FUNDAMENTO: Os peptídeos natriuréticos são liberados pelo coração em resposta ao estresse da parede. OBJETIVO: As concentrações de NT-Pro-BNP em pacientes com Fluxo Lento Coronariano (FLC) foram avaliadas antes e depois do teste de exercício e comparados com os valores dos controles saudáveis. MÉTODOS: A população do estudo foi de 34 pacientes com FLC [22 homens (64,7%), com idade 51,0 ± 6,2 anos], e 34 indivíduos normais com artérias coronarianas normais [21 homens (61,8%), com idade 53,2 ± 6,6 anos]. As taxas de fluxo coronariano dos pacientes e controles foram determinadas pelo escore TIMI Trombólise no Infarto do Miocárdio (Thrombolysis in Myocardial Infarction). As amostras de sangue foram coletadas em repouso e após o teste ergométrico. RESULTADOS: As concentrações basais de NT-Pro-BNP nos pacientes com FLC foram superiores às dos indivíduos-controle (NT-Pro-BNP: 49,7 ± 14,2 pg/mL vs. 25,3 ± 4,6 pg/mL p <0,0001, respectivamente), e essa diferença entre os grupos aumentou após o teste de exercício (NT-Pro-BNP: 69,5 ± 18,6 pg/mL vs. 30,9 ± 6,4 pg/mL, p <0,0001). No grupo FLC após o exercício, a concentração de NT-Pro-BNP em 15 pacientes com angina foi maior do que aqueles sem angina (76,8 ± 17,8 pg/mL vs. 63,8 ± 17,5 pg/mL, p = 0,041).A concentração de NT-Pro-BNP em 11 pacientes com infradesnivelamento do segmento ST foi também maior do que aqueles sem infradesnivelamento do segmento ST (82,4 ± 17,3 pg/mL vs. 63,3 ± 16,1 pg/mL, p = 0,004). Os aumentos na mediana pós-exercício no NT-Pro-BNP (Δ NT-Pro-BNP) foram maiores no grupo FLC do que no grupo de controle (Δ NT-Pro-BNP: 19,8 ± 7,7 pg/mL vs. 5,7 ± 4,5 pg/mL, p < 0,0001). CONCLUSÃO: Os resultados deste estudo sugerem que pode haver uma ligação fisiopatológica importante entre a gravidade do FLC (microvascular ou disfunção da artéria coronária epicárdica) e o nível de circulação de NT-Pro-BNP em pacientes com FLC.
Association between Angiotensin-Converting Enzyme Inhibitors and Troponin in Acute Coronary Syndrome

Background:Cardiovascular disease is the leading cause of mortality in the western world and its treatment should be optimized to decrease severe adverse events.Objective:To determine the effect of previous use of angiotensin-converting enzyme inhibitors on cardiac troponin I measurement in patients with acute coronary syndrome without ST-segment elevation and evaluate clinical outcomes at 180 days.Methods:Prospective, observational study, carried out in a tertiary center, in patients with acute coronary syndrome without ST-segment elevation. Clinical, electrocardiographic and laboratory variables were analyzed, with emphasis on previous use of angiotensin-converting enzyme inhibitors and cardiac troponin I. The Pearson chi-square tests (Pereira) or Fisher's exact test (Armitage) were used, as well as the non-parametric Mann-Whitney's test. Variables with significance levels of <10% were submitted to multiple logistic regression model.Results:A total of 457 patients with a mean age of 62.1 years, of whom 63.7% were males, were included. Risk factors such as hypertension (85.3%) and dyslipidemia (75.9%) were the most prevalent, with 35% of diabetics. In the evaluation of events at 180 days, there were 28 deaths (6.2%). The statistical analysis showed that the variables that interfered with troponin elevation (> 0.5 ng / mL) were high blood glucose at admission (p = 0.0034) and ST-segment depression ≥ 0.5 mm in one or more leads (p = 0.0016). The use of angiotensin-converting inhibitors prior to hospitalization was associated with troponin ≤ 0.5 ng / mL (p = 0.0482). The C-statistics for this model was 0.77.Conclusion:This study showed a correlation between prior use of angiotensin-converting enzyme inhibitors and reduction in the myocardial necrosis marker troponin I in patients admitted for acute coronary syndrome without ST-segment elevation. However, there are no data available yet to state that this reduction could lead to fewer severe clinical events such as death and re-infarction at 180 days.

Introduction:Atrial fibrillation and atrial flutter account for one third of hospitalizations due to arrhythmias, determining great social and economic impacts. In Brazil, data on hospital care of these patients is scarce.Objective:To investigate the arrhythmia subtype of atrial fibrillation and flutter patients in the emergency setting and compare the clinical profile, thromboembolic risk and anticoagulants use.Methods:Cross-sectional retrospective study, with data collection from medical records of every patient treated for atrial fibrillation and flutter in the emergency department of Instituto de Cardiologia do Rio Grande do Sul during the first trimester of 2012.Results:We included 407 patients (356 had atrial fibrillation and 51 had flutter). Patients with paroxysmal atrial fibrillation were in average 5 years younger than those with persistent atrial fibrillation. Compared to paroxysmal atrial fibrillation patients, those with persistent atrial fibrillation and flutter had larger atrial diameter (48.6 ± 7.2 vs. 47.2 ± 6.2 vs. 42.3 ± 6.4; p < 0.01) and lower left ventricular ejection fraction (66.8 ± 11 vs. 53.9 ± 17 vs. 57.4 ± 16; p < 0.01). The prevalence of stroke and heart failure was higher in persistent atrial fibrillation and flutter patients. Those with paroxysmal atrial fibrillation and flutter had higher prevalence of CHADS2 score of zero when compared to those with persistent atrial fibrillation (27.8% vs. 18% vs. 4.9%; p < 0.01). The prevalence of anticoagulation in patients with CHA2DS2-Vasc ≤ 2 was 40%.Conclusions:The population in our registry was similar in its comorbidities and demographic profile to those of North American and European registries. Despite the high thromboembolic risk, the use of anticoagulants was low, revealing difficulties for incorporating guideline recommendations. Public health strategies should be adopted in order to improve these rates.

Material: Studies were made mainly with Ascaris megalocephála Cloq. univalens and bivalens, and also with Tityus bahiensis Perty. 1) Somatic pairing of heterochromatic regions. The heterochromatic ends of the somatic chromosomes in Ascaris show a very strong tendency for unspecifical somatic pairing which may occur between parts of different chromosomes (Figs. 1, 2, 3, 7, 10, 11, 12, 13, 14, 16, 18,), between the two ends of the same chromosome either directly (Figs. 4, 5, 7, 8, 11, 12, 13, 15, 16, 17, 18) or inversely (Fig. 8, in the arrow) and also within a same chromosomal arm (Fig. 6). 2) During the early first cleavage division the chomosomes are an isodiametric cylinder (Figs. 6, 9, 11, 13, 14). But in later metaphase the ends become club shaped (Figs. 1, 2, 3, 4, 5, 7, 10) which is interpreted as the beginning of migration of chromatic substance from the central euchromatic region towards the heterochromatic regions. This migration becomes more and accentuated in anaphase (Figs. 19, 22, 23) and in the vegetative cells where euchromatic region looses more and more staing power, especially in the intersititial zones between the individual small spherical chromosomes into which the euchromatic region desintegrates. The emigrated chromatin material is finally eliminated with the heterochromatic chromosome ends (Fig. 23 and 24). 3) It seems a general rule that during mitotic anaphase all chromosomes with diffuse or multiple spindle fiber attachement (Ascaris, Tityus, Luzula, Steatococcus, Homoptera and Heteroptera in general) move to the poles in the form of an U with precedence of the chromosomal ends. In Ascaris, the heterocromatic regions are pulled passively towards the poles and only the euchromatic central portion may be U-shaped (Fig. 19, 22, 25). While in the other species this U-shape is perfect since the beginning of anaphase, giving the impression that movement towards the poles begins at both ends of a chromosome simultaneously, this is not the case in Ascaris. There the euchromatic region is at first U-shaped, passing then to form a straight or zig-zag line and becoming again U-shaped during late anaphase. This is explained by the fact that the ends of the euchromatic regions have to pull the weight of the passive heterochromatic portions. 4) While it is generally accepted that, during first meio-tic division untill second anaphase, all attachement regions remain either undivided or at least united closely, this is not the case in chromosomes with diffused or multiple attachment. Here one clearly sees in all cases so far studied four parallel chromatids at first metaphase. In Luzula and Tityus (for Tityus all figs. 26 to 31) this division is allready quite clear in paraphase (pro-metaphase) and it cannot be said wether in other species the division in sister chromatids is allready present, but not visible at this stage. During first anaphase the sister chromatids of Titbits remain more or less in contact, while in Luzula and especially in Ascaris they are quite separated. Thus one can count in late anaphase or telophase of Ascaris megalocephala bivalens, nearly allways, four separate chromosomes near each pole, or a total of eight chromatids per division figure (Figs. 35, 36, 37, 38, 39, 40, 41).

Num ensaio de adubação com N, P, K e estêrco (E) de mudas de eucalipto (Eucalyptus saligna Sm.) em "torrão paulista" nos viveiros da Cia. Paulista de Estrada de Ferro, em Rio Claro, SP, foi usado um delineamento fatorial de 3x3x3x2, com resultados estatisticamente significativos para N, P e estêrco. As alturas médias das mudas, em centímetros, 3(1/2) meses após a repicagem para os torrões, foram as seguintes. The authors carried out 3 experiments on the sampling of sugar cane for technological determinations, one with each of the varieties Co 419, CB 40-69 and CB 41-58, in Piracicaba, State of São Paulo, Brasil. The main intent of the project was to compare 2 methods of sampling, namely: 1) Method A, where the sample is a hill (CATANI et al, 1959) or, more generally, 20 stalks all together in a randomly selected point of the furrow; 2) Method B, where 20 stalks are taken, from 20 points evenly spread but on the whole plot. Further computations led to the following coefficients of variation. For available sucrose, which is probably the most important characteristic studied, the average coefficient of variation for the 3 varieties was 2.7%, for the case of method B, that is, 20 stalk samples, one stalk per hill. Assuming this coefficient of variation, in a trial with 5 treatments and 6 replications, in randomised blocks, the least significant difference among treatment means, at the 5% level, would be 4.7% of available sucrose by Tukey's test, and 3.3% by the t test. For the case of method A the average coefficient of variation is 7.0% and, in similar conditions, the least significant difference would be 15.1% by Tukey's test, and 12.1% by the t test. Since differences of available sucrose among treatments in experiments with fertilizers seldom are higher than 3 or 4% of the mean (PIMENTEL GOMES & CARDOSO, 1958), method B with a 20 stalk sample per plot gives more or less the minimum amount of cane to be sampled for technological determinations. In experiments with varieties, however, where differences may be assumed to be higher, a sample of 10 to 20 stalks one per hill, can be enough.

Na presente nota prévia os autores apresentam os primeiros dados acerca da extração e exportação de nutrientes através do cajueiro a partir de 1 a 14 anos de idade. As plantas foram colhidas de uma plantação localizada no Município de Terezinha, BA, situadas em um latossolo de baixa fertilidade, exceto em K. As plantas foram pesadas e analisadas para N, P, K, Ca, Mg, S, B, Cu, Fe, Mn e Zn. Constataram os autores que o cajueiro cresce continuamente até aos 14 anos. Uma árvore aos 10 anos de idade contém: N- 1,5 kg; P- 0,2 kg; K- 1,1 kg; Ca-0,2 kg;Mg-0,3 kg; S- 0,1 kg; B- 4,0 g;Cu-1,4 g;Fe- 2,0 g;Mn- 7,8 g;Zn- 2,6 g. Aquilataram os autores que a exportação de nutrientes através de 1 kg de frutos é a seguinte: N- 13,9 g; P- 1,3 g; K- 6,2 g; Ca- 0,3 g; Mg- 1,3 g; S- 0,5 g; B-18,5 mg; Cu-16,2 mg; Fe- 36,9 mg; Mn-13,2 mg. Finalmente observaram que a concentração de N, Mg, S, Cu, Fe e Zn não varia nas folhas em função da idade da planta; sendo que a concentração em P, K, Ca, B e Mn varia com a idade da planta.

São relatadas as observações a respeito dos hábitos de defecação e de sucção em 6 espécies de hemípteros hematófagos. O quadro abaixo reúne os resultados mais gerais obtidos, apreciados em conjunto: Espécies de exemplares utilizados; % de insetos que defecaram durante ou logo após a picada; N.º médio de defecações nas 3 primeiras horas após o repasto; Duração média de sucção (minutos); % de insetos que sugaram sem interromper a picada. R. prolixus 2 machos, 8 fêmeas e 10 ninfas (50,0; 13,7; 14,2; 20,0). R. neglectus 2 machos, 8 fêmeas e 10 ninfas (30,0; 9,6; 18,5; 20,0). T. infestans 2 machos, 11 fêmeas e 20 ninfas (30,0; 7,1; 15,5; 47,5). P. megistus 6 machos, 10 fêmeas e 12 ninfas (22,7; 3,4; 22,7; 82,1). T. sordida 13 machos e 11 ninfas (12,5; 4,5; 20,0; 79,2). T. vitticeps 11 ninfas (0; 6,2; 26,8; 90,9). De modo geral, a capacidade de defecar no ato da picada mostrou-se diretamente proporcional ao número de defecações (nas três primeiras horas) e à freqüência das interrupções da picada, e inversamente proporcional ao tempo de duração da sucção. Os barbeiros adultos se mostraram mais aptos a defecar no ato da picada do que em fase de ninfa, as fêmeas mais do que os machos. Das espécies observadas, o R. prolixus foi a que melhores condições demonstrou para realizar a contaminação fecal do hospedeiro vertebrado. Sugere-se que um estudo mais aprofundado sôbre a eficácia contaminativa dos diversos transmissores do S. cruzi seja feito de preferência no verão e à noite, em zonas onde grassa endêmicamente a doença de Chagas.

Neste trabalho é feita a descrição anatômica e histológica do aparelho genital de Succiena meridionalis Orbigy, 1846, considerando-se também a concha, mandíbula, rádula e câmara paleal. As variações da concha, rádula e "talon", bem como a intensidade de pigmentação de diversas estruturas também são discutidas. Além dêstes aspectos, são relatados alguns dados biológicos da espécie e disposição anatômica do aparelho copulador por ocasião do ato sexual. Como caracteres importantes para diagnose desta espécie, dentro do gênero, consideramos: 1 - Conchas geralmente translúcidas e frágeis, como comprimento, nos adultos, variando de 6,0 a 13,1 mm e número de espiras compreendida entre 2 1/6 e 3 1/6. 2 - Ovotestis constituído por numerosos folículo reunidos num só grupo. 3 - Canal coletor do ovotestis unido à vesícula seminal subterminalmente. 4 - Vagina curta e mais dilatada na porção proximal, podendo formar um ângulo reto ou agudo com o oviduto. 5 - Bainha muscular do pênis envolvendo a porção distal do falus e sendo atravessada subrterminalmente pelo canal deferente. 6 - Porção proximal do falus bem dilatada, tendo dobrada contra si o epifalus. 7 - Presença de uma papila na comunicação do epifalus com a porção proximal do falus e que se projeta para a luz dêste último. 8 - Inserção terminal do músculo retrtor no epifalus.

Indices pulicidianos, anoplurianos e acarianos, globais e específicos foram determinados para os ectoparasitos de Rattus norvegicus norvegicus capturados em zona urbana de Belo Horizonte, Minas Gerais, Brasil, no período de junho de 1980 a setembro de 1982. Tendo-se em vista os valores limites ou críticos atribuídos aos índices pulicidianos, sobretudo ao índice "cheopis" e propostos por diversos autores como medida complementar de vigilância epidemiológica para peste bubônica, a comunidade de Belo Horizonte poderia ter estado exposta a esta infecção, uma vez que os índices globais anuais de 0,3 a 2,4 e a pulga prevalente foi Xenopsylla cheopis (99,2%), com os maiores índices coincidindo com o final da estação seca-fria. Em duas ocasiões, a comunidade poderia ter permanecido altamente exposta à infecção, já que os índices-limites tolerados foram suplantados: 8,8 (outubro 1980) e 6,2 (setembro 1982). Sugere-se que medidas profiláticas como anti-ratização e desinsetização sejam eficazmente aplicadas ao final da estação seca-fria, ou anteriormente à chegada das chuvas, sendo sucedidas pela desratização. Eimeria lagunculata, Eimeria mammiformis and Eimeria podocnemis n. spp., are described from the faeces of the fresh-water turtle Podocnemis expansa, in Pará State, north Brasil. Oocysts of E. lagunculata are ellipsoidal, 19.2 x 12.8 (17.0-20.7 x 11.8-14.1) mum, shape-index (= length/ width) 1.5 (1.4-1.7). Oocyst wall about 0.5-0.7 mum thick, with a prominent stopper-like micropyle at one pole. No oocyst residuum and no polar body. Sporocysts elongate ellipsoidal, 11.0 x 5.4 (10.4-11.8 x 5.2-6.0) mum, shape-index 2.0 (1.8-2.1): no Stieda body. A compact, ellipsoidal sporocyst residuum lies between the two sporozoites, which possess a posterior and an anterior refractile body. Oocysts of E. mammiformis broadly ellipsoidal, 30.0 x 19.4 (23.0-37.0 x 16.3-21.5) mum, shape-index 1.5 (1.1-1.9). Oocyst wall about 0.7 mum thick, with a prominent micropyle: no oocyst residuum and rarely a single polar body. Sporocysts ellipsoidal, 15.3 x 7.9 (14.8-17.0 x 7.4-9.6) mum, shape-index 2.0 (1.8-2.2), with a tiny Stieda body. Sporocyst residuum bulky, ellipsoidal: sporozoites with two conspicuous refractile bodies. E. podocnemis has broadly ellipsoidal oocysts, 17.0 x 12.8 (14.8-19.2 x 11.8-14.1) mum, shape-index 1.3 (1.1-1.4). Oocyst wall about 0.5-0.7 mum thick, with no micropyle. No oocyst residuum, but always a single polar body. Sporocysts ellipsoidal, 9.7 x 5.2 (8.9-10.4 x 4.4-6.0) mum, shape-index 1.9 (1.6-2.0), with no Stieda body. Sporocyst residuum bulky, ellipsoidal: sporocysts with 2 refractile bodies. Eimeria carinii n. sp., is recorded from the tortoise Geochelone denticulata, also from Pará. Oocyst wall about 1.2 mum thicl. No micropyle. Oocyst residuum limited to a number (about 10-20) of scattered granules: no polar body. Sporocysts broadly ellipsoidal, and with no Stieda body: they measure 8,8 x 7.3 (8.0-9.0 x 7.0-7.5) mum, shape-index 1.2 (1.1-1.3). Sporocyst residuum bulky, spherical to ellipsoidal: sporozoites possess both posterior and anterior refractile bodies.

Two kinds of small extrachromosomal nucleic acid elements were found in the bovine babesias, Babesia bovis and B. bigemina. One element with an apparent size of 5.5 kilobase pairs (kbp) is a double stranded RNA related to virus like particles. Another molecule is a double stranded DNA with a molecular size of about 6.2 kbp. Southern blot comparison of restriction DNA fragments of the latter molecule, which is present in both B. bovis and B. bigemina is described.

The larval and pupal stages of Anopheles (Nyssorhynchus) rondoni (Neiva and Pinto) (Albimanus Section) are fully described and illustrated for the first time. The larval stage is similar to An. (Nys.) strodei Root. It can be recognized by the following combination of characters: clypeal index, 1.6-2.9; single, aciculate setae 2,3-C; seta 3-C 0.5-0.7 the length of 2-C; setae 1,2-P rarely sharing a common tubercle; seta 1-P with narrow leaflets. The pupal stage is distinguished from other Nyssorhynchus by having setae 1-IV-VII and 5-V-VII branched. Only minor variation was found in setal counts between specimens from Peixoto de Azevedo, State of Mato Grosso and Bocaina, State of São Paulo, Brazil

The presence of Vibrio cholerae non-O1 in water supplies for human consumption in the city of Campeche and rural locality of Bécal was investigated. V. cholerae non-O1 was detected in 5.9% of the samples obtained in deep pools of Campeche. Studies conducted in Bécal and neighbourhood of Morelos in Campeche indicated that collected samples harbored V. cholerae non-O1 in 31.5% and 8.7% respectively. There was a particular pattern of distribution of V. cholerae non-O1 serotypes among different studied regions. Accordingly, V. cholerae non-O1 serotype O14 predominated in the deep pools of Campeche and together with V. cholerae non-O1, O155 were preferentially founds in samples taken from intradomiciliary faucets in the neighbourhood of Morelos. Samples from Bécal predominantly presented the serotype O112. 60% and 53.8% of all studied strains of V. cholerae non-O1 proved to be resistant to ampicillin and carbenicillin. 3.1%, 7.7% and 6.2% presented resistant to doxycycline, trimethoprim-sulfamethoxazole and erythromycin respectively. The oocyst is described of Eimeria peltocephali n.sp. from faeces of the freshwater turtle Peltocephalus dumerilianus from Barcelos, State of Amazonas, Brazil. Sporulation is exogenous and fully developed oocysts are elongate, ellipsoidal or cylindrical, frequently curved to a banana-shape, 54.4 x19.1 (37.5 - 68.7 x 18.7-20.0 µm), shape-index 2.8 (1.8 -3.9). The oocyst wall is a single thin, colourless layer about 1 µm thick, with no micropyle. There is a bulky oocyst residuum, at first spherical to ellipsoidal, 19 x 16 (16. 2 -26.2 x 16 - 21.5µm) , but becoming dispersed on maturation. There are no polar bodies. The sporocysts, 19.1 x 6.8 ( 17.5 -21.2 x 6.2 -7.5 µm), shape- index 2.8 (2.3 -3.2), are usually disposed in pairs at each end of the oocyst, and bear an inconspicuous Stieda body in the form of a flat cap. The sporozoites are elongate and slightly curved around the residuum. No refractile bodies were seen. Eimeria molossi n.sp., is described from the molossid bat Molossus ater. Sporulation is exogenous and the mature oocysts are predominantly broadly ellipsoidal, 23.4 x 17.5 (18-30 x 15-22.5 µm), shape-index 1.3 (1-1.6). The oocyst wall is about 2 µm thick, and of three layers: an inner thin, colourless one and two outer layers which are thicker, yellowish-brown, prominently striated and in close apposition. There is no micropyle or oocyst residuum, but one and occasionally two polar bodies are usually present. Sporocysts are ellipsoidal, 10.2 x 7.5 (10-12.5 x 7.5 µm), shape-index 1.4 (1.3-1.7) with an inconspicuous Stieda body. Endogenous stages are described in the epithelial cells of the small intestine
