ABSTRACT OBJECTIVE To validate an instrument designed to assess health promotion in the school environment. METHODS A questionnaire, based on guidelines from the World Health Organization and in line with the Brazilian school health context, was developed to validate the research instrument. There were 60 items in the instrument that included 40 questions for the school manager and 20 items with direct observations made by the interviewer. The items’ content validation was performed using the Delphi technique, with the instrument being applied in 53 schools from two medium-sized cities in the South region of Brazil. Reliability (Cronbach’s alpha and split-half) and validity (principal component analysis) analyses were performed. RESULTS The final instrument remained composed of 28 items, distributed into three dimensions: pedagogical, structural and relational. The resulting components showed good factorial loads (> 0.4) and acceptable reliability (> 0.6) for most items. The pedagogical dimension identifies educational activities regarding drugs and sexuality, violence and prejudice, auto care and peace and quality of life. The structural dimension is comprised of access, sanitary structure, and conservation and equipment. The relational dimension includes relationships within the school and with the community. CONCLUSIONS The proposed instrument presents satisfactory validity and reliability values, which include aspects relevant to promote health in schools. Its use allows the description of the health promotion conditions to which students from each educational institution are exposed. Because this instrument includes items directly observed by the investigator, it should only be used during periods when there are full and regular activities at the school in question.

É apresentado um caso de megaíleo de natureza chagásica. O paciente, portador da Doença de Chagas, branco com 41 anos, apresentava história de 3 anos de episódios de eólicas abdominais, distensão e diarréia, que foram se tornando mais intensas e mais freqüentes. Fora das crises, apresentava-se assintomático. O diagnóstico de megaíleo foi estabelecido por meio do estudo radiológico contrastado do intestino delgado. O estudo histológico realizado em fragmento obtido à intervenção cirúrgica mostrou diminuição do número de células ganglionares dos plexos mientéricos.

The indeterminate form of Chagas' disease is characterized by positive serology for the disease in the absence of clinical findings and in the presence of both normal esophagogram and electrocardiogram. When more sensitive methods were used, abnormalities have been described either in the esophagus or in the heart. The authors have studied simultaneously the esophagus and the heart in the same subjects. In thirteen adults with diagnosis of indeterminate form and nine adult controls, the esophageal manometry both in basal conditions and after stimulus (bethanecol) and vectorcardiogram were performed. In the control group none of the subjects presented concomitant esophageal and cardiac alterations while in the chagasic group 92,3% of the patients presented results simultaneously altered. It is concluded that the studied patients showed indications of parasympathetic denervation manifested by simultaneously esophageal and heart alterations.

During March 1994 cases of a exanthematic acute disease were reported in the municipalities of Itagemirim, Eunápolis and Belmonte, state of Bahia. Dengue fever was confirmed by serology (MAC-ELISA) and by dengue virus type 2 isolation, genotype Jamaica. Signs and symptoms of classic dengue fever were observed with a high percentual of rash (73.8%) and pruritus (50.5%). Major haemorrhagic manifestations were unfrequent and only bleeding gum was reported. Dengue virus activity spreaded rapidly to important tourism counties like Porto Seguro, Ilhéus, Santa Cruz de Cabrália, Prado, Alcobaça and others, representing a risk for the spreading of dengue virus into the country and abroad.

The antimicrobial activity of plant hidroethanolic extracts on bacteria Gram positive, Gram negative, yeasts, Mycobacterium tuberculosis H37 and Mycobacterium bovis was evaluated by using the technique of Agar diffusion and microdilution in broth. Among the extracts evaluated by Agar diffusion, the extract of Bidens pilosa leaf presented the most expressive average of haloes of growth inhibition to the microorganisms, followed by the extract of B. pilosa flower, of Eugenia pyriformis' leaf and seed, of Plinia cauliflora leaf which statistically presented the same average of haloes inhibitory formation on bacteria Gram positive, Gram negative and yeasts. The extracts of Heliconia rostrata did not present activity. Mycobacterium tuberculosis H37 and Mycobacterium bovis(BCG) appeared resistant to all the extracts. The susceptibility profile of Candida albicans and Saccharomyces cerevisiae fungi were compared to one another and to the Gram positive Bacillus subtilis, Enterococcus faecalis and the Gram negative Salmonella typhimurium bacteria (p > 0.05). The evaluation of cytotoxicity was carried out on C6-36 larvae cells of the Aedes albopictus mosquito. The extracts of stem and flower of Heliconia rostrata, leaf and stem of Plinia cauliflora, seed of Anonna crassiflora and stem, flower and root of B. pilosa did not present toxicity in the analyzed concentrations. The highest rates of selectivity appeared in the extracts of stem of A. crassiflora and flower of B. pilosa to Staphylococcus aureus, presenting potential for future studies about a new drug development.

Aim: The objective was to describe an outbreak of bloodstream infections by Burkholderia cepacia complex (Bcc) in bone marrow transplant and hematology outpatients.Methods: On February 15, 2008 a Bcc outbreak was suspected. 24 cases were identified. Demographic and clinical data were evaluated. Environment and healthcare workers' (HCW) hands were cultured. Species were determined and typed. Reinforcement of hand hygiene, central venous catheter (CVC) care, infusion therapy, and maintenance of laminar flow cabinet were undertaken. 16 different HCWs had cared for the CVCs. Multi-dose heparin and saline were prepared on counter common to both units.Findings: 14 patients had B. multivorans(one patient had also B. cenopacia), six non-multivorans Bcc and one did not belong to Bcc. Clone A B. multivorans occurred in 12 patients (from Hematology); in 10 their CVC had been used on February 11/12. Environmental and HCW cultures were negative. All patients were treated with meropenem, and ceftazidime lock-therapy. Eight patients (30%) were hospitalized. No deaths occurred. After control measures (multidose vial for single patient; CVC lock with ceftazidime; cleaning of laminar flow cabinet; hand hygiene improvement; use of cabinet to store prepared medication), no new cases occurred.Conclusions: This polyclonal outbreak may be explained by a common source containing multiple species of Bcc, maybe the laminar flow cabinet common to both units. There may have been contamination by B. multivorans (clone A) of multi-dose vials.

Introduction: Hepatic capillariosis, caused by Capillaria hepatica (Calodium hepaticum) (Bancroft, 1893), Travassos, 1915 (Nematoda, Trichinelloidea, Capillariidae), is a common zoonosis in rodents but is rare in humans. Seventy-two cases in humans have been reported worldwide since the first case was described by MACARTHUR in 192417,27. This study aimed to determine the prevalence of Capillaria hepatica in humans and rodents in an urban area of Porto Velho, the capital of Rondônia, in Brazil. Methods: After conducting a census of the area, 490 residents were randomly selected, and, after signing a term of consent, provided blood samples that were screened for anti-Capillaria hepatica antibodies. Simultaneously, rats were captured to assess the prevalence of this parasite in rodents by histopathological examination in liver sections. Results: A prevalence of 1.8% was found among residents who had specific antibodies at a dilution of 1:150, indicating exposure to parasite eggs; 0.8% of the subjects also had positive titers at a dilution of 1:400, indicating true infection. The prevalence in rats was 2%. Conclusions: The prevalence of infection with this parasite among humans and rats was low. While the prevalence encountered among humans was within the limits reported in the literature, the prevalence among rodents was much lower.

The evaluation of workers as potential reservoirs and disseminators of pathogenic bacteria has been described as a strategy for the prevention and control of healthcare-associated infections (HAIs). The aim of this study was to evaluate the presence of Enterobacteriaceae in the oral cavity of workers at an oncology hospital in the Midwest region of Brazil, as well as to characterize the phenotypic profile of the isolates. Saliva samples of 294 workers from the hospital’s healthcare and support teams were collected. Microbiological procedures were performed according to standard techniques. Among the participants, 55 (18.7%) were colonized by Enterobacteriaceae in the oral cavity. A total of 64 bacteria were isolated, including potentially pathogenic species. The most prevalent species was Enterobacter gergoviae (17.2%). The highest rates of resistance were observed for β-lactams, and 48.4% of the isolates were considered multiresistant. Regarding the enterobacteria isolated, the production of ESBL and KPC was negative. Nevertheless, among the 43 isolates of the CESP group, 51.2% were considered AmpC β-lactamase producers by induction, and 48.8% were hyper-producing mutants. The significant prevalence of carriers of Enterobacteriaceae and the phenotypic profile of the isolates represents a concern, especially due to the multiresistance and production of AmpC β-lactamases.

This cross-sectional study, performed in an oncology hospital in Goiania, aimed to characterize the prevalence of oral colonization and antimicrobial susceptibility of Pseudomonas spp. isolated from the saliva of healthcare workers. Microorganisms were subjected to biochemical tests, susceptibility profile, and phenotypic detection. Of 76 participants colonized with Gram negative bacilli, 12 (15.8%) harbored Pseudomonas spp. Of all isolates, P. aeruginosa (75.0%), P. stutzeri (16.7%), and P. fluorescens (8.3%), were resistant to cefoxitin, and therefore likely to be AmpC producers. The results are clinically relevant and emphasize the importance of surveillance to minimize bacterial dissemination and multiresistance.

Os autores apresentem uma revisão de 45 casos de tuberculose viiliar em adultos, sendo que 42 foram selecionados de urn total de 4.000 necropsias. Uma correlação clínico-patológica é feita senão nitidamente evidenciado c baixo grau de suspeita clinica da doenca e a relativa freqüência desta infecção em Hospitais Gerais conseqüente à mimetização com outras patologias. Uma classificação histovatológica da condição é feita, assim como os principais órgãos atingidos são assinalados. Discussão a respeito dos mecanismos patogênicos é tentada. Finalizam com uma revisão a respeito dos problemas gerais envolvidos na tuberculose miliar.

Amostra de T. cruzi, mediante a inoculação em camundongos brancos jovens, foi isolada de fezes de exemplares de T. infestans capturados em Vitichi, Bolívia e denominada amostra Bolívia. Essa amostra, que se comporta de um modo peculiar em reações de aglutinação, segundo Siqueira, Ribeiro e Fernandes, em 1973, mostrou-se patogênica para camundongos, infectando 100% dos animais inoculados, quer com formas metacíclicas de triatomíneos, quer com formas sanguícolas de doadores com infecção aguda. As formas sanguícolas, em camundongos, medem 23,37 μ de comprimento total médio e tem índice nuclear de 0,94. Nos camundongos mortos durante a fase aguda os ninhos de leishmânias são abundantes, sobretudo no coração. A infecção dos camundongos é grave com curto período prepatente, parasitemia elevada e alta taxa de letalidade. A amostra Bolívia confere, aos animais que sobrevivem, alto grau de resistência contra reinfecção pela amostra Y. Cultiva-se bem em meios líquidos (Warren) e difásicos (NNN) e evolui regularmente em várias espécies de triatomíneos testadas.

A cidade do Salvador, capital do Estado da Bahia, apresenta a população com maior prevalência da infecção pelo HTLV-I no Brasil. Todavia, somente um estudo incluiu uma cidade do interior deste Estado, mesmo assim com número amostral pequeno. O objetivo foi o de avaliar a prevalência de anticorpos anti-HTLV-I/II na população de quatro cidades do interior do Estado da Bahia. As amostras de soro proveninentes de 1.539 indivíduos residentes em Catolândia, Ipupiara, Jacobina e Prado foram triadas através do ELISA, e a confirmação dos resultados nas amostras repetidamente positivas foi realizada através do "Western blot". Quarenta e sete (3,1%) amostras foram positivas pelo ELISA, e 44 destas foram submetidas ao Western blot, com 5 resultados positivos (0,3%), 8 (0,5%) indeterminados (todos da cidade de Jacobina) e 31 negativos. A prevalência geral de anticorpos anti-HTLV-I, nas cidades estudadas, foi de 0,3%. Esta prevalência variou de 0,0% (Prado) a 0,7% (Jacobina), porém não houve diferença estatisticamente significante (p > 0,21). Nenhum indivíduo apresentou anticorpos anti-HTLV-II. Em conclusão, a prevalência da infecção pelo HTLV-I no interior do Estado da Bahia foi baixa, contudo, a população da cidade de Jacobina apresentou a maior prevalência. No entanto, outros estudos epidemiológicos, clínicos e virológicos serão necessários para a melhor compreensão da história natural desta infecção em Jacobina.

A ocorrência de casos de febre amarela (FA) e a ampla distribuição do A. aegypti no Brasil, motivou o estudo da estimativa da proteção imune contra o vírus amarílico vacinal (17D) em moradores de duas cidades do Estado da Bahia, Ipupiara (n = 461) e Prado (n = 228). Nesta área não-endêmica de FA, a pesquisa de anticorpos séricos contra o 17D (Ac17D) e contra 18 outros arbovírus, foi realizada pelo método da inibição da hemaglutinação (IH). Somente 1,2% (8/689) dos indivíduos apresentaram Ac17D, sendo seis com resposta monotípica. A resposta sorológica do tipo heterotípica para Flavivírus (FLV) foi interpretada também como associada à resposta imune ao 17D, sendo mais freqüente em Prado (30,3%) do que em Ipupiara (23,2%). A idade > ou = 50 anos e moradia em outros Estados foram associadas com a soropositividade para FLV, do mesmo modo que a história de vacinação (17D). Mas, a história de vacinação apresentou baixos percentuais de sensibilidade (£ 45,4%) e de valor preditivo-positivo (<= 38,4%), sendo altos os valores da especificidade ( > ou = 70,8%) e do valor preditivo-negativo ( > ou = 78,8%). Em conclusão, foi baixa a freqüência (1,2%) de moradores com Ac17D, apesar da freqüência maior (25,5%) de portadores de anticorpos FLV, o que significa que 26,7% da população estudada pode apresentar proteção contra o vírus da FA.

Como parte de um estudo para avaliar as relações entre mudanças ambientais e ocorrência de infecções, estudou-se a prevalência e intensidade de parasitas intestinais em uma amostra de crianças em idade escolar, residentes em Salvador. A prevalência de infectados por pelo menos um protozoário/helminto foi 66,1%. A prevalência da infecção por helmintos, se eleva com o aumento da idade das crianças, sendo os meninos mais intensamente infectados.
