Adenovirus (AdV) respiratory infections are usually described as being associated with high mortality rates. Laboratory diagnosis is essential for the establishment of the appropriate therapy, and for guiding the implementation of preventive measures in order to prevent the spread of the infection. Aiming to analyze the sensitivity and specificity of the laboratorial diagnosis methods available, we compared antigen detection by indirect immunofluorescence assay (IF), and a specific nested polymerase chain reaction (PCR), to detect AdV in respiratory samples collected from patients admitted to hospital with acute respiratory disease. Positive samples were inoculated into a cell culture to confirm the results. We analyzed 381 samples from the nasopharyngeal aspirates collected during the year 2008; of these, 2.6% tested were positive for adenovirus through IF and 10% through PCR; positive isolation was obtained in 40% and 26% of these cases, respectively. Most infected patients were children under six months of age, and despite of the fact that a significant number of patients required intensive care, the mortality rate was low (5%). In conclusion, molecular methods were found to be useful for rapid diagnosis of adenovirus infections with higher sensitivity than antigen detection; their introduction permitted a significant increase in diagnoses of adenovirus infections.

OBJECTIVE: To evaluate the prevalence of the urinary excretion of BKV and JCV in HIV-infected patients without neurological symptoms. METHODS: Urine samples from HIV-infected patients without neurological symptoms were tested for JC virus and BK virus by PCR. Samples were screened for the presence of polyomavirus with sets of primers complementary to the early region of JCV and BKV genome (AgT). The presence of JC virus or BK virus were confirmed by two other PCR assays using sets of primers complementary to the VP1 gene of each virus. Analysis of the data was performed by the Kruskal-Wallis test for numerical data and Pearson or Yates for categorical variables. RESULTS: A total of 75 patients were included in the study. The overall prevalence of polyomavirus DNA urinary shedding was 67/75 (89.3%). Only BKV DNA was detected in 14/75 (18.7%) urine samples, and only JCV DNA was detected in 11/75 (14.7%) samples. Both BKV and JCV DNA were present in 42/75 (56.0%) samples. CONCLUSION: In this study we found high rates of excretion of JCV, BKV, and simultaneous excretion in HIV+ patients. Also these results differ from the others available on the literature.

Coccidioidomycosis is an emerging fungal disease in Brazil; adequate maintenance and authentication of Coccidioides isolates are essential for research into genetic diversity of the environmental organisms, as well as for understanding the human disease. Seventeen Coccidioides isolates maintained under mineral oil since 1975 in the Instituto de Medicina Tropical de São Paulo (IMTSP) culture collection, Brazil, were evaluated with respect to their viability, morphological characteristics and genetic features in order to authenticate these fungal cultures. Only five isolates were viable after almost 30 years, showing typical morphological characteristics, and sequencing analysis using Coi-F and Coi-R primers revealed 99% identity with Coccidioides genera. These five isolates were then preserved in liquid nitrogen and sterile water, and remained viable after two years of storage under these conditions, maintaining the same features.

The authors report a case of bilateral Tinea nigra plantaris treated through a double-blind study with the topical antifungal agents Isoconazole and Terbinafine. The objective of the study was to clinically compare the efficacy of these two topical antifungal agents on days 10, 20 and 30 of the treatment. No significant clinical differences were found, as all the plantar lesions regressed completely by the end of the treatment. Our conclusion was that in the case reported, the topical antifungal agents Isoconazole and Terbinafine demonstrated identical efficacy as a clinical cure. We also suggest the inclusion of injuries caused by arthropods of the Diplopoda Class in the differential diagnosis of Tinea nigra plantaris, due to the persistent acral hyperpigmentation.

The reactivation of Chagas disease in HIV infected patients presents high mortality and morbidity. We present the case of a female patient with confirmed Chagasic meningoencephalitis as AIDS-defining illness. Interestingly, her TCD4+ lymphocyte cell count was 318 cells/mm3. After two months of induction therapy, one year of maintenance with benznidazol, and early introduction of highly active antiretroviral therapy (HAART), the patient had good clinical, parasitological and radiological evolution. We used a qualitative polymerase chain reaction for the monitoring of T. cruzi parasitemia during and after the treatment. We emphasize the potential value of molecular techniques along with clinical and radiological parameters in the follow-up of patients with Chagas disease and HIV infection. Early introduction of HAART, prolonged induction and maintenance of antiparasitic therapy, and its discontinuation are feasible, in the current management of reactivation of Chagas disease.

SUMMARYAIDS-related cryptococcal meningitis continues to cause a substantial burden of death in low and middle income countries. The diagnostic use for detection of cryptococcal capsular polysaccharide antigen (CrAg) in serum and cerebrospinal fluid by latex agglutination test (CrAg-latex) or enzyme-linked immunoassay (EIA) has been available for over decades. Better diagnostics in asymptomatic and symptomatic phases of cryptococcosis are key components to reduce mortality. Recently, the cryptococcal antigen lateral flow assay (CrAg LFA) was included in the armamentarium for diagnosis. Unlike the other tests, the CrAg LFA is a dipstick immunochromatographic assay, in a format similar to the home pregnancy test, and requires little or no lab infrastructure. This test meets all of the World Health Organization ASSURED criteria (Affordable, Sensitive, Specific, User friendly, Rapid/robust, Equipment-free, and Delivered). CrAg LFA in serum, plasma, whole blood, or cerebrospinal fluid is useful for the diagnosis of disease caused by Cryptococcusspecies. The CrAg LFA has better analytical sensitivity for C. gattii than CrAg-latex or EIA. Prevention of cryptococcal disease is new application of CrAg LFA via screening of blood for subclinical infection in asymptomatic HIV-infected persons with CD4 counts < 100 cells/mL who are not receiving effective antiretroviral therapy. CrAg screening of leftover plasma specimens after CD4 testing can identify persons with asymptomatic infection who urgently require pre-emptive fluconazole, who will otherwise progress to symptomatic infection and/or die.

The spectrum of neurological complications associated with human immunodeficiency virus type 1 (HIV-1) infection is broad. The most frequent etiologies include primary diseases (caused by HIV itself) or secondary diseases (opportunistic infections or neoplasms). Despite these conditions, HIV-infected patients are susceptible to other infections observed in patients without HIV infection. Here we report a rare case of a brain abscess caused by Staphylococcus aureus in an HIV-infected patient. After drainage of the abscess and treatment with oxacilin, the patient had a favorable outcome. This case reinforces the importance of a timely neurosurgical procedure that supported adequate management of an unusual cause of expansive brain lesions in HIV-1 infected patients.

O limiar anaeróbio (LA) medido pela análise da curva-V é a melhor medida da reserva funcional na insuficiência cardíaca e conseqüentemente a medida que melhor retrata o grau de comprometimento cardíaco. No presente trabalho foram estudados 28 pacientes (18 homens; idade média de 47 ± 6,7 anos) com sorologia positiva para a doença de Chagas, com graus variáveis de comprometimento cardíaco segundo a classificação de Los Andes e sem outras doenças associadas. A análise de variância mostrou que o LA dos pacientes do grupo IA (ECG e ventriculograma no ECO normal) é significativamente mais altos do que o valor observado nos demais grupos (p = 0,001). Enquanto o teste de comparações múltiplas de Tukey revelou que as médias do LA nos pacientes dos grupos IB, II e III (IB:ECG normal; ventriculograma no ECO anormal; II: ECG e venticulograma no ECO anormal sem ICC e III com ICC) não diferem significativamente entre si. O presente estudo demonstra que a ocorrência de alterações ventriculográficas iniciais já se associa a significativa redução da reserva cardiopulmonar e este comprometimento é progressivo e paralelo ao grau de comprometimento hemodinâmico cardíaco. A causa mais provável da redução do LA no grupo IB é a disfunção diastólica constatada ao ecocardiograma.

Se analizan los brotes de leptospirosis ocurridos en la provincia de Ciego de Ávila en el periodo de 1980 a 1995. En la etapa se notifican 40 brotes. Las actividades principales vinculadas a los mismos fueron la atención al cultivo de la caña de azúcar, al cultivo del plátano, el baño en río y las inundaciones. Se nota un incremento de brotes a partir del mes de junio. En los meses de octubre y noviembre se reportan las mayores incidencias. Los grupos de edades que más casos aportaron fueron de 10-14 años, 15-19 años y 30-34 años. El sexo más afectado fue el masculino. Los grupos más afectados fueron los estudiantes, pobladores urbanos y trabajadores agrícolas cañeros. De los 40 brotes, 21 fueron confirmados por medio de la prueba de microaglutinación y 19 por la prueba hemolitica, siendo los serogrupos más frecuentes Pomona y Australis.

Se analiza la morbilidad por leptospirosis humana en niños de la Provincia de Ciego de Ávila, Cuba en el período de 1982 a 1995 donde se diagnosticaron 253 casos. Los casos aislados predominaron sobre los presentados en brotes. Con respecto a los grupos de edades predominó el de 10 a 14 años seguido del grupo de 5 a 9 años. Fue más frecuente el sexo masculino que el femenino, en esta etapa se presentó un fallecido. En la presentación de casos por meses se reporta un incremento de enfermos a partir del mes de julio, siendo agosto, octubre y noviembre los meses de mayor incidencia; los síntomas y signos de mayor frecuencia fueron fiebre, cefalea y mialgia; el 92% de los casos eran anictéricos. Los diagnósticos presuntivos más planteados fueron síndrome febril agudo, leptospirosis y meningoencefalitis viral. En las posibles fuentes de infección el contacto con terrenos bajos y el baño en fuentes de agua dulce, presentaron el mayor número de casos. En lo referente al diagnóstico 162 se diagnosticaron por microaglutinación y 91 por prueba hemolítica. En la primera los serogrupos Canícola Australis y Pomona tuvieron el mayor reporte.

We have compared results of Plasmodium species identification obtained with conventional on-site microscopy of Giemsa-stained thick smears (GTS) and a semi-nested polymerase chain reaction (PCR) in 96 malaria patients from Rondônia, Western Brazilian Amazon. Mixed-species infections were detected by PCR in 30% patients, but no such case had been found on GTS. Moreover, P. malariae infections were detected in 9 of 96 patients (10%) by PCR, but were not identified by local microscopists. The potential impact of species misidentification on malaria treatment and control is discussed.

Os acidentes por animais aquáticos venenosos e traumatizantes podem provocar morbidez importante em humanos. Em 236 ocorrências por animais marinhos observadas pelo autor, os ouriços-do-mar causaram cerca de 50%, os cnidários (cubomedusas e caravelas) 25% e peixes venenosos (bagres, arraias e peixes-escorpião) 25% dos acidentes. Nos rios e lagos, as arraias, bagres e mandis causam acidentes que têm mecanismo do envenenamento e efeitos das toxinas semelhantes às espécies marinhas. Em uma série de cerca de 200 acidentes em pescadores de água doce, quase 40% foram causados por bagres e mandis, 5% por arraias de água doce e 55% por peixes traumatogênicos, como as piranhas e as traíras. O autor demonstra os principais animais aquáticos que causam acidentes no Brasil, apresenta aspectos clínicos dos envenenamentos e discute medidas terapêuticas para o controle da intensa sintomatologia observada principalmente nos acidentes causados por cnidários e peixes venenosos.

Mollusks of the genus Conus present a venomous apparatus composed of radulae, a chitin structure linked to glands, which injects potent neurotoxic peptides, causing serious human envenomation and even death, associated with the blockage of certain receptors and muscular paralysis. No reported envenomation has occurred in Brazil, but certain populations are at risk of accidents.
