OBJETIVO Analizar el efecto de un programa piloto de tele-asistencia y tele-alarma en la autopercepción del estado de salud de los adultos mayores y conocer la satisfacción y aceptación del programa. METODOS Estudio transversal que incluyó entrevistas a 378 adultos mayores, 294 cuidadores/familiares y 53 profesionales de la salud, realizado en la Ciudad de México en 2010. Variables sociodemográficas, de autopercepción del estado de salud, satisfacción y aceptación del programa se obtuvieron por medio de cuestionario estandarizado. Se realizó análisis descriptivo y se utilizaron pruebas no paramétricas. RESULTADOS Se observaron efectos significativos en la autopercepción del estado de salud, 6,17 (DE 17,9 p < 0,05) puntos por encima de la media. El programa tuvo una buena aceptación por parte de los adultos mayores, de sus cuidadores familiares y de los profesionales de la salud. CONCLUSIONES El programa contribuye a mantener o mejorar la calidad de vida, permite la atención preventiva y es un medio ideal para dar apoyo psicosocial a los adultos mayores.

OBJETIVO: Caracterizar o consumo alimentar mais frequente da população brasileira. MÉTODOS: Foram analisados dados referentes ao primeiro dia de registro alimentar de 34.003 indivíduos com dez anos ou mais de idade que responderam ao Inquérito Nacional de Alimentação, composto por amostra probabilística da Pesquisa de Orçamentos Familiares 2008-2009. O padrão de consumo foi analisado segundo sexo, grupo etário, região e faixa de renda familiar per capita. RESULTADOS: Os alimentos mais frequentemente referidos pela população brasileira foram arroz (84,0%), café (79,0%), feijão (72,8%), pão de sal (63,0%) e carne bovina (48,7%), destacando-se também o consumo de sucos e refrescos (39,8%), refrigerantes (23,0%) e menor presença de frutas (16,0%) e hortaliças (16,0%). Essa configuração apresenta pouca variação quando se consideram os estratos de sexo e faixa etária; contudo, observa-se que os adolescentes foram o único grupo etário que deixou de citar qualquer hortaliça e que incluiu doces, bebida láctea e biscoitos doces entre os itens mais consumidos. Alimentos marcadamente de consumo regional incluem a farinha de mandioca no Norte e Nordeste e o chá na região Sul. Houve discrepâncias no consumo alimentar entre os estratos de menor e maior renda: indivíduos no quarto de renda mais elevada referiram sanduíches, tomate e alface e aqueles no primeiro quarto de renda citaram os peixes e preparações à base de peixe e farinha de mandioca entre os alimentos mais referidos. CONCLUSÕES: Existe um padrão básico do consumo alimentar no Brasil que inclui entre os alimentos mais consumidos arroz, café, feijão, pão de sal e carne bovina, associado ao consumo regional de alguns poucos itens. Particularmente entre os adolescentes, alimentos ricos em gordura e açúcar são também de consumo frequente.

OBJETIVO: Estimar a prevalência de ingestão inadequada de nutrientes na população idosa brasileira. MÉTODOS: Foram analisados dados do Inquérito Nacional de Alimentação como parte da Pesquisa de Orçamentos Familiares, em 2008-2009. Dados de consumo alimentar individual de 4.322 indivíduos com 60 anos ou mais foram obtidos por meio do registro alimentar de dois dias não consecutivos. A ingestão habitual para cada nutriente foi estimada pelo método do National Cancer Institute, cujos modelos tiveram como covariáveis sexo e região. As prevalências de inadequação de ingestão de micronutrientes foram estimadas segundo sexo e região utilizando o método da EAR como ponte de corte. RESULTADOS: Elevadas prevalências de inadequação (> 50%) foram observadas para as vitaminas E, D, A, cálcio, magnésio e piridoxina em ambos os sexos. Em todas as regiões, observou-se 100% de inadequação de vitamina E. Vitamina D obteve percentuais de inadequação próximos de 100% em todas as regiões, exceto para a região Norte. As prevalências de inadequação de vitamina A foram superiores a 70% nas regiões Norte, Nordeste e Centro-Oeste. Cálcio e magnésio foram os minerais com maior prevalência de ingestão inadequada (> 80%) em todas as regiões. CONCLUSÕES: Idosos brasileiros apresentam elevada inadequação da ingestão de nutrientes, reconhecidos como protetores contra doenças crônicas.

O objetivo dessa comunicação foi analisar a variação do perfil dos benefícios previdenciários por transtornos mentais e comportamentais e sua relação com o trabalho. Foram utilizados dados secundários da Previdência Social brasileira de 2008 a 2011. Foram calculadas taxas de variação média anual no período acerca da população economicamente ativa, número de segurados, benefícios auxílio-doença concedidos em geral e, especificamente, os relativos a transtornos mentais e comportamentais. Os transtornos mentais mantêm-se como a terceira causa das concessões. Houve aumento médio anual de 0,3% de novas concessões, com queda de 2,5% da incidência média anual. Foram considerados relacionados ao trabalho 6,2% dos casos, na média, principalmente decorrentes de transtornos de humor.As autoridades governamentais devem usar os dados da Previdência Social para auxiliar no debate sobre políticas públicas de saúde mental.

OBJECTIVE To analyze conditional and unconditional healthy life expectancy among older Brazilian women.METHODS This cross-sectional study used the intercensal technique to estimate, in the absence of longitudinal data, healthy life expectancy that is conditional and unconditional on the individual’s current health status. The data used were obtained from the Pesquisa Nacional por Amostra de Domicílios (National Household Sample Survey) of 1998, 2003, and 2008. This sample comprised 11,171; 13,694; and 16,259 women aged 65 years or more, respectively. Complete mortality tables from the Brazilian Institute of Geography and Statistics for the years 2001 and 2006 were also used. The definition of health status was based on the difficulty in performing activities of daily living.RESULTS The remaining lifetime was strongly dependent on the current health status of the older women. Between 1998 and 2003, the amount of time lived with disability for healthy women at age 65 was 9.8%. This percentage increased to 66.2% when the women already presented some disability at age 65. Temporal analysis showed that the active life expectancy of the women at age 65 increased between 1998-2003 (19.3 years) and 2003-2008 (19.4 years). However, life years gained have been mainly focused on the unhealthy state.CONCLUSIONS Analysis of conditional and unconditional life expectancy indicated that live years gained are a result of the decline of mortality in unhealthy states. This pattern suggests that there has been no reduction in morbidity among older women in Brazil between 1998 and 2008.

OBJECTIVE To analyze the characteristics of health diagnosis according to the ecohealth approach in rural and urban communities in Mexico.METHODS Health diagnosis were conducted in La Nopalera, from December 2007 to October 2008, and in Atlihuayan, from December 2010 to October 2011. The research was based on three principles of the ecohealth approach: transdisciplinarity, community participation, gender and equity. To collect information, a joint methodology and several techniques were used to stimulate the participation of inhabitants. The diagnostic exercise was carried out in five phases that went from collecting information to prioritization of problems.RESULTS The constitution of the transdisciplinary team, as well as the participation of the population and the principle of gender/equity were differentials between the communities. In the rural community, the active participation of inhabitants and authorities was achieved and the principles of transdisciplinarity and gender/equity were incorporated.CONCLUSIONS With all the difficulties that entails the boost in participation, the incorporation of gender/equity and transdisciplinarity in health diagnosis allowed a holistic public health approach closer to the needs of the population.

Se presentan 105 casos de criptococosis diagnosticados en la República Argentina, entre 1981 y 1990. El número anual de casos fue hasta 1987, de 4 a 8, aumentó desde 1988 por influencia del SIDA y llegó a 35 casos en 1990. La criptococosis no asociada al SIDA se mantuvo entre 3 y 7 casos anuales. Globalmente, el grupo etario más afectado fue el de 20-39 años y la distribución por edad fue diferente en las poblaciones con y sin SIDA. La mediana de la edad de las poblaciones total, asociada al SIDA y a otras causas predisponentes fue de 30, 30 y 45 años, respectivamente. El predominio del sexo masculino fue mucho más evidente entre los pacientes HIV+ que en los que no padecían esta última infección. La causa predisponente fue el SIDA en 57 pacientes, otra en 20 y era desconocida en 28 casos. Estimando indirectamente, el porcentaje de pacientes con SIDA que padecieron criptococosis en este período fue 6,19% (57 casos en 920 HIV+). Cryptococcus neoformans variedad neoformans fue aislado de 101 pacientes y la variedad gattii (serotipo B) de los 4 restantes. Los datos obtenidos son similares a los observados en Europa y Estados Unidos.

We report data related to arbovirus antibodies detected in wild birds periodically captured from January 1978 to December 1990 in the counties of Salesópolis (Casa Grande Station), Itapetininga and Ribeira Valley, considering the different capture environments. Plasmas were examined using hemagglutination-inhibition (HI) tests. Only monotypic reactions were considered, except for two heterotypic reactions in which a significant difference in titer was observed for a determined virus of the same antigenic group. Among a total of 39,911 birds, 269 birds (0.7%) belonging to 66 species and 22 families were found to have a monotypic reaction for Eastern equine encephalitis (EEE), Venezuelan equine encephalitis (VEE), Western equine encephalitis (WEE), Ilheus (ILH), Rocio (ROC), St. Louis encephalitis (SLE), SP An 71686, or Caraparu (CAR) viruses. Analysis of the data provided information of epidemiologic interest with respect to these agents. Birds with positive serology were distributed among different habitats, with a predominance of unforested habitats. The greatest diversity of positive reactions was observed among species which concentrate in culture fields.

Sandflies attracted by human bait were caught in an endemic focus of localized cutaneous leishmaniasis in the state of Campeche, Mexico. Catches were carried out monthly from February 1994 to January 1995 between 18:00 and 22:00 h. Lutzomyia cruciata was the only species caught. The highest population peak of Lu. cruciata was found in March with lesser peaks in February, December 1994, and January 1995. Maximum biting rate of Lu. cruciata was found between 18:00 and 19:00 h. The host-seeking females of Lu. cruciata were directly related to levels of humidity between 88 and 100%. Low and high temperature had a negative effect upon Lu. cruciata activity. The possible role of Lu. cruciata as vector of leishmaniasis in the state of Campeche, Mexico is discussed.

In order to determine Trypanosoma cruzi infection among mammals in Yucatan, Mexico, 372 animals, both wild and synanthropic including carnivores, marsupials and rodents were studied. Serological studies by indirect haemagglutination (IHA) were carried out to detect antibodies to T. cruzi and a parasitological study was also performed (blood smear and histopathology). Of all the animals tested 18.54% were serologically positive, with a significantly higher frequency among the wild ones (33.33%) compared to the synanthropic ones (17.79%). To determine T. cruzi in positive animals, blood was inoculated into a white mouse (webster type) to prove myocardium colonization. The serological and parasitological positivity of these animals, as well as their behavior in the environment, taken together with the socioeconomic and cultural characteristics of the population, suggest that in Yucatan, Mexico, Canis familiaris, Didelphis marsupialis and Rattus rattus act as a link with the wild cycle.

A large epidemic of serogroup B meningococcal disease (MD), has been occurring in greater São Paulo, Brazil, since 1988.21 A Cuban-produced vaccine, based on outer-membrane-protein (OMP) from serogroup B: serotype 4: serosubtype P1.15 (B:4:P1.15) Neisseria meningitidis, was given to about 2.4 million children aged from 3 months to 6 years during 1989 and 1990. The administration of vaccine had little or no measurable effects on this outbreak. In order to detect clonal changes that could explain the continued increase in the incidence of disease after the vaccination, we serotyped isolates recovered between 1990 and 1996 from 834 patients with systemic disease. Strains B:4:P1.15, which was detected in the area as early as 1977, has been the most prevalent phenotype since 1988. These strains are still prevalent in the area and were responsible for about 68% of 834 serogroup B cases in the last 7 years. We analyzed 438 (52%) of these strains by restriction fragment length polymorphism (RFLPs) of rRNA genes (ribotyping). The most frequent pattern obtained was referred to as Rb1 (68%). We concluded that the same clone of B:4:P1.15-Rb1 strains was the most prevalent strain and responsible for the continued increase of incidence of serogroup B MD cases in greater São Paulo during the last 7 years in spite of the vaccination trial.

Localized Cutaneous Leishmaniasis (LCL) known as "chiclero's ulcer" in southeast Mexico, was described by SEIDELIN in 1912. Since then the sylvatic region of the Yucatan peninsula has been documented as an endemic focus of LCL. This study of 73 biopsies from parasitological confirmed lesions of LCL cases of Leishmania (Leishmania) mexicana infection was undertaken: 1) to examine host response at tissue level; and 2) to relate manifestations of this response to some characteristics of clinical presentation. Based on Magalhães' classification we found that the most common pattern in our LCL cases caused by L. (L.) mexicana was predominantly characterized by the presence of unorganized granuloma without necrosis, (43.8%). Another important finding to be highlighted is the fact that in 50/73 (68.5%) parasite identification was positive. There was direct relation between the size of the lesion and time of evolution (r s = 0.3079, p = 0.03), and inverse correlation between size of the lesion and abundance of amastigotes (r s = -0.2467, p = 0.03). In view of the complexity of clinical and histopathological findings, cell-mediated immune response of the disease related to clinical and histopathological features, as so genetic background should be studied.

In the Yucatan Peninsula of Mexico, 95% of the human cases of Cutaneous Leishmaniasis are caused by Leishmania (Leishmania) mexicana with an incidence rate of 5.08 per 100,000 inhabitants. Transmission is limited to the winter months (November to March). One study on wild rodents has incriminated Ototylomys phyllotis and Peromyscus yucatanicus as primary reservoirs of L. (L.) mexicana in the focus of La Libertad, Campeche. In the present study, the prevalence of both infection and disease caused by L. (L.) mexicana in small terrestrial mammals were documented during five transmission seasons (1994-2004) in five foci of Leishmaniasis in the state of Campeche. Foci separated by only 100 km, with similar relative abundances of small mammals, were found to differ significantly in their prevalence of both symptoms and infection. Transmission rates and reservoir species seemed to change in space as well as in time which limited the implementation of effective control measures of the disease even in a small endemic area such as the south of the Yucatan Peninsula.
