OBJETIVO: Desenvolver uma metodologia para correção da magnitude dos óbitos por câncer do colo do útero no Brasil. MÉTODOS: Os dados sobre os 9.607.177 óbitos foram obtidos do Sistema de Informação sobre Mortalidade, para o período de 1996 a 2005. Para a correção do sub-registro, foram utilizados os fatores de expansão gerados pelo Projeto Carga Global de Doença no Brasil - 1998. Para correção das categorias de diagnósticos desconhecidos, incompletos ou mal definidos de óbitos, foi aplicada redistribuição proporcional. Os dados ausentes de idade foram corrigidos por imputação. As correções foram aplicadas por Unidade Federativa e os resultados apresentados para o Brasil, região e áreas geográficas (capital, demais municípios das regiões metropolitanas e interior) por meio do percentual de variabilidade da magnitude das taxas, antes e após a correção dos óbitos. O comportamento das correções foi analisado por modelo de regressão linear multivariada com termos de interação entre região do País e área geográfica. RESULTADOS: As taxas corrigidas de mortalidade por câncer do colo do útero no Brasil mostraram um acréscimo de 103,4%, variando de 35% para as capitais da região Sul a 339% para o interior da região Nordeste. A redistribuição dos óbitos por câncer de útero sem especificação de localização anatômica promoveu os maiores acréscimos na magnitude das taxas. Os percentuais de correção, segundo ano de ocorrência do óbito, mostraram tendência estacionária no Brasil. CONCLUSÕES: Os resultados permitem concluir que a metodologia proposta foi adequada para corrigir a magnitude das taxas de mortalidade por câncer do colo do útero no País, mostrando que a mortalidade por esse câncer é ainda maior do que o observado nos informes oficiais.

OBJETIVO: Descrever características sociodemográficas e analisar a tendência temporal da mortalidade de motociclistas traumatizados em acidentes de transporte. MÉTODOS: Estudo de séries temporais com dados de 580 óbitos de motociclistas do Distrito Federal, de 1996 a 2007, obtidos do Sistema de Informações sobre Mortalidade. Foram calculadas as taxas de mortalidade específicas segundo idade e sexo, as taxas padronizadas (método direto) e a razão de óbitos por frota (motocicletas). A média móvel centralizada da taxa padronizada de mortalidade de homens foi calculada para o período de três anos e um modelo de regressão linear foi construído para estudar a evolução temporal da mortalidade. Para calcular o incremento anual da taxa de mortalidade padronizada utilizou-se o método joinpoint (ponto de inflexão). RESULTADOS: A maior parte dos motociclistas mortos era do sexo masculino, (94,3%), pardo (71,0%) e tinha entre 20 e 39 anos (73,8%). A taxa padronizada de mortalidade de motociclistas (homens) residentes foi de 1,9 para 7,2 óbitos/100 mil homens entre 1996 e 2007. Entre 1998 e 2007, a razão de óbitos por frota passou de 2,0 óbitos/10 mil motocicletas para 10,0 óbitos/10 mil motocicletas entre os homens. Estimou-se incremento anual de 0,48 óbito/100 mil homens (IC95% 0,31;0,65; p < 0,001). O incremento percentual anual da taxa padronizada de mortalidade para o sexo masculino foi de 36,2% no período 1998-2007 (IC 95% 21,2%;53,2%; p < 0,05). CONCLUSÕES: A taxa de mortalidade de motociclistas decorrente de acidentes de transporte aumentou expressivamente. Esse aumento é explicado apenas em parte pelo aumento da frota de motocicletas. Características individuais dos condutores, bem como as condições locais do tráfego, necessitam ser investigadas para o planejamento de políticas preventivas.

OBJETIVO Analisar a tendência da mortalidade por acidentes de motocicleta no Brasil. MÉTODOS Estudo descritivo de séries temporais sobre a taxa de mortalidade de acidentes de motocicleta no Brasil, segundo unidades federativas e faixas etárias entre 1996 e 2009. Os dados de óbitos foram obtidos no Sistema de Informação sobre Mortalidade do Ministério da Saúde e da população no Instituto Brasileiro de Geografia Estatística. Taxas de mortalidade padronizadas foram calculadas no período para o Brasil como um todo e Unidades Federativas. Variações anuais das taxas de mortalidade foram estimadas pelo método de Prais-Winsten de regressão linear. RESULTADOS A taxa de mortalidade por acidentes de motocicleta aumentou de 0,5 para 4,5/100.000 habitantes de 1996 a 2009 (aumento de 800% no período e 19% ao ano). Estados com maiores taxas em 2009 foram: Piauí, Tocantins, Sergipe e Mato Grosso. As maiores taxas de crescimento foram observadas nos Estados das regiões Norte, Nordeste e Centro-Oeste. CONCLUSÕES Houve grande aumento das taxas de mortalidade por acidente de motocicleta em todo o Brasil no período, principalmente nos Estados do Nordeste.

OBJECTIVE To analyze the spatial distribution of homicide mortality in the state of Bahia, Northeastern Brazil. METHODS Ecological study of the 15 to 39-year old male population in the state of Bahia in the period 1996-2010. Data from the Mortality Information System, relating to homicide (X85-Y09) and population estimates from the Brazilian Institute of Geography and Statistics were used. The existence of spatial correlation, the presence of clusters and critical areas of the event studied were analyzed using Moran’s I Global and Local indices. RESULTS A non-random spatial pattern was observed in the distribution of rates, as was the presence of three clusters, the first in the north health district, the second in the eastern region, and the third cluster included townships in the south and the far south of Bahia. CONCLUSIONS The homicide mortality in the three different critical areas requires further studies that consider the socioeconomic, cultural and environmental characteristics in order to guide specific preventive and interventionist practices.

OBJECTIVE To analyze temporal trends and distribution patterns of unsafe abortion in Brazil. METHODS Ecological study based on records of hospital admissions of women due to abortion in Brazil between 1996 and 2012, obtained from the Hospital Information System of the Ministry of Health. We estimated the number of unsafe abortions stratified by place of residence, using indirect estimate techniques. The following indicators were calculated: ratio of unsafe abortions/100 live births and rate of unsafe abortion/1,000 women of childbearing age. We analyzed temporal trends through polynomial regression and spatial distribution using municipalities as the unit of analysis. RESULTS In the study period, a total of 4,007,327 hospital admissions due to abortions were recorded in Brazil. We estimated a total of 16,905,911 unsafe abortions in the country, with an annual mean of 994,465 abortions (mean unsafe abortion rate: 17.0 abortions/1,000 women of childbearing age; ratio of unsafe abortions: 33.2/100 live births). Unsafe abortion presented a declining trend at national level (R2: 94.0%, p < 0.001), with unequal patterns between regions. There was a significant reduction of unsafe abortion in the Northeast (R2: 93.0%, p < 0.001), Southeast (R2: 92.0%, p < 0.001) and Central-West regions (R2: 64.0%, p < 0.001), whereas the North (R2: 39.0%, p = 0.030) presented an increase, and the South (R2: 22.0%, p = 0.340) remained stable. Spatial analysis identified the presence of clusters of municipalities with high values for unsafe abortion, located mainly in states of the North, Northeast and Southeast Regions. CONCLUSIONS Unsafe abortion remains a public health problem in Brazil, with marked regional differences, mainly concentrated in the socioeconomically disadvantaged regions of the country. Qualification of attention to women’s health, especially to reproductive aspects and attention to pre- and post-abortion processes, are necessary and urgent strategies to be implemented in the country.

ABSTRACT This study aimed to describe the digital disease detection and participatory surveillance in different countries. The systems or platforms consolidated in the scientific field were analyzed by describing the strategy, type of data source, main objectives, and manner of interaction with users. Eleven systems or platforms, developed from 1996 to 2016, were analyzed. There was a higher frequency of data mining on the web and active crowdsourcing as well as a trend in the use of mobile applications. It is important to provoke debate in the academia and health services for the evolution of methods and insights into participatory surveillance in the digital age.

Desde 1977, o Instituto Adolfo Lutz (IAL) vem promovendo a sorotipagem do S. pneumoniae ou pneumococo de infecções causadas por esta bacteria. As cepas isoladas têm sido encaminhadas ao WHO Pneumococcal Reference Center, Pensilvania, E.U.A.. De 1977 a 1988, 1.000 cepas de pneumococo isoladas de LCR foram sorotipadas, de acordo com a nomenclatura dinamarquesa, e 60 sorotipos foram identificados. A maior freqüência foi do sorotipo 1, secundado por 6B, 18C, 14, 5, 3, 6A, 23F, 19F e 38. Estes sorotipos distribuídos segundo faixas etárias demonstraram incidência variável, notando-se uma certa peculiaridade, ou seja, a predominância do sorotipo 6B na faixa de zero a menos de dois anos; do sorotipo 1 na faixa de 2 até 50 anos e do sorotipo 3 no grupo acima de 50 anos. Nos 12 anos considerados, 25 sorotipos apresentaram uma certa uniformidade na freqüência e o mesmo foi observado com relação às estações climáticas, apenas com um número maior de infecções meníngeas nos meses mais frios. Considerando a gravidade das infecções pneumocócicas notadamente as meningites, e a pouca informação relativa aos sorotipos pneumocócicos que ocorrem na região, julgamos importante essa informação relativa aos sorotipos, uma vez que tem sido usadas, com sucesso, vacinas polissacarídicas na prevenção dessas infecções.

Field studies in the western Amazon region (state of Acre, Brazil) indicate that the 4-aminoquinolines, as well as the combined regimen with sulfadoxine-pyrimethamine, can no longer be recomended for the treatment and prophylaxis of P. falciparum infections in this region. Quinine remains an effective drug when used correctly. However, compliance problems arise due to the often occurring side-effects during a ten day regimen. Prospects of overcoming these constraints by combining a short course of quinine with other drugs are limited, because of the lack of suitable partner compounds. For this reason quinine/clindamycin appears to be a more practical therapy of P. falciparum malaria. In vitro data from this study suggest that mefloquine is another effective alternative for the treatment of falciparum malaria in this Amazon region.

Neutralizing antibodies to EEE (6.7%), WEE (1.2%), ILH (26.6%), MAG (28.2%) and TCM (15.7%) viruses were found in sera of 432 equines of the Brazilian Pantanal, area where undiagnosed horse deaths are frequently observed. A 4-fold rise in CF titer to EEE virus was detected in acute and convalescent sera of an encephalitis horse sacrified in 1992. Antibodies to EEE, ILH, MAG and TCM viruses were detected in horses less than 2 years old indicating recent circulation of these viruses in the Pantanal. The evidence of recent equine encephalitis associated with rising CF titer to EEE warrants a more intensive study with attempts to isolate virus from horses with clinical manifestations of encephalitis.

We have detected antibodies, in the sera of Chagas disease, Kala-azar and Mucocutaneous leishmaniasis patients, that bind multiple antigens shared between the three causative agents. The Chagas disease sera showed 98 to 100% positive results by ELISA when the Leishmania braziliensis and Leishmania chagasi antigens were used, respectively. The Kala-azar sera showed 100% positive results with Trypanosoma cruzi or L. braziliensis antigens by immunofluorescence assays. The antibodies in the sera of Mucocutaneous leishmaniasis patients showed 100% positive results by ELISA assays with T. cruzi or L. chagasi antigens. Furthermore, the direct agglutination of L. chagasi promastigotes showed that 95% of Kala-azar and 35% of Mucocutaneous leishmaniasis sera agglutinated the parasite in dilutions above 1:512. In contrast, 15% of Chagas sera agglutinated the parasite in dilutions 1:16 and below. Western blot analysis showed that the Chagas sera that formed at least 24 bands with the T. cruzi also formed 13 bands with the L. chagasi and 17 bands with the L. braziliensis. The Kala-azar sera that recognized at least 29 bands with the homologous antigen also formed 14 bands with the T. cruzi and 10 bands with the L. braziliensis antigens. Finally, the Mucocutaneous leishmaniasis sera that formed at least 17 bands with the homologous antigen also formed 10 bands with the T. cruzi and four bands with the L. chagasi antigens. These results indicate the presence of common antigenic determinants in several protozoal proteins and, therefore, explain the serologic cross-reactions reported here.

Hepatitis G virus/ GB virus C is a novel flavivirus recently detected in hepatitis non A-E cases. In this study, the presence of this virus in chronic non-B, non-C hepatitis patients was evaluated using GBV-C specific PCR and this virus was detected in one out of thirteen patients. This patient has presented a severe liver failure, has lived for a long time in the Western Amazon basin and no other cause for this clinical picture was reported. The impact of the discovery of this new agent is still under evaluation throughout the world. The study of the prevalence of this virus among chronic hepatitis patients and healthy individuals (as blood donors) will furnish subside to evaluate its real pathogenicity.

Early in 1995 the first case of Hantavirus Pulmonary Syndrome was serologically confirmed in El Bolsón (Province of Río Negro, Argentina), corresponding to the third outbreak reported in Argentina. A total of 26 cases of HPS related to the Andean region of Rio Negro Province, were reported from 1993 to 1996, 17 in El Bolsón, 4 in San Carlos de Bariloche, and 5 in Buenos Aires. The incidence rate was 5.03 x 100000 with a mortality rate of 51.85 x 100. The occurrence of cases was mainly seasonal, with a significantly greater number in the spring, and the persons affected mainly lived in urban or periurban areas. In four cases, the affected individuals were members of a couple, spouses or live-in contacts. Seven cases were Health workers (physicians, nurses or administrative staff). Twelve cases were related among them, due to an outbreak of 80 days. Two of them did not visit the Andean region. A total of 139 rodents were captured and seven of them, Olygoryzomys longicaudatus, were found to be serologically positive. The possibility of infection by contact with rodents or fecal matter is being analyzed and also hypothesis related with interhuman transmission

Visceral larva migrans (VLM) is a clinical syndrome caused by infection of man by Toxocara spp, the common roundworm of dogs and cats. Tissue migration of larval stages causes illness specially in children. Because larvae are difficult to detect in tissues, diagnosis is mostly based on serology. After the introduction of the enzyme-linked immunosorbent assay (ELISA) using the larval excretory-secretory antigen of T. canis (TES), the diagnosis specificity was greatly improved although cross-reactivity with other helminths are still being reported. In Brazil, diagnosis is routinely made after absorption of serum samples with Ascaris suum antigens, a nematode antigenicaly related with Ascaris lumbricoides which is a common intestinal nematode of children. In order to identify T. canis antigens that cross react to A. suum antigens we analyzed TES antigen by SDS-PAGE and Western blotting techniques. When we used serum samples from patients suspected of VLM and positive result by ELISA as well as a reference serum sample numerous bands were seen (molecular weight of 210-200 kDa, 116-97 kDa, 55-50 kDa and 35-29 kDa). Among these there is at least one band with molecular weight around 55-66 kDa that seem to be responsible for the cross-reactivity between T. canis e A. suum once it disappears when previous absorption of serum samples with A. suum antigens is performed

Twenty-seven mycologically proven cases of paracoccidioidomycosis (PCM) were treated with itraconazole (100-200 mg/day in month 1 and 100 mg/day until month 6-8) and evaluated clinically and serologically, up to 3.5 years post-therapy, using Dot-blot and ELISA for measuring the titers of IgG, IgA and IgM anti-P. brasiliensis antibodies and Western-blot for determining IgG, IgA and IgM antibodies against the antigen components of the fungus. Before treatment, 81.5% (Dot-blot) and 84% (ELISA) of the patients presented elevated IgG anti-P. brasiliensis antibody titers which dropped slightly with treatment. On the other hand, the percentages of pre-treatment high-titered sera for IgA and IgM anti-P.brasiliensis were lower (5l.9% and 5l.8%: Dot-blot; 16.5 and 36%: ELISA, respectively) but the titers tended to become negative more frequently with treatment. Prior to treatment, the percentages of positivity for IgG, IgA and IgM anti-P.brasiliensis antibodies in Western-blot were 96%, 20.8% and 41.6%, respectively. Antigens with molecular weights varying from 16-78 kDa, from 21-76 kDa and from 27-78 kDa were reactive for IgG, IgA and IgM antibodies, respectively. The most frequently reactive antigenic components had molecular weights of 27, 33 and 43 kDa for IgG, and 70 for IgA and IgM antibodies. During the period of study, the patients responded well to treatment. The present data confirm the diversity and complexity of the humoral response in PCM, and the importance of utilizing different serological tests to detect IgG, IgA and IgM anti-P. brasiliensis antibodies
