiplo utilizando a técnica de regressão logística de Poisson. RESULTADOS: A prevalência de morbimortalidade perinatal foi de 51,4%. Nesse total estão computados 2,3% de óbitos fetais (2 casos) e 9,2% de óbitos neonatais (8 casos). As variáveis que apresentaram associação no modelo múltiplo final com morbimortalidade foram: número de consultas de pré-natal ≥6, com razão de prevalência (RP) 0,5 e intervalo de confiança de 95% (IC95%) 0,3-0,9, idade gestacional ≥30 semanas (RP=0,6; IC95% 0,4-0,8), baixo peso ao nascer (RP=2,9; IC95% 1,5-5,4) e necessidade de ventilação mecânica (RP=3,8; IC95% 2,0-7,2).CONCLUSÃO: Observou-se elevada morbimortalidade perinatal entre casos que cursaram com amniorrexe prematura. A morbimortalidade esteve associada a fatores como menor número de consultas de pré-natal, extrema prematuridade e o baixo peso.

PURPOSE:To verify the existence of associations between different maternal ages and the perinatal outcomes of preterm birth and intrauterine growth restriction in the city of São Luís, Maranhão, Northeastern Brazil.METHODS:A cross-sectional study using a sample of 5,063 hospital births was conducted in São Luís, from January to December 2010. The participants comprise the birth cohort for the study "Etiological factors of preterm birth and consequences of perinatal factors for infant health: birth cohorts from two Brazilian cities" (BRISA). Frequencies and 95% confidence intervals were used to describe the results. Multiple logistic regression models were applied to assess the adjusted odds ratio (OR) of maternal age associated with the following outcomes: preterm birth and intrauterine growth restriction.RESULTS:The percentage of early teenage pregnancy (12–15 years old) was 2.2%, and of late (16–19 years old) was 16.4%, while pregnancy at an advanced maternal age (>35 years) was 5.9%. Multivariate analyses showed a statistically significant increase in preterm births among females aged 12–15 years old (OR=1.6; p=0.04) compared with those aged 20–35 years. There was also a higher rate in preterm births among females aged 16–19 years old (OR=1.3; p=0.01). Among those with advanced maternal age (>35 years old), the increase in the prevalence of preterm birth had only borderline statistical significance (OR=1.4; p=0.05). There was no statistically significant association between maternal age and increased prevalence of intrauterine growth restriction.

As causas de mortalidade perinatal em ovinos foram estudadas de março de 2002 a outubro 2004 em 27 fazendas da região semi-árida da Paraíba. De 90 cordeiros necropsiados, 41,1% morreram de infecções neonatais, 23,3% por malformações, 10% por inanição/hipotermia, 10% por distocia, 2,2% por predação e 4,4% foram abortos sem causa identificada. Em relação ao momento da morte, 4,4% dos cordeiros morreram antes do parto, 10% durante o parto, 30% no primeiro dia de vida, 20% entre o 2º e 5º dia e 35,6% entre o 4º e 28º dia após o parto. A assistência das ovelhas durante o parto, a desinfecção do umbigo dos cordeiros, a ingestão de colostro 2 a 6 horas após o parto, e a manutenção das ovelhas em locais adequados durante e após o parto contribuiriam para diminuir as mortes perinatais por distocia e infecções neonatais. A alta freqüência de malformações, em diferentes raças, sugere que esses defeitos sejam causados por uma planta tóxica. Os principais defeitos observados foram a flexão permanente dos membros anteriores, braquignatismo, fenda palatina e outras alterações dos ossos da cabeça. Recentemente foi demonstrado o efeito teratogênico de Mimosa tenuiflora ("jurema-preta"), uma planta muito comum na região semi-árida, nas áreas de caatinga, que aparentemente é responsável pelas malformações. Os cordeiros mortos por inanição/hipotermia tiveram baixo peso ao nascimento (1,37 ± 0,7kg) o que sugere que a principal causa dessas mortes é a deficiente nutrição da mãe durante o último terço da gestação. Considerando-se que na região nordeste, na maioria das fazendas, os carneiros permanecem com as ovelhas durante todo o ano, a adoção de uma estação de monta definida contribuiria para a diminuição da mortalidade perinatal.

As causas de mortalidade perinatal em cabritos foram estudadas de maio de 2002 a agosto de 2004. Em 118 cabritos necropsiados as causas de morte foram: infecção neonatal (50%), distocia (12,71%), complexo hipotermia/inanição (11,86%), malformações (7,62%), síndrome do cabrito mole (6,77%) e abortos (1,69%). Com relação ao momento da morte 1,69% dos cabritos morreram antes do parto, 16,94% durante o parto e 81,34 % após o nascimento. A alta ocorrência de infecções neonatais, distocias e hipotermia/inanição é provavelmente devido a fatores relacionados com erros no manejo sanitário, reprodutivo e nutricional. Artogripose dos membros anteriores foi a principal malformação observada. Este defeito é endêmico em rebanhos de caprinos no semi-árido do Brasil. A maioria das mortes ocorreu após o nascimento (25,42%) e do quarto ao vigésimo dia de vida (38,98%) sugerindo que o cuidado com os cabritos durante os primeiros 28 dias de vida é importante para melhorar as taxas de sobrevivência dos mesmos.

Experiments were designed to study in-vivo effects of sodium cyanide on biochemical endpoints in the freshwater fish Labeo rohita. Fish were exposed to two sublethal concentrations (0.106 and 0.064mg/L) for a period of 15 days. Levels of glycogen, pyruvate, lactate and the enzymatic activities of lactate dehydrogenase (LDH), succinate dehydrogenase (SDH), glucose-6-phosphate dehydrogenase (G6PDH), phosphorylase, alkaline phosphatase (ALP), acid phosphatase (AcP) were assessed in different tissues (liver, muscle and gills). Result indicated a steady decrease in glycogen, pyruvate, SDH, ALP and AcP activity with a concomitant increase in the lactate, phosphorylase, LDH and G6PD activity in all selected tissues. The alterations in all the above biochemical parameters were significantly (p<0.05) time and dose dependent. In all the above parameters, liver pointing out the intensity of cyanide intoxication compare to muscle and gills. Study revealed change in the metabolic energy by means of altered metabolic profile of the fish. Further, these observations indicated that even sublethal concentrations of sodium cyanide might not be fully devoid of deleterious influence on metabolism in L. rohita.

The effects of postnatal amitraz exposure on physical and behavioral parameters were studied in Wistar rats, whose lactating dams received the pesticide (10 mg/kg) orally on days 1, 4, 7, 10, 13, 16 and 19 of lactation; control dams received distilled water (1 ml/kg) on the same days. A total of 18 different litters (9 of them control and 9 experimental) born after a 21-day gestation were used. The results showed that the median effective time (ET50) for fur development, eye opening, testis descent and onset of the startle response were increased in rats postnatally exposed to amitraz (2.7, 15.1, 21.6 and 15.3 days, respectively) compared to those of the control pups (1.8, 14.0, 19.9 and 12.9 days, respectively). The ages of incisor eruption, total unfolding of the external ears, vaginal and ear opening and the time taken to perform the grasping hindlimb reflex were not affected by amitraz exposure. Pups from dams treated with amitraz during lactation took more time (in seconds) to perform the surface righting reflex on postnatal days (PND) 3 (25.0 ± 2.0), 4 (12.3 ± 1.2) and 5 (8.7 ± 0.9) in relation to controls (10.6 ± 1.2; 4.5 ± 0.6 and 3.4 ± 0.4, respectively); the climbing response was not changed by amitraz. Postnatal amitraz exposure increased spontaneous motor activity of male and female pups in the open-field on PND 16 (140 ± 11) and 17 (124 ± 12), and 16 (104 ± 9), 17 (137 ± 9) and 18 (106 ± 8), respectively. Data on spontaneous motor activity of the control male and female pups were 59 ± 11 and 69 ± 10 for days 16 and 17 and 49 ± 9, 48 ± 7 and 56 ± 7 for days 16, 17 and 18, respectively. Some qualitative differences were also observed in spontaneous motor behavior; thus, raising the head, shoulder and pelvis matured one or two days later in the amitraz-treated offspring. Postnatal amitraz exposure did not change locomotion and rearing frequencies or immobility time in the open-field on PND 30, 60 and 90. The present findings indicate that postnatal exposure to amitraz caused transient developmental and behavioral changes in the exposed offspring and suggest that further investigation of the potential health risk of amitraz exposure to developing human and animal offsprings may be warranted.

The human anti-rabies pre-exposure treatment currently used in Brazil, employing a 1-ml dose of suckling mouse brain vaccine (SMBV) administered on days 0, 2, 4 and 28, was compared to an alternative treatment with two 1 ml-doses on day 0, and one 1 ml-dose injected on days 7 and 21. The latter induced higher virus-neutralizing antibody (VNA) titers on day 21. Both Brazilian rabies vaccines produced with PV or CVS rabies virus strains were tested. Two additional volunteer vaccinee groups, receiving the pre-exposure and the abbreviated post-exposure schedules recommended by the WHO using cell-culture vaccine (CCV) produced with PM rabies virus strain, were included as reference. The VNA were measured against both PV and CVS strains on days 21, 42 and 180 by the cell-culture neutralization microtest. The PV-SMBV elicited higher seroconversion rates and VNA by day 21 than the CVS-SMBV. Both, however, failed to induce a long-term immunity, since VNA titers were <0.5 IU/ml on day 180, regardless of the schedule used. Cell-culture vaccine always elicited very high VNA on all days of collection. When serum samples from people receiving mouse brain tissue were titrated against the PV and CVS strains, the VNA obtained were similar, regardless of the vaccinal strain and the virus used in the neutralization test. These results contrast with those obtained with sera from people receiving PM-CCV, whose VNA were significantly higher when tested against the CVS strain.

Hepatitis C virus (HCV) infection is widespread and responsible for more than 60% of chronic hepatitis cases. HCV presents a genetic variability which has led to viral classification into at least 6 genotypes and a series of subtypes. These variants present characteristic geographical distribution, but their association with different responses to treatment with interferon and severity of disease still remains controversial. The aim of this study was to investigate the patterns of distribution of HCV genotypes among different exposure categories in Brazil. Two hundred and fifty anti-HCV positive samples were submitted to HCV-RNA detection by RT-PCR and their genotype was determined by restriction fragment length polymorphism (RFLP) analysis. In addition, the genotype/subtype of 60 samples was also determined by a reverse hybridization assay. HCV 1 was the most prevalent (72.0%), followed by type 3 (25.3%), HCV 2 (2.0%) and HCV 4 (0.7%). The HCV genotype distribution varied among the different exposure categories, with HCV 1 being more frequent among blood donors, hemophiliacs and hemodialysis patients. A high frequency of HCV 3 was observed in cirrhotic patients, blood donors from the South of Brazil and injecting drug users (IDUs). The general distribution of the HCV genotype in Brazil is similar to that in other regions of the world.

Early stimulation has been shown to produce long-lasting effects in many species. Prenatal exposure to some strong stressors may affect development of the nervous system leading to behavioral impairment in adult life. The purpose of the present work was to study the postnatal harmful effects of exposure to variable mild stresses in rats during pregnancy. Female Holtzman rats were submitted daily to one session of a chronic variable stress (CVS) during pregnancy (prenatal stress; PS group). Control pregnant rats (C group) were undisturbed. The pups of PS and C dams were weighed and separated into two groups 48 h after delivery. One group was maintained with their own dams (PS group, N = 70; C group, N = 36) while the other PS pups were cross-fostered with C dams (PSF group, N = 47) and the other C pups were cross-fostered with PS dams (CF group, N = 58). Pups were undisturbed until weaning (postnatal day 28). The male offspring underwent motor activity tests (day 28), enriched environment tests (day 37) and social interaction tests (day 42) in an animal activity monitor. Body weight was recorded on days 2, 28 and 60. The PS pups showed lower birth weight than C pups (Duncan's test, P<0.05). The PS pups suckling with their stressed mothers displayed greater preweaning mortality (C: 23%, PS: 60%; c2 test, P<0.05) and lower body weight than controls at days 28 and 60 (Duncan's test, P<0.05 and P<0.01, respectively). The PS, PSF and CF groups showed lower motor activity scores than controls when tested at day 28 (Duncan's test, P<0.01 for PS group and P<0.05 for CF and PSF groups). In the enriched environment test performed on day 37, between-group differences in total motor activity were not detected; however, the PS, CF and PSF groups displayed less exploration time than controls (Duncan's test, P<0.05). Only the PS group showed impaired motor activity and impaired social behavior at day 42 (Duncan's test, P<0.05). In fact, CVS treatment during gestation plus suckling with a previously stressed mother caused long-lasting physical and behavioral changes in rats. Cross-fostering PS-exposed pups to a dam which was not submitted to stress counteracted most of the harmful effects of the treatment. It is probable that prenatal stress plus suckling from a previously stressed mother can induce long-lasting changes in the neurotransmitter systems involved in emotional regulation. Further experiments using neurochemical and pharmacological approaches would be interesting in this model.

The objective of the present study was to determine if exposure of cervical mucus to air during specular examination could modify mucus pH. Detection of changes is justified because of their possible interference with sperm-mucus interaction, since an acidic pH is unfavorable to sperm penetration and is associated with infertility due to the cervical factor. Twenty women with good quality mucus were evaluated. pH measurements of ecto- and endocervical mucus were made in situ using a glass electrode after 0-, 5- and 10-min exposure to air. There was a progressive alkalinization of mucus pH. Mean values of ectocervical mucus pH were 6.91, 7.16 and 7.27, while mean values of endocervical mucus pH were 7.09, 7.34 and 7.46 at 0, 5 and 10 min, respectively. Significant differences were found between the mean values obtained at 0 and 5 min, and at 0 and 10 min (P<0.05), whereas the differences in mean values at 5 and 10 min were not significant at either site. We conclude that 5 to 10 min of exposure to atmospheric air affects cervical mucus pH in a significant way. Since tests used to evaluate sperm-mucus interaction generally have not considered this possibility, we suggest that they should be performed immediately after mucus collection in order to avoid misinterpretation of the results.
