Background: Conventional surgical repair of thoracic aortic dissections is a challenge due to mortality and morbidity risks. Objectives: We analyzed our experience in hybrid aortic arch repair for complex dissections of the aortic arch. Methods: Between 2009 and 2013, 18 patients (the mean age of 67 ± 8 years-old) underwent hybrid aortic arch repair. The procedural strategy was determined on the individual patient. Results: Thirteen patients had type I repair using trifurcation and another patient with bifurcation graft. Two patients had type II repair with replacement of the ascending aorta. Two patients received extra-anatomic bypass grafting to left carotid artery allowing covering of zone 1. Stent graft deployment rate was 100%. No patients experienced stroke. One patient with total debranching of the aortic arch following an acute dissection of the proximal arch expired 3 months after TEVAR due to heart failure. There were no early to midterm endoleaks. The median follow-up was 20 ± 8 months with patency rate of 100%. Conclusion: Various debranching solutions for different complex scenarios of the aortic arch serve as less invasive procedures than conventional open surgery enabling safe and effective treatment of this highly selected subgroup of patients with complex aortic pathologies.

Background:Primary graft dysfunction is the main cause of early mortality after heart transplantation. Mechanical circulatory support has been used to treat this syndrome.Objective:Describe the experience with extracorporeal membrane oxygenation to treat post-transplant primary cardiac graft dysfunction.Methods:Between January 2007 and December 2013, a total of 71 orthotopic heart transplantations were performed in patients with advanced heart failure. Eleven (15.5%) of these patients who presented primary graft dysfunction constituted the population of this study. Primary graft dysfunction manifested in our population as failure to wean from cardiopulmonary bypass in six (54.5%) patients, severe hemodynamic instability in the immediate postoperative period with severe cardiac dysfunction in three (27.3%), and cardiac arrest (18.2%). The average ischemia time was 151 ± 82 minutes. Once the diagnosis of primary graft dysfunction was established, we installed a mechanical circulatory support to stabilize the severe hemodynamic condition of the patients and followed their progression longitudinally.Results:The average duration of extracorporeal membrane oxygenation support was 76 ± 47.4 hours (range 32 to 144 hours). Weaning with cardiac recovery was successful in nine (81.8%) patients. However, two patients who presented cardiac recovery did not survive to hospital discharge.Conclusion:Mechanical circulatory support with central extracorporeal membrane oxygenation promoted cardiac recovery within a few days in most patients.

AbstractBackground:Acute coronary syndrome (ACS) is defined as a “group of clinical symptoms compatible with acute myocardial ischemia”, representing the leading cause of death worldwide, with a high clinical and financial impact. In this sense, the development of economic studies assessing the costs related to the treatment of ACS should be considered.Objective:To evaluate costs and length of hospital stay between groups of patients treated for ACS undergoing angioplasty with or without stent implantation (stent+ / stent-), coronary artery bypass surgery (CABG) and treated only clinically (Clinical) from the perspective of the Brazilian Supplementary Health System (SHS).Methods:A retrospective analysis of medical claims of beneficiaries of health plans was performed considering hospitalization costs and length of hospital stay for management of patients undergoing different types of treatment for ACS, between Jan/2010 and Jun/2012.Results:The average costs per patient were R$ 18,261.77, R$ 30,611.07, R$ 37,454.94 and R$ 40,883.37 in the following groups: Clinical, stent-, stent+ and CABG, respectively. The average costs per day of hospitalization were R$ 1,987.03, R$ 4,024.72, R$ 6,033.40 and R$ 2,663.82, respectively. The average results for length of stay were 9.19 days, 7.61 days, 6.19 days and 15.20 days in these same groups. The differences were significant between all groups except Clinical and stent- and between stent + and CABG groups for cost analysis.Conclusion:Hospitalization costs of SCA are high in the Brazilian SHS, being significantly higher when interventional procedures are required.

AbstractBackground:Risk scores for cardiac surgery cannot continue to be neglected.Objective:To assess the performance of “Age, Creatinine and Ejection Fraction Score” (ACEF Score) to predict mortality in patients submitted to elective coronary artery bypass graft and/or heart valve surgery, and to compare it to other scores.Methods:A prospective cohort study was carried out with the database of a Brazilian tertiary care center. A total of 2,565 patients submitted to elective surgeries between May 2007 and July 2009 were assessed. For a more detailed analysis, the ACEF Score performance was compared to the InsCor’s and EuroSCORE’s performance through correlation, calibration and discrimination tests.Results:Patients were stratified into mild, moderate and severe for all models. Calibration was inadequate for ACEF Score (p = 0.046) and adequate for InsCor (p = 0.460) and EuroSCORE (p = 0.750). As for discrimination, the area under the ROC curve was questionable for the ACEF Score (0.625) and adequate for InsCor (0.744) and EuroSCORE (0.763).Conclusion:Although simple to use and practical, the ACEF Score, unlike InsCor and EuroSCORE, was not accurate for predicting mortality in patients submitted to elective coronary artery bypass graft and/or heart valve surgery in a Brazilian tertiary care center. Abstract Background: The revascularization strategy of the left main disease is determinant for clinical outcomes. Objective: We sought to 1) validate and compare the performance of the SYNTAX Score 1 and 2 for predicting major cardiovascular events at 4 years in patients who underwent unprotected left main angioplasty and 2) evaluate the long-term outcome according to the SYNTAX score 2-recommended revascularization strategy. Methods: We retrospectively studied 132 patients from a single-centre registry who underwent unprotected left main angioplasty between March 1999 and December 2010. Discrimination and calibration of both models were assessed by ROC curve analysis, calibration curves and the Hosmer-Lemeshow test. Results: Total event rate was 26.5% at 4 years.The AUC for the SYNTAX Score 1 and SYNTAX Score 2 for percutaneous coronary intervention, was 0.61 (95% CI: 0.49-0.73) and 0.67 (95% CI: 0.57-0.78), respectively. Despite a good overall adjustment for both models, the SYNTAX Score 2 tended to underpredict risk. In the 47 patients (36%) who should have undergone surgery according to the SYNTAX Score 2, event rate was numerically higher (30% vs. 25%; p=0.54), and for those with a higher difference between the two SYNTAX Score 2 scores (Percutaneous coronary intervention vs. Coronary artery by-pass graft risk estimation greater than 5.7%), event rate was almost double (40% vs. 22%; p=0.2). Conclusion: The SYNTAX Score 2 may allow a better and individualized risk stratification of patients who need revascularization of an unprotected left main coronary artery. Prospective studies are needed for further validation.

This paper deals with one genus and three species of dorylaimid nematodes found inhabiting soil in Brazil, as follows: Eudorylaimus ibiti Lordello, 1965. Closely resembling E. ibiti are E. humilis (Thorne & Swanger, 1936) Andrássy, 1959, E. diadematus (Cobb, 1936) Andrássy, 1959, and E. santosi (Meyl, 1957) Andrássy, 1959. it differs from E. humilis in the following aspects: a) longer and thicker body (1,126.0-1,520.8: 1,000 microns; a=21.0-26.0 : a= 31); b) less prominent lips; and, c) tail terminus decidedly acute; differs from E. diadematus in having: a) less prominent lips; b) posterior region of body ventrally concave; and, c) a different organization in the walls of the pre-rectum; differs from E. santosi in having: a) longer body (1,126.0-1,520.8 : 900-1,000 microns); b) spear with undiscernible aperture; c) a different organization in the guiding-ring of spear; and, d) caudal papillae closer together and located in front of the middle of the tail. Mesodorylaymus pizai Lordello, 1965. M. pizai most closely resembling species is M. mesonyctius (Kreis, 1930) Andrássy, 1959, from which it differs in having: a) lip region amalgamated, continuous with neck contour (lateral view); b) males with 11-12 supplements; and, c) females with longitudinal vulva. Metaporcelaimus Lordello, 1965. This genus differs from Aporcelaimus Thorne & Swanger, 1936, in having oesophagus made up of three regions, a cardia like structure being seen between the posterior and middle parts. Type species: M.mombucae Lordello, 1965.

The genus Homodiaetus Eigenmann & Ward, 1907 is revised and four species are recognized. Its distribution is restricted to southeastern South America, from Uruguay to Paraguay river at west to the coastal drainages of Rio de Janeiro State, Brazil. Homodiaetus is currently distinguished from other genus of Stegophilinae by the combination of the following characters: origin of ventral-fin at midlength between the snout tip and the caudal-fin origin; opercle with three or more odontodes; and gill membranes confluent with the istmus. Homodiaetus anisitsi Eigenmann & Ward, 1907, is diagnosed by the caudal-fin with black middle rays, margin of upper and lower procurrent caudal-fin rays with dark stripes extending to the caudal-fin, and 3-6 opercular odontodes; H. passarellii (Ribeiro, 1944) with 6-7 opercular odontodes, 21-24 lower procurrent caudal-fin rays and 23-26 upper procurrent caudal-fin rays; H. banguela sp. nov. with 9 opercular odontodes, 17-19 lower procurrent caudal-fin rays, 17-22 upper procurrent caudal-fin rays, reduction of fourth pharyngobranchial with only three teeth and untoothed fifth ceratobranchial; and H. graciosa sp. nov. with 5-6 dentary rows, 7-9 opercular odontodes and 16-23 upper procurrent caudal-fin rays.

Bothriurus pichicuy, a new scorpion species from the center coast of Chile, is described. It belongs to the vittatus species-group, and differs from the others species in the group by the pigmentation patterns of the prosoma, mesosoma and venter of metasoma, by the development and arrangement of the ventral keels of caudal segment V, and for its small size. The species is the only representative of the vittatus species-group that lives in sympatry with another Bothriurus species: B. coriaceus Pocock, 1893, from the coastal desert of Chile. Records of B. pichicuy came from the provinces of Petorca and Choapa.

Pimelodus tetramerus sp. nov. difere das outras espécies de Pimelodus Lacepède por sua coloração típica, que consiste de quatro faixas escuras sobre os lados do corpo, a primeira, mais dorsal, iniciando logo após o final da placa pré-dorsal e terminando no final da base da nadadeira adiposa; a segunda, mais larga que a primeira, logo acima da linha lateral, percorre desde a borda lateral do processo supra-occipital até a base da nadadeira caudal. Ambas faixas estão fragmentadas em sua porção anterior, formando pontos escuros que, ocasionalmente, podem estar unidos, compondo um padrão vermiculado. Tais pontos escuros e/ou padrão vermiculado se distribui sobre a região pré-dorsal e cabeça. A terceira faixa, tão larga quanto a segunda, logo abaixo da linha lateral, inicia junto à região superior do processo pós-cleitral e estende-se até a base da nadadeira caudal e a quarta, de menor comprimento, podendo estar fraca ou ausente, inicia-se logo abaixo do processo pós-cleitral e finaliza até próximo à nadadeira pélvica. Além disso, constituem caracteres diagnósticos da nova espécie a presença de uma estreita faixa clara sobre a linha lateral, com aproximadamente 1/3 da largura da terceira faixa escura, e a presença de uma barra escura percorrendo longitudinalmente o lobo ventral da nadadeira caudal. A nova espécie compartilha o forâmen trigeminal tripartido com outras espécies de pimelodídeos.

O presente estudo caracterizou o desenvolvimento inicial de Brycon hilarii (Valenciennes, 1850) através de caracteres morfométricos e merísticos e analisou as relações entre as características morfométricas das larvas ao longo do desenvolvimento usando modelos de regressão linear, quadrática e linear por partes. O material foi obtido nas bacias dos rios Cuiabá e Manso, Mato Grosso, Brasil, entre março de 2000 e março de 2004. Os indivíduos foram identificados e separados de acordo com o grau de desenvolvimento da notocorda e, posteriormente, medidos e contados os caracteres morfométricos e merísticos, respectivamente. O comprimento padrão variou entre 3,25 e 26,00 mm. Inicialmente, as larvas apresentaram maior concentração de pigmentos dendríticos ao longo do intestino e, em flexão e pós-flexão, intensificaram-se no dorso do corpo. Uma mancha umeral e outra no pedúnculo caudal foram observadas no final do estágio de pós-flexão. O intestino apresentou-se longo, o focinho curto, o órgão adesivo presente em larval vitelino e pré-flexão e a boca terminal por todo o desenvolvimento. O número total de miômeros variou de 41 a 50 (23 a 30 pré e 16 a 24 pós-anal). O número de raios das nadadeiras foi: dorsal 11; anal 30; peitoral 15 e ventral oito. A análise do crescimento indicou maior metamorfose no estágio de flexão.

La regulación de ríos y arroyos mediante diques modifica parámetros que influyen directamente sobre la biota. En la cuenca endorreica Achiras-del Gato ha finalizado recientemente la construcción de una presa de mediana envergadura que alterará las características limnológicas del sistema y la estructura de las comunidades bentónicas. Este estudio aporta los primeros registros del macrobentos de este sistema lótico y analiza la variación espacio-temporal de la estructura de la comunidad en tramos pedemontanos del arroyo Achiras. Durante un año, antes de la construcción de la presa, se colectaron muestras de zoobentos y se registraron variables abióticas en correderas y rabiones de dos sitios del arroyo, uno situado río arriba del área del dique y otro aguas abajo de la misma. Para cada sitio, hábitat y estación del año se determinó abundancia, frecuencia, riqueza taxonómica, Índice de Shannon y equidad, y se analizaron mediante ANOVA de una y tres vías. Los atributos de las comunidades de referencia se complementaron con la determinación de taxa exclusivos e indicadores y detectores ecológicos. En el primer sitio se registraron 73 taxa, Simulium fue más abundante y Leptohyphes eximius Eaton, 1882, más frecuente. En el segundo se hallaron 75 taxa, Caenis fue más abundante y Cricotopus sp. 3 más frecuente. La abundancia y la riqueza fueron significativamente mayores en primavera, y la diversidad en rabiones de invierno. Se determinó que los atributos de la comunidad estuvieron afectados por la época de muestreo. El arroyo Achiras posee un régimen hídrico freático-pluvial, con caudales bajos en invierno y altos en verano, por lo que se esperan cambios en la biota nativa y la integridad del sistema en respuesta a la reducción de la variabilidad hidráulica cuando el funcionamiento de la presa altere el caudal natural.

A morfologia macro e microscópica da traqueia e pulmões de Amphisbaena vermicularis Wagler, 1824 e Amphisbaena microcephala (Wagler, 1824), assim como a ultraestrutura das câmaras respiratórias, foram descritas pela primeira vez neste estudo. A traqueia não se ramifica e seu segmento caudal, situado entre os pulmões, foi denominado brônquio. O pulmão esquerdo é alongado, saculiforme e unicameral, com parênquima faveolar na porção cranial e trabecular, na porção caudal. Câmaras respiratórias estão presentes em ambas as regiões do pulmão, mas é possível que a região caudal funcione também como reservatório de ar. O pulmão direito está reduzido nas duas espécies, no entanto em A. vermicularis a redução é bastante acentuada e apenas um vestígio deste órgão pode ser observado, mas em A. microcephala o pulmão direito é um órgão com limites definidos que se comunica com a porção caudal do tubo traqueal, através de dois orifícios. Pneumócitos tipo I e tipo II estão presentes nas câmaras respiratórias. As lâminas basais dos pneumócitos I e das células endoteliais encontram-se fundidas, de forma a diminuir a barreira ar-sangue, que é de aproximadamente 0,5 µm em A. microcephala. As características morfológicas descritas neste estudo podem representar adaptações que permitem a sobrevivência dos espécimes de Amphisbaenia nas galerias subterrâneas, onde passam a maior parte de suas vidas sob condições de baixa renovação de ar, níveis de umidade relativamente variáveis e partículas em suspensão.

In Brazil all the fishes belonging to the sub-family Curimatinae are called « saguirú ». The present work gives a biological study of the Curimatus elegans Steind., a small fish without any economical importance, which is to be found along the whole brazilian coast, down till Paraguay. The specimens utilized for the present study come from Fortaleza (Ceará, north-eastern Brazil). The C. elegans is « ilyophagus », that means, it feeds itself exclusively with those organic materials to be found in mud, specially with microscopical algae. The intestines are very extent, some of them measuring about 9 to 11 times body's length. Studies have been made about growth and age of the C. elegans; the biggest sizes found were of 153 mm. for females and 88 mm. for males. The C. elegans shows developed sexual glands during a long period (April to September). The movements of the spermatozoa, in contact with water is of 40 to 50 seconds of intense movements, ceasing after 70 to 100 seconds. In contact with 0.5% NaCl-solution spermatozoa show a big increase in movements-time, that can last till about 25 minutes. The eggs' diameter measures 0.70 to 0.73 mm., mature and hydrated it attains 0.93 to 1,00 mm. There is a certain correlation between the size of the body and the quantity of eggs. Big specimens can produce a total of 200.000 eggs. The average quantity contained in 1 gr. and 1 cc. is 6018 and 6229 eggs, respectively. Maturity and spawning in laboratory has been obtained due to injections of suspension of fish-hypophysis. Three or four hours after the injection, fishes show more movement and evident signs of excitation, proceeding spawning after 5 to 6 hours. Males, persecuting females, describe successive circles (merry-go-round) - carroussel), swimming side by side with females up to water's surface, where sexual products are start beating dry, for there is no blood yet. Circulation-scheme is to be found on fig. 4 and 5. The swim-bladder and the stomach are but delineated; the intestine is formed by a cylindric tube, all closed. At the place, where later on there will open the mouth, we find a group of ciliary hairs that produce a liquid current, very evident by the semi-circle formed by attached solid particles. After 36 hours, opening of the mouth and formation of the gill slits begin. At the age of 90 hours (4 mm.) the larvas swim well and start to feed themselves; the digestive tube is now all open and the swimbladder works already. During the first days of life, larvas have an adhesive organ situated at their frontal region (fig. 7) in form of a crescent, by means of which they hang to surrounding vegetation (fig. 6). When the larva begins to swim and to feed itself and its yolk are having been absorbed. the adhesive organ retracts and disappears. While larvas and alevins feed themselves with plancton, they have small eye-teeth, which disappear,. when fishes become « ilyophagus ». There exist too, during their life as larvas, pharyngeal-teeth. The lateral line appears in the larva after 16 to 18 days; more or less at the same time all fins are completely developed. Shortly after, first scales appear (20 to 23 days). Evolution of intestines twisting followed (fig. 9). Larvas show at different parts of their bodies small of organs excretory functions, that are constituted by bottons in serial disposition, every one with an excretory canal that opens towards the outside. These formations disappear suddenly when larvas attain their phase of alevin. The existence of a great number of said formations at the caudal fin (fig. 12) is of great interest. In our experiences of breeding we have employed several thousands of C. elegans larvas in different environs and we made conditions of surrounding change (illumination), depth of water, temperature, presence of sand at bottom of aquariums and without sand, food). In this way we could compare the results obtained, estimate the action of each factor for the realisation of a good bring-up of larvas.

The A. describes the life history of Flebotomus (Brumptomyia) travassosi Mangabeira, 1942. The paper deals with the morphology of the eggs, the larvae in all stages, the pupae and the female. Also gives the differences between the 3 known species of that subgenus (avellari, travassosi and guimarãisi) pointing out the characters in common, principally the presence of only two caudal hairs in all stages. The pupae do not present specifical characters.

The present morphological study of A. glabratus was based on the observation of shell, radula, renal region and genitalia of 50 specimens having a shell diameter of 18 mm. In this summary we record the data pertaining to the chracteristics that can be used in systematics. The numerals refere to the mean and their standard deviation; no special reference being made, they correspond to length measurements. Shell: 18 mm in diameter, 5.59 ± 0.24 mm in greatest width, 5 to 6 whorls. Right side umbilicated, left one weakly depressed. Last whorl about thrice as tall as the penultimate one at the aperture, the measurements being taken on the right side. Aperture perpendicular or a little oblique. Body, extended: 47.06 ± 3.31 mm. Renal tube: Narrow and elongated, 23.84 ± 1.90 mm, showing a pigmented ridge along its ventral surface. Ovotestis: 12.78 ± 1.50 mm. Mainly trifurcate diverticula attaching in fan-like manner to the collecting canal (this arrangement is seen to best advantage in the cephalic middle of the ovotestis). The collecting canal greatly swells at the cephalic end, narrowing suddenly as it leaves the ovotestis. Ovisperm duct: 13.70 ± 1.68 mm, including the non-unwound seminal vesicle. The latter, situated about 1 mm from the beginning af the ovisperm duct, was 1.14 ± 0.29 mm in greatest diameter, and is beset by numerous short diverticula. Sperm duct: 14.16 ± 1.27 mm, pursuing a sinous course along the oviduct. Prostate: Prostate duct 5.53 ± 0.74 mm, collecting a row of long diverticula, the latter 21.6 ± 3.5 in number. Last diverticulum generally simple or bifurcate, penultimate generally arborescent, bifurcate or simple, antepenultimate nearly always arborescent, the remaining ones arborescent. The arborescent diverticula frequently give off secondary branches. Vas deferens: 17.50 ± 2.05 mm. The ratio vas deferens/vergic sac was 4.7 ± 0.6. Verge: 3.70 ± 0.54 mm long, 0.12 ± 0.03 mm wide. Free end tapering to a point where the sperm canal opens. No penial stylet. Vergic sac: 3.77 ± 0.50 mm long, 0.19 ± 0.01 mm wide. The length ratio vergic sac/preputium was 1 ± 0.02. Preputium: Deeply pigmented, 3.79 ± 0.40 mm long, 0.89 ± 0.12 mm wide in the middle. Muscular diaphragm between it and the vergic sac. Two muscular pilasters along its lateral walls. Oviduct: 10.24 ± 1.29 mm, suddenly swollen at the cephalic end so that it forms a folded pouch capping the beginning of the uterus. Uterus: 10.58 ± 1.18 mm. Vagina: 2.06 ± 0.15 mm long, 0.32 ± 0.05 mm wide, showing a swelling at its caudal portion, just above the opening of the spermathecal duct. Spermatheca: 1.57 ± 0.41 mm long, 0.92 ± 0.23 mm wide. Spermathecal duct 1.15 ± 0.23 mm. Radula: 125 to 163 rows of teeth (mean 141.4 ± 9.8). Radula formula 27-1-27 to 34-1-34 (mean 30.9 ± 1.7).
