OBJETIVO: Avaliar o impacto da iniciativa Hospital Amigo da Criança sobre a prática do aleitamento materno durante a internação de recém-nascidos em unidade de neonatologia e durante os primeiros seis meses de vida. MÉTODOS: Foram consultados todos os prontuários de recém-nascidos internados em unidade de neonatologia de um hospital de ensino durante os anos de 1994 (N=285) e 1998 (N=368). Foram analisadas informações sobre o regime alimentar durante a internação e após a alta hospitalar, até os primeiros seis meses de vida. Para estimar a duração do aleitamento materno e do aleitamento materno exclusivo e as diferenças entre os dois anos, foram utilizados a técnica de Kaplan-Meier e o teste de Log-Rank. Análises de regressão logística e de Cox foram aplicadas para controlar variáveis de confusão. RESULTADOS: Durante o período de internação, houve aumento expressivo do percentual de crianças em aleitamento materno exclusivo (de 1,9%, em 1994, para 41,7%, em 1998) e alimentação exclusiva por fórmula, que era de 17,9%, em 1994, e deixou de existir em 1998. Quanto à amamentação nos primeiros seis meses de vida, o tempo mediano do aleitamento materno exclusivo aumentou de 12 para 45 dias. Para o aleitamento materno, que inclui a ingestão de outros alimentos, não houve diferença significativa. CONCLUSÕES: A implantação da iniciativa do hospital estudado contribuiu para aumentar o índice de aleitamento materno exclusivo durante a internação de recém-nascidos em uma unidade de neonatologia e nos primeiros seis meses de vida.

Cryptosporidiosis occurrence was determined in HIV+ patient assisted in the Clinic of Infect-parasitic Diseases in a hospital of Nova Iguaçu, Rio de Janeiro, Brazil, in the period from Juy/1998 to March/1999. In order to research, seventy-five patient, carriers of diarrhea or not, were appraised. The samples of feces were collected and placed in saline solution with formaldehyde (5%). The Modified Ritchie technique was used for the oocysts research, and the smears were stained with Safranine O methylene blue. The results verified 9.33% of positive samples, with higher frequency of cases in patients of the masculine sex from 20 to 50 years old, however without significant difference.

OBJECTIVE: To obtain population estimates and profile risk factors for infant mortality in two birth cohorts and compare them among cities of different regions in Brazil. METHODS: In Ribeirão Preto, southeast Brazil, infant mortality was determined in a third of hospital live births (2,846 singleton deliveries) in 1994. In São Luís, northeast Brazil, data were obtained using systematic sampling of births stratified by maternity unit (2,443 singleton deliveries) in 1997-1998. Mothers answered standardized questionnaires shortly after delivery and information on infant deaths was retrieved from hospitals, registries and the States Health Secretarys' Office. The relative risk (RR) was estimated by Poisson regression. RESULTS: In São Luís, the infant mortality rate was 26.6/1,000 live births, the neonatal mortality rate was 18.4/1,000 and the post-neonatal mortality rate was 8.2/1,000, all higher than those observed in Ribeirão Preto (16.9, 10.9 and 6.0 per 1,000, respectively). Adjusted analysis revealed that previous stillbirths (RR=3.67 vs 4.13) and maternal age <18 years (RR=2.62 vs 2.59) were risk factors for infant mortality in the two cities. Inadequate prenatal care (RR=2.00) and male sex (RR=1.79) were risk factors in São Luís only, and a dwelling with 5 or more residents was a protective factor (RR=0.53). In Ribeirão Preto, maternal smoking was associated with infant mortality (RR=2.64). CONCLUSIONS: In addition to socioeconomic inequalities, differences in access to and quality of medical care between cities had an impact on infant mortality rates.

OBJETIVO: O enterococo resistente à vancomicina é atualmente um dos principais microorganismos implicados em infecções nosocomiais. Assim, realizou-se estudo com o objetivo de avaliar sua epidemiologia em um hospital terciário de ensino. MÉTODOS: Trata-se de um estudo epidemiológico retrospectivo, realizado de 2000 a 2002, que analisou amostras de culturas clínicas positivas para enterococo resistente à vancomicina (VRE) em um hospital universitário com 660 leitos. Procurou-se definir sua incidência e os principais sítios e unidades de isolamento. Foi verificada a significância entre as variáveis nos três anos de estudo, sendo considerado como significante p<0,05. RESULTADOS: Houve aumento progressivo na resistência à vancomicina nas culturas clínicas positivas para Enterococcus spp. nos três anos de estudo. Em 2000, 9,5% das amostras eram resistentes à vancomicina, com aumento para 14,7% em 2001 e 15,8% em 2002. As unidades com maior número de isolados foram respectivamente: pronto-socorro (19,5%) e UTI geral (15%); os sítios mais isolados foram: urina (36%) e sangue (20%). CONCLUSÕES: Com o aumento progressivo na incidência de resistência à vancomicina e da taxa de VRE, concluiu-se ser necessárias medidas de controle mais efetivas para deter a disseminação do VRE.

OBJETIVO: Descrever a atividade docente assistencial, cujo objetivo é proporcionar experiência de promoção da saúde bucal coletiva a estudantes concluintes do curso de odontologia. MÉTODOS: Tal experiência fundamenta-se na avaliação do desempenho do aluno, como educador em saúde bucal, à medida que ele tem, entre outras tarefas, a de motivar pacientes internados e seus acompanhantes, na geração de hábitos saudáveis, visando à assistência integral e mais humanizada do paciente hospitalizado. RESULTADOS: Os resultados mostraram que as metas desejadas foram atingidas, visto que a higiene bucal dos pacientes já se tornou uma tarefa incorporada à rotina hospitalar, considerando-se os índices de biofilme dentário: inicial, 1,72; e final, 1,17, respectivamente. Essa diferença, de acordo com a estatística U-Mann-Whitney, mostrou-se significativa, pois o valor de p<0,0001 traduz o alto nível de motivação, alcançado pelo binômio mãe-criança, no que se refere à higiene oral. CONCLUSÕES: Conclui-se que as experiências de ensino-aprendizagem, oriundas das atividades interdisciplinares e multiprofissionais, têm permitido melhor entendimento do processo saúde-doença, por parte do aluno de odontologia. Servem ainda como oportunidade para o seu aprendizado sobre o planejamento e execução de atividades educativo-preventivas, que complementam sua vivência técnico-profissional e despertam a sensibilidade social, tão necessária à sua formação acadêmica.

OBJECTIVE: To assess factors associated with infant feeding practices on the first day at home after hospital discharge. METHODS: A total of 209 women, who had a child aged four months or less and were living in Itapira, Brazil, were interviewed during the National Immunization Campaign Day in 1999. Statistical analysis was performed using the Chi-square test and a logistic regression model was used for verifying an association between dependent and independent variables. RESULTS: Women aged 25.5 years on average and 18.2% were teenagers. Fifty-three percent of the women delivered vaginally and most vaginal deliveries (78.5%) took place in the public hospital. The prevalence of exclusive breastfeeding on the first day at home was 78.1% and 11.6% of the infants were receiving formula at this time. The only factor associated with EBF on the first day at home was being a teenaged-primiparous mother (OR=9.40; 95% CI: 1.24-71.27). This association remained statistically significant even after controlling for type of delivery and hospital where the birth took place. Feeding formula on the first day at home was only significantly associated with the hospital (i.e., birth at the city hospital was a protective factor (OR=0.33; 95% CI: 0.13-0.86), even after controlling for vaginal delivery. CONCLUSIONS: On the first day at home after hospital discharge, teenaged-primiparous mothers were more likely to exclusive breastfeeding as well as those infants born in the municipal public hospital. Further studies are needed from a multidisciplinary approach.

OBJECTIVE: Myocardial infarction is an acute and severe cardiovascular disease that generally leads to patient admissions to intensive care units and few cases are initially admitted to infirmaries. The objective of the study was to assess whether estimates of air pollution effects on myocardial infarction morbidity are modified by the source of health information. METHODS: The study was carried out in hospitals of the Brazilian Health System in the city of São Paulo, Southern Brazil. A time series study (1998-1999) was performed using two outcomes: infarction admissions to infirmaries and to intensive care units, both for people older than 64 years of age. Generalized linear models controlling for seasonality (long and short-term trends) and weather were used. The eight-day cumulative effects of air pollutants were assessed using third degree polynomial distributed lag models. RESULTS: Almost 70% of daily hospital admissions due to myocardial infarction were to infirmaries. Despite that, the effects of air pollutants on infarction were higher for intensive care units admissions. All pollutants were positively associated with the study outcomes but SO2 presented the strongest statistically significant association. An interquartile range increase on SO2 concentration was associated with increases of 13% (95% CI: 6-19) and 8% (95% CI: 2-13) of intensive care units and infirmary infarction admissions, respectively. CONCLUSIONS: It may be assumed there is a misclassification of myocardial infarction admissions to infirmaries leading to overestimation. Also, despite the absolute number of events, admissions to intensive care units data provides a more adequate estimate of the magnitude of air pollution effects on infarction admissions.

OBJETIVO: Antibióticos são os medicamentos que mais causam eventos adversos, gerando problemas aos pacientes e custos adicionais ao sistema de saúde. Assim, objetivou-se analisar a ocorrência de eventos adversos a antibióticos em pacientes internados em um hospital. MÉTODOS: Realizou-se monitoramento intensivo do uso de antibióticos em pacientes adultos internados no município de Maringá, Paraná, de setembro de 2002 a fevereiro de 2003. Foram pesquisadas variáveis relativas aos medicamentos em uso, em particular aos antibióticos e aos eventos adversos. Com base em critérios para a avaliação do uso correto dos antibióticos, os eventos observados foram classificados como reações adversas, erros de medicação e "quase erros". Para relação de causalidade entre a administração do fármaco e o surgimento dos eventos utilizou-se o algoritmo de Naranjo. RESULTADOS: Foram acompanhados 87 pacientes e identificados 91 eventos adversos, sendo três deles (3,3%) reações adversas a medicamentos, sete (7,7%) erros de medicação, e 81 (89,0%) "quase erros". As reações a medicamentos ocorreram devido ao uso de quinolonas e foram consideradas "prováveis" utilizando-se o algoritmo de Naranjo. Os sete erros de medicação ocorreram devido a quatro prescrições incorretas de dose e três interações medicamentosas. CONCLUSÕES: Os resultados sugerem que a falta de conhecimento do medicamento ou a falta de informação sobre o paciente no momento da prescrição tenham sido os principais fatores envolvidos na ocorrência das reações a medicamentos.

OBJECTIVE: To assess risk factors for antepartum fetal deaths. METHODS: A population-based case-control study was carried out in the city of São Paulo from August 2000 to January 2001. Subjects were selected from a birth cohort from a linked birth and death certificate database. Cases were 164 antepartum fetal deaths and controls were drawn from a random sample of 313 births surviving at least 28 days. Information was collected from birth and death certificates, hospital records and home interviews. A hierarchical conceptual framework guided the logistic regression analysis. RESULTS: Statistically significant factors associated with antepartum fetal death were: mother without or recent marital union; mother's education under four years; mothers with previous low birth weight infant; mothers with hypertension, diabetes, bleeding during pregnancy; no or inadequate prenatal care; congenital malformation and intrauterine growth restriction. The highest population attributable fractions were for inadequacy of prenatal care (40%), hypertension (27%), intrauterine growth restriction (30%) and absence of a long-standing union (26%). CONCLUSIONS: Proximal biological risk factors are most important in antepartum fetal deaths. However, distal factors - mother's low education and marital status - are also significant. Improving access to and quality of prenatal care could have a large impact on fetal mortality.

OBJECTIVE: To describe the demographic profile, social functioning, and quality of life of a population of long-stay care patients in a psychiatric hospital. METHODS: A study was carried out in Porto Alegre, Southern Brazil, in 2002. A total of 584 (96%) long-stay patients were assessed by means of the following instruments: the World Health Organization Quality of Life, the Social Behavior Schedule, the Independent Living Skills Survey, the Brief Psychiatric Rating Scale and another instrument for assessing disability (Questionnaire for Assessing Physical Disability). RESULTS: The average hospital stay was 26 years (SD: 15.8) and 46.6% of inpatients had no physical disability. Patients had their social functioning skills and autonomy largely impaired. Few of them (27.7%) answered the instrument for assessing quality of life, and showed significant impairments in all domains. The Brief Psychiatric Rating Scale evidenced a low prevalence of positive symptoms in this population. CONCLUSIONS: The institutionalized population studied presented significantly impaired social functioning, autonomy, and quality of life. These aspects need to be taken into consideration while planning for their deinstitutionalization.

OBJECTIVE: To assess the association between oral health and hygiene practices and oral cancer. METHODS: Hospital-based case-control study in the metropolitan area of São Paulo, southeastern Brazil, from 1998 to 2002. A total 309 patients with squamous cell carcinoma of the mouth and the pharynx and 468 controls matched by sex and age were included in the study. Cases were recruited in seven reference hospitals and controls were selected in five out of the seven participating hospitals. Detailed information on smoking, alcohol consumption, schooling, oral health status and hygiene practices were obtained through interviews. Odds ratios (OR) and 95% confidence intervals (95% CI), adjusted by sex, age, schooling, smoking, alcohol consumption as well as the variables oral health status and hygiene practices were estimated using unconditional logistic regression analyses. RESULTS: The use of complete dental prosthesis was not associated with oral cancer but regular gum bleeding showed a strong association (OR 3.1; 95% CI 1.2-7.9). Those who never attended a dental visit were more likely to have oral cancer (OR 2.5; 95% CI 1.3-4.8). Daily mouthwash use showed a stronger association to pharynx (OR 4.7; 95% CI 1.8-12.5) than mouth cancer (OR 3.2; 95% CI 1.6-6.3). CONCLUSIONS: Gum bleeding, no dental care, and daily mouthwash use were factors associated with oral cancer regardless of tobacco and alcohol consumption.

OBJECTIVE: To develop a Charlson-like comorbidity index based on clinical conditions and weights of the original Charlson comorbidity index. METHODS: Clinical conditions and weights were adapted from the International Classification of Diseases, 10th revision and applied to a single hospital admission diagnosis. The study included 3,733 patients over 18 years of age who were admitted to a public general hospital in the city of Rio de Janeiro, southeast Brazil, between Jan 2001 and Jan 2003. The index distribution was analyzed by gender, type of admission, blood transfusion, intensive care unit admission, age and length of hospital stay. Two logistic regression models were developed to predict in-hospital mortality including: a) the aforementioned variables and the risk-adjustment index (full model); and b) the risk-adjustment index and patient's age (reduced model). RESULTS: Of all patients analyzed, 22.3% had risk scores >1, and their mortality rate was 4.5% (66.0% of them had scores >1). Except for gender and type of admission, all variables were retained in the logistic regression. The models including the developed risk index had an area under the receiver operating characteristic curve of 0.86 (full model), and 0.76 (reduced model). Each unit increase in the risk score was associated with nearly 50% increase in the odds of in-hospital death. CONCLUSIONS: The risk index developed was able to effectively discriminate the odds of in-hospital death which can be useful when limited information is available from hospital databases.

OBJECTIVE: To estimate the spatial intensity of urban violence events using wavelet-based methods and emergency room data. METHODS: Information on victims attended at the emergency room of a public hospital in the city of São Paulo, Southeastern Brazil, from January 1, 2002 to January 11, 2003 were obtained from hospital records. The spatial distribution of 3,540 events was recorded and a uniform random procedure was used to allocate records with incomplete addresses. Point processes and wavelet analysis technique were used to estimate the spatial intensity, defined as the expected number of events by unit area. RESULTS: Of all georeferenced points, 59% were accidents and 40% were assaults. There is a non-homogeneous spatial distribution of the events with high concentration in two districts and three large avenues in the southern area of the city of São Paulo. CONCLUSIONS: Hospital records combined with methodological tools to estimate intensity of events are useful to study urban violence. The wavelet analysis is useful in the computation of the expected number of events and their respective confidence bands for any sub-region and, consequently, in the specification of risk estimates that could be used in decision-making processes for public policies.

OBJETIVO: Estimar a freqüência de ingestão alcoólica em vítimas de causas externas atendidas em hospital. MÉTODOS: Estudo realizado com vítimas atendidas em um hospital geral universitário em Uberlândia (MG), de fevereiro a agosto de 2004. A alcoolemia foi determinada em 85 pacientes no pronto-socorro e entrevistaram-se outros 301 internados nas enfermarias sobre possível ingestão alcoólica previamente ao trauma; em ambos os grupos foi aplicado o questionário Cut-down, Annoyed by criticism, Guilty and Eye-opener (CAGE). Para as comparações das freqüências foi utilizado o teste exato de Fisher. RESULTADOS: A alcoolemia foi positiva em 31,8% dos pacientes testados, os quais mais freqüentemente necessitaram de internação (70,4% versus 37,9%; p<0,05). Proporcionalmente, alcoolemia positiva foi mais freqüente (p<0,05) entre as vítimas de agressão física (57,1%) do que as de queda (18,2%) ou de acidente de trânsito (29,3%). Nas enfermarias, 29,9% dos pacientes referiram ingestão alcoólica, proporcionalmente mais freqüente (p<0,01) entre as vítimas de agressão física (67,4%) do que entre as de acidente de trânsito (27,8%) ou queda (19,3%). Entre aqueles que ingeriram álcool, abordados no pronto-socorro e nas enfermarias, observou-se, respectivamente: que a maioria era homens (85,2% e 80,4%), a ocorrência de trauma foi maior (p<0,05) nos finais de semana (63% e 57,8%) e no período noturno (59,3% e 57,8%), e o questionário CAGE foi positivo em 81,5% e 82,2%. CONCLUSÕES: Cerca de um terço dos pacientes ingeriu bebidas alcoólicas previamente ao trauma e, entre eles, a maioria era homens. Proporcionalmente, a ingestão prévia de bebidas alcoólicas foi mais freqüente entre os pacientes vítimas de violência. Os resultados da aplicação do CAGE mostra que a maioria dos pacientes vítimas de causas externas após ingestão etílica não era alcoolista ocasional, e sim provável usuário crônico ou dependente de álcool.
