OBJETIVO: Observar, num grupo de pacientes na forma indeterminada da doença de Chagas, o aparecimento de doenças cardiovasculares e sua possível relação com a doença de base. MÉTODOS: Foram seguidos, prospectivamente, 160 pacientes por até 177 meses com avaliações clínicas trimestrais. RESULTADOS: Tornaram-se hipertensos 23 (14,4%) pacientes, sendo 21 (13,2%) com pressão arterial diastólica <110mmHg. Duas pacientes (1,2%) hipertensas tiveram acidente vascular cerebral isquêmico (AVCI). Uma (0,6%) paciente teve hemorragia meníngea por ruptura de aneurisma cerebral. Quatro (2,4%) pacientes apresentaram arritmia clinicamente, dois (1,2%) extra-sístoles ventriculares, um (0,6%) extra-sístoles supraventriculares e um (0,6%) fibrilação atrial aguda. Dois (1,2%) pacientes desenvolveram coronariopatia comprovada angiograficamente, um (0,6%) com infarto agudo do miocárdio, outro com angina estável. Um (0,6%) paciente desenvolveu sinais e sintomas de insuficiência cardíaca, juntamente com o aparecimento de hipertensão arterial sistêmica (HAS). CONCLUSÃO: A doença cardiovascular mais freqüente foi a HAS. Duas hipertensas apresentaram AVCI. As arritmias observadas não foram mais freqüentes que na população normal e a coronariopatia também ocorreu raramente, confirmando um bom prognóstico clínico a longo prazo desse grupo de pacientes.

OBJETIVO: Determinar o índice de sucesso e complicações das intervenções coronárias realizadas em hospital geral do Piauí, em um laboratório de hemodinâmica com baixo volume de procedimentos por ano. MÉTODOS: Cento e vinte e dois pacientes, submetidos a 146 procedimentos de intervenção coronária, de agosto/91 a janeiro/98 tiveram seus dados clínicos e angiográficos analisados retrospectivamente. As cineangiocoronariografias e as intervenções foram realizadas em aparelho com sistema de fluoroscopia com câmara e monitor de TV de 525 linhas. As variáveis analisadas foram o sucesso inicial do procedimento (estenose residual <50% e fluxo TIMI III) e as complicações maiores (oclusão aguda, infarto, cirurgia de urgência e óbito). Foram revisados os dados clínicos e as características morfológicas das lesões (ACC/AHA Task Force). RESULTADOS: A média etária foi de 61 anos, variando de 25 a 85, 67% dos pacientes eram do sexo masculino, 85% apresentavam síndrome isquêmica aguda (infarto agudo do miocárdio, angina pós infarto ou angina de repouso), 5% apresentavam-se em choque cardiogênico. 88% das lesões eram tipo B. O índice de sucesso imediato foi de 93%. A taxa de complicações maiores (oclusão aguda, cirurgia de urgência, infarto e óbito) foi de 3,5%. CONCLUSÃO: A despeito de um baixo volume de procedimentos por ano e um equipamento de cineangiocoronariografia convencional, o índice de sucesso desta instituição foi excelente em uma população de pacientes diversa e complexa com taxa de complicações semelhante à encontrada na literatura.

OBJECTIVE: To analyze the initial clinical experience of transmyocardial laser revascularization (TMLR) in patients with severe diffuse coronary artery disease. METHODS: Between February, 1998 and February, 1999, 20 patients were submitted to TMLR at the Heart Institute (InCor), University of São Paulo Medical School, Brazil, isolated or in association with conventional coronary artery bypass graft (CABG). All patients had severe diffuse coronary artery disease, with angina functional class III/IV (Canadian Cardiovascular Society score) unresponsive to medical therapy. Fourteen patients were submitted to TMLR as the sole therapy, whereas 6 underwent concomitant CABG. Fifty per cent of the patients had either been previously submitted to a CABG or to a percutaneous transluminal coronary angioplasty (PTCA). Mean age was 60 years, ranging from 45 to 74 years. RESULTS: All patients had three-vessel disease, with normal or mildly impaired left ventricular global function. Follow-up ranged from 1 to 13 months (mean 6.6 months), with no postoperative short or long term mortality. There was significant symptom improvement after the procedure, with 85% of the patients free of angina, and the remaining 15 % of the patients showing improvement in functional class, as well as in exercise tolerance. CONCLUSION: This novel technique can be considered a low risk alternative for a highly selected group of patients not suitable for conventional revascularization procedures.

OBJECTIVE: To verify if adaptive left ventricle (LV) characteristics are also present in individuals under 70 years of age with severe aortic stenosis (AS). METHODS: The study comprised 40 consecutive patients under 70 years of age with AS and no associated coronary artery disease, referred for valve surgery. Out of the 40 patients, 22 were men and 18 women, and the mean age was 49.8±14.3 years. Cardiac symptoms, presence of systemic hypertension (SH), functional class according to the New York Heart Association (NYHA), and valve lesion etiology were considered. LV cavity dimensions, ejection fraction (EF), fractional shortening (FS), mass (MS), and relative diastolic thickness (RDT) were examined by Doppler echocardiography. RESULTS: Fourteen (63.6%) men and 11 (61.6%) women were classified as NYHA class III/IV (p=0.70). There was no difference in the frequency of angina, syncope or dyspnea between genders. The incidence of SH was greater in women than in men (10 versus 2, p=0.0044). Women had a smaller LV end-diastolic diameter index (32.1±6.5 x 36.5±5.3mm/m², p=0.027), LV end-systolic diameter index (19.9±5.9 x 26.5±6.4mm/m², p=0.0022) and LV mass index (MS) (211.4±71.1 x 270.9±74.9g/m², p=0.017) when compared with men. EF (66.2±13.4 x 52.0±14.6%, p=0.0032), FS (37.6±10.7 x 27.9±9.6%, p=0.0046) and RDT (0.58±0.22 x 0.44±0.09, p=0.0095) were significantly greater in women than in men. CONCLUSION: It is the patient gender rather than age that influences left ventricular adaptive response to AS.

PURPOSE: To determine the frequency of coronary artery disease, microalbuminuria and the relation to lipid profile disorders, blood pressure and clinical and metabolic features. METHODS: Fifty-five type 2 diabetic patients (32 females, 23 males), aged 59.9±9 years and with known diabetes duration of 11±7.3 years were studied. Coronary artery disease (CAD) was defined as a positive history of myocardial infarction, typical angina, myocardial revascularization or a positive stress testing. Microalbuminuria was defined when two out of three overnight urine samples had a urinary albumin excretion ranging 20 - 200µg/min. RESULTS: CAD was present in 24 patients (43,6%). High blood pressure (HBP) present in 32 patients (58.2%) and was more frequent in CAD group (p=0.05) HBP. Increased the risk of CAD 3.7 times (CI[1.14-12]). Microalbuminuria was present in 25 patients (45.5%) and tended to associate with higher systolic blood pressure (SBP) (p = 0.06), presence of hypertension (p = 0.06) and know diabetes duration (p = 0.08). In the stepwise multiple logistic regression the systolic blood pressure was the only variable that influenced UAE (r = 0.39, r² = 0.14, p = 0.01). The h ypertensive patients had higher cholesterol levels (p = 0.04). CONCLUSION: In our sample the frequency of microalbuminuria, hypertension, hypercholesterolemia and CHD was high. Since diabetes is an independent risk factor for cardiovascular disease, the association of others risk factors suggest the need for an intensive therapeutic intervention in primary and in secundary prevention.

OBJECTIVE: Anomalous origin of the left coronary artery from the right pulmonary artery (AOLCARPA), is a rare entity that is usually associated with other defects. Of the 20 cases of AOLCARPA reported in the literature, 14 (70%) had associations. We describe four patients with AOLCARPA without associated defects, but with a peculiar intramural aortic trajectory. METHODS: Fifty-five patients with anomalous origin of the left coronary artery were operated upon at INCOR-FMUSP. Four of the patients had the anomalous origin from the right pulmonary artery (RPA) without associated defects but with intramural aortic trajectory. Clinical and laboratory examinations were analyzed, as well as surgical findings. RESULTS: All patients had congestive heart failure (CHF) and 3 also had angina pectoris. Two patients had a murmur of mitral regurgitation, signs of myocardial infarction on the ECG and cardiomegaly. The shortening fraction varied from 9% to 23%. The hemodynamic study confirmed the diagnosis of anomalous origin of the coronary artery, but the intramural trajectory and the origin from the RPA were established only at surgery. In 3 patients, the technique of side-to-side anastomosis was performed with a good outcome. One patient, who underwent end-to-side anastomosis, died 6 months after the surgery. CONCLUSION: Association with other defects usually occurs in the AOLCARPA, and the intramural aortic trajectory is difficult to clinically diagnose but easy to surgically correct.

OBJECTIVE - This study compared the early and late results of the use of one single stent with those of the use of multiple stents in patients with lesions longer than 20mm. METHODS - Prospective assessment of patients electively treated with stents, with optimal stent deployment and followed-up for more than 3 months. From February '94 to January '98, 215 patients with lesions >20mm were treated. These patients were divided into 2 groups as follows: Group A - 105 patients (49%) with one stent implanted; Group B - 110 patients (51%) with multiple stents implanted. RESULTS - The mean length of the lesions was 26mm in group A (21-48mm) versus 29mm in group B (21-52mm) (p=0.01). Major complications occurred in one patient (0.9%) in group A (subacute thrombosis, myocardial infarctionand death) and in 2 patients (1.8%) in group B (one emergency surgery and one myocardial infarction) (p=NS). The results of the late follow-up period (>6 months) were similar for both groups (group A = 82% vs group B = 76%; p=NS), and we observed an event-free survical in 89% of the patients in group A and in 91% of the patients in group B (p=NS). Angina (group A = 11% vs group B = 7%) and lesion revascularization (group A = 5% vs group B = 6%; p=NS) also occurred in a similar percentage. No infarction or death was observed in the late follow-up period; restenosis was identified in 33% and 29% of the patients in groups A and B, respectively (p=NS). CONCLUSION - The results obtained using one stent and using multiple stents were similar; the greater cost-effectiveness of one stent implantation, however, seems to make this strategy the first choice.

OBJECTIVE: To identify characteristics of the hospitalizations due to ischemic heart disease (IHD) made by the Single Health System -- "Sistema Único de Saúde (SUS) in Brazil from 1993 to 1997. METHODS: The information used came from records of permissions for hospitalization due to IHD (diseases codified from 410 to 414 by the International Disease Classification -- 9th Revision) furnished by the data bank DATASUS. The material studied was classified according to age, sex and length of hospitalization of the patients, and expenses to the system for IHD. RESULTS: IHD represents 1.0% of total hospitalizations. Angina pectoris was the most frequent type, occurring in 53.3% of the cases, followed by acute myocardial infarct (26.6%). This later was more frequent in men and angina in women . The majority of patients with IHD stayed hospitalized from 5 to 8 days. In the years of 1997 the expenses due to hospital treatment for IHD reach to 0.01% of Brazil's Gross Internal Product. In the studied period (1993-1997), IHD was responsible by 1.0% of hospitalizations, however it was 3.3% of the expenses of SUS. CONCLUSION: IHD is an important cause of hospitalization by the SUS; it has a rather high cost, indicating the need for preventive measures aimed at reducing exposure to risk factors and to decrease the incidence of this group of diseases in the nation.

OBJECTIVE: To analyze the effects of in-hospital reocclusion of reperfused AMI culprit coronary arteries in mortality and to identify the predictors. METHODS: The present study comprises a sample of 155 patients with AMI who underwent successful mechanical reperfusion by direct coronary angioplasty and angiographic control during hospitalization or before discharge. Patients were classified into group A: reoccluded patients (n=30) and group B: non-reoccluded patients (n=125). RESULTS: We identified in-hospital reocclusion predictors and found a greater significance in mortality among reoccluded patients (23,3% x 1.6%; p=0.00004). Silent reocclusion or typical angina at reocclusion had a good prognosis. The independent predictors of in-hospital mortality were hypertension, multiarterial lesions, totally occluded AMI culprit lesions, failed redilatation, failed redilatation in comparison with no intention to redilate, no redilatation in comparison with no atempt to redilate, and reocclusion within the first 48 to 72 hours. The decision to redilate, independently of the result, led to a 50.0% reduction in hospital mortality (p=0.0366). CONCLUSION: In-hospital AMI culprit coronary artery reocclusion had an adverse effect similar to that reported in clinical studies with high mortality rates (23.3% x 1.6%; p=0.00004). The major contribution of this study is to recommend the reopening of reoccluded AMI culprit coronary arteries as a means for the management of coronary artery reocclusion.

OBJECTIVE: To assess the relation between coronary artery disease and the calcification index on helical computed tomography. METHOD: We studied 22 patients (ages ranging from 40 to 70 years) who underwent coronary angiography because of chest pain suggestive of angina pectoris. Findings on coronary angiography were classified as follows: significant obstructive disease (stenosis > or = 50%), nonobstructive disease (stenosis <50%), and no disease. With no previous knowledge of the results of the coronary angiography and within 7 days, helical computed tomography of the chest was performed. Then, data of the coronary angiography were correlated with the calcification index obtained by helical computed tomography. RESULTS: The sensitivity of helical computed tomography to the presence of significant obstructive lesions on coronary angiography was 87.5%, specificity was 100%, and negative and positive predictive values were 75% and 100%, respectively. The mean calcification index was greater in patients with severe coronary lesions, mainly when involvement of 2 or 3 vessels occurred, than that in patients with no coronary artery disease or with nonobstructive coronary artery lesions (p<0.05). CONCLUSION: Helical computed tomography is an effective method for detecting and quantifying coronary artery calcification, and it has proved to be sensitive to and specific for the noninvasive diagnosis of coronary artery stenosis.

OBJECTIVE: To assess the association between cardiovascular risk factors and acute myocardial infarction as the first manifestation of ischemic heart disease, correlating them with coronary angiographic findings. METHODS: We carried out a cross-sectional study of 104 patients with previous acute myocardial infarction, who were divided into 2 groups according to the presence or absence of angina prior to acute myocardial infarction. We assessed the presence of angina preceding acute myocardial infarction and risk factors, such as age >55 years, male sex, smoking, systemic arterial hypertension, lipid profile, diabetes mellitus, obesity, sedentary lifestyle, and familial history of ischemic heart disease. On coronary angiography, the severity of coronary heart disease and presence of left ventricular hypertrophy were assessed. RESULTS: Of the 104 patients studied, 72.1% were males, 90.4% were white, 73.1% were older than 55 years, and 53.8% were hypertensive. Acute myocardial infarction was the first manifestation of ischemic heart disease in 49% of the patients. The associated risk factors were systemic arterial hypertension (RR=0.19; 95% CI=0.06-0.59; P=0.04) and left ventricular hypertrophy (RR=0.27; 95% CI=0,.8-0.88; P=0.03). The remaining risk factors were not statistically significant. CONCLUSION: Prevalence of acute myocardial infarction as the first manifestation of ischemic heart disease is high, approximately 50%. Hypertensive individuals more frequently have symptoms preceding acute myocardial infarction, probably due to ventricular hypertrophy associated with high blood pressure levels.

OBJECTIVE - Cardiac evaluation (clinical, electrocardiographic and echocardiographic) of 25 Brazilian patients with clinical diagnosis of Friedreich's ataxia (FA) related to the frequency and the size of GAA repeats (unstable expansion of trinucleotide repeats that results in the disease). METHODS - Clinical and cardiac study including electrocardiogram and echocardiogram of all patients and molecular analysis to detect the frequency and the size of GAA expansion, by polymerase chain reaction analysis. RESULTS - Homozygous GAA expansion was detected in 17 patients (68%) -- all typical cases. In 8 (32%) cases (6 atypical and 2 typical), no GAA expansion was observed, therefore it was not considered Friedreich's ataxia. All patients with GAA expansion (100%) had electrocardiographic abnormalities, and only 25% of the cases without GAA expansion had some abnormality on this exam. However, only 6% of all patients revealed some signals/symptoms suggestive of cardiac involvement. CONCLUSION - A molecular analysis is essential to confirm the diagnosis of Friedreich's ataxia; however, an adequate cardiac evaluation, including an electrocardiogram, was extremely useful to better screening the patients which should perform these molecular analysis.

OBJECTIVE: To verify the possible role of adventitial inflammation in atherosclerotic plaque vulnerability and coronary artery remodelling. METHODS: We compared the mean numbers of lymphocytes in the adventitia and in the plaque of ruptured thrombosed and stable equi-stenotic coronary segments of 34 patients who died due to acute myocardial infarction. We also analysed adventitial microvessels, adventitial fibrosis and the external elastic membrane. RESULTS: In the adventitia, the numbers of lymphocytes and microvessels/mm² were 69.5±88.3 and 60.9± 32.1 in culprit lesions and 16.4 ± 21.1 and 44.3±16.1 in stable lesions (p<0.05); within the plaques, the mean number of lymphocytes was 24±40.8 in culprit lesions and 10.9±13.2 in stable ones (p=0.17). The mean percent area of adventitial fibrosis/cross-sectional area of the vessel was significantly lower in unstable plaques (p<0.001). The confocal images showed holes in the external elastic membrane. CONCLUSION: Unstable plaques exhibit chronic pan-arteritis, accompanied by enlargement, medial thinning, and less fibrosis than in stable lesions, which is compatible with vessel aneurysm. Adventitial inflammation may contribute significantly to atheroma instability.

Doença cardíaca isquêmica é uma complicação rara e, só recentemente reconhecida, de irradiação mediastínica para tratamento de tumores nesta região. É relatado caso de uma mulher de 51 anos com angina do peito, rapidamente progressiva, em que o achado angiográfico foi representado por lesão suboclusiva ostial de tronco de coronária esquerda. A história pregressa era marcada por uso de radioterapia para tratamento de linfoma Hodgkin mediastínico, com íntima relação com ventrículo direito, ressecado cirurgicamente e tratado em seqüência com irradiação e quimioterapia, dois anos antes. A indução de estenoses coronarianas nesses pacientes pode ser dependente ou não de aterosclerose focal e é mediada, principalmente, por espessamento intimal decorrente de fibrose tissular, sem que haja alteração na camada média e com predileção pelas porções proximais (ostiais) das artérias principais. O reconhecimento desta condição (radioterapia torácica), como fator isolado e independente para doença coronariana, deve ser considerada na programação de medidas para prevenção, detecção e tratamento precoce.

OBJETIVO: Investigar o critério ultra-sonográfico de área mínima da luz (AML), com valor de corte igual a 4,0 mm² na tentativa de diferençar as lesões que devem ou não ser tratadas. MÉTODOS: Incluímos 173 pacientes consecutivos com lesões angiograficamente moderadas (porcentual de estenose entre 40 e 70) submetidos à realização de ultra-som, divididos em 2 grupos: grupo 1 clínico (AML > 4,0 mm²) e grupo 2 revascularização (AML < 4,0 mm²), que foram acompanhados para determinar as taxas de eventos cardíacos maiores (ECM) em dois anos, a necessidade de revascularização da lesão-alvo e identificar os preditores clínicos, angiográficos e ultra-sonográficos dos eventos. RESULTADOS: Apresentaram AML > 4,0 mm² 75 (43%) pacientes, mantidos clinicamente e 98 (57%) pacientes AML < 4,0 mm², tratados com stents coronarianos. Pela angiografia coronariana quantitativa não houve diferença significante entre o porcentual de estenose do vaso [grupo 1: 48% vs grupo 2: 53%; p=0,06]. Ao contrário das mensurações ultra-sonográficas, pois a AML mostrou-se significativamente maior no grupo 1 quando comparada ao grupo 2 [4,54 mm² vs 2,45 mm²; p<0,001)]. O impacto clínico da tomada de decisão foi favorável, não verificando-se diferença na ocorrência de eventos cardíacos maiores: [grupo 1: 5 (7%) vs grupo 2: 14 (15%); p= 0,09]. A necessidade de revascularização da lesão-alvo também não diferiu (grupo 1: 3 (4%) vs grupo 2: 11 (12%); p=0,07). As variáveis preditoras para os ECM foram: diabetes, angina CFIII pré-hospitalização e a AML avaliada pelo ultra-som. CONCLUSÃO: A estratégia de decisão de tratamento assegura baixas taxas de ECM em ambos os grupos no seguimento de 24 meses, garantindo reduzidas taxas de revascularização, sendo as variáveis preditoras de eventos cardíacos maiores: diabetes melito, angina classe funcional III e a AML ao ultra-som intracoronariano.
