PURPOSE: To evaluate whether climacteric women undergoing liver transplantation had higher prevalence of decreased bone mass than those without any liver disease. METHODS: A cross-sectional study with 48 women receiving follow-up care at a university hospital in Southeastern Brazil, from February 4th 2009 to January 5th 2011, was conducted. Of these women, 24 were 35 years or older and had undergone liver transplantation at least one year before study entry. The remaining 24 women had no liver disease and their ages and menstrual patterns were similar to those of transplanted patients. Laboratorial tests (follicle-stimulating hormone and estradiol) and bone density measurements of the lumbar spine and femur (equipment Hologic, Discovery WI) were performed. Statistical analysis was carried out by Fisher's exact test, simple Odds Ratio (OR), and multiple logistic regression. RESULTS: Mean age of the women included in the study was 52.8 (±10.7) years-old, 27.1% were premenopausal and 72.9% were peri/postmenopausal. Approximately 14.6% of these women exhibited osteoporosis and 35.4% had low bone mass. The following items were associated with decreased bone mass: being postmenopausal (OR=71.4; 95%CI 3.8 - 1,339.7; p<0.0001), current age over 49 years-old (OR=11.4; 95%CI 2.9 - 44.0; p=0.0002), and serum estradiol levels lower than 44.5 pg/mL (OR=18.3; 95%CI 3.4 - 97.0; p<0.0001). Having a history of liver transplantation was not associated with decreased bone mass (OR=1.4; 95%CI 0.4 - 4.3; p=0.56). CONCLUSION: Liver transplantation was not associated with decreased bone mass in this group of climacteric women.

OBJETIVO: Avaliar a associação entre sintomas clínicos e malignidade em mulheres com tumores anexiais, submetidas à cirurgia. MÉTODOS: Estudo de corte transversal com coleta prospectiva, no qual foram incluídas 105 mulheres, atendidas em um hospital de ensino do Estado de São Paulo de novembro de 2009 a março de 2011, devido ao tumor anexial e à indicação de laparotomia/laparoscopia. Todas foram submetidas a uma entrevista estruturada sobre a ocorrência de 18 sintomas associados ao câncer de ovário. A entrevista incluiu gravidade, frequência e duração dos sintomas nos 12 meses prévios à primeira consulta. Também foram avaliados os níveis de CA125 e a classificação ultrassonográfica. Foi calculada para cada sintoma a razão de prevalência com intervalo de confiança de 95%. O padrão-ouro foi o resultado do exame anatomopatológico das peças cirúrgicas. RESULTADOS: Das 105 mulheres incluídas, 75 (71,4%) apresentaram tumores benignos e 30 (28,6%), malignos. Em mulheres com tumores malignos, os sintomas foram mais frequentes, dentre eles: inchaço abdominal (70%), aumento do volume abdominal (67%), dor pélvica (60%), irregularidade menstrual (60%), empachamento (53%), dor abdominal (50%), dor nas costas (50%) e saciedade precoce (50%). As mulheres com tumores benignos apresentaram essencialmente dor pélvica (61%), irregularidade menstrual (61%) e inchaço abdominal (47%). Os sintomas significativamente associados com malignidade foram: sensação de inchaço abdominal (RP=2,0; IC95% 1,01 - 3,94), aumento objetivo do volume abdominal (RP=2,16; IC95% 1,12 - 4,16), dor nas costas (RP=1,97; IC95% 1,09 - 3,55), empachamento (RP=2,25; IC95% 1,25 - 4,07), saciedade precoce (RP=2,06; IC95% 1,14 - 3,70), massa abdominal (RP=1,83; IC95% 1,01 - 3,30), dificuldade para deglutir (RP=1,98; IC95% 1,10 - 3,56) e sangramento pós-menopausa (RP=2,91; IC95% 1,55 - 5,44). A presença de dor pélvica, constipação, dispareunia, fadiga, dor abdominal, náusea e/ou vômito, irregularidade menstrual, perda de peso, diarreia e sinusorragia foram semelhantes nos dois grupos. CONCLUSÕES: Em mulheres com tumores anexiais com indicação cirúrgica, a avaliação pré-operatória dos sintomas pode auxiliar na predição da malignidade.

PURPOSE: To assess quality of life and climacteric symptoms in women with and without liver transplants. METHODS: This was a cross-sectional study of 52 women undergoing follow-up at a university hospital in southeastern Brazil from February 4th, 2009 to January 5th, 2011. Twenty-four of these women were 35 years old or older and had undergone liver transplantation at least one year before study entry. The remaining 28 women had no liver disease and were matched by age and menstrual patterns to the patients with transplants. The abbreviated version of the World Health Organization (WHOQOL-BREF) questionnaire was used to assess quality of life. Menopausal symptoms were assessed using the Menopause Rating Scale (MRS). Statistical analysis was carried out by Student's t-test, Mann-Whitney test and analysis of variance. Correlations between MRS and the WHOQOL-BREF were established by correlation coefficients. RESULTS: The mean age of the women included in the study was 52.2 (±10.4) years and the mean time since transplantation was 6.1 (±3.3) years. Women with liver transplants had better quality of life scores in the environment domain (p=0.01). No difference was noted between the two groups in any domain of the MRS. For women in the comparison group, there was a strongly negative correlation between somatic symptoms in the MRS and the physical domain of the WHOQOL-BREF (p<0.01; r=-0.8). In contrast, there was only a moderate association for women with liver transplants (p<0.01; r=-0.5). CONCLUSIONS: Women with liver transplants had better quality of life scores in the domain related to environment and did not exhibit more intense climacteric symptoms than did those with no liver disease. Climacteric symptoms negatively influenced quality of life in liver transplant recipients, although less intensely than in women without a history of liver disease.

OBJETIVO: Avaliar os parâmetros clínicos, ultrassonográficos, bioquímicos e as alterações metabólicas em adolescentes com síndrome dos ovários policísticos (SOP). MÉTODOS: Estudo retrospectivo realizado com 44 adolescentes entre 12 e 19 anos, com diagnóstico de SOP pelo Consenso de Rotterdam. As alterações metabólicas foram avaliadas de acordo com as recomendações da Federação Internacional de Diabetes, sendo consideradas: circunferência da cintura (CC) >percentil 90 (10-15 anos de idade) ou >80 cm (idade >16 anos); glicemia de jejum >100 mg/dL; triglicerídios >150 mg/dL; HDL <40 mg/dL, e pressão arterial >Hg 130/85 mm. RESULTADOS: A média de idade foi de 16,7±2,2 anos e da idade da menarca 11,8±1,4 anos. A irregularidade menstrual mais observada foi amenorreia (72,7%) seguida de oligomenorréia (27,3%); hirsutismo foi observado em 86,4% e acne em 56,8%. Ovários policisticos ao ultrassom observados apenas em 27,3%. A média do IMC foi de 30,3±6,6 kg/m². De acordo com o IMC, 52,3% das adolescentes eram obesas, 13,6% estavam com sobrepeso e 6,8% eram eutróficas. O aumento da circunferência da cintura (63,6%, 28/44) e a redução do HDL-C (34,1%, 15/44) foram as alterações metabólicas mais observadas. Triglicerídios aumentados foram observados em 27,3% (12/44), pressão arterial e aumento da glicemia de jejum alterada foram encontrados em 9,1% (4/44) e 4,5% (2/44) dos casos, respectivamente. Acantosis nigricans foi observada em 52,3% das adolescentes com SOP e a resistência insulinica encontrada em 62,8%. A sindrome metabólica foi identificada em seis adolescentes (13,6%), sendo todas obesas ou com sobrepeso. CONCLUSÃO: Entre as adolescentes com SOP do estudo, a irregularidade menstrual e o hirsutismo são as manifestações clínicas mais frequentes, enquanto os achados ultrassonográficos compatíveis com ovários policísticos são os menos prevalentes. A obesidade associada à resistência à insulina predispõe estas adolescentes à maior frequência de alterações metabólicas.

OBJETIVO: Caracterizar e comparar variáveis clínicas, antropométricas e bioquímico-metabólicas de pacientes com síndrome dos ovários policísticos (SOP), estratificadas segundo o índice de massa corpórea (IMC). MÉTODOS: Estudo transversal com 78 mulheres entre 18 e 45 anos com diagnóstico de SOP, pelos Critérios de Rotterdam. As pacientes foram estratificadas segundo o IMC. As variáveis analisadas foram: idade, estado civil, sedentarismo, irregularidade menstrual, pressão arterial (PA), medidas antropométricas, perfil lipídico, glicemia em jejum e dosagens hormonais. Para comparar as variáveis analisadas entre os diferentes IMC, usou-se a Análise de Variância e o Teste de Kruskal-Wallis. O nível de significância para todos os testes foi de 5%. RESULTADOS: As pacientes apresentaram média de idade de 26,3 anos, sendo 79,5% classificadas como sedentárias e 68% com hiperandrogenismo. A circunferência da cintura, a Razão cintura/quadril, a Razão cintura/estatura e a porcentagem de gordura corporal foram maiores no grupo de obesas. A presença de marcadores de risco cardiovascular (RCV - glicemia de jejum, PA sistólica e diastólica e LDL-colesterol) foi diretamente proporcional ao IMC, enquanto que os níveis de HDL-colesterol e SHBG foram inversamente proporcionais ao IMC. CONCLUSÃO: A presença de marcadores de RCV aumentou proporcionalmente ao IMC, evidenciando que o perfil metabólico das mulheres obesas com SOP é mais desfavorável do que n não obesas.

OBJETIVOS: Avaliar a qualidade de vida das mulheres com síndrome dos ovários policísticos (SOP) e compreender a experiência vivida por essas mulheres diante dos sintomas que apresentam. MÉTODOS: Este estudo compreendeu duas abordagens metodológicas - quantitativa e qualitativa, de forma complementar. Foi avaliada a qualidade de vida de 213 mulheres (abordagem quantitativa) por meio do SF-36, sendo 109 com SOP (Grupo Caso: 26,8±5,4 anos) e 104 mulheres saudáveis (Grupo Controle: 23,9±6,7 anos). A análise estatística compreendeu a utilização dos testes t de Student e qui-quadrado, além dos testes de correlação de Pearson. O nível de significância adotado foi de 5%. Das mulheres do Grupo SOP, 15 participaram do estudo qualitativo, tendo sido entrevistadas mediante uso de roteiro semiestruturado. Os dados qualitativos foram analisados por meio da técnica análise de conteúdo temática categorial. RESULTADOS: Mulheres com SOP apresentaram comprometimento na qualidade de vida quando comparadas ao Grupo Controle (capacidade funcional: 76,5±20,5 e 84,6±15,9, respectivamente; aspectos físicos: 56,4±43,3 e 72,6±33,3; estado geral de saúde: 55,2±21,0 e 62,5±17,2; vitalidade: 49,6±21,3 e 55,3±21,3; aspectos sociais: 55,3±32,4 e 66,2±26,7; aspectos emocionais: 34,2±39,7 e 52,9±38,2; saúde mental: 50,6±22,8 e 59,2±20,2). Em relação aos dados qualitativos, a análise temática categorial aponta que sentimentos de "anormalidade", tristeza, medo e ansiedade estiveram associados aos principais sintomas da SOP: hirsutismo, irregularidade menstrual, infertilidade e obesidade. Esses sintomas repercutiram na vida social, na esfera profissional e no relacionamento conjugal dessas mulheres. CONCLUSÃO: A SOP compromete a qualidade de vida das mulheres, levando-as a se sentirem diferentes das outras mulheres. Por causa disso, a mulher com SOP não necessita apenas de tratamento médico para as repercussões reprodutivas, estéticas e metabólicas, mas de atendimento multiprofissional.

PURPOSE: To compare salivary and serum cortisol levels, salivary alpha-amylase (sAA), and unstimulated whole saliva (UWS) flow rate in pregnant and non-pregnant women. METHOD: A longitudinal study was conducted at a health promotion center of a university hospital. Nine pregnant and 12 non-pregnant women participated in the study. Serum and UWS were collected and analyzed every trimester and twice a month during the menstrual cycle. The salivary and serum cortisol levels were determined by chemiluminescence assay and the sAA was processed in an automated biochemistry analyzer. RESULTS: Significant differences between the pregnant and non-pregnant groups were found in median [interquartile range] levels of serum cortisol (23.8 µL/dL [19.4-29.4] versus 12.3 [9.6-16.8], p<0.001) and sAA (56.7 U/L [30.9-82.2] versus 31.8 [18.1-53.2], p<0.001). Differences in salivary and serum cortisol (µL/dL) and sAA levels in the follicular versus luteal phase were observed (p<0.001). Median UWS flow rates were similar in pregnant (0.26 [0.15-0.30] mL/min) and non-pregnant subjects (0.23 [0.20-0.32] mL/min). Significant correlations were found between salivary and serum cortisol (p=0.02) and between salivary cortisol and sAA (p=0.01). CONCLUSIONS: Serum cortisol and sAA levels are increased during pregnancy. During the luteal phase of the ovarian cycle, salivary cortisol levels increase, whereas serum cortisol and sAA levels decline.

OBJETIVO: Avaliar os resultados reprodutivos após septoplastia histeroscópica.MÉTODOS: Estudo retrospetivo observacional por meio de análise dos registos clínicos de 28 mulheres com antecedentes de infertilidade ou de abortos recorrentes, submetidas à septoplastia histeroscópica. Para a avaliação do desfecho reprodutivo foram consultados os registos informáticos dos Cuidados de Saúde Primários e do nosso Centro Hospitalar, no período compreendido entre a septoplastia e a primeira gravidez ou o presente. Os outcomes primários foram a taxa de gravidez, de recém-nascidos e de abortos após septoplastia. O septo uterino foi diagnosticado por ecografia 2D ou 3D e classificado de acordo com a classificação da American Fertility Society. A septoplastia foi realizada na fase folicular do ciclo menstrual, com recurso da energia monopolar, bipolar e/ou microtesoura. Os programas Microsoft Excel e SPSS versão 17 foram utilizados para comparação do desfecho reprodutivo prévio e posterior à septoplastia. Foi utilizado o teste exato de Fisher, considerando significado estatístico quando p<0,05.RESULTADOS: Foi realizada septoplastia histeroscópica em 20 doentes (72%) com infertilidade secundária e 8 (28%) com infertilidade primária, tendo sido necessária segunda intervenção para remoção completa do septo em 5 casos (18%). Ocorreu um caso perfuração uterina minor. Após septoplastia histeroscópica, 64% das mulheres engravidaram, obtendo-se uma taxa de nados vivos de 48%; gravidez tubária de 4%; e 19% das doentes tiveram abortos espontâneos.CONCLUSÕES: Os resultados do presente estudo estão de acordo com o descrito na literatura, tendo-se obtido uma melhoria significativa dos desfechos reprodutivos, com uma redução da taxa de aborto espontâneo de cinco vezes após a septoplastia histeroscópica.

OBJETIVO: Avaliar a resposta ovariana após uso de ciclofosfamida (CFM) em pacientes com lúpus eritematoso sistêmico (LES) e correlacionar os achados de tempo de doença e idade no período de utilização de CFM e dose cumulativa com alterações no ciclo menstrual e/ou evolução para insuficiência ovariana (IO). MÉTODOS: Foi um estudo transversal, retrospectivo, com 50 pacientes com diagnóstico de LES e que fizeram tratamento com CFM com seguimento clínico de, pelo menos, 1 ano. Foram incluídas pacientes com idade entre 12 e 40 anos e que apresentavam ciclos menstruais regulares prévios ao tratamento. Foram excluídas pacientes que descontinuaram o seguimento, ou este foi menor do que um ano, além daquelas que apresentaram irregularidade/ausência menstrual antes do uso do fármaco. Todas as mulheres estudadas foram submetidas à entrevista e à aplicação de questionário. Neste foram abordadas questões relevantes de padrão de ciclo menstrual antes e posterior à terapia, assim como períodos gestacionais e método contraceptivo. Foi questionado se as pacientes foram orientadas sobre os efeitos colaterais e as consequências da CFM. Para análise estatística, foram utilizados os testes t de Student, Mann-Whitney, do χ2 e o não paramétrico de Kolmogorov-Smirnov. RESULTADOS: A média de idade das pacientes incluídas no do estudo foi de 30,8 anos, e a média de idade no momento do uso de CFM, de 25,3 anos. Após a CFM, 24% das pacientes não menstruaram mais, 28% voltaram a ter ciclos regulares e 48% delas permaneceram com ciclos irregulares. Verificou-se que as pacientes que evoluíram com falência ovariana tinham maior tempo de doença (12,3 anos) do que aquelas que não evoluíram (8,9 anos). Treze pacientes tiveram gestação após a CFM, em todas ocorreu de forma espontânea; no entanto, 66% evoluíram com abortamento. A média de idade das pacientes que fizeram uso de CFM e evoluíram com falência ovariana foi de 28,1 anos. A amenorreia ocorreu em 50% das pacientes que tinham idade entre 31 e 40 anos, 22,2%, entre 21 e 30 anos, e 7,7%, entre 12 e 20 anos. Nosso estudo não mostrou correlação estatística entre dose cumulativa e falência ovariana, apesar de que doses cumulativas maiores que 11 g tendem a promover algum tipo de irregularidade menstrual. CONCLUSÃO: O tempo de doença do LES, a idade no momento do tratamento e as maiores doses cumulativas são importantes fatores preditores da IO pós-terapia com CFM. Gestação em pacientes lúpicas após o uso do quimioterápico tem maiores chances de evoluir com abortamento. Viu-se que pequena parte das doentes estava ciente de todas as repercussões que a droga poderia gerar. Por isso, novos estudos devem ser realizados para o conhecimento e a conscientização, por parte do meio médico, sobre a importância da anticoncepção e da preservação do tecido ovariano.

Endometrium is one of the fastest growing human tissues. Sex hormones, estrogen and progesterone, in interaction with several growth factors, control its growth and differentiation. Insulin-like growth factor 1 (IGF-1) interacts with cell surface receptors and also with specific soluble binding proteins. IGF-binding proteins (IGF-BP) have been shown to modulate IGF-1 action. Of six known isoforms, IGF-BP-1 has been characterized as a marker produced by endometrial stromal cells in the late secretory phase and in the decidua. In the current study, IGF-1-BP concentration and affinity in the proliferative and secretory phase of the menstrual cycle were measured. Endometrial samples were from patients of reproductive age with regular menstrual cycles and taking no steroid hormones. Cytosolic fractions were prepared and binding of 125I-labeled IGF-1 performed. Cross-linking reaction products were analyzed by SDS-polyacrylamide gel electrophoresis (7.5%) followed by autoradiography. 125I-IGF-1 affinity to cytosolic proteins was not statistically different between the proliferative and secretory endometrium. An approximately 35-kDa binding protein was identified when 125I-IGF-1 was cross-linked to cytosol proteins. Secretory endometrium had significantly more IGF-1-BP when compared to proliferative endometrium. The specificity of the cross-linking process was evaluated by the addition of 100 nM unlabeled IGF-1 or insulin. Unlabeled IGF-1 totally abolished the radioactivity from the band, indicating specific binding. Insulin had no apparent effect on the intensity of the labeled band. These results suggest that IGF-BP could modulate the action of IGF-1 throughout the menstrual cycle. It would be interesting to study this binding protein in other pathologic conditions of the endometrium such as adenocarcinomas and hyperplasia.

Human and animal immune functions present sex dimorphism that seems to be mainly regulated by sex hormones. In the present study, the activities of the antioxidant enzymes total superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase (GSH-Px) were measured in intraperitoneal resident macrophages from adult male and female rats. In addition to comparing males and females, we also examined the regulation of these enzyme activities in macrophages by sex steroids. GSH-Px activity did not differ between male and female macrophages. However, both total SOD and CAT activities were markedly higher in females than in males (83 and 180%). Removal of the gonads in both males and females (comparison between castrated groups) increased the difference in SOD activity from 83 to 138% and reduced the difference in CAT activity from 180 to 86%. Castration and testosterone administration did not significantly modify the activities of the antioxidant enzymes in male macrophages. Ovariectomy did not affect SOD or GSH-Px activity but markedly reduced (48%) CAT activity. This latter change was fully reversed by estrogen administration, whereas progesterone had a smaller effect. These results led us to conclude that differences in the SOD and CAT activities may partially explain some of the differences in immune function reported for males and females. Also, estrogen is a potent regulator of CAT in macrophages and therefore this enzyme activity in macrophages may vary considerably during the menstrual cycle.

The aim of the present study was to identify the risk factors for ovarian failure in patients with systemic lupus erythematosus. Seventy-one women aged 17 to 45 years with systemic lupus erythematosus were studied. Patients were interviewed and their medical records reviewed. Demographic characteristics, clinical and serologic profiles, and menstrual and obstetric histories were recorded. Disease activity was measured by the systemic lupus erythematosus disease activity index. Serum FSH, LH, estradiol, progesterone, TSH, prolactin, and antimicrosomal and antithyroglobulin antibodies were measured. Patients who developed ovarian failure were compared to those who did not. Ovarian failure occurred in 11 patients (15.5%) and nine had premature menopause (11.3%). Cyclophosphamide administration and older patient age were found to be associated with ovarian failure. The cumulative cyclophosphamide dose was significantly higher in patients with ovarian failure than in those without this condition (18.9 vs 9.1 g; P = 0.04). The relative risk for ovarian failure in patients with cumulative cyclophosphamide dose higher than 10 g was 3.2. TSH levels were high in 100% of patients with ovarian failure who had received pulse cyclophosphamide. Ovarian failure, and premature menopause in particular, is common in patients with systemic lupus erythematosus, with the most important risk factors being cyclophosphamide dose and age. Thyroid problems may be another risk factor for ovarian failure in patients with lupus.

Estrogen has multiple effects on lipid and lipoprotein metabolism. We investigated the association between the four common single nucleotide polymorphisms in the estrogen receptor 1 (ESR1) gene locus, -1989T>G, +261G>C, IVS1-397T>C and IVS1-351A>G, and lipid and lipoprotein levels in southern Brazilians. The sample consisted in 150 men and 187 premenopausal women. The women were considered premenopausal if they had regular menstrual bleeding within the previous 3 months and were 18-50 years of age. Exclusion criteria were pregnancy, secondary hyperlipidemia due to renal, hepatic or thyroid disease, and diabetes. Smoking status was self-reported; subjects were classified as never smoked and current smokers. DNA was amplified by PCR and was subsequently digested with the appropriate restriction enzymes. Statistical analysis was carried out for men and women separately. In the study population, major allele frequencies were _1989*T (0.83), +261*G (0.96), IVS1-397*T (0.58), and IVS1-351*A (0.65). Multiple linear regression analyses indicated that an interaction between +261G>C polymorphism and smoking was a significant factor affecting high-density lipoprotein cholesterol (HDL-C) levels (P = 0.028) in women. Nonsmoking women with genotype G/C of +261G>C polymorphism had mean HDL-C levels higher than those with G/G genotype (1.40 ± 0.33 vs 1.22 ± 0.26 mmol/L; P = 0.033). No significant associations with lipid and lipoprotein levels in women and men were detected for other polymorphisms. In conclusion, the +261G>C polymorphism might influence lipoprotein and lipid levels in premenopausal women, but these effects seem to be modulated by smoking, whereas in men ESR1 polymorphisms were not associated with high lipoprotein levels.

Adipose tissue secretes a variety of adipokines, including leptin and adiponectin, which are involved in endocrine processes regulating glucose and fatty metabolism, energy expenditure, inflammatory response, immunity, cardiovascular function, and reproduction. The present article describes the fluctuations in circulating leptin and adiponectin as well as their patterns of secretion in women from birth to menopause. During pregnancy, leptin and adiponectin seem to act in an autocrine/paracrine fashion in the placenta and adipose tissue, playing a role in the maternal-fetal interface and contributing to glucose metabolism and fetal development. In newborns, adiponectin levels are two to three times higher than in adults. Full-term newborns have significantly higher leptin and adiponectin levels than preterms, whereas small-for-gestational-age infants have lower levels of these adipokines than adequate-for-gestational-age newborns. However, with weight gain, leptin concentrations increase significantly. Children between 5 and 8 years of age experience an increase in leptin and a decrease in adiponectin regardless of body mass index, with a reversal of the newborn pattern for adiponectin: plasma adiponectin levels at age five are inversely correlated with percentage of body fat. In puberty, leptin plays a role in the regulation of menstrual cycles. In adults, it has been suggested that obese individuals exhibit both leptin resistance and decreased serum adiponectin levels. In conclusion, a progressive increase in adiposity throughout life seems to influence the relationship between leptin and adiponectin in women.

The female brain operates in a constantly changing chemical milieu caused by cyclical changes in gonadal hormones during the estrous cycle (menstrual cycle in women). Such hormones are highly lipophilic and pass readily from the plasma to the brain where they can influence neuronal function. It is becoming clear that the rapid reduction in peripheral circulating progesterone, which occurs during the late diestrous phase of the cycle, can trigger a withdrawal-like response, in which changes in GABA A receptor expression render hyper-responsive certain brain areas involved in processing responses to stressful stimuli. The periaqueductal gray matter (PAG) is recognised as an important region for integrating anxiety/defence responses. Withdrawal from progesterone, via actions of its neuroactive metabolite allopregnanolone, triggers up-regulation of extrasynaptic GABA A receptors on GABAergic neurons in the PAG. As a consequence, ongoing GABAergic tone on the output cells decreases, leading to an increase in functional excitability of the circuitry and enhanced responsiveness to stressful stimuli during the late diestrous phase. These changes during late diestrus could be prevented by short-term neurosteroid administration, timed to produce a more gradual fall in the peripheral concentration of allopregnanolone than the rapid decrease that occurs naturally, thus removing the trigger for the central withdrawal response.
