Choledochal cyst is a rare congenital malformation of the biliary tree, and aproximately 25% of them are diagnosed in adults. Appropriate surgical management of these lesions depends on the anatomic site and extension of the cystic process. The recognized association of the bile duct cysts with hepatobiliary malignant disease has important surgical implications. Total cyst removal and cholecistectomy with Roux-en- Y hepaticojejunostomy was performed in a 47 year old female with a common bile duct cyst tipe 1 postoperative period was uneventful. A review of literature is presented.

Spontaneous perforation of the common bile duct is rare. It happens predominantly in children and it is related to obstructive disease of the biliary tract. We present a case of an 18 year-old male patient, with ulcerative rectocolitis associated with malignant tumor of the head of pancreas. The patient developed an acute abdomen syndrome and laparotomy, a spontaneous perforation of common bile duct was evidenced. The authors make a revision of the clinical aspects of that pathology.

Biliary duct cystoadenomas are rare neoplasms, with about 120 cases described in the literature, including cystoadenomas and cystoadenocarcinomas. The authors report a case of cystoadenoma of the common bile duct in a 45-year-old woman with history of jaundice. Ultrasound revealed a cystic mass located in the common bile duct. Endoscopic retrograde cholangiopancreatography showed a negative filling defect in the proximal third of the common bile duct, a finding unique to our case. Total excision of the mass, cholecystectomy and an end-total anastomosis with a T-tube choledochostomy, were performed. Histological examination revealed a multicystic lesion with cavities lined by mucinous columnar non-ciliated epithelium, with surrounding densely cellular stroma resembling ovarian stroma. Six years after surgery the patient is alive and well, with no complains referring to the hepatobiliary tract. No abnormalities are presently detected in the biliary tree, ultrasonographically.

Literature has been showing a tendency of reducing the limits of Whipple's resection. The main technical advance was the pylorus preserving resection, technique proposed by Traverso and Longmire in 1978. The pancreticoduodenectomy for chronic pancreatitis, is probably the best opportunity to apply this type of procedure. In these specific patients, the author preserved the pylorus and the third portion of the duodenum. The gastrointestinal transit was reconstructed by the duodenumduodenal anastomosis and the bile duct and the pancreas were drained in a Roux-en-way loop . Follow-up showed no important complication, with no problems related to gastric emptying and without pain.

Management of common bile duct stones in the era of laparoscopic surgery is controversial. The biliary anastomosis is indicated in case of large common bile duct, recurrent stones, giant stones and concomitant common bile duct stricture and duct stones. The development of laparoscopic techniques permits to perform this type of surgery laparoscopically as well as the open procedure.

A 33-year-old man presented to our department with a 4-month history of right quadrant abdominal pain. Physical examination was normal. A chest X-ray showed no remarkable findings. Ultrasonography demonstrated a hypoechoic mass measuring 6 cm in the head of the pancreas. Computed tomography confirmed a solid mass in the pancreas without Wirsung or bile duct dilatation. At laparotomy, excision a 6 cm egg-shaped and hypervascular mass in the head of the pancreas was performed. Histologically, the features were consistent with Castleman disease. Castleman's disease is a rare, usually benign lymphoid condition described by Castleman (1954) and characterized by giant lymph nodes. Surgical resection is diagnostic and curative.

Our objective is to report a case of laparoscopic cholecystectomy in a patient with duplicated cystic duct. A 34 year old male presented with episodic pain in the upper rigth quadrant of the abdomen. Murphy' s sign was not present. Ultrassonography showed gallbladder with multiple calculi and a thickened wall. At laparoscopic cholecystectomy, a duplicated cystic duct was found. Careful dissection and intraoperative cholangiography were performed to rule out common bile duct injury.

Perforation of common bile duct (CBD) is usually associated with invasive procedures. Spontaneous perforation is rare, and more often described in neonates. We report a case of a spontaneous perforation of CBD in an adult with a family history of cholelithiasis, but with no calculus found during exploration of the biliary tree. The patient was successfully treated by cholecystectomy and T - tube drainage of the CBD.

Laparoscopic resection of the left lateral segment of the liver in donors of living liver transplantation. The authors present a case of laparoscopic resection of the left lateral segment of the liver in a donor of living liver transplantation. The procedure was done in six hours and the left lateral segment of the liver was removed through a 15 cm right subcostal incision. The patient was discharged on the 5th post-operative day. A 40 mL intrabdominal collection of bile was percutaneously drained guided by ultra-sonography. The drain was removed after five days. Afterwards, the patient had good recovery with no other complication.

OBJECTIVE: To verify whether the ileal exclusion interferes with liver and kidney functional changes secondary to extrahepatic cholestasis.METHODS: We studied 24 rats, divided into three groups with eight individuals each: Group 1 (control), Group 2 (ligation of the hepatic duct combined with internal biliary drainage), and Group 3 (bile duct ligation combined with internal biliary drainage and exclusion of the terminal ileum). Animals in Group 1 (control) underwent sham laparotomy. The animals of groups 2 and 3 underwent ligation and section of the hepatic duct and were kept in cholestasis for four weeks. Next, they underwent an internal biliary bypass. In Group 3, besides the biliary-enteric bypass, we associated the exclusion of the last ten centimeters of the terminal ileum and carried out an ileocolic anastomosis. After four weeks of monitoring, blood was collected from all animals of the three groups for liver and kidney biochemical evaluation (albumin, ALT, AST, direct and indirect bilirubin, alkaline phosphatase, cGT, creatinine and urea).RESULTS: there were increased values of ALT, AST, direct bilirubin, cGT, creatinine and urea in rats from Group 3 (p < 0.05).CONCLUSION: ileal exclusion worsened liver and kidney functions in the murine model of extrahepatic cholestasis, being disadvantageous as therapeutic procedure for cholestatic disorders.

Objective: To analyze and discuss the clinical data, diagnosis and treatment of a number of patients with cystic dilatation of the common bile duct of a Brazilian pediatric hospital.Methods: We analyzed 30 patients treated at the Martagão Gesteira Institute of Pediatrics and Child Care of the Federal University of Rio de Janeiro for 23 years ,with statistical analysis of epidemiological data, clinical manifestations, diagnosis, treatment and postoperative outcome.Results: We observed a marked female predominance (73.4% of cases), the diagnosis being made in the first decade of life in 90% of patients. The most prevalent clinical manifestation was jaundice (70% of cases) and the classic triad of choledochal cyst was not observed. Abdominal ultrasound was the first imaging examination performed, with a sensitivity of 56.6%, with diagnostic definition in 17 children. Two patients (6.6%) had prenatal diagnosis. All patients underwent surgical treatment, cyst resection with Roux-en-Y hepaticojejunostomy being performed in 80% of cases. The incidence of postoperative complications was 13.3% and the mortality rate was 6.6%, ie two patients were diagnosed with Caroli's disease.Conclusion: The non-observance of the classic triad of choledochal cyst suggests that its incidence is lower than that reported in the medical literature. The surgical treatment of choledochal cysts, with resection and bilioenteric anastomosis, is safe even for small children.

Herlyn-Werner-Wunderlich (HWW) syndrome is a rare congenital disorder of the Müllerian ducts in which there is uterus didelphys, obstructed hemivagina and unilateral renal agenesis. The most common presentation is an abdominal mass secondary to hematocolpos, pain and dysmenorrhea. However, in some cases, such as the one we present here, menses are normal due to an obstructed hemivagina, and diagnosis can be delayed. We describe evaluation and surgical management of a 13-year-old girl with this condition who was diagnosed by computed tomography (CT) scan and confirmed by pelvic ultrasound and surgical exploration, as well as a review of the literature.

Descreve-se a ocorrência de um surto de intoxicação espontânea por Senecio brasiliensis em ovinos em um estabelecimento do município de Mata, Estado do Rio Grande do Sul, em meados de janeiro de 1997. De um total de 94 ovinos, 51 (54,25%) animais adoeceram e 50 (53,2%) morreram. Esse rebanho permaneceu durante aproximadamente 7 meses (de junho de 1996 a janeiro de 1997) em piquetes de pastagem nativa onde havia grande quantidade de S. brasiliensis. O quadro clínico manifestado pelos animais afetados consistia em fotossensibilização, emagrecimento progressivo, apatia, fraqueza, perturbações neurológicas como depressão, andar a esmo e desequilibrado, icterícia e hemoglobinúria. Houve melhora das lesões de pele naqueles ovinos que desenvolveram fotossensibilização hepatógena depois que foram retirados do sol. As principais lesões macroscópicas observadas em 9 dos 10 ovinos necropsiados incluíam fígado diminuído de tamanho, firme, difusamente marrom amarelado ou esverdeado, com quantidades variáveis de nódulos de 1-3 mm de diâmetro, bem circunscritos, salientes na cápsula, amarelados, distribuídos aleatoriamente por todo o parênquima. A vesícula biliar estava repleta e preenchida por bile verde escura e espessa. Havia também derrames cavitários (hidropericárdio e ascite). Crise hemolítica aguda fatal associada à intoxicação crônica hepatógena por cobre foi observada em cinco ovinos. Além das lesões hepáticas macroscópicas já mencionadas, foi observada icterícia generalizada da carcaça, rins tumefeitos, friáveis, difusamente escurecidos ou com fino pontilhado enegrecido; a urina era marrom escura (hemoglobinúria). As principais lesões microscópicas foram observadas no fígado e consistiam em hepatomegalocitose, proliferação de ductos biliares (hiperplasia ductal) e fibrose periportal moderada acompanhada de infiltrado inflamatório mononuclear. Macrófagos carregados de pigmento acastanhado formavam aglomerados nas tríades portais ou estavam dispersos entre os hepatócitos remanescentes. O material armazenado no citoplasma desses macrófagos correspondia a ceróide e cobre, positivo nas técnicas de PAS e rodanina, respectivamente. Nos rins de cinco animais, havia nefrose hemoglobinúrica caracterizada por degeneração e necrose do epitélio tubular, presença de hemoglobina e hemossiderina no citoplasma das células epiteliais dos túbulos contorcidos e cilindros de hemoglobina na luz tubular. Evidência morfológica de encefalopatia hepática incluía degeneração esponjosa (status spongiosus) da substância branca do encéfalo. Achados ultra-estruturais no fígado incluíam graus variáveis de degeneração hepatocelular caracterizada pelo acúmulo de numerosas gotas lipídicas no citoplasma das células hepáticas e presença de lisossomos carregados de material eletrodenso que, na maioria dos casos, correspondia à lipofuscina-ceróide. Adicionalmente, havia discreta dilatação do retículo endoplasmático rugoso e moderada hiperplasia do retículo endoplasmático liso em algumas regiões do citoplasma dos hepatócitos. No epitélio dos túbulos contorcidos proximais do rim foi observado edema intracelular e diversas alterações mitocondriais de caráter degenerativo que incluíam tumefação, desorganização e ruptura das cristas, matriz finamente granular, acúmulo de gotículas de gordura e ruptura das membranas em alguns casos. Lisossomos contendo material fortemente eletrodenso foram observados em muitas células tubulares renais. O exame laboratorial de fragmentos de fígado e rim dos ovinos afetados revelou níveis elevados de cobre que variaram respectivamente de 369 ppm a 1248 ppm e 152 ppm a 687 ppm com base na matéria seca. O diagnóstico de intoxicação por Senecio brasiliensis baseou-se em dados epidemiológicos, clínicos, de necropsia, histopatológicos e laboratoriais.

Foi realizado um estudo retrospectivo dos casos de tristeza parasitária bovina (TPB) ocorridos no sul do Rio Grande do Sul, área de influência do Laboratório Regional de Diagnóstico (LRD) da Faculdade de Veterinária da Universidade Federal de Pelotas entre 1978 e 2005. De um total de 4.884 materiais de bovinos provenientes de necropsias realizadas e órgãos ou sangue enviados ao laboratório 231 (4,7%) tiveram o diagnóstico de TPB. Desses 231 surtos foram resgatados os dados de 221 diagnósticos dos quais 91 (41,1%) foram causados por Babesia bovis, 11 (4,9%) por Babesia bigemina, e 65 (29,41%) por Anaplasma marginale. Em outros 33 (14,93%) surtos de babesiose não foi informada a espécie de Babesia e em 21 (9,5%) surtos foi detectada infecção mista por Babesia sp e A. marginale. Os índices gerais médios de morbidade, mortalidade e letalidade, resgatados em 149 dos 221 surtos da doença, foram de 11,17%, 6,81% e 70,04%, respectivamente. Verificou-se que, na região estudada, a maioria dos surtos ocorre durante os meses de verão e outono, e que os animais com um a três anos de idade são os mais afetados. Os sinais clínicos nos surtos caracterizaram-se por apatia, orelhas caídas, debilidade, febre, anorexia e emagrecimento. Os valores de hematócrito eram baixos. Hemoglobinúria foi frequentemente observada nos casos de babesiose. Sinais neurológicos estavam presentes nos casos de babesiose por B. bovis e se caracterizaram por transtornos da locomoção, tremores musculares, agressividade e quedas com movimentos de pedalagem. As lesões macroscópicas principais relatadas nos casos de babesiose foram esplenomegalia, hepatomegalia, fígado amarelo, hemoglobinúria, icterícia, hemorragias cardíacas e bile espessa. Congestão do córtex cerebral foi relatada nos casos de babesiose por B. bovis. Nesta região, com população de bovinos de aproximadamente 2.630.000 cabeças as perdas anuais por morte de bovinos pela enfermidade podem ser estimadas em 6.220 cabeças por ano representando um prejuízo econômico anual aproximado de R$3.732.000,00 evidenciando a necessidade de medidas preventivas que evitem a exposição de animais desprote-gidos aos agentes da TPB.

Avaliou-se a incidência de Vibrio spp. a partir de lesões superficiais em mamíferos marinhos encalhados ou capturados em redes de pesca nas regiões litorâneas do Sudeste (Rio de Janeiro) e Sul (RS) do Brasil. Foram coletadas 198 amostras, pelas instituições de pesquisa DEENSP, GEMARS e Ceclimar, as quais foram enviadas ao Labent/IOC/FIOCruz, onde foram submetidas ao enriquecimento em Água Peptonada Alcalina (APA) adicionada de 1% e 3% de NaCl e in-cubadas a 37ºC por 18-24 horas. Em seqüência foram semeadas em meio Agar Tiossulfato Citrato Bile Sacarose (TCBS) e as colônias suspeitas submetidas à caracterização bioquímica. Foram isoladas 108 cepas bacterianas, destacando-se Vibrio alginolyticus, V. parahaemolyticus, V. vulnificus e V. fluvialis como os principais patógenos isolados. Os resultados obtidos apontam para a necessidade de implementar atividades de vigilância e monitorização bacteriológica, particularmente de espécies selvagens, e reforçar os programas de proteção ambiental em casos de mamíferos marinhos ameaçados de extinção.
