OBJECTIVE: To evaluate a comprehensive MRI protocol that investigates for cancer, vascular disease, and degenerative/inflammatory disease from the head to the pelvis in less than 40 minutes on a new generation 48-channel 3T system. MATERIALS AND METHODS: All MR studies were performed on a 48-channel 3T MR scanner. A 20-channel head/neck coil, two 18-channel body arrays, and a 32-channel spine array were employed. A total of 4 healthy individuals were studied. The designed protocol included a combination of single-shot T2-weighted sequences, T1-weighted 3D gradient-echo pre- and post-gadolinium. All images were retrospectively evaluated by two radiologists independently for overall image quality. RESULTS: The image quality for cancer was rated as excellent in the liver, pancreas, kidneys, lungs, pelvic organs, and brain, and rated as fair in the colon and breast. For vascular diseases ratings were excellent in the aorta, major branch vessel origins, inferior vena cava, portal and hepatic veins, rated as good in pulmonary arteries, and as poor in the coronary arteries. For degenerative/inflammatory diseases ratings were excellent in the brain, liver and pancreas. The inter-observer agreement was excellent. CONCLUSION: A comprehensive and time efficient screening for important categories of disease processes may be achieved with high quality imaging in a new generation 48-channel 3T system.

Cochlear implant is the method of choice in the treatment of deep sensorineural hypoacusis, particularly in patients where conventional amplification devices do not imply noticeable clinical improvement. Imaging findings are crucial in the indication or contraindication for such surgical procedure. In the assessment of the temporal bone, radiologists should be familiar with relative or absolute contraindication factors, as well as with factors that might significantly complicate the implantation. Some criteria such as cochlear nerve aplasia, labyrinthine and/or cochlear aplasia are still considered as absolute contraindications, in spite of studies bringing such criteria into question. Cochlear dysplasias constitute relative contraindications, among them labyrinthitis ossificans is highlighted. Other alterations may be mentioned as complicating agents in the temporal bone assessment, namely, hypoplasia of the mastoid process, aberrant facial nerve, otomastoiditis, otosclerosis, dehiscent jugular bulb, enlarged endolymphatic duct and sac. The experienced radiologist assumes an important role in the evaluation of this condition.

Inferior vena cava anomalies are rare, occurring in up to 8.7% of the population, as left renal vein anomalies are considered. The inferior vena cava develops from the sixth to the eighth gestational weeks, originating from three paired embryonic veins, namely the subcardinal, supracardinal and postcardinal veins. This complex ontogenesis of the inferior vena cava, with multiple anastomoses between the pairs of embryonic veins, leads to a number of anatomic variations in the venous return from the abdomen and lower limbs. Some of such variations have significant clinical and surgical implications related to other cardiovascular anomalies and in some cases associated with venous thrombosis of lower limbs, particularly in young adults. The authors reviewed images of ten patients with inferior vena cava anomalies, three of them with deep venous thrombosis. The authors highlight the major findings of inferior vena cava anomalies at multidetector computed tomography and magnetic resonance imaging, correlating them the embryonic development and demonstrating the main alternative pathways for venous drainage. The knowledge on the inferior vena cava anomalies is critical in the assessment of abdominal images to avoid misdiagnosis and to indicate the possibility of associated anomalies, besides clinical and surgical implications.

Complications from central venous catheterization include infectious conditions, pneumothorax, hemothorax and venous thrombosis. Pericardial effusion with cardiac tamponade hardly occurs, and in infants is generally caused by umbilical catheterization. The authors describe the case of cardiac tamponade occurred in an infant during chest computed tomography with contrast infusion through a central venous catheter inserted into the right internal jugular vein.

The present report describes the case of a child that after blunt abdominal trauma presented with portal thrombosis followed by progressive splenomegaly and jaundice. Ultrasonography and percutaneous cholangiography revealed biliary dilatation secondary to choledochal stenosis caused by dilated peribiliary veins, characterizing a case of portal biliopathy. The present case report is aimed at presenting an uncommon cause of this condition.

Traditional biomarker parameters and aromatic compounds were applied to characterize and classify ten Cuban asphaltites (asphaltene-rich petroleum occurring as seeps or filling veins, joints, cavities and fissures). Genetic molecular parameters were compared in order to establish oil-oil correlations between samples. Thermal evolution was investigated using saturated biomarker and aromatic maturity parameters. All samples seem to represent petroleum in the early catagenetic stage. Statistical procedures used as auxiliary techniques show that they represent oils of Family II (marine anoxic carbonate sourced oils), except for 2 samples interpreted as belonging to Family III oils (normal marine siliciclastic suboxic sourced oils).

The Mal de Río Cuarto disease is caused by Mal de Río Cuarto virus (MRCV) transmitted by Delphacodes kuscheli. Comparative studies were carried out on the cytopathological alterations produced by MRCV in corn (Zea mays), wheat (Triticum aestivum) and barley (Hordeum vulgare), as seen with a transmission electron microscope. Corn plants were infected with viruliferous D. kuscheli collected from the endemic disease area (i.e. Río Cuarto County, Córdoba, Argentina). For the viral transmission to small grain cereal plants, laboratory rared insects were used. In this case, the inoculum source was wheat and barley plants infected with MRCV isolate grown in a greenhouse. Leaf samples with conspicuous symptoms were collected: enations and size reduction in corn; crenatures, swelling veins and dark green color in small grain cereals. Viral infection was corroborated by DAS-ELISA. Viroplasms containing complete and incomplete virus particles and fibrillar material were found in the cytoplasm of infected cells in all species. Mature virions were between 60 and 70 nm diameter. In wheat and barley, viroplasms and dispersed particles were observed only in phloem, while in corn virions were also found in cells of the bundle sheath. Crystalline arrays of particles were detected in corn enation constitutive cells. Tubular inclusions were found only in wheat samples. The three species showed abnormalities in the chloroplasts of affected cells. The results showed that MRCV cytopathology has similarities with other viruses from the genus Fijivirus, family family Reoviridae, but slight differences depending upon the host plant.

During surveys undertaken from 1998 to 2003 in the major vegetable growing areas of the city of São Paulo green belt, lettuce (Lactuca sativa) and endive (Cichorium endivia) plants were observed, which showed chlorotic thickening of foliar veins, defective growth and, in some cases, failure to form complete heads. Biological and serological [DAS-Enzyme linked immunosorbent assay (Elisa)] tests together with electron microscope observations, revealed the presence of Lettuce big-vein virus and Mirafiori lettuce virus, in these plants both responsible for the lettuce big-vein syndrome.

Intestinal complications after laparoscopic cholecystectomy are rare and usually caused by direct injury sustained on trocar insertion. However, intestinal ischaemia has been reported as an unusual complication of the pneumoperitoneum. We describe a 55-years-old patient who underwent an uneventful laparoscopic cholecystectomy after an episode of acute cholecystitis. Initial recovery was complicated by development of increasing abdominal pain which led to open laparotomy on day 2. Gangrene of the distal ileum and right-sided colon was detected and small bowel resection with right colectomy and primary anastomosis was performed. Histological examination of the resected ileum showed features of venous hemorragic infarction and trombosis. In view of the proximity of the operation it is assumed that ileal ischaemia was precipitated by carbon dioxide pneumoperitoneum. Some studies have been demonstrated that, within 30 minutes of establishing a pneumoperitoneum at an intraabdominal pressure of 16 mmHg, cardiac output, blood flow in the superior mesenteric artery and portal vein decrease progressively. Carbon dioxide pneumoperitoneum may lead to mechanical compression of the splanchnic veins and mesenteric vasoconstriction as a result of carbon dioxide absortion. The distribution of the ischaemic segment of intestine is also unusual as the most precarious blood supply is traditionally at the splenic flexure of the colon. It has been suggested that intermittent decompression of the abdomen reduces the risk of mesenteric ischaemia during penumoperitoneum especially in patients with predisposing clinical features for arteriosclerosis intestinal. In present patient was observed intestinal venous infarction what remains unclear but we think the carbon dioxide pneumoperitoneum have been related to it.

Neste trabalho, são analisadas retrospectivamente as complicações locais e sistêmicas associadas ao implante e uso de catéteres venosos centrais (CVC) de longa permanência. Num período de oito anos, foram implantados 500 CVC para quimioterapia de doenças malignas ou para suporte em transplantes de medula óssea. Dois tipos de CVC foram usados: 322 CVC totalmente implantáveis (com reservatório subcutâneo) e 178 CVC semi-implantáveis (com segmento externo). Os implantes foram feitos por via percutânea ou por dissecção venosa cirúrgica. As veias de acesso foram: jugular interna, subclávia, cefálica, e safena magna. OS CVC foram usados de três dias a 75 meses (média de 4,8 meses). Foram analisadas as complicações que necessitaram de tratamento, prolongaram a estadia hospitalar ou levaram à retirada do catéter. Complicações menores foram excluídas deste estudo. Os tipos e os números de complicações observadas foram: Anestésicas: broncoespasmo grave (um); enfisema do pescoço por perfuração traqueal (um). Cirúrgicas: deiscência da incisão (duas); hematoma do pescoço (três); infecção aguda no local de implante (duas); lesão temporária do nervo vago (três}; linfocele (uma); fístula do ducto torácico (uma). Venosas: trombose aguda da jugular (três) e da veia subclávia (cinco); síndrome da cava superior (três). Do próprio catéter: bacteremia tardia (39); trombose do catéter (48); erosão da pele sobre o catéter/reservatório(três); torção do reservatório (duas); fratura do catéter (duas). No total, ocorreram 119 complicações, para uma taxa cumulativa de 23,8%. Nenhuma complicação foi fatal. O implante e uso dos CVC de longa permanência estão associados a complicações freqüentes, que podem ser graves. Mas os benefícios trazidos por estes CVC nos pacientes que necessitam de acesso venoso confiável por tempo prolongado são certamente muito maiores do que os riscos das complicações.

Revisamos nossa experiência com carcinoma epidermóide metastático (CEM) para o pescoço com tumor primário desconhecido com a intenção de evidenciar quando o tratamento radioterápico exclusivo ou o tratamento cirúrgico seguido de radioterapia teriam impacto positivo sobre a sobrevida. Este é um estudo retrospectivo de 54 pacientes com CEM tratados na Seção de Cirurgia de Cabeça e Pescoço do Hospital do Câncer/INCa entre 1986 e 1992. Quarenta e oito pacientes (89%) eram do sexo masculino, a idade média foi de 54 anos. Quarenta pacientes tinham metástase para linfonodos cervicais da cadeia jugular interna alta (nível 2). Utilizamos a classificação TNM da UICC de 1992 para estagiar os pacientes, onde oito pacientes foram classificados como N1, vinte como N2, 22 como N3, sendo que quatro pacientes permaneceram não classificados. Todos foram submetidos a endoscopia do trato aerodigestivo superior e raio X de tórax. Trinta e cinco pacientes foram submetidos a biópsia de aspiração com agulha fina. Trinta e oito pacientes tiveram tratamento com intenção curativa e 1.6 tiveram tratamento paliativo com radioterapia. Dos pacientes tratados com intenção curativa, dez foram submetidos a esvaziamento cervical e 28 tiveram tratamento exclusivo com radioterapia. Os 16 pacientes tratados com intenção paliativa foram excluídos dos cálculos de sobrevida e análise das recidivas. As recidivas cervicais foram analisadas usando o método do qui-quadrado, e as curvas de sobrevida foram comparadas usando-se o teste de Wilcoxon. A biópsia aspirativa com agulha fina alcançou o diagnóstico em 85% dos casos. Oito pacientes (15%) apresentaram metástase à distância. O tumor primário foi identificado subseqüentemente em 9% dos pacientes. Dezoito pacientes (64%) tratados com radioterapia exclusiva tiveram recidivas no pescoço, e três pacientes (33%) tratados com cirurgia + radioterapia tiveram recidivas no pescoço. (p=0,05) Os pacientes classificados como N2/N3 tratados com cirurgia + radioterapia tiveram melhores resultados do que os tratados com radioterapia exclusiva (respectivamente p=0,05 e p=0,09). Os pacientes Nl tiveram melhor sobrevida livre de doença do que os pacientes N2/N3 (respectivamente p=0,007 e p=0,OO7). A sobrevida livre de doença em cinco anos foi de 69% para os pacientes Nl, 11 % para os pacientes N2 e 15% para os pacientes N3. A sobrevida livre de doença para todos os estágios foi de 28%. A biópsia aspirativa com agulha fina é um bom meio para diagnóstico e deve ser usada rotineiramente.

Superior mesenteric vein invasion has historically been considered a contraindication for pancreatic cancer resection. Several studies have shown that in selected cases vascular resection can be performed safely. Many techniques have been used to reconstruct the venous flow. We describe one case of mesenteric superior vein resection and reconstrution of mesenteric-portal venous flow using the splenic vein during a pancreatoduodenectomy for pancreatic adenocarcinoma. The patient presented an extensive involvement of the superior mesenteric vein. A segmental resection with an end-to-end anastomosis of this vein and the splenic vein was accomplished after splenectomy. The patient had histologically confirmed negative margins. There was no hospital complications. These results show that the splenic vein can be an option for venous flow reconstruction when a segmental vascular resection is required during at pancreaticoduodenectomy.

BACKGROUND: Liver vascular isolation is essential for the treatment of the retrohepatic vena cava and hepatic veins. Triple vessel occlusion (TVO - occlusion of the portal triad, the inferior vena cava above the renal veins and within the pericardium) is the easiest isolation method for the surgeon. Unfortunately, this technique cannot be applied to hypovolemic and/or shock (cardiac arrest) patients as it compromises venous return. OBJECTIVES: Our objective is to demonstrate that in the above mentioned patients, establishing a previous hypervolemic state allows the safe use of TVO. METHODS: The method includes efficient injury tamponade with aggressive fluid replacement until normal blood volume is reached (resuscitation). Normal blood volume is recognized by a return of arterial blood pressure to normal levels, inferior vena cava filling and an increase in aortic wall tension. Following this procedure, hypervolemia is obtained by the rapid additional infusion of 1.500 to 2.000 ml of fluids. TVO in this situation does not alter the heart rhythm and maintains a clear operative field which is essential for hepatotomy, venorrhaphy and or venous ligation. RESULTS: Three patients were successfully operated.

An isolated hepatic caudate lobectomy was performed in a fifty-seven-year-old white woman presenting a colo-rectal metastasis to hepatic segment I. The resection was performed under total liver vascular exclusion due to the proximity to hepatic veins. The patient presented an uneventful hospitalization being discharged on day seven.

OBJETIVO: Demonstrar os acessos vasculares para hemodiálise mais utilizados em 23 unidades de hemodiálise, distribuídas em sete estados brasileiros. MÉTODO: Entre outubro de 1999 a agosto de 2000, foram avaliados 2559 pacientes em 23 unidades de hemodiálise distribuídas em 23 estados brasileiros onde foi observado: A - A freqüência da utilização do acesso vascular, se externo através de cateteres ou se interno através de fístula arteriovenosa (FAV). B - Os tipos de cateteres, se de curta permanência ou de longa permanência, assim como os locais anatômicos utilizados para sua inserção. C - Os tipos de FAV, se direta ou com interposição de algum tipo de prótese e os locais anatômicos onde foram construídas, se distais ou proximais. D - O custo financeiro com os acessos vasculares. RESULTADOS: Constatou-se que 93,4% dos pacientes tinham um acesso vascular através de FAV e 6,6% através de cateter. As FAV diretas distais foram as mais utilizadas em 74,8% dos pacientes; as FAV diretas proximais foram construídas em 21,7% das vezes; as FAV com politetrafluoretileno expandido (PTFE) 3,2% da totalidade; a veia safena foi utilizada em 0,1% e as FAV consideradas como outras em 0,2%. Os cateteres de longa permanência foram utilizados em 8,7% da totalidade dos cateteres e os de curta permanência em 91,3%. Como via de acesso a veia jugular foi utilizada em 42,4%, a veia subclávia em 42,4% e a veia femoral em 6,5%. O custo financeiro com acesso para hemodiálise foi de 1% do custo total das unidades. CONCLUSÕES: O acesso vascular no Brasil tem características próprias. O acesso mais utilizado é a FAV distal e o uso do PTFE é baixo.
