O objetivo deste trabalho foi estudar mudanças na prevalência de lesões intra-epiteliais escamosas cervicais entre mulheres HIV-1 positivas após a introdução da terapia antiretroviral altamente eficaz e avaliar modificações na prevalência de fatores de risco para lesões intra-epiteliais escamosas cervicais. Foram estudadas 50 pacientes em 1995-1999 e 120 pacientes em 2006-2007. Coletaram-se dados demográficos, comportamentais, laboratoriais. Calculou-se a prevalência de lesões intra-epiteliais escamosas cervicais entre os dois períodos, assim como as prevalências dos outros fatores de risco. No primeiro período, encontrou-se uma prevalência de lesões intra-epiteliais escamosas cervicais de 66% e no segundo de 43% (p=0,007). A média do CD4 em 1995-1999 foi de 275,71 (DP 283,23); a média do CD4 em 2006-2007 foi de 463,32 (DP 231.90), (p=0,001). Houve mudanças significativas nos fatores idade, cor, estado conjugal e fumo entre os dois períodos. A diminuição da prevalência de lesões intra-epiteliais escamosas cervicais pode estar relacionada ao uso da estratégia de terapia antiretroviral altamente eficaz assim como à mudança de fatores de risco para lesões intra-epiteliais escamosas cervicais ao longo do tempo.

O objetivo deste estudo foi descrever a prevalência de sífilis e HIV em parturientes atendidas nas maternidades públicas, Vitória, ES. No período de janeiro a maio 2007, elas responderam a entrevista contendo dados sócio-demográficos, comportamentais e clínicos e realizaram teste rápido, ELISA e imunofluorescência indireta para HIV; e teste rápido, VDRL e MHA-TP para sífilis. Um total de 1.380 parturientes foi incluído. A média de idade foi 24,2 (DP 6,1) anos e escolaridade 8,5 (DP 2,6) anos. A prevalência de HIV foi 0,6% (IC95% 0,2%-1,1%) e sífilis 0,4% (IC95% 0,2%-0,9%). O teste rápido para HIV foi concordante com o ELISA e a imunofluorescência indireta em todos os casos. O teste rápido para sífilis foi positivo em seis parturientes, sendo que dois resultados não foram confirmados pelo VDRL e MHA-TP. Entre 71 (5,1%) parturientes que não realizaram pré-natal, o teste rápido para sífilis foi positivo em uma e o HIV em duas delas. Os resultados indicam a importância do teste rápido para o diagnóstico de sífilis e HIV, pois há parturientes que não realizam pré-natal ou que não tem acesso ao resultado ou ao tratamento durante o pré-natal.

INTRODUCTION: This work aimed to study the community structure of sandflies, with regard to the richness, constancy, abundance, and monthly frequency of the species with a focus on the transmission of leishmaniasis. METHODS: The study was conducted in the rural villages of Bom Jardim and Santa Maria, situated on the edge of a tropical rain forest in the municipality of São Jose de Ribamar, Maranhão, Brazil. The phlebotomines were captured in the intradomiciles and peridomiciles of each village, with Centers for Disease Control (CDC) light traps set in 10 homes in each village, for 1 year, once a month, from 18h to 6h. RESULTS: We collected 1,378 individuals of 16 sandfly species. The capture success rate was higher in Bom Jardim (0.61 specimens/hour/trap) than that of Santa Maria (0.35/specimens/hour/trap). The sandflies were more abundant in the peridomiciles (86.1%) and in the rainy season (77%). Five species were considered constants (occurring in more than 50% of samples), 5 accessory (25%-50%), and 6 accidental (<25%). The most abundant species were Lutzomyia longipalpis (59.7%) and L whitmani (28%). The permutation analysis showed differences between the species composition of the villages and no separation between the intradomicile and peridomicile of each village. The species that most contributed to the dissimilarity between the light traps of the 2 villages were L. longipalpis, L. whitmani, and L. evandroi, contributing to 80.8% of the variation among groups. CONCLUSIONS: The high level of richness and abundance of species and the presence of competent vectors throughout the year and around houses justify the occurrence of leishmaniasis cases reported in the area.

Introduction American cutaneous leishmaniasis (ACL) has been reported in every municipality of the State of Mato Grosso, Brazil, but the transmission epidemiology remains poorly understood. Our study was developed in a rural area of the Nova Mutum municipality where four autochthonous cases of ACL were reported in 2009. Our aims were to describe the local phlebotomine sandfly fauna and to investigate the infection rates and infecting Leishmania species in the captured sandflies. Methods Entomological captures were performed bimonthly at 10 fixed sites close to the edge of a forested area between June 2011 and April 2012. Results A total of 3,743 phlebotomine sandflies belonging to 31 distinct species were captured. Approximately 75% of the specimens were females. The most abundant species (45.4%) was Lutzomyia antunesi, which was consistently captured at every site. Species that are epidemiologically important for ACL, such as L. flaviscutellata, L. whitmani and L. umbratilis, were also captured. L. antunesi and L. ubiquitalis were naturally infected by Leishmania braziliensis or Le. guyanensis, with minimum infection rates of 0.88% and 6.67%, respectively. Surprisingly, L. antunesi was infected by Le. infantum (synonym chagasi). Conclusions The natural infection of L. antunesi and L. ubiquitalis by Leishmania sp. suggests that these species might play a role in the zoonotic cycle of ACL in Nova Mutum. The presence of Le. infantum in L. antunesi suggests that there may be a risk of an outbreak of visceral leishmaniasis (VL) in Nova Mutum.

Exacerbation of the immune response against Mycobacterium leprae can lead to neuritis, which is commonly treated via immunosuppression with corticosteroids. Early neurolysis may be performed concurrently, especially in young patients with a risk of functional sequelae. We report the case of a young patient experienced intense pain in the left elbow one year after the treatment of tuberculoid-tuberculoid leprosy. The pain was associated with paresthesias in the ulnar edge and left ulnar claw. After evaluation, the diagnosis was changed to borderline tuberculoid leprosy accompanied with neuritis of the left ulnar nerve. Early neurolysis resulted in rapid reduction of the pain and recovery of motor function.

Left ventricular hypertrophy following volume overload is regarded as an example of cardiac remodeling without increased fibrosis accumulation. However, infarction is associated with increased fibrosis within the noninfarcted, hypertrophied myocardium, particularly in the subendocardial regions. It is conceivable to suppose that, as also occurs postinfarction, low coronary driving pressure may also interfere with accumulation of myocardial fibrosis following aortocaval fistula. PURPOSE: To investigate the role of acute hemodynamic changes in subsequent deposition of cardiac fibrosis in response to aortocaval fistula. METHOD: Aortocaval fistula were created in 4 groups of Wistar rats that were followed over 4 and 8 weeks: aortocaval fistula 4 and aortocaval fistula 8 (10 rats each) and their respective controls (sham-operated controls - Sh), Sh4 and Sh8 (8 rats each). Hemodynamic measurements were performed 1 week after surgery. Hypertrophy and fibrosis were quantified by myocyte diameter and collagen volume fraction at the end of follow up. RESULT: Compared with Sh4 and Sh8, pulse pressure, left ventricular end-diastolic pressure, and +dP/dt were higher in aortocaval fistula 4 and aortocaval fistula 8, but -dP/dt was similar. Coronary driving pressure (mm Hg), used as an estimate of perfusion pressure, was lower in aortocaval fistula 8 (52.6 ± 4.1) than in Sh8 (100.8 ± 1.3), but comparable between aortocaval fistula 4 (50.0 ± 8.9) and Sh4 (84.8 ± 2.3). Myocyte diameter was greater in aortocaval fistula 8, whereas interstitial and subendocardial fibrosis were greater in aortocaval fistula 4 and aortocaval fistula 8. Coronary driving pressure correlated inversely and independently with subendocardial fibrosis (r² = .86, P <.001), whereas left ventricular systolic pressure (r² = 0.73, P = .004) and end-diastolic pressure (r² = 0.55, P = 012) correlated positively and independently with interstitial fibrosis. CONCLUSION: Coronary driving pressure falls and ventricular pressures increase early after aortocaval fistula and are associated with subsequent myocardial fibrosis deposition.

The taxonomic histort of the genus Capirona is presented. Complete synonymies, descriptions, common names, distributions and discussions are given for Capirona and its two species C. descorticans and C. leiophloea. The generic distribution is the Amazon valley and eastern Guianas and coincides with that of the genus Hevea. C. descorticans is know from the western and southeastern Amazon valley; C. leiophloea is reported from the eastern Guianas and the northeastern Amazon valley. The zone of contact between the two species is along the southern edge oh the Amazon River and the lower drainage of the Rio Tapajoz in Pará, Brazil.

Recent studies have described widespread statigraphic units of Late Pleistocene and Holocene age in the western part of the Amazon Basin. The recognition of deltaic sedimentation in the uppermost these units near Rio Branco, Brazil, at a modern elevation of approximately 500 feett leads to the conclusion that this area was situated on the edge of a large Amazonian lake that existed in the recent past when Andean tectonism caused active downwarping of the western edge of the Amazon Basin. The ramifications of this "Lago Anazonas" hypothesis extend into every area of modern Amazonian geology and biology.

Population densities of six primate species (Saguinus midas, Pithecia pithecia, Cebus apella, Chiropotes satanas, Alouatta seniculus and Ateles paniscus) were estimated in continuous forest and in isolated reserves (one of 100 ha and four of 10 ha). Saguinusdensities in the continuous forest were found to be low, probably due to the lack of edge habitat and second growth favoured by them; Pithecia, Cebus and Ateles populations are also low, possibly because of more widely distributed and/or less abundant food sources than is true for other Amazonian regions, although hunting in the past, particularly of Ateles may also be a contributing factor; and Chiropotes and Alouatta densities were found to be similar to those observed in other areas of Amazonas forests. Ateles and Chiropotes, which occupy ranges on the order of three km2 were excluded from the 100-ka reserve at the time of its isolation. Unfortunately populations were not known prior to isolation of this reserve but during isolation there remained four groups of Saguinus, two Pitheciagroups, one Cebus groups and five Alouatta groups. One Saguinus group disappeared two months later, and one year post-isolation the Cebus group also left the reserve. Single Alouatta groups survive in the isolated 10-ha reserves. Saguinus, present in the four 10-ha reserves following isolation, have disappeared from two of them. One 10-ha reserve retains a group of Pithecia.

Seed dispersal of Hymenaea courbaril was studied by following the fate of 585 seeds embedded with small magnets and set in displays in and near a logged strip in rain forest in the Peruvian Amazonian. Mammals took fruits from all displays, which were located in the forest, edge, and cleared strip. Overall removal rates were low - a median of 8.1 fruits / month from displays maintained with 8-10 fruits - but were higher in August than in earlier months. Most fruits were dropped near the display or had their seeds eaten, but > 13% were successfully dispersed. Most of the dispersed seeds were buried, which increases probability of germination. Maximum dispersal distance of live seeds was 12.1 m (median 3.1 m), but other magnets were transported up to 34 m, indicating seeds were dispersed further, but then eaten. Acouchies (most likely Myoprocta pratti) and agoutis (Dasyprocta fuliginosa) were apparently the main dispersal agents. Dispersal of seeds from the forest into the logged strip was rare, suggesting that although rodents disperse H. courbaril, they cannot be relied on for the reseeding this and similar species in recent clearings.

Densidade básica (g.cm-3: peso seco / volume úmido) da madeira foi determinada para 13 das principais espécies arbóreo-arbustivas de um ecossistema de "campina" (caatinga amazônica) situado em Roraima, extremo norte da Amazônia brasileira. As amostras de madeira para o cálculo da densidade básica de cada espécie foram compostas por "discos amostrais" (casca, alburno e cerne) de diferentes classes diamétricas. No total, foram amostrados 98 indivíduos, perfazendo 150 peças de madeira (52 com diâmetro < 1,6cm; 63 entre 1,6-3,2cm, 27 entre 3,2-4,8cm e 8 > 4,8cm). A espécie de maior densidade média foi Matayba arborescens (Aubl.) Radlk. (0,68 g.cm-3), seguida de Humiria balsamifera (Aubl.) St. Hill. (0,67 g.cm-3) e Pera schomburgkiana Müel. Arg. (0,64 g.cm-3). As classes de menor diâmetro foram também as de menor valor absoluto: 0,59 ± 0,06 (DP) g.cm-3 (< 1,6cm), 0,64 ± 0,08 g.cm-3 (1,6-3,2cm), 0,67 ± 0,06 g.cm-3 (3,2-4,8cm) e 0,69 ± 0,10 g.cm-3 (> 4,8cm). A média ponderada pela biomassa do ecossistema foi de 0,64 ± 0,08 g.cm-3. Este resultado é 15,2% inferior ao comumente utilizado para transformação de volume de madeira em biomassa para ecossistemas de "campina" na Amazônia, influenciando diretamente nos cálculos de emissão de gases do efeito estufa.

Construction of hydroelectric dams in tropical regions has been contributing significantly to forest fragmentation. Alterations at edges of forest fragments impact plant communities that suffer increases in tree damage and dead, and decreases in seedling recruitment. This study aimed to test the core-area model in a fragmented landscape caused by construction of a hydroelectric power plant in the Brazilian Amazon. We studied variations in forest structure between the margin and interiors of 17 islands of 8-100 hectares in the Tucuruí dam reservoir, in two plots (30 and >100m from the margin) per island. Mean tree density, basal area, seedling density and forest cover did not significantly differ between marginal and interior island plots. Also, no significant differences were found in liana density, dead tree or damage for margin and interior plots. The peculiar topographic conditions associated with the matrix habitat and shapes of the island seem to extend edge effects to the islands' centers independently of the island size, giving the interior similar physical microclimatic conditions as at the edges. We propose a protocol for assessing the ecological impacts of edge effects in fragments of natural habitat surrounded by induced (artificial) edges. The protocol involves three steps: (1) identification of focal taxa of particular conservation or management interest, (2) measurement of an "edge function" that describes the response of these taxa to induced edges, and (3) use of a "Core-Area Model" to extrapolate edge function parameters to existing or novel situations.

Radiometric changes observed in multi-temporal optical satellite images have an important role in efforts to characterize selective-logging areas. The aim of this study was to analyze the multi-temporal behavior of spectral-mixture responses in satellite images in simulated selective-logging areas in the Amazon forest, considering red/near-infrared spectral relationships. Forest edges were used to infer the selective-logging infrastructure using differently oriented edges in the transition between forest and deforested areas in satellite images. TM/Landsat-5 images acquired at three dates with different solar-illumination geometries were used in this analysis. The method assumed that the radiometric responses between forest with selective-logging effects and forest edges in contact with recent clear-cuts are related. The spatial frequency attributes of red/near infrared bands for edge areas were analyzed. Analysis of dispersion diagrams showed two groups of pixels that represent selective-logging areas. The attributes for size and radiometric distance representing these two groups were related to solar-elevation angle. The results suggest that detection of timber exploitation areas is limited because of the complexity of the selective-logging radiometric response. Thus, the accuracy of detecting selective logging can be influenced by the solar-elevation angle at the time of image acquisition. We conclude that images with lower solar-elevation angles are less reliable for delineation of selecting logging.

OBJETIVO: O presente estudo teve como objetivo conhecer evidências de validade fatorial e preditiva de uma medida de atitudes ante o uso de drogas em geral (EAAUD). MÉTODO: Participaram voluntariamente 276 estudantes universitários de vários cursos, provenientes de uma universidade pública (94,2%) e outra particular (5,8%) da cidade de João Pessoa (PB). Estes tinham idade média de 21 anos (DP = 3,40; 93,5% eram de jovens de 17 a 25 anos), a maioria solteira (91%) e do sexo feminino (65,6%). Além de um conjunto de perguntas demográficas, os participantes preencheram a EAAUD. Esta compreende uma medida formada por quatro itens/adjetivos bipolares (positivo/negativo, gosto/desgosto, bom/ruim e desejável/indesejável) respondidos em escala do tipo diferencial semântico, de 9 pontos, variando de - 4 a + 4. RESULTADOS: De acordo com a análise de componentes principais, um único componente emergiu, com todos os itens apresentando saturação acima de |0,40|; este teve valor próprio de 3,17, explicando 79,3% da variância total. Sua consistência interna (alfa de Cronbach) foi de 0,91. Por meio de regressão logística, comprovou-se que as pontuações na EAAUD predisseram significativamente a condição de ser um usuário de drogas, B = 0,17, Wald (1) = 8,45, p = 0,004. CONCLUSÃO: Tais resultados indicam que esta medida reúne evidências de validade fatorial e preditiva, podendo ser empregada para conhecer o potencial envolvimento dos jovens com drogas. Não obstante, sugerem-se novas pesquisas com amostras maiores e mais diversificadas.

OBJETIVO: Buscou-se avaliar a prevalência da sonolência excessiva diurna (SED) na população geral de um município brasileiro. MÉTODO: Foram feitas 198 entrevistas domiciliares entre os adultos, em amostra representativa da população geral de Ribeirão do Largo, BA. A amostragem foi aleatória simples. Tinham SED as pessoas com 11 ou mais pontos na Escala de Sonolência de Epworth (ESE). Usaram-se os testes de qui-quadrado, Fisher e ANOVA; nível de significância 5%. RESULTADOS: Tinham SED 21,5% da população (DP = 2,9%; IC 15,8% a 27,2%); não houve associação significativa entre SED e idade (p = 0,924), nem IMC (p = 0,197), sexo (p = 0,095), instrução (p = 0,700), estado civil (p = 0,414) e uso de hipnóticos (p = 0,176). Houve associação com o despertar precoce (p = 0,046). CONCLUSÃO: Foi alta a prevalência de SED na população estudada.
