1. A estrutura externa e interna do esqueleto crânico de Triatoma infestans é descrita e microanatomia da cabeça é apresentada por meio de uma série de cortes transversais. Surgem algumas observações que estão em contradição aos dados da literatura mais nova (posição dos côndilos da cabeça posterior; pontos de origem de alguns músculos, tendão bem desenvolvido da bomba salivar, etc.). 2. Os órgãos da cabeça que servem diretamente ou indiretamente à alimentação são examinados detalhadamente: Tentório, faringe, aparelho salivar, partes bucais (mandíbulas, maxilas e lábio) e músculos sa cabeça. É explicado que a estrutura de algumas partes é mais fácil de ser compreendida quando ela é considerada como resultado de invaginações e evaginações. Dentro desta descrição não é referida a filogenia das partes do esqueleto. 3. O tentório sòmente serve à fixação das partes internas da cabeça mas sim também á condução dos estiletes. Trata-se de uma invaginação da parede crãnica anterior cujo lume pode ser comprovado. 4. Por meio dos resultados microanatômicos e das observações "in vivo" em larvas de bactrodes, é demonstrado o mecanismo do engulhamento. 5. O aparelho salivar é explicado como invaginação complicada do fundo da hipofaringe. 6. O pistilo da bomba salivar possui um tendão, no qual o retrator insere-se em forma de penas. A contra-inserção encontra-se exclusivamente na parte posterior do tentório. 7. A cúpula da bomba tem uma válvula de entrada e outra de saída. Em frente do desembocamento do canal condutor encontra-se uma segunda válvula que aparentemente impede um flutuamento da saliva já injetada na maxila para dentro do canal condutor. 8. A cúpula possui em frente e em baixo do canal produtor de saliva um aumento secundário que representa o começo da invaginação do aparelho salivar e deve ser considerado funcionalmente como reservatório equilibrador. 9. As mandíbulas têm para o protrator uma alavanca de articulação que reforça o efeito do músculo. A alavanca mostra-se como parte final posterior e engrossada do desdobramento do tentório que daqui em diante deve ser chamado "invaginação". A alavanca insere-se por intermédio de um arco de tonofibrilas indiretamente na parte final da mandíbula. Esta inserção é apresentada por uma reconstrução (fig. 35). O retrator da mandíbula insere-se como um tendão na parede do canal do tentório colada á mandíbula. O retrator tem a sua contra-inserção na parede superior e posterior da cabeça. 10. A inserção dos protratores das maxilas também é indireta (mas se malavanca de articulação, veja reconstrução em fig. 36), a dos retratores é direita. o contra-inserção dêstes últimos encontra-se na parede superior e posterior da cabeça. 11. As pontas das mandíbulas são especializadas numa forma que na picada fecham os dois canais (canal de alimentação e de saliva) abrindo-se sòmente no momento da sucção. (fig. 30). 12. Ao contrário aos dados de literatura, é observado que o aparelho salivar tem função própria e que a secreção é expelida através do canal salivar maxilar.

Foram descritas anatômica e histològicamente as glândulas labiais modificadas que são um par de complexos glandulares, cada um composto de três glândulas. Cada glândula tem uma parede formada por uma camada unicelular limitando uma cavidade relativamente grande, o reservatório. As duas primeiras glândulas (D 1 e D 2) encontram-se justapostas uma à outra. Entre elas é formado um hilo que tem um canal auxiliar que o liga à terceira glãndula ( D 3), situada mais para trás. Pelo hilo as três secreções das glândulas são automàticamente misturadas, sempre na mesma proporção, de modo que a composição da saliva só varia dentro de pequenos limites. Tôdas as glândulas são recobertas por uma musculatura e possuem válvulas que se abrem por meio dos músculos. O nervo (nervus glandulae salivaris, alias labialis) sai da parte posterior do gânglio subesofagiano (segmento labial do cérebro) encosta-se no canal salivar principal (entre hilo e bombasalivar) e emite ramos para as três glândulas. A parte ectodermal do aparelho salivar termina no centro das válulas das três glândulas. Os canais do hilo são revestidos por uma cutícula. As células de tôdas as três glândulas são binucleadas. Cada glândula apresenta um tipo de secreção característico. Animais famintos possuem quase exclusivamente glândulas no estado de regeneração, ou melhor em repouso. A expulsão da secreção das células para o interior da cavidade glandular realiza-se durante ou imediatamente depois da picada. Com isto, a quantidade de saliva gasta pela picada, vai ser substituída. Ficou provada uma correlação entre núcleo e protoplasma durante e, depois da secreção. Foi confirmada a opinião de HEIDENHAIN de que a formação das secreções representa uma função autônoma do protoplasma enquanto que a regeneração do mesmo realiza-se por intermédio de substâncias líquidas expulsas do núcleo para dentro do protoplasma no momento da saída das secreções. Saliva of blood-sucking arthropods contains substances that counteract the host's hemostatic and inflammatory reactions, allowing the arthropod to locate blood and keep it flowing during the blood meal. Parasites may manipulate this system in order to achieve increased transmission, both to vertebrate and to invertebrate hosts. Additionally, salivary pharmacological substances may locally immunosupress the delivery site, allowing initial colonization of the vertebrate host by the parasite.

Rats experimentally infected with Trypanosoma cruzi Y strain exhibited hypertrophy of the submandibular gland at 18 days after infection.SDS-PAGE of infected rats saliva revealed the presence of an additional band with an apparent molecular weight of about 13KDa. Electrophoresis of protein salivaand immunochemical analysis with antibody against rat cystatin S confirmed that the protein was identical to that induced by beta adrenergic stimulation.

Previous reports showed that Lutzomyia longipalpis saliva exacerbate Leishmania braziliensis infection in mice. The sand fly Lu. whitmani is one of the vectors of L. (Viannia) braziliensis (LVb), a causative agent of cutaneous leishmaniasis in the State of Ceará, Brazil. To determine whether saliva of Lu. whitmani could increase the infectivity of LVb in mice, we inoculated groups of BALB/c mice with LVb promastigotes in the presence or absence of the salivary glands lysate from Lu. whitmani. We found that coinjection with Lu. whitmani saliva increased size but not longevity of cutaneous LVb lesions in BALB/c mice, since the formed lesions gradually resolved. The mechanism(s) by which Lu. whitmani saliva might exacerbate LVb infection in BALB/c mice is speculated.

The first steps in leishmaniasis are critical in determining the evolution of the disease. Major advances have recently been done in understanding this crucial moment. Fundamental research in parasite-vector interaction, parasite biology, insect saliva, and vertebrate host response have shed new light and uncovered a most fascinating and complex moment in leishmaniasis. We review here some of these aspects and we try to connect them in a logical framework.

One of the main opportunistic fungal infections amongst immunocompromised individuals is oral candidosis, which has been found in up to 90% of human immunodeficiency virus (HIV)-infected patients. This study employed yeasts isolated from the saliva and oral cavities of 114 HIV-infected patients living in Campinas, São Paulo. Of the isolates, 57.8% were identified as Candida albicans and 42.1% as non-C. albicans. The latter isolates were subsequently identified as C. krusei (7.5%), C. lusitaniae (5.2%), C. tropicalis (4.6%), C. parapsilosis (4.6%), C. glabrata (2.8%), C. kefyr (1.7%), C. guilliermondii (1.7%), C. intermedia (1.1%), C. norvegensis (0.5%), and Rhodotorula rubra (1.7%). Susceptibility of the isolates to amphotericin B, fluconazole, miconazole, and itraconazole was also determined by a microdilution method adopted by the National Committee for Clinical Laboratory Standards. The isolates demonstrated various susceptibilities to the antifungal agents. In particular 29 C. albicans and 13 non-C. albicans isolates showed low susceptibility to FLCZ (> 64 µg/ml). This study revealed huge diversity of Candida species, in particular the increasing emergence of non-C. albicans associated with the oral flora of HIV-infected patients.

This study was aimed at investigating alternate methods for serodiagnosis of tuberculosis (TB), which are needed because bacteriologic diagnosis of childhood TB is difficult. A selection of 80 serum and saliva samples were tested from Warao indigenous children under 15 years of age; 34 high TB suspects (28 positive and 6 negative for the tuberculin skin test, TST) and 46 healthy contact children (32 positive and 14 negative for the TST). Several enzyme-linked immunosorbent assay (ELISA) serological tests were developed to test for Mycobacterium tuberculosis-specific antibodies, including serum IgA, IgG, IgE, and secretory IgA (sIgA) in saliva against 3 specific antigens (PPD, HSP60, 38 kDa). Of these, 2 antigens, PPD and 38 kDa, showed significantly higher reactivity. The sensitivity and specificity of these tests for diagnosis remained limited, between 26.5% and 38.2%, and 77.4% and 97%, respectively. Of all the samples studied and combinations realized between all isotypes and antigens combined with 3 isotypes (anti-PPD IgG, IgE, and anti-38kDa sIgA) managed to detect the largest number of patients, showing an improved sensitivity level of 64.7%, although specificity levels dropped to 81.8%. These results were compared with the Omega diagnostics commercial kit results. The commercial kits showed significantly lower reactivity (sensitivity of 20% and 13.33% to Myco G and Complex Plus, respectively) and a specificity of 100%. This study shows that in indigenous populations of Venezuela, where invasive procedures cannot be used to select samples but evaluation with a chest X-ray for radiological studies is available, the combination of 3 specific isotypes may be a useful tool to increase diagnostic accuracy with pulmonary TB in this population, when used together with clinical and epidemiological criteria.

An historical review is given of American visceral leishmaniasis (AVL), with particular reference to the eco-epidemiology of the disease in Brazil. Following the first records of AVL in this country, in 1934, the sandfly Lutzomyia longipalpis (Lutz and Neiva, 1912) was incriminated as the principal vector. It is now generally accepted, however, that there exist a number of cryptic species under the name of Lu. longipalpis s.l. and that variations in the quantity of the vasodilatory peptide maxadilan in the saliva of flies from different populations of Lu. longipalpis s.l., may account for the variable clinical manifestations of AVL seen in different geographic regions. Distribution of AVL has been shown to extend throughout most of South and Central America, with the domestic dog serving as the principal reservoir of infection for man. However, while one hypothesis suggests that the causative parasite is Leishmania infantum, imported from Europe with the Portuguese and Spanish colonists, the demonstration of a high rate of benign, inapparent infection in foxes in Amazonian Brazil raised an opposing suggestion that the parasite is indigenous to the Americas. Recent reports of similar infections in native marsupials, and possibly rodents, tend to support this view, particularly as Lu. longipalpis is primordially a silvatic sandfly. Although effective control measures in foci of the disease will diminish the number of canine and human infections, the presence of such an enzootic in a variety of native animals will render the total eradication of AVL unlikely.

In this report, we examine the adaptability of commercially available serological kits to detect antibodies markers for viral hepatitis in oral fluid samples. We also assessed the prevalence of hepatitis A, B, and C virus-specific antibodies, and related risk factors for these infectious diseases through sensitivity of the tests in saliva samples to evaluate if oral fluid can be an alternative tool to substitute serum in diagnosis of acute viral hepatitis and in epidemiological studies. One hundred and ten paired serum and saliva specimens from suspect patients of having acute hepatitis were collected to detect antibodies to hepatitis A (total and IgM), hepatitis B (anti-HBs, total anti-HBc and IgM anti-HBc), and hepatitis C (anti-HCV) using commercially available enzyme-linked immunossorbent assay (EIA). In relation to serum samples, oral fluid assay sensitivity and specificity were as follows: 87 and 100% for total anti-HAV, 79 and 100% for anti-HAV IgM, 6 and 95% for anti-HBs, 13 and 100% for total anti-HBc, 100 and 100% for anti-HBc IgM, and 75 and 100% for anti-HCV. The consistency observed between antibodies tests in saliva and expected risk factors for hepatitis A and C suggests that the saliva method could replace serum in epidemiological studies for hepatitis A and C.

Bihar, India has been in the grip of kala-azar for many years. Its rampant and severe spread has made life miserable in most parts of the state. Such conditions require a comprehensive understanding of this affliction. The numbers coming out of the districts prone to the disease in the north and south Ganges have provided us with several startling revelations, as there are striking uniformities on both sides, including similar vegetation, water storage facilities, house construction and little change in risk factors. The northern areas have been regularly sprayed with DDT since 1977, but eradication of the disease appears to be a distant dream. In 2007 alone, there were as many as 37,738 cases in that region. In contrast, the southern districts of Patna and Nalanda have never had the disease in its epidemic form and endemic disease has been present in only some pockets of the two districts. In those cases, two rounds of spraying with DDT had very positive results, with successful control and no new established foci. In addition, an eleven-year longitudinal study of the man hour density and house index for the vector Phlebotomus argentipes demonstrated that they were quite high in Patna and Nalanda and quite low in north Bihar. Given these facts, an attempt has been made to unravel the role of P. argentipes saliva (salivary gland) in the epidemiology of kala-azar. It was determined that patchy DDT spraying should be avoided for effective control of kala-azar.

Helicobacter pylori infection is associated with peptic ulcer and gastric carcinoma. The oral cavity may be a reservoir for H. pylori; however, the results of studies on this subject are controversial. We employed single-step and nested polymerase chain reactions (PCR) to detect the presence of the vacA, ureA and 16S rDNA genes of H. pylori in the stomach, saliva and dental plaque of 30 subjects. The results were confirmed by sequencing. Nested 16S rDNA and ureA amplification was achieved in 80% of gastric, 30% of saliva and 20% of dental plaque specimens. Sequencing of 10, seven and four 16S rDNA products from stomach, saliva and dental plaque, respectively, showed > 99% identity with H. pylori. Sequencing of the other four oral cavity PCR products showed similarity with Campylobacter and Wolinella species. Additionally, the vacA genotype identified in the samples of different sites was the same within a given subject.H. pylori may be found in the oral cavity of patients with gastric infection, thus it could be a source of transmission. However, results obtained with detection methods based only on PCR should be interpreted with caution because other microorganisms that are phylogenetically very close to H. pylori are also present in the mouth.

The objective of this study was to evaluate the feeding behavior of Triatoma vitticeps through the identification of its food sources and the characterization of the blood ingestion process. In addition, we aimed to verify if the saliva of this vector interferes with the perception of the host during the feedings by creating a nervous impulse. Here, we demonstrated that the T. vitticeps saliva reduces, gradually and irreversibly, the amplitude of the compound action potential of the nervous fibre, which helps decrease the perception of the insect by the host. The precipitin reaction demonstrated the feeding eclecticism of this vector, with the identification of eight food sources - most of them found simultaneously in the same insect. The analysis of the electrical signals produced by the cibarial pump during meals demonstrated that the best feeding performance of T. vitticeps nymphs that fed on pigeons is mainly due to the higher contraction frequency of the pump. The longer contact period with the host to obtain a complete meal compared with other triatominae species of the same instar could favor the occurrence of multiple blood sources in T. vitticeps under natural conditions, as it was evidenced by the precipitin test.

Transcripts similar to those that encode the nonstructural (NS) proteins NS3 and NS5 from flaviviruses were found in a salivary gland (SG) complementary DNA (cDNA) library from the cattle tick Rhipicephalus microplus.Tick extracts were cultured with cells to enable the isolation of viruses capable of replicating in cultured invertebrate and vertebrate cells. Deep sequencing of the viral RNA isolated from culture supernatants provided the complete coding sequences for the NS3 and NS5 proteins and their molecular characterisation confirmed similarity with the NS3 and NS5 sequences from other flaviviruses. Despite this similarity, phylogenetic analyses revealed that this potentially novel virus may be a highly divergent member of the genus Flavivirus. Interestingly, we detected the divergent NS3 and NS5 sequences in ticks collected from several dairy farms widely distributed throughout three regions of Brazil. This is the first report of flavivirus-like transcripts inR. microplus ticks. This novel virus is a potential arbovirus because it replicated in arthropod and mammalian cells; furthermore, it was detected in a cDNA library from tick SGs and therefore may be present in tick saliva. It is important to determine whether and by what means this potential virus is transmissible and to monitor the virus as a potential emerging tick-borne zoonotic pathogen.

RESUMO Objetivo Descrever a frequência de profissionais de enfermagem de unidades hospitalares com alteração no padrão diurno de secreção de cortisol. Método Foram incluídos 56 profissionais de enfermagem randomicamente selecionados, alocados nas unidades ambulatório, clínica médica, clínica cirúrgica, centro cirúrgico, pronto socorro infantil e adulto, unidade de terapia intensiva adulto e pediátrica de um hospital universitário. Para avaliação do padrão diurno de secreção de cortisol foram coletadas amostras de saliva em dois dias úteis consecutivos de trabalho. Resultados 42,5% dos profissionais de enfermagem apresentaram padrão atípico de secreção de cortisol. Além disso, quanto maior o tempo de trabalho na profissão, maior a concentração de cortisol (r=0,346; p=0,020). Conclusão Mais de um terço da amostra de profissionais de enfermagem apresentou padrões atípicos de secreção de cortisol, sugerindo que estes profissionais podem estar expostos a uma sobrecarga não apenas mental, mas biológica, estando expostos ao risco para o adoecimento por doenças relacionadas ao estresse.
