ABSTRACT OBJECTIVE To estimate the prevalence of hypertension among adolescent Brazilian students. METHODS A systematic review of school-based cross-sectional studies was conducted. The articles were searched in the databases MEDLINE, Embase, Scopus, LILACS, SciELO, Web of Science, CAPES thesis database and Trip Database. In addition, we examined the lists of references of relevant studies to identify potentially eligible articles. No restrictions regarding publication date, language, or status applied. The studies were selected by two independent evaluators, who also extracted the data and assessed the methodological quality following eight criteria related to sampling, measuring blood pressure, and presenting results. The meta-analysis was calculated using a random effects model and analyses were performed to investigate heterogeneity. RESULTS We retrieved 1,577 articles from the search and included 22 in the review. The included articles corresponded to 14,115 adolescents, 51.2% (n = 7,230) female. We observed a variety of techniques, equipment, and references used. The prevalence of hypertension was 8.0% (95%CI 5.0–11.0; I2 = 97.6%), 9.3% (95%CI 5.6–13.6; I2 = 96.4%) in males and 6.5% (95%CI 4.2–9.1; I2 = 94.2%) in females. The meta-regression failed to identify the causes of the heterogeneity among studies. CONCLUSIONS Despite the differences found in the methodologies of the included studies, the results of this systematic review indicate that hypertension is prevalent in the Brazilian adolescent school population. For future investigations, we suggest the standardization of techniques, equipment, and references, aiming at improving the methodological quality of the studies.

Due to the scarce information about the epidemiological features of schistosomiasis in which the vector is Biomphalaria tenagophila, an investigation was carried in Pedro de Toledo in 1980 where such peculiarity is observed. Stool examinations (Kato-Katz method) were performed in 4,741 individuals (22.8% positive to Schistosoma mansoni eggs) of this 583 had previously received chemoterapy and 4,158 remainders, untreated. The schistosomiasis prevalence in those two groups where respectively 31.7% and 21.6%. Epidemiological investigation showed that 83.6% were autochthonous cases from the studied area: the autochthonous prevalence rate, and the intensity of infection in the untreated autochthonous cases were higher in males than in females; the intensity in the latter untreated group was low, 58.5 eggs/g feces (geometric mean). Moreover, according to the age groups the intensity of infections correlated well (r s = 0.745) with the prevalence rates. Schistosomiasis was verified to occur mostly during the leisure time and by the use of water streams for housework in rural zone. Only 0.4% out of 1,137 snails was positive for S. mansoni cercariae, apparently unchanged from the 1978 study when the human prevalence was 12.0%. The studied area presented differences and similarities in relation to the other Brazilian areas were the main intermediate host is B. glabrata.

Materiais colhidos das presas, das bainhas das presas e do veneno de 15 Bothrops jararaca recém-capturadas, aparentemente saudáveis, foram submetidos a exame bacterioscópico e cultura aeróbia a anaeróbia. As bactérias mais freqüentemente isoladas foram os estreptococos do grupo D (1.2 serpentes), Enterobacter sp. (6), Providencia rettgeri (6), Providencia sp. (4), Escherichia coli (4), Morganella morganii (3) e Clostridium sp. (5). Como estas bactérias são semelhantes às encontradas nos abscessos de pacientes picados por serpentes do gênero Bothrops, é válido considerar a possibilidade de que bactérias da boca da serpente sejam inoculadas no momento da picada e, encontrando condições favoráveis de multiplicação, causem infecção.

Adenovirus are important pathogen primarily associated to respiratory infections of children and military personnel, even though it is also associated to cases of conjunctivitis and keratoconjunctivitis. We analyzed respiratory secretion collected from subjects with and without respiratory infection symptoms, being 181 civilians and 221 military subjects. The samples were inoculated in HEp-2 and/or A549 tissue cultures for viral isolation. Samples presenting cytopathogenic effect (CPE) in any tissue culture were tested by a polymerase chain reaction (PCR) assay to confirm adenovirus isolation. The isolates confirmed as adenovirus were further analyzed by restriction endonuclease assay for determination of viral species. Three isolates were identified as specie A (two from civilian and one from military), one isolate from military was identified as specie C, and one isolate from civilian was identified as specie D. For two isolates the specie could not be identified.

In the late 1960s, Melanoides tuberculatus snails were introduced in Brazil from North/East Africa and Southeast Asia. The first records of specimens infected with cercariae were registered in Rio de Janeiro State in 2001. The present study reports the occurrence of M. tuberculatus infected with larval trematodes in Rio de Janeiro City. Bottom sediment was collected with dip nets and sieved through 0.25 inch-mesh screening. Snails were transported to the laboratory in vials with stream water, then measured and individually isolated in glass vials with distilled water. They were exposed to artificial light and temperature to induce cercarial emergence. The most actively emerging cercariae were processed by differential staining and silver nitrate impregnation methods. Negative snails were subsequently dissected. Approximately 700 snails were collected. Snail total lengths ranged from 1.2 to 3.3 cm. The prevalence rate was 15.76% although 53.76% of the snails were found infected in one of the sites. Infected snails were infected with rediae and pleurolophocercous cercariae. Cercarial morphology and chaetotaxy were consistent with those of the family Heterophyidae mostly due to the presence of median dorsal and ventral fins on the tail and the absence of CI dorsal sensory receptors.

The aim of this study was to perform a retrospective analysis of cases of candidemia in a Brazilian hospital in the city of Fortaleza, Ceará. A total of 50 blood cultures were analyzed from 40 candidemic patients. The mycological diagnosis was based on the phenotypical analysis and the patients' data were recorded in appropriate files. The most frequent species were Candida parapsilosis (n = 18), followed by C. albicans (n = 14), C. tropicalis (n = 8), C. guillermondii (n = 6), C. glabrata (n = 2), and Candida spp. (n = 2). A detailed descriptive study was undertaken with 21 patients whose medical records were complete. The candidemia episodes occurred in eight male patients and 13 female patients. The most representative risk factors implicated in candidemia were prior antibiotic therapy, central venous catheters, parenteral nutrition, gastric probes and mechanical ventilation. Death occurred in 13 of the 21-candidemic patients. This study demonstrated the emergence of candidemia caused by C. parapsilosis in a Brazilian hospital in the city of Fortaleza, Ceará.

The present study intended to analyze the seroprevalence of Helicobacter pylori, IgG, and its relation to dyspepsia in a population from the western Amazon region. During the "Projeto Bandeira Científica", a University of São Paulo Medical School program, in Monte Negro's rural areas, state of Rondônia, 266 blood samples were collected from volunteers. The material was tested for IgG antibodies anti-Helicobacter pylori by ELISA method and the participants were also interviewed on dyspepsia, hygiene and social aspects. Participants aged between five and 81 years old (34 years on average), 149 (56%) were female and 117 (44%) male. We found 210 (78.9%) positive, 50 (18.8%) negative and six (2.3%) undetermined samples. Dyspeptic complaints were found in 226 cases (85.2%). There was no statistical association between dyspepsia and positive serology for H. pylori. We concluded that the seroprevalence in all age categories is similar to results found in other studies conducted in developing countries, including those from Brazil. On the other hand, the seroprevalence found in Monte Negro was higher than that reported in developed countries. As expected, there was a progressive increase in the positivity for H. pylori in older age groups.

SUMMARYTo commemorate Prof. Carlos da Silva Lacaz's centennial anniversary, the authors have written a brief account of a few, out of hundreds, biological, ecological, molecular and phylogenetic studies that led to the arrival of Paracoccidioides lutzii, hidden for more than a century within Paracoccidioides brasiliensis. Lacaz's permanent interest in this fungus, and particularly his conviction on the benefits that research on paracoccidioidomycosis would bring to patients, were pivotal in the development of the field.

A raiva humana se mantém como problema de Saúde Pública, especialmente nas áreas urbanas, onde o cão atua como principal reservatório e fonte imediata de infecção. A deficiência de programas de controle de cães vadios, de imunização sistemática e de educação sanitária, contribui para a perpetuação da cadeia epidemiológioa. O presente estudo analisa todos os casos de raiva humana, observados em habitantes do Estado da Guanabara, no período 1965-1969, segundo as variáveis: idade, sexo, côr, variação estacional, período de incubação e de duração da fase clínica, fonte de infecção, local de mordedura e uso de vacina posterior à exposição. Os principais resultados foram: 1. A ravia na Guanabara, parece ser mais comum nos grupos etários mais jovens e nos homens. 2. Não se observou variação estacional. 3. O período de incubação mediano foi de 45 dias e de duração da fase clínica de 3 dias. 4. Ocorreu uma associação entre período de incubação menor e vacinação incompleta.

Um grupo de 199 crianças de 1 mês a 1 ano de idade, residentes em Parintins, AM, divididas em casos e controles de DDA, foram comparadas segundo: idade; ocupação do chefe da família; condições de moradia; condições demográficas; participação social da família; nutrição; percepção de suscetibilidade e gravidade da DDA; crenças sobre a causalidade da DDA. Foram aceitas as hipóteses nulas de não-associação entre as variáveis independentes e a presença de DDA, exceto idade. Questiona-se a representatividade da amostra tomada. Propõe-se o prosseguimento dos estudos relativos aos fatores que condicionam a manutenção da DDA como principal causa da mortalidade em nosso meio.

Através do teste PPD procurou-se estabelecer o comportamento da infecção tuberculosa em uma fábrica de beneficiamento de juta, examinando 355 operários, áveis: divididos segundo as variádade: idade, sexo, tempo e local de serviço. Observou-se associação significativa entre maior prevalência de reatoresfortes e as variáveis: sexo e trabalho em local "não-aglomerado". Em termos globais, a prevalência de reatores fortes foi igual a 38%.

Com o objetiiro de identificar a espécie de Leishmania envolvida nas afecções cutâneas de indivíduos residentes nos municípios de Cosmópolis e Indaiatuba, região de Campinas, Estado de São Paulo, correlacionamos dados clínicos, histopatológicos e testes de bibridização in situ. Os expressivos índices de incidência de leishmaniose tegumentar americana obsewados nesses municípios nos levaram a iniciar esses estudos. Nesse trabalho, apresentamos os dados relativos a seis indivíduos. As características das lesões, ulceradas, de dificil cicatrização e presentes em locais expostos do corpo sugeriram quadros de leishmaniose tegumentar americana. Os testes de intradermoreação de Montenegro, positivos em cinco dos seis pacientes analisados também reforçaram a suspeita de leishmaniose tegumentar americana. Os padrões histopatológicos, como reação exsudativa celular e granulomatosa e a dificuldade de isolamento dos parasitas obtidos de biópsias de lesões são compatíveis com aqueles descritos para Leishmania (Viannia) braziliensis. Em testes de bibridização in situ do DNA do cinetoplasto de amastigotas das lesões dos seis pacientes, obseivamos que quatro deles apresentavam sinais de bibridização com a sonda de L. (Viannia) braziliensis, confirmando as suspeitas de que a Leishmania responsável pelas afecções cutâneas nos pacientes analisados era do subgenêro Viannia e do complexo braziliensis.

Estudou-se as prevalências de Gíardia duodenalís e Cryptosporidium parvum, conforme sexo e faixa etária. em crianças de 2-60 meses (n = 310), atendidas em 8 creches. Giardiose ocoireu em 42 (13,5%) das crianças e foi mais freqüente na faixa etãria de 19-24 meses, embora crianças nos primeiros seis meses de vida apresentassem amostras fecais positivas. Teste estatístico não revelou associação entre sexo e parasitismo (p > 0,05). C. parvum foi encontrado em 20 crianças (6,4%) com maior frequência no grupo de 7-12 meses. Crianças entre 25-30 meses e acima de 36 meses não apresentaram oocistos nas fezes. Oocistos foram detectados em 4 crianças na faixa etãria de 0-6 meses. Não houve significância estatística entre criptosporidiose e sexo (p = 0,09) e, entre creches (p = 0,068). Ocorreu associação entre idade e a presença do coccídio (p = 0,004). Abordou-se os aspectos epidemiológicos por meio de inquérito familiar, exames parasitolõgicos entre contatos dos casos-índices e dos animais presentes no domicílio. Prevalência de C. parvum foi maior quando pelo menos um dos pais era profissional da Área de Saúde (73,6% positivas), hai:ia relato de doença concomitante (52,6%) positivas) e diairéia era o principal sintoma por ocasião de exame fecal (78,9%) positivas). Exames de fezes dos animais domésticos foram negativos para estes dois parasitos.

Massive destruction of parasitized splenic macrophages was histologically observed at the height of a virulent infection caused by Trypanosoma cruzi (Y strain) in the mouse. This was coincident with a sudden drop in parasitemic curve. Most of the animals died at this point, probably due to the liberation of toxic products, such as TNF, following the massive destruction of parasitized cells. However, parasitized-cell destruction indicated the transition from susceptibility to resistance. Although it has been extensively studied in vitro, this study contributes with the morphological counterpart observed in vivo by optical and electron microscopy. When infected animals were specifically treated during early infection transition to chronic phase was immediately observed without splenic parasitism. Animals that apparently recovered from massive cell-destruction in the spleen showed evidences of a rapid restoration of splenic architecture.
