We report the case of a 35-year-old homeless alcoholic and illicit drug user, with AIDS, who was admitted to the emergency unit complaining of asthenia and a weight loss of 30kg over the preceding three months. Clinical and laboratory data confirmed a diagnosis of marasmus, bacterial pneumonia, chorioretinitis caused by Toxoplasma gondii and oral Candida infection. The patient also presented loss of tongue papillae, gingival hypertrophy, perifollicular hyperkeratosis and hemorrhage, coiled, corkscrew-like hair, anemia, hypoalbuminemia, increased C-reactive protein levels and low serum vitamin C levels. The patient developed severe gastric hemorrhage, with hemodynamic instability and terminal disseminated intravascular coagulopathy.

INTRODUCTION: Despite all efforts to restrict its transmission, Chagas' disease remains a severe public health problem in Latin America, affecting 8-12 million individuals. Chronic Chagas' heart disease, the chief factor in the high mortality rate associated with the illness, affects more than half a million Brazilians. Its evolution may result in severe heart failure associated with loss of functional capacity and quality of life, with important social and medical/labor consequences. Many studies have shown the beneficial effect of regular exercise on cardiac patients, but few of them have focused on chronic Chagas' heart disease. METHODS: This study evaluated the effects of an exercise program on the functional capacity of patients with chronic Chagas' disease who were treated in outpatient clinics at the Evandro Chagas Institute of Clinical Research and the National Institute of Cardiology, Rio de Janeiro, Brazil. The exercises were performed 3 times a week for 1 h (30 min of aerobic activity and 30 min of resistance exercises and extension) over 6 months in 2010. Functional capacity was evaluated by comparing the direct measurement of the O2 uptake volume (VO2) obtained by a cardiopulmonary exercise test before and after the program (p < 0.05). RESULTS: Eighteen patients (13 females) were followed, with minimum and maximum ages of 30 and 72 years, respectively. We observed an average increase of VO2peak > 10% (p = 0.01949). CONCLUSIONS: The results suggest a statistically significant improvement in functional capacity with regular exercise of the right intensity.

Introduction Our objective was to evaluate the influence of rainfall regime on the population dynamics of Biomphalaria in a potential urban focus of schistosomiasis in Aracaju, Brazil, during 2009-2010. Methods Snails were collected monthly and were counted, measured and identified; the level of infection and fecal contamination at the sampling sites was determined; rainfall data were obtained. Results High levels of fecal contamination were observed, and the abundance of Biomphalaria glabrata increased during the rainy and post-rainy seasons. The snails' size was variable, and infected snails were identified independently of rainfall. Conclusions These results provide evidence of anthropogenic and climate interference in an urban focus of schistosomiasis in the Aracaju metropolitan area.

Introduction Leprosy is a chronic infectious disease that is caused by Mycobacterium leprae. The objective of this study was to evaluate the risk factors that are associated with neural alterations and physical disabilities in leprosy patients at the time of diagnosis. Methods A prospective cross-sectional study was conducted on 155 leprosy patients who participated in a program that aimed to eliminate leprosy from São Luis, State of Maranhão. Results Patients who were 31-45 years of age, were older than 60 years of age or had a partner were more likely to have a disability. Patients with partners were 1.14 times more likely (p = 0.025) to have disabilities of the hands. The frequency of disabilities in the feet among the patients with different clinical forms of leprosy was statistically significant. Conclusions The identification of risk factors that are associated with neural alterations and physical disabilities in leprosy patients is important for diagnosing the disease because this approach enables physicians to plan and prioritize actions for the treatment and monitoring of patients.

Aspergillosis is an infection caused by saprophytic fungi of the genus Aspergillus, which typically occurs in immunosuppressed individuals, but has also been reported in immunocompetent patients. The main routes of entry are the respiratory tract, skin, cornea, and ear, and the infection may be localized or disseminated by contiguity or vascular invasion. We report a severe case of rhinosinusitis with cutaneous involvement, caused by invasive aspergillosis, in an immunocompetent user of inhaled cocaine. Invasive aspergillosis related to cocaine abuse has not yet been reported in the literature. After itraconazole treatment and surgical debridement, complete clinical remission was achieved. Nasal reconstruction with a skin graft over a silicone prosthesis resulted in a satisfactory esthetic outcome.

Chagas disease (CD) is a parasitic infection that originated in the Americas and is caused by Trypanosoma cruzi. In the last few years, the disease has spread to countries in North America, Asia and Europe due to the migration of Latin Americans. In the Brazilian Amazon, CD has an endemic transmission, especially in the Rio Negro region, where an occupational hazard was described for piaçaveiros (piassaba gatherers). In the State of Amazonas, the first chagasic infection was reported in 1977, and the first acute CD case was recorded in 1980. After initiatives to integrate acute CD diagnostics with the malaria laboratories network, reports of acute CD cases have increased. Most of these cases are associated with oral transmission by the consumption of contaminated food. Chronic cases have also been diagnosed, mostly in the indeterminate form. These cases were detected by serological surveys in cardiologic outpatient clinics and during blood donor screening. Considering that the control mechanisms adopted in Brazil's classic transmission areas are not fully applicable in the Amazon, it is important to understand the disease behavior in this region, both in the acute and chronic cases. Therefore, the pursuit of control measures for the Amazon region should be a priority given that CD represents a challenge to preserving the way of life of the Amazon's inhabitants.

Abstract: INTRODUCTION: Hantavirus diseases are emerging human diseases caused by Hantavirus spp. of the Bunnyaviridae family. Hantavirus pulmonary syndrome (HPS) has been detected in the Federal District (DF) of Brazil since 2004. Among the 27 Brazilian Federal Units, DF has the highest fatality rate. More than 10 years have already passed since then, with confirmation of cases caused by the Araraquara and Paranoa species. The reservoir is Necromys lasiurus. METHODS: Local surveillance data of the confirmed cases were analyzed, including age, sex, month and year of occurrence, clinical symptoms, syndromes and outcomes, and probable transmission place (PTP). The cases were mainly confirmed by IgM detection with a capture enzyme immunoassay. The cases were classified as autochthonous if PTPs were in the DF area. RESULTS: From 2004 to 2013, in the DF, 126 cases of hantavirus were confirmed, and the cumulative incidence was 5.0 per 100,000 inhabitants. The occurrence of cases was predominantly from April to August. At least 75% of the cases were autochthonous. Acute respiratory failure was reported in 47.5% of cases, and the fatality rate was 40%. CONCLUSIONS: In the DF, the cumulative incidence of HPS was one of the highest worldwide. A seasonal pattern of hantavirus disease in the dry season is clear. There was a high frequency of severe clinical signals and symptoms as well as a high fatality rate. For the near future, visitors and inhabitants of DF rural areas, particularly male adults, should receive continuous education about hantavirus transmission and prevention.

Este trabalho discute os efeitos das mudanças do uso do solo na biogequímica dos rios da bacia de drenagem do rio Ji-Paraná (Rondônia). Nesta região, a distribuição espacial do desmatamento e das propriedades do solo resultam em sinais diferentes, possibilitando a divisão dos sistemas fluviais em três grupos: rios com águas pobres em íons e baixo impacto; rios com conteúdo iônico intermediário e impacto médio e rios com elevados conteúdo iônico e impacto antropogênico. As características biogeoquímicas dos rios têm relação significativa com a área de pasto, melhor parâmetro para prever a condutividade elétrica (r² = 0,87) e as concentrações de sódio (r² = 0,75), cloreto (r² = 0,69), potássio (r² = 0,63), fosfato (r² = 0.78), nitrogênio inorgânico (r² = 0.52), carbono inorgânico (r² = 0.81) e carbono orgânico (rain ² = 0.51) dissolvidos. Cálcio e magnésio tiveram sua variância explicada pelas características do solo e pastagem. Nossos resultados indicam que as mudanças observadas na micro-escala constituem "sinais biogeoquímicos" gerados pelo processamento do material nas margens dos rios. A medida em que os rios evoluem para ordens superiores, os sinais persistentes nos canais fluviais estão mais associdados às características da bacia de drenagem (solos e uso da terra). Apesar dos efeitos das mudanças observadas no uso do solo não serem ainda detectáveis na macro-escala (bacia amazônica), a disrupção da estrutura e funcionamento dos ecossistemas é detectável nas micro e meso escalas, com alterações significativas na ciclagem de nutrientes nos ecossistemas fluviais.

O efeito da intensidade do primeiro desbaste seletivo, após um ano, foi avaliado em dois povoamentos de teca sob espaçamento 3,0 x 3,0 m, e em duas diferentes idades. No povoamento com quatro anos de idade, o desbaste foi de 20%, 30% e 40%, em número de indivíduos, e no de cinco anos, de 30%, 40% e 50%, e as respectivas testemunhas (0%). Cada tratamento foi aplicado ao acaso, com oito repetições, contendo cada parcela 35 árvores, além da bordadura. Um ano após o desbaste aplicado no povoamento com quatro anos, as variáveis DAP, altura total (H) e as médias por árvore de área transversal (der=0 src="/img/revistas/aa/v38n2/a05G.gif" align=absmiddle>) e de volume (der=0 src="/img/revistas/aa/v38n2/a05V.gif" align=absmiddle>) não apresentaram diferenças significativas entre os tratamentos; o incremento corrente anual (ICA V) não foi afetado pelas intensidades. No povoamento desbastado aos cinco anos, o ICA V e H foram semelhantes em todos tratamentos. As intensidade de desbaste testadas não afetaram o crescimento em DAP, der=0 src="/img/revistas/aa/v38n2/a05G.gif" align=absmiddle>e der=0 src="/img/revistas/aa/v38n2/a05V.gif" align=absmiddle>, mas apresentaram diferenças significativas em relação à testemunha. O primeiro desbaste seletivo pode ser aplicado aos cinco anos de idade, em povoamentos de Tectona grandis com condições semelhantes, com qualquer das intensidades testadas.

Females of simuliid black flies are haematophagous insects and vectors of several pathogenic agents of human diseases such as the filarial worms Mansonella ozzardi and Onchocerca volvulus. The genus Cerqueirellum is one of the most important groups of vectors of mansonellosis and onchocerciasis diseases in South America, and the genera Coscaroniellum and Shelleyellum are phylogenetically close to Cerqueirellum. There is not yet an agreement among authors about the generic classification of the species which compose these three genera, being all lumped by some taxonomists within Psaroniocompsa. A cladistic analysis of all species of Coscaroniellum, Cerqueirellum, and Shelleyellum, based on 41 morphological characters were done. Species closely related to Cerqueirellum were included in the analysis. The genera Cerqueirellum, Coscaroniellum and Shelleyellum were demonstrated as consistent basal entities and well-defined monophyletic clades.

A new species of the genus Cerqueirellum Py-Daniel, 1983 (Diptera: Simuliidae) is described. The adults are similar to the species C. oyapockense (Floch & Abonnenc, 1946) and C. roraimense (Nunes de Mello, 1974), of which the females are similar, and the males present discrete differences. The main differences of this new species to others of the genus Cerqueirellum are the integument of the larva recovered from stout spines and long cephalic trichomes in the pupa. Some females were infected with Mansonella ozzardii (Manson, 1897) (Nematoda, Onchocercidae) and probably transmit mansonelliasis in the Ituxi river, state of Amazonas, Brazil.

O objetivo deste estudo foi avaliar o desempenho do feijoeiro-comum (Phaseolus vulgaris L) em função da inoculação de sementes com Rhizobium tropici e das adubações nitrogenada e molibdica. Utilizou-se o delineamento em blocos casualizados, com quatro repetições, em arranjo fatorial 2x2x2, constituindo-se das combinações de ausência e presença de inoculação de sementes, da adubação molíbdica e da adubação nitrogenada. A inoculação das sementes foi imediatamente antes da semeadura com inoculante turfoso composto da estirpe CIAT 899 (SEMIA 4077); a adubação nitrogenada foi na semeadura (10 kg.ha-1) e em cobertura (50 kg.ha-1), quando as plantas apresentaram a terceira folha trifoliolada expandida e o molibdênio (Mo) foi aplicado em pulverização foliar na dose de 60 g.ha-1. Não foram observados efeitos dos tratamentos para a massa seca de raízes e o número de vagens por planta. A adubação nitrogenada reduziu a nodulação nos feijoeiros. Todavia, com a adubação nitrogenada foi verificado incremento na altura e na massa seca da parte aérea dos feijoeiros. Grãos de feijão mais pesados foram observados em feijoeiros adubados com N na ausência de Mo. As interações entre adubação nitrogenada e molibdica com inoculação de sementes afetaram, também, o teor de N foliar e o número de grãos por vagem A inoculação de sementes proporcionou nos feijoeiros rendimentos de grãos semelhantes aos fertilizados com N.

Lesmas do gênero Omalonyx d'Orbigny, 1837 são hermafroditas, herbívoras, de distribuição neotropical e vivem em plantas aquáticas, nas demais vegetações adjacentes e em solo úmido próximo a ambientes de água doce. No presente trabalho reporta-se a ocorrência atípica de O. pattersonae Tillier, 1981 e de Omalonyx sp. em área de terra firme, distante de ambiente aquático. Estas espécies aqui reportadas são simpátricas e devido à alta densidade populacional e prejuízos causados às folhas do capim-elefante Pennisetum purpureum Schumach, são caracterizadas como pragas agrícolas. No período da noite as lesmas se alimentavam das folhas do capim elefante e durante o dia permaneciam escondidas na base do caule, próximo a superfície úmida do solo. A aplicação de cal hidratada sobre agregados de indivíduos de Omalonyx spp foi um método efetivo para o controle das populações. As alterações ambientais dos ecossistemas amazônicos para uso agrícola e/ou urbanização tem promovido o aumento populacional de espécies que se adaptam a novos habitats e geralmente se tornam pragas de difícil controle.
