FUNDAMENTO: A redução da mortalidade por doenças cardiovasculares é observada no Brasil há anos, atribuída à queda nos fatores de risco, melhora na terapêutica e diminuição da mortalidade hospitalar. OBJETIVO: Comparar a mortalidade populacional, o coeficiente de internação e a mortalidade hospitalar em unidades do Sistema Único de Saúde, para doença isquêmica do coração (DIC), doenças cerebrovasculares (DCBV) e insuficiência cardíaca (IC), no município de Niterói, entre 1998 e 2007. MÉTODOS: Foram utilizados o número de óbitos e o de internações e mortalidade hospitalar de residentes em Niterói para o capítulo IX do CID10 e causas específicas disponíveis no Datasus, na população de 30 anos e mais. A diferença entre a magnitude dos indicadores foi calculada para homens e mulheres considerando a média do primeiro triênio menos a média do segundo triênio. RESULTADOS: Houve queda dos coeficientes de mortalidade populacional em homens e mulheres, para todas as faixas etárias, nos três grupos de causas e para o capítulo IX. A tendência à queda dos coeficientes diminuiu com a idade. Para DIC houve queda na mortalidade hospitalar. Para DCBV e IC, aumento. Os coeficientes de internação hospitalar por DIC diminuíram, com exceção de algumas faixas. CONCLUSÃO: O presente estudo permitiu esclarecer alguns aspectos da morbimortalidade cardiovascular em Niterói. A redução da mortalidade populacional e hospitalar por DIC indica haver melhor qualidade na abordagem dessa condição. O aumento da mortalidade hospitalar por DCBV e IC aponta para a necessidade de se dar maior atenção à qualidade do cuidado hospitalar para esses grupos de doenças.

FUNDAMENTO: O Infarto Agudo do Miocárdio (IAM) é a principal causa isolada de óbito entre as doenças crônicas não transmissíveis no Brasil. O conhecimento das tendências de mortalidade é necessário para o planejamento de estratégias de prevenção. OBJETIVO: Avaliar a tendência de mortalidade por infarto do miocárdio no período de 1998 a 2009 na cidade de Curitiba (PR), sua distribuição por gênero, faixa etária e seu impacto na redução do número absoluto de mortes por essa doença nesse período. MÉTODOS: Dados demográficos foram obtidos do Instituto Brasileiro de Geografia e Estatística e dados de óbitos foram obtidos no Sistema de Informação de Mortalidade do Ministério da Saúde, considerando gênero, faixa etária e residência. A partir do ajuste de um modelo de Regressão de Poisson foram estimadas taxas de mortalidade e número de mortes esperadas que não foram observadas. RESULTADOS: Foi encontrada tendência de declínio significativa (p < 0,001) no período. A estimativa da redução média na taxa de óbito por IAM a cada ano foi de 3,8% (IC 95%: 3,2% - 4,5%). Não houve diferença significativa entre os gêneros (p = 0,238); entretanto, a evolução das taxas padronizadas específicas por idade diferiu significativamente entre as faixas etárias (p = 0,018). Estima-se que a redução anual de 3,8% na taxa de mortalidade tenha resultado em 2.168 mortes aquém do número esperado, considerando a taxa de mortalidade observada em 1998 e projetando esse número sobre o crescimento populacional ocorrido no período estudado. CONCLUSÃO: Embora permaneça como causa importante de óbito, a mortalidade por IAM apresentou queda significativa no período avaliado

FUNDAMENTO: Ainda não foram analisadas as características epidemiológicas das doenças da aorta torácica (DAT) no estado de São Paulo e no Brasil, assim como o seu impacto na sobrevida desses pacientes. OBJETIVOS: Avaliar o impacto da mortalidade das DAT e caracterizá-la epidemiologicamente. MÉTODOS: Análise retrospectiva dos dados do Sistema Único de Saúde para os códigos de DAT do registro de internações, de procedimentos e dos óbitos, a partir do Código Internacional de Doenças (CID-10), registrados na Secretaria de Saúde do Estado de São Paulo durante o período de janeiro de 1998 a dezembro de 2007. RESULTADOS: Foram 9.465 óbitos por DAT, 5.500 homens (58,1%) e 3.965 mulheres (41,9%); 6.721 dissecções (71%) e 2.744 aneurismas, 86,3% diagnosticados no IML. Foram 6.109 internações, 67,9% do sexo masculino, sendo que 21,2% evoluíram a óbito (69% homens), com proporções semelhantes de dissecção e aneurisma entre os sexos, respectivamente 54% e 46%, porém com mortalidade distinta. Os homens com DAT morrem mais que as mulheres (OR = 1,5). A distribuição etária para óbitos e internações foi semelhante, com predomínio na sexta década. Foram 3.572 operações (58% das internações) com mortalidade de 20,3% (os pacientes mantidos em tratamento medicamentoso apresentaram mortalidade de 22,6%; p = 0,047). O número de internações, de cirurgias, de óbitos dos pacientes internados e geral de óbitos por DAT foi progressivamente superior ao aumento populacional no decorrer do tempo. CONCLUSÕES: Atuações específicas na identificação precoce desses pacientes, assim como a viabilização do seu atendimento, devem ser implementadas para reduzir a aparente progressiva mortalidade por DAT imposta à nossa população.

This paper presents epidemiological, laboratory, and clinical data on 12 years of dengue virus activity in the State of Rio de Janeiro from the time the disease was first confirmed virologically in April 1986 through April 1998. DEN-1 and DEN-2 viruses are the serotypes circulating in the state and were responsible for the epidemics reported during the last 12 years. The results published here show both the impact of dengue virus infections on the population and laboratory advances that have improved dengue diagnosis.

We analyzed the respiratory syncytial virus (RSV) groups and their epidemiological pattern that were detected over the course of seven years in southern Brazil. The two RSV groups co-circulated each year, but frequencies of groups A and B varied both between and within yearly outbreaks. In 1991, group A predominated over group B (p=0.0016). RSV outbreaks analyzed showed a temperature-dependent pattern and no association with rainfall, similarly to other countries from southern South America. Knowledge of the variants is important in terms of both diagnosis and definition of a vaccine composition.

The epidemiology of the transmission of malaria parasites varies ecologically. To observe some entomological aspects of the malaria transmission in an urban environment, a longitudinal survey of anopheline fauna was performed in Boa Vista, Roraima, Brazil. A total of 7,263 anophelines was collected in human bait at 13 de Setembro and Caranã districts: Anopheles albitarsis sensu lato (82.8%), An. darlingi (10.3%), An. braziliensis (5.5%), An. peryassui (0.9%) and An. nuneztovari (0.5%). Nightly 12 h collections showed that An. albitarsis was actively biting throughout the night with peak activities at sunset and at midnight. An. darlingi bit during all night and did not demonstrate a defined biting peak. Highest biting indices, entomological inoculation rates and malaria cases were observed seasonally during the rainy season (April-November). Hourly collections showed host seek activity for all mosquitoes peaked during the first hour after sunset. An. darlingi showed the highest plasmodial malaria infection rate followed by An. albitarsis, An. braziliensis and An. nuneztovari (8.5%, 4.6%, 3% and 2.6%, respectively). An. albitarsis was the most frequently collected anopheline, presented the highest biting index and it was the second most frequently collected infected species infected with malaria parasites. An. albitarsis and An. darlingi respectively, are the primary vectors of malaria throughout Boa Vista.

The first epidemic tegumentary leishmaniasis´ outbreak in the province of Misiones was recorded in 1998, in the locality of Puerto Esperanza. Phlebotominae collected in the region, previously or simultaneously to the outbreak (September 1993-December 1998) showed that the species Lutzomyia intermedia s. l. was prevalent (94%, n 6,150) at all the sites sampled with miniature light trap (10) and Shannon trap (3). L. pessoai, L. whitmani, L. migonei, L. shannoni, L. fischeri, L. misionensis, Brumptomyia avellari and B. guimaraesi were also captured. Sand fly distribution in time and space suggests that in the province of Misiones (1) the species already present before 1990 could give rise to the epidemic by the density/dispersion fluctuation of their local populations; (2) the abundance of L. intermedia s. l. was associated with environments with ecotones of primary-secondary vegetation, close to water bodies and with moderate human disturbance; (3) this species showed, towards the end of 1997, peaks of exceptional abundance, subsequent to rainfall peaks in 1996. This increase in abundance of potential vector sand fly populations close to houses with colonizable surroundings could have generated the 1998 epidemic outbreak.

Eggs and nymphs of Triatoma jurbergi were described using optical microscopy and scanning electron microscopy. T. jurbergi is a wild species, found in State of Mato Grosso (15ºS and 300 m.a.s.l), Brazil. Eggs showed the operculum and surface with pentagonal and hexagonal cells, with small fractures and punctuations randomly distributed. Differences were found in the five nymphal stages of T. jurbergi, that allow their to be distinguished from the similar species T. guazu. The diagnostic characters most useful for differentiation were the general color of the insect, abdomen shape and its length.

Plasmodium falciparum sensitivity to chloroquine (CHL), amodiaquine (AMO) and sulfadoxine/pyrimethamine (SDX/PYR) was assessed in vivo and in vitro in a representative sample from the population of Zaragoza in El Bajo Cauca region (Antioquia-Colombia). There were 94 patients with P. falciparum evaluated. For the in vivo test the patients were followed by clinical examination and microscopy, during 7 days. The in vitro test was performed following the recommendations of the World Health Organization. The in vivo prevalence of resistance to CHL was 67%, to AMO 3% and to SDX/PYR 9%. The in vitro test showed sensitivity to all antimalarials evaluated. Concordance for CHL between the in vivo and in vitro tests was 33%. For AMO and SDX/PYR, the concordance was 100%. We conclude that a high percentage of patients are resistant to CHL (in vivo). A high rate of intestinal parasitism might explain in part, the differences observed between the in vivo and the in vitro results. Therefore, new policies and treatment regimens should be proposed for the treatment of the infection in the region. Nationwide studies assessing the degree of resistance are needed.

The ultrastructural morphology of the ventral region of the head (rostrum and buccula) and proesternum (stridulatory sulcus) of nymphs from the 1st to 5th instars of Triatoma guazu Lent & Wygodzinsky, 1979 and Triatoma jurbergi Carcavallo, Galvão & Lent, 1998 was described. Morphological differences between the two species and of the five nymphal stages development of each species were observed. These structures showed systematic differential characteristics of the studied species and may be used to increase their taxonomic range.

Mycobacterium kansasii is the most common cause of pulmonary nontuberculous mycobacteria infection and classical identification of this pathogen needs a time consuming phenotypic tests. Polymerase chain reaction-restriction fragment lenght polymorphism analysis (PRA) of the gene enconding for the 65kDa heat shock (hsp65) protein offers an easy, rapid, and inexpensive procedure to identify and subtype M. kansasii isolates. In the present study, we performed a retrospective analysis of patients who had mycobacteria identified on the basis of phenotypic tests by means of a review of database at Mycobacteria Laboratory of the Instituto Adolfo Lutz in the period 1995-1998. A total of 9381 clinical isolates were analyzed of which 7777 (82.9%) were identified as M. tuberculosis complex and 1604 (17.1%) as nontuberculous mycobacteria. Of the 296 M. kansasii isolates, 189 (63.8%) isolates obtained from 119 patients were viable and were analyzed by PRA-hsp65. Hundred eight two (98.9%) were classified as M. kansasii type I. Two isolates were classified as type II and III and five isolates were characterized as other Mycobacterium species. Clinical isolates of M. kansasii in the state of São Paulo was almost exclusively subtype I regardless of HIV status.

The prevalence of infection by hepatitis B (HBV) and C (HCV) viruses varies among geographical regions. In order to determine the prevalence of HBV and HCV infection in voluntary blood donors we evaluated the prevalence of HBsAg, anti-HBc, and anti-HCV markers of 128,497 blood donor samples collected from 1998 to 2005 in the state of Rio de Janeiro. These markers were analyzed by immunoenzymatic tests, as determined by the Ministry of Health. Data were obtained from the Sorology Laboratory of the Hemoterapy Service of the Instituto Nacional de Câncer, Rio de Janeiro. Overall prevalence estimates were: 0.27% for HBsAg, 3.68% for anti-HBc, and 0.90% for anti-HCV. There was a significant decrease in the overall prevalence of HBsAg (from 0.36 to 0.14%) and anti-HBc (from 6.12 to 2.05%) in the period encompassed between 1998-2005. Similarly, there was a decline in anti-HCV prevalence rates in Brazilian blood donors, from 1.04% in 1998 to 0.79% in 2004, with an increase of HCV prevalence to 1.09% in 2005. These prevalence estimates were higher than those found in other countries, indicating high rates of infection by HBV and HCV and a persistent risk of HBV and HCV transmission by transfusion.

Significant decrease in human immunodeficiency virus type 1 (HIV-1) vertical transmission has been observed worldwide in centers where interventions such as antiretroviral therapy (ART), elective cesarean section, and avoidance of breastfeeding have been implemented. This prospective cohort study aimed to assess the determinants of and the temporal trends in HIV-1 vertical transmission in the metropolitan area of Belo Horizonte, Brazil from January 1998 to December 2005. The rate of HIV-1 vertical transmission decreased from 20% in 1998 to 3% in 2005. This decline was associated with increased use of more complex ART regimens during pregnancy. Multivariate analysis restricted to clinical variables demonstrated that non ART, neonatal respiratory distress/sepsis and breastfeeding were independently associated with HIV-1 vertical transmission. When laboratory parameters were included in the model, high maternal viral load and non maternal ART were associated with HIV-1 vertical transmission. The results from this study confirm the impact of ART in the reduction of HIV-1 vertical transmission and indicate the need for improvement in the care and monitoring of mother and infant pairs affected by HIV-1.
