OBJECTIVE: To assess the frequency of comorbidities of mental and behavioral disorders (CMBD) in psychoactive substance (PAS)-dependent patients with different periods of abstinence cared for at Alcohol and Other Drug Psychosocial Care Centers (CAPS-ad). METHOD: All patients under treatment in the two CAPS-ad of the city of Uberlândia-MG, between April and September 2010, were consecutively assessed. The ICD-10 symptom checklist was used to diagnose CMBD; additional information was obtained from interviews and medical records. The patients were divided according to duration of abstinence: < 1 week (Group 1); 1-4 weeks (Group 2); and > 4 weeks (Group 3). RESULTS: Of all patients assessed, 62.8% were diagnosed with CMBD, which were more frequent (p < 0.05) in Group 1 (72%) than Group 3 (54.2%), and both groups were similar to Group 2 (61%). Depressive and anxiety disorders were more frequent among patients of Group 1. Mood disorders were more frequent (p < 0.05) in women [22/34 (65%) vs. 54/154 (35.1%)], whereas psychotic disorders were more frequent (p = 0.05) in men [16/154 (10.4%) vs. 0]. The presence of CMBD was associated with more severe clinical conditions. CONCLUSIONS: The higher frequency of diagnosis of CMBD in patients of Group 1 may have resulted from the difficulties in distinguishing mental disorders that are due to PAS intoxication or withdrawal from those that are not. However, to make the diagnosis of CMBD, even during detoxification, can increase the likelihood of better response to treatment.

INTRODUCTION: Schizophrenia is a chronic mental disorder associated with impairment in social functioning. The most widely used scale to measure social functioning is the GAF (Global Assessment of Functioning), but it has the disadvantage of measuring at the same time symptoms and functioning, as described in its anchors. OBJECTIVES:Translation and cultural adaptation of the PSP, proposing a final version in Portuguese for use in Brazil. METHODS: We performed five steps: 1) translation; 2) back translation; 3) formal assessment of semantic equivalence; 4) debriefing; 5) analysis by experts. Interrater reliability (Intraclass correlation, ICC) between two raters was also measured. RESULTS: The final version was applied by two independent investigators in 18 adults with schizophrenia (DSM-IV-TR). The interrater reliability (ICC) was 0.812 (p < 0.001). CONCLUSION: The translation and adaptation of the PSP had an adequate level of semantic equivalence between the Portuguese version and the original English version. There were no difficulties related to understanding the content expressed in the translated texts and terms. Its application was easy and it showed a good interrater reliability. The PSP is a valid instrument for the measurement of personal and social functioning in schizophrenia.
Violência verbal contra indivíduos com transtorno mental no Brasil: prevalência e fatores associados

Objetivo Avaliar a satisfação dos pacientes e familiares atendidos em um serviço ambulatorial de saúde mental da cidade de Rio Branco, Acre. Métodos Foi realizado um estudo transversal com uma amostra de 160 pacientes e 160 familiares. Para coleta de dados, foram utilizadas as versões abreviadas das Escalas de Satisfação com os Serviços de Saúde Mental – SATIS-BR para pacientes e familiares, e um questionário sociodemográfico e clínico. Foram feitos análises estatísticas descritivas, cálculos das médias e desvios-padrão dos escores de satisfação global e das subescalas, e análises bivariadas utilizando o programa SPSS, versão 17. Resultados Os resultados da média de satisfação global dos pacientes e familiares revelaram que eles estão satisfeitos com o serviço de saúde mental. As subescalas dos pacientes: competência e compreensão da equipe e acolhida da equipe e ajuda recebida foram elevadas. No entanto, a subescala condições físicas e conforto do serviço apresentou uma menor média de satisfação. Também apresentaram um elevado nível de satisfação as subescalas para os familiares: resultados do tratamento, acolhida e competência da equipe e privacidade e confidencialidade. Foi identificado que pacientes mais velhos e que não tinham tido crises estavam mais satisfeitos. Assim como os familiares mais jovens também tinham maior nível de satisfação. Conclusão Os resultados apontam para necessidades de melhorias nos aspectos relacionados a infraestrutura, conforto e aparência dos serviços, bem como a criação de estratégias que favoreçam maior participação do familiar no tratamento do paciente.

OBJECTIVE: To analyze associations between levels of physical activity, cardiorespiratory fitness, dietary content, and risk factors that cause a predisposition towards cardiovascular disease. METHODS: Sixty-two individuals aged between 20 and 45 years were evaluated. Levels of physical activity were established by estimates of energy demand corresponding to everyday activity; indices for cardiorespiratory fitness were obtained from estimates of maximal oxygen consumption; information about dietary content was obtained from dietary records kept on seven consecutive days. To indicate risk factors that cause a predisposition towards cardiovascular disease, use was made of body mass indexes, waist-hip circumference relationships, levels of arterial pressure and of plasma lipid-lipoprotein concentration. To establish associations between the variables studied, multiple regression analysis was used. RESULTS: Physical activity levels and cardiorespiratory fitness levels were inversely correlated with the amount and distribution of body fat and arterial pressure. Taken together, the two variables were responsible for between 16% and 19% of the variation in arterial pressure. Total and saturated fat ingestion was associated with higher serum lipid levels. Both dietary components were responsible for between 49% and 61% of the variation in LDL-cholesterol. CONCLUSION: High ingestion of food rich in total and saturated fat and decreased levels of physical activity and of cardiorespiratory fitness are associated with an increased risk of cardiovascular disease, which supports previous data.

OBJECTIVE: To assess, in myocardium specimens obtained from necropsies, the correlation between the concentration of hydroxyproline, measured with the photocolorimetric method, and the intensity of fibrosis, determined with the morphometric method. METHODS: Left ventricle myocardium samples were obtained from 45 patients who had undergone necropsy, some of them with a variety of cardiopathies and others without any heart disease. The concentrations of hydroxyproline were determined with the photocolorimetric method. In the histologic sections from each heart, the myocardial fibrosis was quantified by using a light microscope with an integrating ocular lens. RESULTS: A median of, respectively, 4.5 and 4.3 mug of hydroxyproline/mg of dry weight was found in fixed and nonfixed left ventricle myocardium fragments. A positive correlation occurred between the hydroxyproline concentrations and the intensity of fibrosis, both in the fixed (Sr=+0.25; p=0.099) and in the nonfixed (Sr=+0.32; p=0.03) specimens. CONCLUSION: The biochemical methodology was proven to be adequate, and manual morphometry was shown to have limitations that may interfere with the statistical significance of correlations for the estimate of fibrosis intensity in the human myocardium.

OBJETIVO: Estudar a reatividade vascular mediante a análise do fluxo sangüíneo e da resistência vascular periférica em repouso e estresse mental na gravidez de mulheres com estenose mitral. MÉTODOS: Foram estudadas 22 mulheres portadoras de estenose mitral, 13 grávidas (GE) e 9 não-grávidas (EM), e 9 gestantes saudáveis voluntárias (GN). Na gestação, 9 (GEB) das 13 do GE necessitaram de betabloqueador e 4 restantes (GESB) evoluíram sem medicação. A plestimografia em repouso e sob estresse mental analisou o fluxo sangüíneo muscular, resistência vascular periférica (RVP), pressão arterial média (PAM) e freqüência cardíaca (FC) na gestação e no puerpério. RESULTADOS: Na gestação do GESB, os valores do fluxo sangüíneo muscular e da FC foram maiores em 1,6% e 20,5% (p = 0,05), e os da RVP e da PAM foram menores em 19,3% e 4,4%, respectivamente, em comparação ao puerpério; no estresse mental, o fluxo sangüíneo muscular aumentou em 55,9%, a FC reduziu em 30,2% e houve semelhança da RVP e PAM. No GEB, os valores do fluxo e da FC foram maiores em 5,9% e 14,9% (p = 0,001) e os da PAM e da RVP menores em 10,3% e 9,1%, respectivamente, quando comparados ao puerpério; no estresse mental o fluxo e a PAM aumentaram em 69,8% e 174,1%, respectivamente, a FC foi semelhante e a RVP reduziu em 53,7%. O estudo comparativo mostrou que no grupo GN o fluxo sangüíneo muscular foi maior, a RVP menor e houve semelhança na PAM e FC em relação ao grupo GE e que os grupos GE, GN, EM apresentaram semelhança na resposta ao estresse mental. CONCLUSÕES: A reatividade vascular na gestação de mulheres com estenose mitral foi preservada e a análise das medidas mostrou valores menores de fluxo sangüíneo muscular e maiores da RVP quando comparados às gestantes saudáveis.

A dor torácica é um sintoma comum nos serviços de emergência e a cintilografia de estresse representa uma das etapas da estratificação de risco nesses indivíduos. Entretanto, um grupo de pacientes com exames funcionais negativos após o estresse físico ou farmacológico desenvolve isquemia miocárdica durante estresse mental. As alterações do tônus vascular em resposta a mecanismos endógenos são a base fisiopatológica dessas alterações. Relata-se um caso que ilustra como a cintilografia de estresse mental tem o potencial para ser utilizada na avaliação de isquemia miocárdica, não detectada pelos métodos convencionais, em pacientes com suspeita de dor torácica isquêmica.

FUNDAMENTOS: A cintilografia miocárdica com estresse mental parece induzir isquemia através de uma fisiopatologia particular quando comparada com a cintilografia, utilizando o estresse físico ou farmacológico. OBJETIVO: Avaliar a prevalência de isquemia miocárdica induzida por estresse mental, em pacientes com dor torácica e cintilografia com estresse convencional normal, utilizando 99mTc-Sestamibi. MÉTODOS: 22 PAcientes foram admitidos com dor torácica na emergência, ou foram encaminhados ambulatorialmente ao serviço de medicina nuclear da nossa instituição, onde realizaram cintilografia miocárdica de estresse e repouso sem alterações isquêmicas. Então, foram convidados a realizar uma fase adicional com indução de estresse mental através do conflito de cores (Stroop Color Test) com o objetivo de detectar isquemia miocárdica. Dois cardiologistas e médicos nucleares realizaram a análise cega dos dados perfusionais e consequente quantificação através do SDS (Summed Diference Score), pontuando os segmentos com alteração perfusional após o estresse mental e comparando com a imagem de repouso. A presença de isquemia miocárdica foi considerada com SDS > 3. RESULTADOS: A prevalência de isquemia miocárdica induzida por estresse mental foi de 40% (9 pacientes positivos). Nos 22 pacientes estudados não houve diferença estatística quanto ao número de fatores de risco, alterações hemodinâmicas induzidas pelo estresse mental, uso de medicações, sintomas apresentados, presença ou ausência de doença coronariana e variações da fração de ejeção e volume sistólico final do Gated SPECT. CONCLUSÃO: EM Uma amostra selecionada de pacientes com dor torácica e cintilografia miocárdica convencional normal, a pesquisa de isquemia miocárdica induzida pelo estresse mental através de cintilografia pode ser positiva em até 40% dos casos.

Workers of Melipona quadrifasciata anthidioides (Lepeletier, 1836) develop their ovaries and lay eggs, therefore the production of vitellogenin is expected. In electrophoretic profiles only fat body extracts from nurse workers and ovary extracts from newly-emerged workers show protein with molecular mass similar to vitellogenin. However, an increase in the protein content was detected in forager fat body. This increase was attributed to storage of vitellogenin or other proteins in the previous phase and not discharged into the hemolymph or to an effect of the increased titre of juvenile hormone in this phase of worker life over the fat body functioning.

RNA was determined in red blood cells of man and other mammals. Our report is based on 41 determinations. Red blood cells of rat showed the highest values in comparison with the blood cells of guinea pig, rabbit, horse and sheep which showed the lowest values, and man with intermediate ones. The method used was a combination of Schimidt and Thanhauser and Schneider extractions with the final reactions of pentose with the orcinol reagent colorimetrically measured.

A case-control study evaluating the association between mental retardation and toxoplasmosis was conducted among 845 school children in Belo Horizonte, MG, Brazil. Cases (450) were mentally retarded children attending a public school for special education. Controls (395) were children from the regular public school system. Clinical and anthropometric examinations and interviews were carried out to determine risk factors for toxoplasmosis and mental retardation. Diagnosis of Toxoplasma gondii infection was based upon an indirect immunofluorescent test (IFA); 55% of cases and 29% of controls were positive. The Relative Odds of mental retardation in children with positive serology was 3.0 (95% CI 2.2-4.0). Maternal exposure to cats and contact with soil were associated with an increased risk of mental retardation. Retinochoroiditis was fourfold more prevalent among cases than controls and was only diagnosed in T. gondii IFA positive participants. Congenital toxoplasmosis, in its subclinical form, appears to be an important component in the etiology of mental retardation, especially in high risk (lower socio-economic) groups. The population attributable risk was estimated as 6.0 - 9.0%, suggesting the amount of mental retardation associated with this infection.
