OBJECTIVE: To determine the prevalence rates of infections among intensive care unit patients, the predominant infecting organisms, and their resistance patterns. To identify the related factors for intensive care unit-acquired infection and mortality rates. DESIGN: A 1-day point-prevalence study. SETTING:A total of 19 intensive care units at the Hospital das Clínicas - University of São Paulo, School of Medicine (HC-FMUSP), a teaching and tertiary hospital, were eligible to participate in the study. PATIENTS: All patients over 16 years old occupying an intensive care unit bed over a 24-hour period. The 19 intensive care unit s provided 126 patient case reports. MAIN OUTCOME MEASURES: Rates of infection, antimicrobial use, microbiological isolates resistance patterns, potential related factors for intensive care unit-acquired infection, and death rates. RESULTS: A total of 126 patients were studied. Eighty-seven patients (69%) received antimicrobials on the day of study, 72 (57%) for treatment, and 15 (12%) for prophylaxis. Community-acquired infection occurred in 15 patients (20.8%), non- intensive care unit nosocomial infection in 24 (33.3%), and intensive care unit-acquired infection in 22 patients (30.6%). Eleven patients (15.3%) had no defined type. The most frequently reported infections were respiratory (58.5%). The most frequently isolated bacteria were Enterobacteriaceae (33.8%), Pseudomonas aeruginosa (26.4%), and Staphylococcus aureus (16.9%; [100% resistant to methicillin]). Multivariate regression analysis revealed 3 risk factors for intensive care unit-acquired infection: age > 60 years (p = 0.007), use of a nasogastric tube (p = 0.017), and postoperative status (p = 0.017). At the end of 4 weeks, overall mortality was 28.8%. Patients with infection had a mortality rate of 34.7%. There was no difference between mortality rates for infected and noninfected patients (p=0.088). CONCLUSION: The rate of nosocomial infection is high in intensive care unit patients, especially for respiratory infections. The predominant bacteria were Enterobacteriaceae, Pseudomonas aeruginosa, and Staphylococcus aureus (resistant organisms). Factors such as nasogastric intubation, postoperative status, and age ³60 years were significantly associated with infection. This study documents the clinical impression that prevalence rates of intensive care unit-acquired infections are high and suggests that preventive measures are important for reducing the occurrence of infection in critically ill patients.

A cross-sectional study of 120 subjects was performed with the purpose of evaluating stress hormones and emotional stress (anxiety) in outpatient and hospitalized subjects. The aims were to determine the degree of objective stress, as well as to correlate this finding with subjective findings, estimated using Beck's Anxiety Inventory.. METHOD: Three populations were investigated, namely outpatient clinical cases (Group I, n = 30), hospitalized clinical individuals (Group II, n = 30), and hospitalized surgical candidates (Group III, n = 30). Controls (Group IV, n = 30) were healthy volunteers who were health-care professionals and students. To avoid hormone interactions, only men were enrolled in all groups. All hospitalized subjects were tested on admission and before therapeutic interventions. Fasting epinephrine, norepinephrine, and cortisol were measured in the morning, and Beck's Anxiety Inventory was adminstered by a trained psychologist. RESULTS: The 3 patient groups displayed higher anxiety levels than the controls. Hormone concentrations did not present remarkable changes and did not correlate with subjective stress (anxiety). CONCLUSIONS: 1) Subjective disorders (as determined with Beck's Anxiety Inventory ) were a common finding in both outpatient and hospitalized populations, without differences between the various groups; 2) Objective stress (as determined by elevated hormone levels) was more difficult to confirm-findings rarely exceeded the reference range; 3) Correlation between the two variables could not be demonstrated; 4) Further studies are necessary to define stress quantification and interpretation in patient populations, especially in relationship with nutritional diagnosis and dietetic prescription.

Avaliação da precisão da amostragem sistemática em relação a amostragem aleatória, ambas utilizadas em inventários florestais, com base no erro padrão estimado e tendo como parâmetro de comparação, a área basal. Os dois tipos de amostragem foram aplicados em uma população florestal, 100% inventariada (DAP maior ou igual a 25 cm), constituída de 4 blocos experimentais de 24 hectares cada e distribuídos aleatoriamente sobre uma bacia hidrográfica de aproximadamente 600 hectares. Essa área experimental faz parte do projeto de Manejo Ecológico (Bacia 3). O menor erro padrão estimado foi obtido através da amostragem sistemática, mesmo considerando que na amostragem aleatória foram feitos 5 sorteios diferentes para a tomada das unidades amostrais do inventário simulado.

In this study, we concentrate on modelling gross primary productivity using two simple approaches to simulate canopy photosynthesis: "big leaf" and "sun/shade" models. Two approaches for calibration are used: scaling up of canopy photosynthetic parameters from the leaf to the canopy level and fitting canopy biochemistry to eddy covariance fluxes. Validation of the models is achieved by using eddy covariance data from the LBA site C14. Comparing the performance of both models we conclude that numerically (in terms of goodness of fit) and qualitatively, (in terms of residual response to different environmental variables) sun/shade does a better job. Compared to the sun/shade model, the big leaf model shows a lower goodness of fit and fails to respond to variations in the diffuse fraction, also having skewed responses to temperature and VPD. The separate treatment of sun and shade leaves in combination with the separation of the incoming light into direct beam and diffuse make sun/shade a strong modelling tool that catches more of the observed variability in canopy fluxes as measured by eddy covariance. In conclusion, the sun/shade approach is a relatively simple and effective tool for modelling photosynthetic carbon uptake that could be easily included in many terrestrial carbon models.

OBJETIVO: Avaliar os resultados imediatos e complicações da valvoplastia mitral percutânea por balão (VMPB), com o balão de Inoue (BI) e com o balão único (BU). MÉTODOS: Dentre 390 procedimentos utilizaram-se o BI em 29 procedimentos e o BU de baixo perfil em 337. Não houve diferença na idade e sexo nos 2 grupos. O grupo BI era menos sintomático (p=0,0015). Não houve diferença na distribuição do escore ecocardiográfico e da área valvar mitral (AVM) pré-VMPB. RESULTADOS: Quando compararam-se os 2 grupos entre si, os resultados nos grupos BI e BU foram, respectivamente: pré-VMPB para pressão pulmonar média (PPM) 36±15 e 39±14mmHg, p=0,2033, para gradiente (GRAD) mitral médio 17±6 e 20±7mmHg, p=0,0396 e AVM 0,9±0,2 e 0,9±9,2cm², p=0,8043, enquanto os valores pós-VMPB foram PPM 25±8 e 28±10mmHg, p=0,2881, GRAD 5±3 e 5±4mmHg, p=0,2778 e AVM 2,2±0,2 e 2,0±0,4cm², p=0,0362. Pré-VMPB a válvula mitral era competente em 26 procedimentos com o BI e 280 dos com o BU e havia regurgitação mitral de +/4 em 3 do grupo BI e em 57 do BU, p=0,3591 e pós-VMPB tivemos, no grupo BI a valva mitral (VM) competente em 18, +/4 em 7 e 2+/4 em 4 e no grupo BU, a VM era competente em 218, +/4 em 80, 2+/4 em 25, 3+/4 em 5 e 4+/4 em 2, p=0,7439. Só houve complicações no grupo BU. CONCLUSÃO: As duas técnicas foram eficientes. Os resultados hemodinâmicos foram semelhantes, embora a AVM pós-VMPB do grupo do BI foi maior.

OBJETIVOS: Comparar a monitorização residencial da pressão arterial (MRPA) e monitorização ambulatorial da pressão arterial (MAPA) com os registros de consultório e correlacionar o índice de massa de ventrículo esquerdo (IMVE) com a MRPA e medida de consultório. MÉTODOS: Protocolo 1 - Sessenta e oito hipertensos (58±12 anos, 37 mulheres) realizaram: a) MRPA durante 7 dias; b) MAPA de 24h; e c) medida da pressão arterial no consultório, pelo médico. Protocolo 2 - 41 hipertensos (48 ± 14 anos, 25 mulheres) além de MRPA e medida no consultório, realizaram ecocardiograma bi-dimensional. RESULTADOS: Protocolo 1 - a medida de consultório (153±24/96±13mmHg) foi maior (p<0,05) do que a obtida pela MAPA diurna (137±17/87±12mmHg) e pela MRPA (133±18/84±12mmHg). Protocolo 2 - o IMVE mostrou correlação com as médias de pressão sistólica/diastólica da MRPA (r= 0,39/0,49, p<0,05) mas não com as de consultório (r= 0,02/0,22, p>0,05). CONCLUSÃO: A MRPA apresentou valores menores do que a medida de consultório e semelhantes aos da MAPA, além de melhor correlação com IMVE do que a medida de consultório.

OBJECTIVE - To assess the incidence of fatal pulmonary embolism (FPE), the accuracy of clinical diagnosis, and the profile of patients who suffered an FPE in a tertiary University Hospital. METHODS - Analysis of the records of 3,890 autopsies performed at the Department of General Pathology from January 1980 to December 1990. RESULTS - Among the 3,980 autopsies, 109 were cases of clinically suspected FPE; of these, 28 cases of FPE were confirmed. FPE accounted for 114 deaths, with clinical suspicion in 28 cases. The incidence of FPE was 2.86%. No difference in sex distribution was noted. Patients in the 6th decade of life were most affected. The following conditions were more commonly related to FPE: neoplasias (20%) and heart failure (18.5%). The conditions most commonly misdiagnosed as FPE were pulmonary edema (16%), pneumonia (15%) and myocardial infarction (10%). The clinical diagnosis of FPE showed a sensitivity of 25.6%, a specificity of 97.9%, and an accuracy of 95.6%. CONCLUSION - The diagnosis of pulmonary embolism made on clinical grounds still has considerable limitations.

OBJECTIVE: To assess whether coronary stenting in diabetic patients provides in-hospital results and clinical evolution similar to those in nondiabetic patients. METHODS: From July `97 to April '99 we performed coronary stent implantation in 386 patients with coronary heart disease, who were divided into two groups: diabetic patients and nondiabetic patients. The in-hospital results and the clinical evolution of each group were retrospectively analyzed. RESULTS: The nondiabetic group comprised 305 (79%) patients and the diabetic group 81 (21%) patients. Basic clinical and angiographic characteristics were similar. Angiographic success was in diabetics = 96.6% vs in nondiabetics = 97.9% (p=ns). Among the major complications in the in-hospital phase, the rate of myocardial infarction was higher in the diabetic group (7.4% vs 1.9%) (p=0.022). In the follow-up, a favorable and homogeneous evolution occurred in regard to asymptomatic patients, myocardial infarction, and death in the groups. A greater need for revascularization, however, existed in the diabetic patients (15% vs 2.4%, p<0.001). CONCLUSION: Coronary stenting in diabetic patients is an efficient procedure, with a high angiographic and clinical success rate similar to that in nondiabetic patients. Diabetic patients, however, had a higher incidence of in-hospital myocardial infarction and a greater need for additional myocardial revascularization.

OBJECTIVE: To study by doppler echocardiography the cardiac systolic and diastolic functions of health, uncomplicated obese subjects. METHODS: Fifty-nine obese women with an average body mass index (BMI) of 35 kg/m² were evaluated and compared with 19 subjects with an average BMI of 23 kg/m² (control group). RESULTS: In the obese group, a clear tendency was observed toward higher systolic pressure, increased wall thickness and, consequently, myocardial mass, elevation on the circumference stress of the left ventricular wall, and an indisputable presence of diastolic abnormalities. Filling abnormalities were observed with impaired relaxation, with prolonged isovolumic relaxation time (IVRT) and augmented atrium contribution representing early indexes of cardiac dysfunction when systolic performance is still normal. CONCLUSION: Obesity is generally a chronic condition, and doppler echocardiography can be used as a noninvasive instrument for early evaluation of left ventricular diastolic indexes.

OBJECTIVE: Our aim was to compare, in a non randomized study, the surgical outcome in elderly patients with mechanical (Group 1; n=83) and bioprosthetic valve implants (Group 2; n=136). METHODS: During a three year period, 219 patients >75 years underwent Aortic Valve Replacement. The groups matched according to age, sex, comorbidity, valve pathology and concomitant Coronary Artery Bypass Surgery. Follow-up was a total of 469 patient-years (mean follow-up 2.1 years, maximum 4,4 years). RESULTS: Operative mortality was zero and the overall early mortality was 2.3 % (within 30 days). Actuarial survival was 87.5 ± 4.0% and 66.1 ± 7.7% (NS) at 4 years in Group 1 and Group 2, respectively. Freedom from valve-related death was 88.9 ± 3.8% in Group 1 and 69.9±7.9% (NS) in Group 2 at 4 years. CONCLUSION: Aortic Valve Replacement in the elderly (>75 years) is a safe procedure even in cases where concomitant coronary artery revascularization is performed. Only a few anticoagulant-related complications were reported and this may indicate that selected groups of elderly patients with significant life expectancy may benefit from mechanical implants .

OBJECTIVE: To report a training program in cardiology emphasizing changes in its pedagogical practice. These changes were put into practice by some teachers at the Medical School of Porto Alegre of the University of Rio Grande do Sul (FAMED/UFRGS) aiming to make faculty and student activities more dynamic and to promote more efficacious learning. The training program is directed at 5th semester medical students and aims at a behavioral change in teachers and students to promote more interaction, to favor exchanges, and to make the teaching-learning process easier, always maintaining the patient in the center of the medical activity. METHODS: The program emphasizes the definition of general and specific objectives for each activity to be developed by the students, with training in the area of admission to the cardiology service, with special emphasis on behavioral change in the cognitive, motor, affective, and attitudinal areas. Knowledge was developed by means of interactive seminars with initial and final assessment tests to identify students' and teachers' performance. The students were evaluated in an immediate, continuous, and progressive way in their daily activities and through comparison of the results of 2 tests, one applied at the beginning of the training and the other at its end. These 2 tests contained the same questions. RESULTS: We systematically assessed 560 students over 4 years. The mean grades of the tests performed prior to and after the 244 seminars were 7.38±1.66 and 9.17± 0.82, respectively (p<0.0001). For the tests applied at the beginning and at the end of the training, the mean grades were 5.61±1.61 and 9.37±0.90, respectively (p<0.0001). CONCLUSION: The program proved to be efficient both for the students' learning and for assessing their performance in a systematic and objective way.

OBJECTIVE: To study the in-hospital evolution of patients aged 65 years and older, with acute myocardial infarction, who were treated by direct coronary angioplasty with no fibrinolytic therapy. METHODS: We studied 885 patients divided into 2 groups as follows: group I (GI) - 293 (33.4%) patients aged ³ 65 years (72±5 years), and group II (GII) - 592 patients aged < 65 years (57±9 years). Multivessel disease was more frequent in GI (63.5% x 49.7%; p=0.001). A greater number of GII patients were class I or II of the clinical Killip-Kimball classification (K) (80.2% x 67.2%; p=0.00002), while a significant number of GI patients were KIII and KIV (24.3% x 12.8%; p=0.00003). RESULTS: Group I had a lower index of success (84.6% x 94%; p=0.0002) and a greater in-hospital mortality (12.2% x 4.7%; p=0.00007). The predictors of mortality in GI were as follows: previous infarction (20.5% x 6.3%; p=0.02), anterior location (13.4% x 6.4%; p=0.03), and male sex (10.4% x 4.4%; p=0.007). CONCLUSION: Elderly patients had more severe acute myocardial infarction and more extensive disease, a lower index of success, and greater in-hospital mortality. Previous infarction, anterior location and male sex were identified as predictors of mortality in the elderly group (GI).

OBJETIVO: Determinar os efeitos do estado e especificidade de treinamento aeróbio na relação entre o percentual do consumo máximo de oxigênio (%VO2max) e o percentual da frequência cardíaca máxima (%FCmax) durante o exercício incremental realizado no cicloergômetro. MÉTODOS: Sete corredores, 9 ciclistas, 11 triatletas e 12 sedentários, todos do sexo masculino e aparentemente saudáveis, foram submetidos a um teste incremental até a exaustão no cicloergômetro. Regressões lineares entre %VO2max e %FCmax foram determinadas para cada indivíduo. Com base nessas regressões, foram calculados %FCmax correspondentes a determinados %VO2max (50, 60, 70, 80 e 90%) de cada participante. RESULTADOS: Não foram encontradas diferenças significantes entre todos os grupos nos %FCmax para cada um dos %VO2max avaliados. Analisando-se os voluntários como um único grupo, as médias dos %FCmax correspondentes a 50, 60, 70, 80 e 90% %VO2max foram 67, 73, 80, 87, e 93%, respectivamente. CONCLUSÃO: Nos grupos analisados, a relação entre o %VO2max e %FCmax durante o exercício incremental no ciclismo não é dependente do estado e especificidade do treinamento aeróbio.

OBJETIVO: Comparar os fluxos livres, os calibres e os comprimentos da artéria torácica interna esquerda (ATIE) pediculada (P) e esqueletizada (E) no transoperatório antes e depois da aplicação do vasodilatador tópico (VT). MÉTODOS: Em ensaio clínico randomizado e cego foram estudados 50 pacientes submetidos a cirurgia de revascularização miocárdica eletivas quanto ao emprego da ATIE in situ nas formas P ou E. Dos 25 pacientes no grupo pediculada (GP), 16 eram do sexo masculino, apresentando angina classe II e III (NYHA) e fração de ejeção (FE) de 50,8 ± 9,2%. No grupo esqueletizada (GE), 19 pacientes eram do sexo masculino, possuindo angina classe II e FE de 46,8 ± 9,3%. As medidas foram realizadas pré-circulação extracorpórea e divididas em duas fases: fase 1 (antes) e fase 2 (após 15 min da aplicação da papaverina tópica a 2,5 mg/ml a 37ºC). Durante as aferições, eram monitoradas a pressão arterial média, pressão venosa central e freqüência cardíaca. RESULTADOS: O GP apresentou fluxo de 46±16 e 77±28 ml/min e calibre de 1,4±0,1 e 1,7±0,1 mm nas fases 1 e 2, e o GE, 57±27 e 97±35 ml/min de fluxo e 1,4±0,1 e 1,8±0,2 mm de calibre, respectivamente, não havendo diferenças significativas no desfecho comprimento. CONCLUSÃO: ATIE E demonstrou um aumento significativo no fluxo e calibre, após o uso de VT com p=0,03 e p=0,01 respectivamente, comparados com a ATIE P.

OBJETIVO: Verificar se o tratamento com levosimendan seria mais dispendioso que o usual com dobutamina, uma vez que o preço dos medicamentos não representa a maior despesa no tratamento da descompensação cardíaca. MÉTODOS: Comparou-se o custo do tratamento de 18 pacientes hospitalizados devido a descompensação cardíaca, 9 tratados com dobutamina (grupo dobuta) e 9 com levosimendan (grupo levo). Os grupos foram semelhantes quanto à idade, sexo, classe funcional e função cardíaca. RESULTADOS: O custo do tratamento foi semelhante para os dois grupos. No grupo levo as despesas com medicamentos foram maiores, mas as relativas ao período de terapia intensiva e do material empregado foram menores. Levo - medicamentos: R$ 5.414,00; materiais: R$ 399,90; diárias hospitalares: R$ 5.061,20; serviços profissionais: R$ 3.241,80; final: R$ 14.117,00. Dobuta - medicamentos: R$ 2.320,10; materiais: R$ 1.665,70; diárias hospitalares: R$ 6.261,90; serviços profissionais: R$ 3.894,30; final: R$ 14.142,00. CONCLUSÃO: Apesar do preço mais elevado da droga, o custo global do tratamento foi semelhante para os pacientes tratados com dobutamina ou levosimendan. O paciente tratado com levosimendan permaneceu menos tempo em terapia intensiva.
