Giardia and Cryptosporidium have caused several outbreaks of gastroenteritis in humans associated with drinking water. Contaminated sewage effluents are recognized as a potential source of waterborne protozoa. Due to the lack of studies about the occurrence of these parasites in sewage samples in Brazil, we compared the efficiency of two procedures for concentrating cysts and oocysts in activated sludge samples of one sewage treatment plant. For this, the samples were submitted to i) concentration by the ether clarification procedure (ECP) and to ii) purification by sucrose flotation method (SFM) and aliquots of the pellets were examined by immunofluorescence. Giardia cysts were present in all samples (100.0%; n = 8) when using ECP and kit 1 reagents, while kit 2 resulted in six positive samples (85.7%; n = 7). As for SFM, cysts were detected in 75.0% and 100.0% of these samples (for kit 1 and 2, respectively). Regarding Cryptosporidium, two samples (25.0%; kit 1 and 28.5% for kit 2) were detected positive by using ECP, while for SFM, only one sample (examined by kit 1) was positive (12.5%). The results of the control trial revealed Giardia and Cryptosporidium recovery efficiency rates for ECP of 54.5% and 9.6%, while SFM was 10.5% and 3.2%, respectively. Considering the high concentration detected, a previous evaluation of the activated sludge before its application in agriculture is recommended and with some improvement, ECP would be an appropriate simple technique for protozoa detection in sewage samples.

Extended-spectrum β-lactamases (ESBL) in enterobacteria are recognized worldwide as a great hospital problem. In this study, 127 ESBL-producing Enterobacteriaceae isolated in one year from inpatients and outpatients at a public teaching hospital at São Paulo, Brazil, were submitted to analysis by PCR with specific primers for blaSHV, blaTEM and blaCTX-M genes. From the 127 isolates, 96 (75.6%) Klebsiella pneumoniae, 12 (9.3%) Escherichia coli, 8 (6.2%) Morganella morganii, 3 (2.3%) Proteus mirabilis, 2 (1.6%) Klebsiella oxytoca, 2 (1.6%) Providencia rettgeri, 2 (1.6%) Providencia stuartti, 1 (0.8%) Enterobacter aerogenes and 1 (0.8%) Enterobacter cloacae were identified as ESBL producers. BlaSHV, blaTEM and blaCTX-M were detected in 63%, 17.3% and 33.9% strains, respectively. Pulsed field gel eletrophoresis genotyping of K. pneumoniae revealed four main molecular patterns and 29 unrelated profiles. PCR results showed a high variety of ESBL groups among strains, in nine different species. The results suggest the spread of resistance genes among genetically different strains of ESBL-producing K. pneumoniae in some hospital wards, and also that some strongly related strains were identified in different hospital wards, suggesting clonal spread in the institutional environment.

We report a case of accidental infection by Trypanosoma cruzi in a 42-year-old female patient who presented an inoculation chagoma. Laboratory confirmation was based on examination of fresh blood, Giemsa-stained blood smear, immunoenzyme test (EIA-IgG), indirect immunofluorescence (IIF-IgM, IgG) and polymerase chain reaction (PCR). Only the PCR gave a positive result, and the EIA test was inconclusive. Two treatments with benznidazole were necessary. PCR was the only technique that continued to give positive results for approximately two months (65 days, or 2.2 months) following the second treatment and negative results from 96 days (3.2 months) to 850 days (28.3 months). We concluded that the presence of an inoculation chagoma and use of PCR were important and decisive for diagnosis and follow-up of the case.

Between April 2003 and May 2009 phlebotomine sandflies were collected in Parque do Sabiá complex, Uberlândia municipality, Minas Gerais State, Brazil, using CDC and Shannon traps. The objective was to associate the sandfly species captured to the risk of the transmission of leishmaniasis in the municipality. The 126 captured specimens belonging to six species of phlebotomine, among which Lutzomyia (Psychodopygus) davisi (Root, 1934) predominated with 113 specimens (89.7%). The remaining captured species were Lutzomyia (Pintomyia) mamedei Oliveira, Afonso, Dias & Brazil, 1994 - five specimens (3.9%); Lutzomyia (Nyssomyia) flaviscutellata (Mangabeira, 1942) - four specimens (3.2%); Lutzomyia lenti (Mangabeira, 1938) - two specimens (1.6%); Brumptomyia avellari (Costa Lima, 1932) - one specimen (0.8%); and Lutzomyia (Nyssomyia) whitmani (Antunes & Coutinho, 1939) - one specimen (0.8%). The collection of species that may be involved in the transmission of Leishmania reveals the need for continuous entomological surveillance.

Salmonella spp. causes diseases in fowls, when species-specific serovars (Salmonella Pullorum and S.Gallinarum) are present in flocks, and public health problems, when non-typhoid serovars are isolated, as well as possible bacterial resistance induced by the preventive and therapeutic use of antimicrobials in animal production. This study describes the serovars and bacterial resistance of 280Salmonella spp. strains isolated from turkey and broiler carcasses in Southern Brazil between 2004 and 2006. SalmonellaEnteritidis was the most prevalent serovar (55.7%), followed by Heidelberg (5.0%), Agona (4.3%), Bredeney (3.9%), Hadar (3.2%), and Typhimurium (2.9%). Tennessee and S. Enterica subspecies enterica(O: 4.5) were isolated only in turkeys, and Hadar (18.6%) was the most prevalent serovar in this species. Antimicrobial susceptibility tests were performed in 178 isolates (43 from turkeys and 135 from broilers). All isolates were sensitive to amoxicillin + clavulanic acid, polymyxin B, ciprofloxacin, and norfloxacin, and were resistant to bacitracin and penicillin. Broiler carcass isolates showed resistance to nalidixic acid (48.9%), nitrofurantoin (34.3%), neomycin (9.6%), tetracycline (5.2%), and kanamycin (8.9%); and turkey carcass isolates were resistant to nalidixic acid (62.8%), tetracycline (34.9%), and neomycin (30.2%), with a significant difference in turkeys when compared to broiler carcass isolates. These results indicate the need for judicious use of antimicrobials in livestock production, given that the serovars identified are potential causes of food poisoning.

Neste trabalho os autores apresentam dados relativos à prevalência e formas clinicas da esquistossomose mansoni, hospedeiro intermediário e determinação do seu índice de infecção em Santa Luzia do Carneiro, área rural do Município de Itanhomi, Vale do Rio Doce, Minas Gerais. O estudo seccional da população revelou 42,1% de positividade para ovos de S.mansoni em um exame de fezes peto método de Lutz (Hoffmann, Ponz, Janer) e um percentual de 6,8% de pacientes com hepato-esplenomegalia. O hospedeiro intermediário encontrado foi a B.glabrata com um índice de infecção de 3,2%.

Em Mambaí, área endêmica de doença de Chagas, foram realizadas abreugrafias do esôfago com filme de 70mm, em 1.145 indivíduos do sexo masculino e 1.184 dofeminino, de 4 a 87 anos, dos quais 1.006eram soropositivos. Os pacientes foram posicionados em oblíqua anterior direita e tomadas duas abreugrafias: uma imediatamente à ingestão de 75 ml de suspensão baritada e outra 60 segundos após. Setenta e seis (3,2%) indivíduos apresentaram esofagopatia, sendo que 71(7%) entre os 1.006 soropositivos e, 5(0,37%) entre os 1.323 soronegativos. Dos 76 esofagopatas 47(61,8%) eram do sexo masculino e 29(38,1%) do sexo feminino. Segundo a classificação de Rezende e cols 48(63,1%) pertenciam ao grupo I, 18(23,7%) ao grupo II, 5(6,6%) ao grupo III e 5(6,6%) ao grupo IV. Entre os infectados a esofagopatia aumentou progressivamente com a idade, principalmente a partir dos 30 anos, alcançando a prevalência de 21,5% após os 59 anos.

Para verificar a freqüência de anisocoria em portadores da fase crônica da doença de Chagas foi feito estudo prospectivo e duplo cego. Foram submetidos a exame oftalmológico 131 pacientes com sorologia positiva para doença de Chagas e 138 indivíduos com sorologia negativa, da população do município de Mambaí (GO), região endêmica de doença de Chagas. Para a avaliação da presença ou não de anisocoria foi realizada a pupilometria com régua. Observamos a presença de anisocoria em 10(7,6%) pacientes chagásicos e em 3(2,1%) controles. O teste do qui-quadrado revelou que as diferenças detectadas foram estatisticamente significantes ao nível de 5%. A doença de Chagas deve ser incluída entre as causas de anisocoria.

Em 1992, foi obsewada lesão cutânea por Leishmania em um cão do município de Sabará, Minas Gerais, onde já haviam sido registrados casos humanos de leishmaniose tegumentar. O parasita foi caracterizado como pertencente ao subgênero Viannia, ao qual pertence a Leishmania braziliensis, principal espécie encontrada tia região sudeste do Brasil. Com o objetivo de determinar o papel do cão no ciclo de transmissão da doença, foi realizado um inquérito canino na área. Foram examinados 631 cães, sendo a soroprevalência para leishmaniose igual a 3,2%. Foi observada a proximidade ou presença no mesmo domicílio de cães e pessoas doentes. Este aspecto fala a favor da transmissão domiciliar ou peridomiciliar, com o cão infectado podendo atuar como fator de risco nesta área periurbana. Entretanto, a baixa soroprevalência encontrada deve-se provavelmente ao pequeno papel deste animal na transmissão da doença neste foco recente da doença.
