Cryptococcus neoformans is the fifth most common opportunistic agent of infection in patients with AIDS in the USA, exceeded only by Candida species, Pneumocystis carinii, cytomegalovirus and Mycobacterium avium1, 2, 6, 10, 11. In Brazil is the sixth, exceeded by Candida species, P. carinii, Mycobacterium species, Toxoplasma gondii, and herpes simplex virus (AIDS, Boletim Epidemiológico, set/nov 96, Ministério da Saúde, Brasil). During 30 years, the treatment of C. neoformans meningitis was based on the use of amphotericin B with or without flucytosine13. Nowadays, with the immunodepression caused by human immunodeficiency virus (HIV) infection and the availability of new antifungal drugs as the triazoles, the concept related to cure and relapses of cryptococcosis has been altered7, 20. Patients are treated with amphotericin B with or without flucytosine as initial therapy, but maintenance therapy is always necessary in AIDS patients with C. neoformans infections

This study evaluated the usefulness of the anti-HBc, hepatitis C virus antibodies (anti-HCV), human T cell lymphotropic virus I and II antibodies (anti-HTLV I/II), serologic tests for syphilis, and surface antigen of hepatitis B virus (HBsAg) as surrogate markers for the risk for HIV infection in 80,284 serum samples from blood donors from the Blood Bank of "Hospital Universitário Regional Norte do Paraná", Londrina, Paraná State, Brazil, analyzed from July 1994 to April 2001. Among 39 blood donors with positive serology for HIV, 12 (30.8%) were anti-HBc positive, 10 (25.6%) for anti-HCV, 1 (2.6%) for anti-HTLV I/I, 1 (2.6%) was positive for syphilis, and 1 (2.6%) for HBsAg. Among the donors with negative serology for HIV, these markers were detected in 8,407 (10.5%), 441 (0.5%), 189 (0.2%), 464 (0.6%), and 473 (0.6%) samples, respectively. The difference was statistically significant (p < 0.001) for anti-HBc and anti-HCV. Although the predictive positive value for these surrogate markers were low for HIV infection, the results confirmed the anti-HBc and anti-HCV as useful surrogate markers for HIV infection thus reinforcing the maintenance of them in the screening for blood donors contributing to the prevention of the small number of cases in which HIV is still transmitted by transfusion.

OBJECTIVES: To determine the frequency of radiological manifestations of chest tuberculosis among the tuberculosis outpatients at the Santa Casa de Misericórdia de São Paulo Hospital, and to correlate these radiological findings with the sputum bacilloscopy. SAMPLE AND METHODS: A review was made of the medical record cards and chest X-rays of all patients attended between January 1996 and December 1998. Patients with a diagnosis of tuberculosis who presented intrathoracic manifestations of the disease and negative anti-HIV serology were selected. RESULTS: The selection included 153 patients, with an average age of 37.5 years, who were predominantly male (60.8%) and white (56.9%). Pulmonary lesions were present in 121 (79.9%) and extrapulmonary lesions in 32 (20.1%). Parenchymal-infiltrate lesions appeared in 56 patients (36.6%), cavity lesions in 55 (36.0%), pleural effusion in 28 (18.3%), isolated nodules in 6 (3.9%), mediastinal enlargement in 4 (2.6%) and miliary pattern in 4 (2.6%). Cavities were present in 45.5% of the patients with pulmonary lesions, generally in association with the parenchymal-infiltrate lesions. Parenchymal infiltrate was present in 86.8% of the patients with pulmonary lesions. There was significant presence of alcohol-acid resistant bacillus in the sputum of patients with cavities (76.4%), in comparison with those without cavities (50%) (p = 0.003). CONCLUSIONS: Parenchymal-infiltrate lesions are the most frequent radiological manifestation of pulmonary tuberculosis, and they are generally associated with cavities. There is a relationship between the presence of acid fast bacilli in sputum and pulmonary cavity lesions.

Serum- and/or- cerebrospinal fluid (CSF) samples obtained from 190 patients suffering from chronic, progressive neurological disease were screened for the presence of human T-cell lymphotropic viruses type I (HTLV-I) and type II (HTLV-II) antibodies over a six-year period (1996 to 2001) in Belém, Pará, Brazil. Patients were of both sexes (male subjects, 52%) with ages ranging from 2 to 79 years (mean, 35.9). Overall, 15 (7.9%) subjects - of whom 12 (80%) were female adults - reacted HTLV-I/II-seropositive when screened by enzyme-linked immunosorbent assay (ELISA). Serum samples from 14 of these patients were also analyzed using a recombinant Western blot (WB) assay that yielded HTLV-I-, HTLV-II-, and HTLV-I/II- reactivities for 10 (71.4%), 3 (21.4%) and 1 (7.2%) of them, respectively. The yearly rates of HTLV-I/II antibodies ranged from 2.6% (2001) to 21.7% (2000), with progressively increasing seropositivities from 1998 to 2000. Altogether, walking difficulty (n = 5 subjects), spasticity (n = 4) and leg weakness (n = 3) accounted for 80% of symptoms recorded among the 15 patients whose sera had antibodies to HTLV-I/II as detected by ELISA. These findings provide evidence that both HTLV-I and HTLV-II play a role in the development of chronic myelopathy in Belém, Pará, Northern Brazil.

Airbone fungi are considered important causes of allergic rhinitis and allergic asthma. The knowledge of these fungi in a city or region is important for the ecological diagnosis and specific treatment of allergic manifestations induced by inhalation of fungal allergens. The airborne fungi of Fortaleza, State of Ceará, Brazil, were studied during a one year period. Five hundred and twenty Petri dishes with Sabouraud dextrose agar medium were exposed at ten different locations in the city. The dishes exposed yielded one thousand and five hundred and twenty one colonies of twenty four genera. The most predominants were: Aspergillus (44.7%), Penicillium (13.3%), Curvularia (9.8%), Cladosporium (6.8%), Mycelia sterilia (6.0%), Fusarium (3.5%), Rhizopus (3.1%), Drechslera (2.6%), Alternaria (2.4%) and Absidia (2.2%). The results shown that Aspergillus, Penicillium, Mycelia sterilia, Fusarium and Alternaria were found during all months in the year. Absidia was more frequent during the dry season. Anemophilous fungi and the high concentration of spores in the air are important because may result in an increased number of people with allergic respiratory disease.

The main purpose of this study was to investigate natural infection by Leishmania in phlebotomine females in a visceral-leishmaniasis focus in Antonio João county in Mato Grosso do Sul State, Brazil. Between June and October 2003, the digestive tracts of 81 females captured in Aldeia Campestre, Aldeia Marangatu and Povoado Campestre were dissected. The females were separated by species, location, area and date of capture into 13 groups and kept in ethanol 70%. To identify the Leishmania species using the PCR technique, amplifications of the ribosomal-DNA (rDNA) and mini-exon genes were analyzed. Of the 81 specimens, 77 (95%) were Lutzomyia longipalpis, making this the most common species; only one specimen of each of the species Brumptomyia avellari, Evandromyia cortelezzii, Evandromyia lenti and Nyssomyia whitmani was found. Trypanosomatids were identified in eight of the nine groups of Lutzomyia longipalpis (10.39%) one group from Aldeia Campestre, one from Aldeia Marangatu and six from Povoado Campestre; of the eight groups, one from Aldeia Marangatu and another, with promastigotes forms also confirmed by dissection (1.23%) from Povoado Campestre, were identified by PCR as Leishmania chagasi (2.6%). The other groups gave negative results. These findings indicate that there is a high risk of leishmaniasis transmission in this area.

Adenovirus (AdV) respiratory infections are usually described as being associated with high mortality rates. Laboratory diagnosis is essential for the establishment of the appropriate therapy, and for guiding the implementation of preventive measures in order to prevent the spread of the infection. Aiming to analyze the sensitivity and specificity of the laboratorial diagnosis methods available, we compared antigen detection by indirect immunofluorescence assay (IF), and a specific nested polymerase chain reaction (PCR), to detect AdV in respiratory samples collected from patients admitted to hospital with acute respiratory disease. Positive samples were inoculated into a cell culture to confirm the results. We analyzed 381 samples from the nasopharyngeal aspirates collected during the year 2008; of these, 2.6% tested were positive for adenovirus through IF and 10% through PCR; positive isolation was obtained in 40% and 26% of these cases, respectively. Most infected patients were children under six months of age, and despite of the fact that a significant number of patients required intensive care, the mortality rate was low (5%). In conclusion, molecular methods were found to be useful for rapid diagnosis of adenovirus infections with higher sensitivity than antigen detection; their introduction permitted a significant increase in diagnoses of adenovirus infections.

SUMMARY Chagas disease is caused by Trypanosoma cruzi and affects about two to three million people in Brazil, still figuring as an important public health problem. A study was conducted in a rural area of the municipality of Limoeiro do Norte - CE, northeastern Brazil, aiming to determine the prevalence of T. cruzi infection. Of the inhabitants, 52% were examined, among whom 2.6% (4/154) were seropositive in at least two serological tests. All seropositive individuals were older than 50 years, farmers, with a low education and a family income of less than three minimum wages. Active surveillance may be an alternative for early detection of this disease.

Procedeu-se a um levantamento coproscópico em alunos de 8 a 15 anos, pertencentes a 3 escolas do Alto da Boa Vista. De 155 alunos examinados, encontraram-se 4 casos positivos (2,6%) de esquistossomose. Entre 25 familiares destes, encontraram-se outros 4 casos positivos (16%). Havia na área uma população de Biomphalaria tenagophila (Orbigny, 1835), amplamente distribuída nas valas de irrigação de hortas de agrião, ligadas ao rio das Fumas. A taxa de infecção dos moluscos foi de 7 (0,29%) em 2.400 examinados. Comparando-se estes resultados com dados anteriores, observa-se que a prevalência da esquistossomose se mantém no Alto da Boa Vista há pelo menos 15 anos.

Thirteen communities from 7 Argentinian provinces were selected for the evaluation of serology as an indicator of transmission of Chagas disease. Of the communities appraised, 6 did not have a history of previous treatment with insecticides and 7 had received sporadic or continuous insecticide treatment. The inhabitants of 20% of the houses of each locality were studied by serology. The samples were obtained byfinger pricking and 50 fil of blood were mixed with 150μl of 50% glycerine solution in tissue culture media to be assayed by Indirect Hemagglutination and Indirect Immunofluorescence tests. In untreated areas, the prevalence of infection in infants 0-4 years old was 17.5%, reaching to over 22% for the 5-9 year old group, and to 33.3% in 10-14 year old individuals. The prevalence in treated and surveyed areas was 2.6% in 0-4 year old children, 5.4% in 5-9 year old and 6,2% in 10-14 year old youngsters. The differences between both areas were statistically significant (p < 0.005). This study favors serology as a valid indicator for the evaluation of transmission of Chagas disease in rural areas.

Ao estudar 30 casos de cromoblastomicose diagnosticados no Hospital dos Servidores do Estado do Maranhão, no período de novembro de 1988 a março de 1993, os autores observaram 2 (6,6 %) casos, que apresentaram associação desta doença com hanseníase. O primeiro paciente desenvolveu as duas doenças concomitantemente, apresentando espessamento no nervo cubital bilateral, mal perfurante plantar em pé direito e lesões em placas verrucóides na perna esquerda, com biópsia de nervo cubital direito positiva para hanseníase dimorfa T e biópsia da lesão em placa, positiva para Fonsccaea pedrosoi. O segundo caso, paciente com história de hanseníase virchowiana há 30 anos, em pausa terapêutica por "cura", com lesões verrugo-confluentes em cotovelo direito há 12 meses, histopatológico e cultura positiva, para cromoblastomicose. Os possíveis fatores para o desenvolvimento da cromoblastomicose nestes pacientes são discutidos.

Após estudo de 30pacientes portadores de cromoblastomicose, acompanhados no hospital dos servidores do Estado do Maranhão, verificou-se em 2 (6,6%) lesões ná região glútea, diferindo do que geralmente se observa, pois na cromoblastomicose existe sempre história de microtraumatismo sofridos durante o trabalho na lavoura, propiciando o desenvolvimento mais freqüente das lesões nas extremidades, principalmente dos membros inferiores. Ambos pacientes, doentes há 10 anos, apresentavam lesões nodulares e verrugo-confluentes em placas coalescentes na região glútea direita. O diagnóstico etiológico foi firmado através de exame histopatológico e cultura, com isolamento de Fonsecaea pedrosoi. Na investigação epidemiológica do tipo de exposição verificou-se que os mesmos desempenhavam a profissão de quebradores de coco-babaçu, atividade relativamente comum no Estado. Parece evidente a relação entre o tipo de atividade profissional e o desenvolvimento da infecção.

Neste trabalho, estudou-se as espécies do gênero Anopheles que se atraem por iscas humanas na Região de Buriticupu, Santa Luzia, Estado do Maranhão. Foram encontradas 7 espécies do subgênero Nyssorhyncus A. (N) darlingi, o principal vetor da malária humana na região, foi a espécie mais freqüente (53,1% da amostra total). Seguem na ordem de importância, A. (N) evansae (21.0%), A. (N) triannulatus (17.4%) e A. (N) nuñeztovari (4,8%). As demais espécies, A. (N) argyritarsis, A. (N) oswaldoi e A. (N) rangeli representaram juntas 3,7% da amostra total. As visitas foram mais frequentes no extra (51,7% da amostra total) do que no peridomicílio (45,7%). A. (N) darlingi e A. (N) evansae foram as únicas espécies encontradas no intradomicílio, numa proporção de aproximadamente 2,6% da amostra total. Os anofelinos ocorreram nas estações seca e chuvosa, com maior frequência na última, coincidindo com os elevados índices de caso de malária naquela região.

No Amazonas, o acidente ofídico é um problema de saúde pública pouco conhecido. Por este motivo, foi realizado um estudo descritivo dos acidentes ofídicos atendidos nas Unidades de Saúde de 34 municípios, um distrito e dois pelotões de fronteira do Estado do Amazonas. As características mais comuns encontradas dentre os pacientes foram: agricultor (50,4%), do sexo masculino (81,3%), em idade produtiva (72,1%), picado no membro inferior (88,5%), por jararaca (48,6%) ou surucucu (46,8%), na zona rural de seu município (70,2%) e que só recebeu atendimento médico em tempo superior a seis horas, após acidente (57,3%). As manifestações locais mais freqüentes foram: edema (76,9%), dor (68,7%), eritema (10,2%) e hemorragia (9,3%). Hemorragia (18,8%) foi a manifestação sistêmica mais freqüente. O antiveneno foi administrado em apenas 65,9% dos pacientes. A via mais utilizada foi a endovenosa (52,3%), sendo relevante o uso de vias não mais recomendadas (47,7%). O antiveneno administrado, na maioria dos pacientes, foi o antibotrópico (66,7%). As complicações mais freqüentes foram abcesso (13,7%), necrose (12,3%), infecção secundária (8,3%), insuficiência renal (2,5%) e gangrena (2,5%). Os procedimentos médicos mais usados para o tratamento das complicações foram: drenagem (52,6%), debridamento (28,9%), amputação (10,5%), limpeza cirúrgica (5,3%) e diálise peritoneal (2,6%). A letalidade foi de 1%.

O objetivo desse trabalho foi determinar a prevalência das parasitoses em escolares de Bambuí, através de exames coprológicos (direto e Kato-Katz) e reavaliar os criadouros de moluscos descritos no município. Dos 2.901 escolares examinados, 20,1% estavam parasitados, sendo que Giardia lamblia, Entamoeba coli, Ascaris lumbricoides e ancilostomídeos foram os parasitas mais freqüentes, com prevalências de 6,2%, 6,2%, 4,8% e 1,4%, respectivamente. Os ancilostomídeos foram significativamente mais freqüentes na zona rural e nos alunos com mais de 14 anos, enquanto a prevalência da E. coli foi maior na zona urbana e a G. lamblia mais freqüente na faixa etária de 0-6 anos. Somente três crianças eliminavam ovos de Schistosoma mansoni. O único hospedeiro intermediário encontrado foi a Biomphalaria glabrata e nenhuma delas estava eliminando cercárias de S. mansoni . Comparando-se estes dados a de levantamentos realizados anteriormente no município, observou-se uma queda na prevalência de todos os parasitas. Algumas hipóteses para tentar explicar esta queda são discutidas tais como: processo intenso de urbanização e a melhoria das condições sócio-sanitárias do município.
