OBJETIVO: Identificar indicadores da institucionalização de idosos. MÉTODOS: Estudo de casos e controles com 991 idosos em Pelotas, RS, de 2007 a 2008. Os casos (idosos institucionalizados; n = 393) foram detectados por meio de um censo em todas as instituições de longa permanência para idosos da cidade. Os controles populacionais (n = 598) foram selecionados de forma aleatória, por meio de um amplo inquérito de saúde. Na comparação dos grupos, foram empregados os testes qui-quadrado de Pearson e tendência linear na análise bruta e o modelo de regressão logística binária na análise ajustada, com medidas de efeito expressas em odds ratio. RESULTADOS: A institucionalização foi mais freqüente no sexo feminino (OR = 1,96; IC95% 1,31;2,95). Idosos com idade avançada (OR = 3,23 e OR = 9,56 para faixas etárias de 70-79 e > 80 anos, respectivamente), que viviam sem companheiro (solteiros, separados e viúvos), não possuíam escolaridade formal e apresentavam incapacidade funcional para atividades básicas da vida diária apresentaram maior probabilidade de institucionalização. Observou-se tendência inversa entre a ocorrência de institucionalização do idoso e o nível de atividade física, em que sujeitos pouco ativos e inativos apresentaram maiores probabilidades de institucionalização (OR = 1,71 e OR = 4,73, respectivamente). CONCLUSÕES: Dentre todas as características investigadas nos idosos, idade > 80 anos, viver sem companheiro e ser fisicamente inativo foram os indicadores mais fortemente associados à ocorrência de institucionalização. O incentivo ao cuidado informal, a partir de ações educativas e culturais focadas no papel da família para o idoso, pode impedir a institucionalização desses indivíduos.

OBJECTIVE: To assess the association between exposure to adverse psychosocial working conditions and poor self-rated health among bank employees. METHODS: A cross-sectional study including a sample of 2,054 employees of a government bank was conducted in 2008. Self-rated health was assessed by a single question: "In general, would you say your health is (...)." Exposure to adverse psychosocial working conditions was evaluated by the effort-reward imbalance model and the demand-control model. Information on other independent variables was obtained through a self-administered semi-structured questionnaire. A multiple logistic regression analysis was performed and odds ratio calculated to assess independent associations between adverse psychosocial working conditions and poor self-rated health. RESULTS: The overall prevalence of poor self-rated health was 9%, with no significant gender difference. Exposure to high demand and low control environment at work was associated with poor self-rated health. Employees with high effort-reward imbalance and overcommitment also reported poor self-rated health, with a dose-response relationship. Social support at work was inversely related to poor self-rated health, with a dose-response relationship. CONCLUSIONS: Exposure to adverse psychosocial work factors assessed based on the effort-reward imbalance model and the demand-control model is independently associated with poor self-rated health among the workers studied.

OBJETIVO: Estimar a prevalência e identificar fatores associados ao acúmulo de comportamentos de risco para doenças cardiovasculares entre adultos. MÉTODOS: Estudo transversal de base populacional com amostra representativa de 2.732 adultos de ambos os sexos de Pelotas, RS, em 2010. Os fatores de risco comportamentais investigados foram: tabagismo; inatividade física no lazer; consumo habitual de gordura aparente da carne; e consumo diário de embutidos, carne vermelha e leite integral. O desfecho do estudo foi o escore de aglomeração de fatores de risco comportamentais, variando de zero a três: nenhum fator de risco comportamental para doenças cardiovasculares ou exposição a 1, 2 ou > 3 fatores de risco comportamentais. Realizou-se regressão logística multinomial para avaliar o efeito ajustado das características individuais sobre o acúmulo de fatores de risco comportamentais, tendo como categoria de referência indivíduos sem qualquer dos fatores. RESULTADOS: A inatividade física foi o fator de risco mais prevalente (75,6%), seguido do consumo habitual de gordura aparente da carne (52,3%). Dois terços da população apresentaram dois ou mais fatores de risco comportamentais. A combinação de inatividade física e consumo habitual de gordura aparente da carne ocorreu em 17,5% da amostra; e inatividade física, consumo habitual de gordura aparente da carne e tabagismo, em 6,7%. Os odds ratios de acúmulo de dois ou mais fatores foram maiores entre homens e associaram-se inversamente com o indicador econômico nacional. CONCLUSÕES: O acúmulo de fatores de risco comportamentais para doenças cardiovasculares é elevado na população estudada. São necessárias intervenções públicas capazes de prevenir a ocorrência simultânea desses fatores.

OBJETIVO: Evaluar la asociación entre frecuencia de ver televisión, sobrepeso y obesidad en una muestra representativa de mujeres peruanas a nivel nacional. MÉTODOS: Análisis secundario de la Encuesta Nacional Demográfica y de Salud Familiar incluyendo mujeres de 15 a 49 años. Las variables resultados fueron obesidad (índice de masa corporal >30 kg/m²) y sobrepeso (>25 peso <30 kg/m²); mientras que la exposición fue la frecuencia de ver televisión (nunca, ocasionalmente, casi todos los días). Se usó regresión logística para análisis de muestras complejas según el diseño de la encuesta ajustando por potenciales confusores. Los resultados fueron presentados como Odds Ratio ajustados (ORa) con sus respectivos intervalos de confianza al 95% (IC95%). RESULTADOS: Un total de 21.712 mujeres fueron incluidas en el análisis. La prevalencia de sobrepeso fue 34,7% (IC95% 33,8;35,7) y de obesidad fue 14,3% (IC95% 13,6;15,1). Las mujeres que veían televisión ocasionalmente y casi todos los días tuvieron mayor probabilidad de tener obesidad: ORa 1,7 (IC95% 1,3;2,3) y ORa 2,6 (IC95% 2,0;3,5), respectivamente comparado con aquellas que nunca veían televisión. La magnitud de la asociación fue menor para el caso de sobrepeso: ORa 1,2 (IC95% 1,3;2,3) y ORa 1,6 (IC95% 1,1;1,4), respectivamente. La fuerza de la asociación fue mayor en el área urbana. CONCLUSIONES: En mujeres peruanas la frecuencia de ver televisión estuvo asociada con presentar obesidad y sobrepeso, y la fuerza de asociación varió de acuerdo al área de residencia. Estos hallazgos deberían orientar las estrategias de prevención de la obesidad en el contexto peruano.

OBJETIVO: To assess factors associated with a low risk perception of zoonoses and to identify the gaps in knowledge about transmission and prevention of zoonoses in immigrant and Italian workers. MÉTODOS: A cross-sectional study with 175 workers in the agro-livestock and agro-food industry in Piemonte, Italy, was carried out. Data were collected with a semi-structured questionnaire based on knowledge, attitudes and practices (KAP) survey. We calculated proportions and used chi-square tests and odds ratios to assess associations. Eight individual interviews with key informants on immigration and public health in Piemonte were carried out. RESULTADOS: Participants were 82 (47%) Italians and 93 (53%) immigrants. Immigrants were from Romania, Morocco, Albania, India, China, Argentina, Peru, Macedonia, Ivory Coast, Ukraine and Colombia. The study revealed significant differences in risk perception at work (p = 0.001). We found associations between "not having correct knowledge about zoonoses" and the following variables: i. "being immigrant" OR = 4.1 (95%CI 1.7;9.8 p ≤ 0.01); ii. "working in the livestock industry" OR = 2.9 (95%CI 1.2;15.4 p = 0.01); and iii. "being an unqualified worker" OR = 4.4 (95%CI 2.9;15.4 p ≤ 0.01). Another strong association was found between being immigrant and having a low job qualification OR = 6.7 (IC95% 2.9 - 15.4 p ≤ 0.01). Asian immigrants were the group with the highest frequency of risky behaviours and the lowest level of knowledge about zoonoses. CONCLUSÕES: Our results indicate that there were differences in risk perception of zoonoses between the groups participating in our study. These results suggest that immigrant status can be considered a risk factor for having lower risk perception and lower level of knowledge of zoonoses at work. There is a relationship between this specific knowledge of zoonoses and lack of training and instruction among migrant populations. Our results stress the need for developing education programs on zoonoses prevention among the immigrant population in Piemonte, Italy.

OBJETIVO: Analisar o aumento do número de autores por artigo em revistas científicas brasileiras de saúde coletiva. MÉTODOS: Foram pesquisados na base de dados LILACS artigos publicados em seis revistas de saúde coletiva e uma revista médica (para comparação), da coleção SciELO, com classificação Qualis, da Capes, igual ou superior a B-1, entre 1999 e 2010. Foram avaliadas a evolução da mediana de números de autores/artigo e a proporção de artigos com mais de quatro autores. Estimou-se a associação entre o triênio de publicação e a presença de quatro ou mais autores por artigo por meio de odds ratio de Mantel-Haenzel, ajustadas para o tipo de revista. RESULTADOS: Houve crescimento da mediana do número de autores e da proporção de artigos com mais de quatro autores para todas as revistas, principalmente no último triênio. As odds ratio para publicação de artigos com quatro autores ou mais, ajustadas para os tipo de revista, foram: segundo triênio: 1,3 (IC95% 1,1;1,4); terceiro triênio: 1,5 (IC95% 1,3;1,8); quarto triênio: 2,39 (IC95% 2,1;2,8). CONCLUSÕES: Periódicos científicos de saúde coletiva têm apresentado aumento no número de autores por artigo ao longo dos anos, independentemente da orientação editorial.

OBJECTIVE: To analyze the strengths and limitations of the Family Health Strategy from the perspective of health care professionals and the community. METHODS: Between June-August 2009, in the city of Vespasiano, Minas Gerais State, Southeastern Brazil, a questionnaire was used to evaluate the Family Health Strategy (ESF) with 77 healthcare professionals and 293 caregivers of children under five. Health care professional training, community access to health care, communication with patients and delivery of health education and pediatric care were the main points of interest in the evaluation. Logistic regression analysis was used to obtain odds ratios and 95% confidence intervals as well as to assess the statistical significance of the variables studied. RESULTS: The majority of health care professionals reported their program training was insufficient in quantity, content and method of delivery. Caregivers and professionals identified similar weaknesses (services not accessible to the community, lack of healthcare professionals, poor training for professionals) and strengths (community health worker-patient communications, provision of educational information, and pediatric care). Recommendations for improvement included: more doctors and specialists, more and better training, and scheduling improvements. Caregiver satisfaction with the ESF was found to be related to perceived benefits such as community health agent household visits (OR 5.8, 95%CI 2.8;12.1), good professional-patient relationships (OR 4.8, 95%CI 2.5;9.3), and family-focused health (OR 4.1, 95%CI 1.6;10.2); and perceived problems such as lack of personnel (OR 0.3, 95%CI 0.2;0.6), difficulty with access (OR 0.2, 95%CI 0.1;0.4), and poor quality of care (OR 0.3, 95%CI 0.1;0.6). Overall, 62% of caregivers reported being generally satisfied with the ESF services. CONCLUSIONS: Identifying the limitations and strengths of the Family Health Strategy from the healthcare professional and caregiver perspective may serve to advance primary community healthcare in Brazil.

OBJETIVO Analisar características sociodemográficas e clínico-epidemiológicas dos casos de tuberculose e fatores associados ao abandono e ao óbito na vigência do tratamento. MÉTODOS Estudo epidemiológico baseado em dados notificados de tuberculose em indígenas e não indígenas, segundo raça/cor, em Mato Grosso do Sul, entre 2001 e 2009. Realizou-se análise descritiva dos casos de acordo com as variáveis sexo, faixa etária, zona de residência, exames empregados para o diagnóstico, forma clínica, tratamento supervisionado e situação de encerramento, segundo raça/cor. Utilizou-se análise univariada e múltipla por meio de regressão logística para identificar preditores de abandono e óbito, e odds ratio como medida de associação. Foi construída série histórica de incidência, segundo raça/cor. RESULTADOS Registraram-se 6.962 casos novos de tuberculose no período, 15,6% entre indígenas. Houve predomínio em homens e adultos (20 a 44 anos) em todos os grupos. A maior parte dos doentes indígenas residia na zona rural (79,8%) e 13,5% dos registros nos indígenas ocorreram em < 10 anos. A incidência média no estado foi 34,5/100.000 habitantes, 209,0; 73,1; 52,7; 23,0 e 22,4 entre indígenas, amarelos, pretos, brancos e pardos, respectivamente. Doentes de 20 a 44 anos (OR = 13,3; IC95% 1,9;96,8), do sexo masculino (OR = 1,6; IC95% 1,1;2,3) e de raça/cor preta (OR = 2,5; IC95% 1,0;6,3) mostraram associação com abandono de tratamento, enquanto doentes > 45 anos (OR = 3,0; IC95% 1,2;7,8) e com a forma mista (OR = 2,3; IC95% 1,1;5,0) apresentaram associação com óbito. Apesar de representarem 3,0% da população, os indígenas foram responsáveis por 15,6% das notificações no período. CONCLUSÕES Houve importantes desigualdades em relação ao adoecimento por tuberculose entre as categorias estudadas. As incidências nos indígenas foram consistentemente maiores, chegando a exceder em mais de seis vezes as médias nacionais. Entre pretos e pardos, piores resultados no tratamento foram observados, pois apresentaram chance de abandono duas vezes maior que os indígenas. O mau desempenho do programa também esteve fortemente associado ao abandono e ao óbito. Acredita-se que, enquanto não se reduzir a pobreza, as desigualdades nos indicadores em saúde permanecerão.

OBJETIVO: Estimar a prevalência e a distribuição geográfica da doença periodontal na população adulta brasileira e sua associação com desigualdades sociais contextuais e individuais. MÉTODOS: Foram utilizados dados dos adultos de 35 a 44 anos de idade da Pesquisa Nacional de Saúde Bucal – SBBrasil 2010 (n = 9.564). O Índice Periodontal Comunitário (CPI) e o Índice de Perda Periodontal (PIP) foram usados para definir a doença periodontal em “moderada a grave” (CPI > 2 e PIP > 0) e “grave” (CPI > 2 e PIP > 1). As desigualdades sociais contextuais incluíram o índice de desenvolvimento humano e a desigualdade de renda (Índice de Gini). Outras variáveis contextuais foram a cobertura de equipes de saúde bucal da Estratégia de Saúde da Família e o percentual de adultos fumantes. Modelos de regressão logística multinível para os participantes com dados completos (n = 4.594) foram usados para estimar odds ratios (OR) e intervalos de 95% de confiança (IC95%) entre desigualdades sociais e doença periodontal. RESULTADOS: A prevalência da doença periodontal “moderada a grave” em brasileiros adultos foi de 15,3% e 5,8% para a condição “grave”, com variações consideráveis entre os municípios. Dentre as variáveis contextuais, a desigualdade de renda foi independentemente associada com a doença periodontal “grave” (OR = 3,0; IC95% 1,5;5,9). A menor cobertura de equipes de saúde bucal foi associada com as duas formas de doença periodontal, enquanto o percentual de fumantes manteve-se associado com a doença periodontal “moderada a grave”. Adultos com idade mais avançada, de cor de pele parda, sexo masculino, menor renda familiar e menor escolaridade apresentaram maiores chances para ambas as condições periodontais investigadas. CONCLUSÕES: No Brasil, a prevalência da doença periodontal variou conforme o município e a definição de doença empregada. A desigualdade de renda teve um papel importante na ocorrência da doença periodontal “grave”. Características individuais de posição social foram associadas com as duas formas de doença periodontal.

OBJECTIVE : To investigate the association between common mental disorders and intimate partner violence during pregnancy. METHODS : A cross sectional study was carried out with 1,120 pregnant women aged 18-49 years old, who were registered in the Family Health Program in the city of Recife, Northeastern Brazil, between 2005 and 2006. Common mental disorders were assessed using the Self-Reporting Questionnaire (SRQ-20). Intimate partner violence was defined as psychologically, physically and sexually abusive acts committed against women by their partners. Crude and adjusted odds ratios were estimated for the association studied utilizing logistic regression analysis. RESULTS : The most common form of partner violence was psychological. The prevalence of common mental disorders was 71.0% among women who reported all form of violence in pregnancy and 33.8% among those who did not report intimate partner violence. Common mental disorders were associated with psychological violence (OR 2.49, 95%CI 1.8;3.5), even without physical or sexual violence. When psychological violence was combined with physical or sexual violence, the risk of common mental disorders was even higher (OR 3.45; 95%CI 2.3;5.2). CONCLUSIONS : Being assaulted by someone with whom you are emotionally involved can trigger feelings of helplessness, low self-esteem and depression. The pregnancy probably increased women`s vulnerability to common mental disorders

OBJECTIVE To describe the health status and access to care of forced-return Mexican migrants deported through the Mexico-United States border and to compare it with the situation of voluntary-return migrants. METHODS Secondary data analysis from the Survey on Migration in Mexico’s Northern Border from 2012. This is a continuous survey, designed to describe migration flows between Mexico and the United States, with a mobile-population sampling design. We analyzed indicators of health and access to care among deported migrants, and compare them with voluntary-return migrants. Our analysis sample included 2,680 voluntary-return migrants, and 6,862 deportees. We employ an ordinal multiple logistic regression model, to compare the adjusted odds of having worst self-reported health between the studied groups. RESULTS As compared to voluntary-return migrants, deportees were less likely to have medical insurance in the United States (OR = 0.05; 95%CI 0.04;0.06). In the regression model a poorer self-perceived health was found to be associated with having been deported (OR = 1.71, 95%CI 1.52;1.92), as well as age (OR = 1.03, 95%CI 1.02;1.03) and years of education (OR = 0.94 95%CI 0.93;0.95). CONCLUSIONS According to our results, deportees had less access to care while in the United States, as compared with voluntary-return migrants. Our results also showed an independent and statistically significant association between deportation and having poorer self-perceived health. To promote the health and access to care of deported Mexican migrants coming back from the United States, new health and social policies are required.

OBJECTIVE To analyze the factors associated with a lack of prenatal care in a large municipality in southern Brazil. METHODS In this case-control age-matched study, 716 women were evaluated; of these, 179 did not receive prenatal care and 537 received prenatal care (controls). These women were identified using the Sistema Nacional de Informação sobre Nascidos Vivos (Live Birth Information System) of Pelotas, RS, Southern Brazil, between 2009 and 2010. Multivariate analysis was performed using conditional logistic regression to estimate the odds ratios (OR). RESULTS In the final model, the variables associated with a lack of prenatal care were the level of education, particularly when it was lesser than four years [OR 4.46; 95% confidence interval (CI) 1.92;10.36], being single (OR 3.61; 95%CI 1.85;7.04), and multiparity (OR 2.89; 95%CI 1.72;4.85). The prevalence of a lack of prenatal care among administrative regions varied between 0.7% and 3.9%. CONCLUSIONS The risk factors identified must be considered when planning actions for the inclusion of women in prenatal care by both the central management and healthcare teams. These indicated the municipal areas with greater deficits in prenatal care. The reorganization of the actions to identify women with risk factors in the community can be considered to be a starting point of this process. In addition, the integration of the activities of local programs that target the mother and child is essential to constantly identify pregnant women without prenatal care.

OBJECTIVE To assess the prevalence and factors associated with intimate partner violence after the diagnosis of sexually transmitted diseases.METHODS This cross-sectional study was conducted in Fortaleza, CE, Northeastern Brazil, in 2012 and involved 221 individuals (40.3% male and 59.7% female) attended to at reference health care units for the treatment of sexually transmitted diseases. Data were collected using a questionnaire applied during interviews with each participant. A multivariate analysis with a logistic regression model was conducted using the stepwise technique. Only the variables with a p value < 0.05 were included in the adjusted analysis. The odds ratio (OR) with 95% confidence interval (CI) was used as the measure of effect.RESULTS A total of 30.3% of the participants reported experiencing some type of violence (27.6%, psychological; 5.9%, physical; and 7.2%, sexual) after the diagnosis of sexually transmitted disease. In the multivariate analysis adjusted to assess intimate partner violence after the revelation of the diagnosis of sexually transmitted diseases, the following variables remained statistically significant: extramarital relations (OR = 3.72; 95%CI 1.91;7.26; p = 0.000), alcohol consumption by the partner (OR = 2.16; 95%CI 1.08;4.33; p = 0.026), history of violence prior to diagnosis (OR = 2.87; 95%CI 1.44;5.69; p = 0.003), and fear of disclosing the diagnosis to the partner (OR = 2.66; 95%CI 1.32;5.32; p = 0.006).CONCLUSIONS Individuals who had extramarital relations, experienced violence prior to the diagnosis of sexually transmitted disease, feared disclosing the diagnosis to the partner, and those whose partner consumed alcohol had an increased likelihood of suffering violence. The high prevalence of intimate partner violence suggests that this population is vulnerable and therefore intervention efforts should be directed to them. Referral health care services for the treatment of sexually transmitted diseases can be strategic places to identify and prevent intimate partner violence.

OBJECTIVE To analyze the association between unintended pregnancy and postpartum depression.METHODS This is a prospective cohort study conducted with 1,121 pregnant aged 18 to 49 years, who attended the prenatal program devised by the Brazilian Family Health Strategy, Recife, PE, Northeastern Brazil, between July 2005 and December 2006. We interviewed 1,121 women during pregnancy and 1,057 after childbirth. Unintended pregnancy was evaluated during the first interview and postpartum depression symptoms were assessed using the Edinburgh Postnatal Depression Screening Scale. The crude and adjusted odds ratios for the studied association were estimated using logistic regression analysis.RESULTS The frequency for unintended pregnancy was 60.2%; 25.9% presented postpartum depression symptoms. Those who had unintended pregnancies had a higher likelihood of presenting this symptoms, even after adjusting for confounding variables (OR = 1.48; 95%CI 1.09;2.01). When the Self Reporting Questionnaire (SRQ-20) variable was included, the association decreased, however, remained statistically significant (OR = 1.42; 95%CI 1.03;1.97).CONCLUSIONS Unintended pregnancy showed association with subsequent postpartum depressive symptoms. This suggests that high values in Edinburgh Postnatal Depression Screening Scale may result from unintended pregnancy.

OBJECTIVE To investigate the association between social capital and social capital and self-perception of health based on examining the influence of health-related behaviors as possible mediators of this relationship.METHODS A cross-sectional study was used with 1,081 subjects, which is representative of the population of individuals aged 40 years or more in a medium-sized city in Southern Brazil. The subjects who perceived their health as fine, bad or very bad were considered to have a negative self-perception of their health. The social capital indicators were: number of friends, people from whom they could borrow money from when needed; the extent of trust in community members; whether or not members of the community helped each other; community safety; and extent of participation in community activities. The behaviors were: physical activity during leisure time, fruits and vegetable consumption, tobacco use and alcohol abuse. The odds ratios (OR) and confidence intervals (CI) 95% were calculated by binary logistic regression. The significance of mediation was verified using the Sobel test.RESULTS Following adjustment for demographic and clinical variables, subjects with fewer friends (OR = 1.39, 95%CI 1.08;1.80), those who perceived less frequently help from people in the neighborhood (OR = 1.30, 95%CI 1.01;1.68), who saw the violent neighborhood (OR = 1.33, 95%CI 1.01;1.74) and who had not participated in any community activity (OR = 1.39, 95%CI 1.07;1.80) had more negative self-perception of their health. Physical activity during leisure time was a significant mediator in the relationship between all social capital indicators (except for the borrowed money variable) and self-perceived health. Fruit and vegetable consumption was a significant mediator of the relationship between the extent of participation in community activities and self-perceived health. Tobacco use and alcohol abuse did not seem to have a mediating role in any relationship.CONCLUSIONS Lifestyle seems to only partially explain the relationship between social capital and self-perceived health. Among the investigated behaviors, physical activity during leisure time is what seems to have the most important role as a mediator of this relationship.
