Plasmodium parasites degrade host hemoglobin to obtain free amino acids, essential for protein synthesis. During this event, free toxic heme moieties crystallize spontaneously to produce a non-toxic pigment called hemozoin or ß-hematin. In this context, a group of azole antimycotics, clotrimazole (CTZ), ketoconazole (KTZ) and fluconazole (FCZ), were investigated for their abilities to inhibit ß-hematin synthesis (IßHS) and hemoglobin proteolysis (IHbP) in vitro. The ß-hematin synthesis was recorded by spectrophotometry at 405 nm and the hemoglobin proteolysis was determined by SDS-PAGE 12.5%, followed by densitometric analysis. Compounds were also assayed in vivo in a malaria murine model. CTZ and KTZ exhibited the maximal effects inhibiting both biochemical events, showing inhibition of β-hematin synthesis (IC50 values of 12.4 ± 0.9 µM and 14.4 ± 1.4 µM respectively) and inhibition of hemoglobin proteolysis (80.1 ± 2.0% and 55.3 ± 3.6%, respectively). There is a broad correlation to the in vivo results, especially CTZ, which reduced the parasitemia (%P) of infected-mice at 4th day post-infection significantly compared to non-treated controls (12.4 ± 3.0% compared to 26.6 ± 3.7%, p = 0.014) and prolonged the survival days post-infection. The results indicated that the inhibition of the hemoglobin metabolism by the azole antimycotics could be responsible for their antimalarial effect.

BACKGROUND: Pemphigus vulgaris (PV) is an autoimmune disease characterized by blistering of the skin and mucosa, which develops due to the interaction between predisposing genetic and environmental factors. Infections caused by members of the Herpesviridae family have been suggested as a possible triggering factor for PV. OBJECTIVE AND METHODS: In this report, we investigate the presence of herpesviruses in refractory lesions on the right upper eyelid. The lesion has persisted despite the treatment with corticosteroids. Polymerase chain reaction (PCR) and DNA sequence analysis have been used to detect the DNA of HSV 1/2, VZV, EBV, CMV, HHV-6, HHV-7, and HHV-8. RESULTS: The sample collected from the right upper eyelid has tested positive for HSV 1/2. Sequence analysis has confirmed the PCR results and allowed the identification of the HSV strain as belonging to type 1. After treatment with acyclovir, the lesion of the right upper eyelid has cleared and not relapsed. CONCLUSION: When patients present PV lesions which are refractory to corticosteroid therapy, herpetic infection should be considered.

Com a finalidade de caracterizar efetivamente a septicemia na salmonelose prolongada, em 6 pacientes, dos quais 2 com resistência antibiótica e quimioterápica, procedeu-se a hemoculturas seriadas, consecutivas ou não, durante um período que oscilou entre 25 e 113 dias. Em cada paciente, numa média de 7 hemoculturas, foram isoladas, respectivamente, as seguintes salmonelas: S. typhi, S. paratyphi C, S. panamá, S. cholerae-suis var. Kunzendorf e S. paratyphi A. No caso em que se isolou a S. paratyphi A, em uma das hemoculturas isolou-se, também, a S. montevideo. A duração média da septicemia nos 6 pacientes foi de 73 dias. A se considerarem procedentes os informes dos pacientes quanto ao início da doença, a septicemia, com tôda a probabilidade, deve ter tido a duração média de 4 meses. Por outro lado, tomando por base a identidade do quadro clínico e a coincidência fisiopatológica dos casos estudados, é possível admitir-se a ocorrência de septicemias de longa duração nos demais 31 casos pelas seguintes salmonelas: S. newport, S. dublin, S. cholerae-suis, S. typhimurium, S. anatum, S. derby, e Salmonellas sp (não sorològicamente identificadas). Com base nestes achados, o A. acredita não ser possível admitir-se, na salmonelose em estudo, a diferença fundamental entre salmonelas adaptadas ou não à espécie humana, conforme preceitua a Doutrina de Kiel; considera, também, limitadas as restrições preconizadas pela Doutrina de Montevidéu aos conceitos fisiopatogenéticos da escola alemã. De fato, salmonelas do grupo tifiparatifi e outras consideradas adaptadas aos animais têm produzido, indistintamente em crianças e em adultos, quadros clínicos indistinguíveis, nos quais transcende a septicemia de longa duração e tornam-se manifestas a disproteinemia marcante e a hiperplasia do S.R.E. Finalmente, discute-se no trabalho o papel representado por uma série de fatores que podem atuar, acarretando alterações no organismo parasitado e criando condições à multiplicação das salmonelas e à manutenção da infecção. Dentre êles foram ressaltados: a associação parasitária, especialmente a esquistossomose mansoni; a desnutrição crônica; a disproteinemia marcante, seja devida à deficiência de prótides, seja em decorrência ao estado reacional do S.R.E., seletivamente agredido pelas salmonelas; modificações do estado imunitário, quer por inibição da anticorpogênese e da fagocitose, quer devida à inadequada propriedade imunológica das globulinas, extraordinariamente aumentadas na doença.

Um levantamento malacológico em coleções hídricas de 13 municípios da microrregião de Belo Horizonte, MG, Brasil, foi efetuado para detectar focos de transmissão de esquistossomose e outras parasitoses. De 1990 a 1996 foram coletados 22.066 moluscos dos quais 378 (1,7%) estavam infectados com trematódeos: Biomphalaria glabrata (7.920), com Schistosoma mansoni (1,9%), com Echinostomatidae (1,2%), com Strigeidae (0,6%), com Cercaria minense (0,1%) e Derogenidae (-0,1%); B. straminea (4.093), com Strigeidae (0,6%), com Echinostomatidae (0,2%), com Clinostomatidae (-0,1%) e duas cercárias desconhecidas; B. tenagophila (1.338), com Strigeidae (0,1%); Physa marmorata (1.776), com Echinostomatidae (1,6%). Os moluscos Biomphalaria peregrina, B. occidentalis, B. schrammi, Drepanotrema depressissimum, D. lucidum, D. cimex, Physa cubensis, Lymnaea columella, Melania tuberculata, Idiopyrgus souleyetianus, Pomacea sp, Anodontites sp e Ancylidae não estavam infectados. Moluscos de 9 municípios estavam infectados com S. mansoni e de 11 com outros trematódeos.

Com o objetivo de caracterizar a situação epidemiológica da filariose linfática em Belém-PA foram analisados dados dos inquéritos hemoscópicos de 1951 a 2003. As informações do período de 1951 a 1994 foram coletadas de relatórios disponibilizados pela Fundação Nacional de Saúde. Os dados de 1995 a 2003 foram obtidos através de inquéritos realizados em 62 bairros, dos oito distritos administrativos da cidade. Observou-se uma queda apreciável ao longo dos anos nos índices de microfilarêmicos. As percentagens de parasitados nas décadas de 1950, 1960, 1970, 1980 e 1990, foram respectivamente: 8,2%, 2,6%, 0,7%, 0,16% e 0,02%. Em 2001, foi diagnosticado um único microfilarêmico, interrompendo uma série de dois anos sem registro de exames positivos na cidade. Em 2002 e 2003, inquéritos hemoscópicos e entomológicos foram realizados, simultaneamente, não sendo detectados indivíduos microfilarêmicos ou mosquitos infectados. Para manter essa tendência, medidas de vigilância devem ser observadas, a fim de detectar e tratar precocemente pacientes, para evitar o risco de ressurgimento dos focos, aparentemente já controlados.

We retrospectively analyzed a series of 151 cases of cutaneous leishmaniasis treated between 1967 and 1982. One-hundred-and-thirty-nine (92%) patients presented with active lesions and were treated with daily doses of meglumine antimoniate: 81 adults received a 5-ml vial IM and 58 children received 1 to 5ml. Forty-five (32.4%) patients underwent continuous treatment with meglumine antimoniate for 25 to 116 days without rest intervals, and 94 (67.6%) intermittent treatment with 2 to 5 series of meglumine antimoniate. Intermittent series could include schedules of daily IM applications for 10 to 25 days each and intervals varying from 10 to 60 days. Antimony dose was calculated for 66 (47.5%) patients and ranged from 3.9 to 28.7 Sb5+/kg/day. Of these, 35 patients received >10mg and 31 patients <10mg Sb5+/kg/day. Median time of healing was longer for lesions on the legs and feet - 67.5 days versus 48.7 days (p < 0.001) for other sites. However, there were no significant differences in the median time of healing between adults and children, intermittent and continuous regimens or high and low antimony doses. Fifty-one patients were reassessed 5 to 14 years after treatment and showed no evidence of disease. These results support further investigation (clinical trials) on treatment using low doses of antimony.

The efficacy of treatment with nifurtimox and/or benznidazole among adults with chronic Chagas disease with no previous electrocardiographic disturbances was evaluated over a mean follow-up of 21 years, by means of conventional serology, xenodiagnosis, clinical examination, electrocardiograms and chest X-ray. One hundred and eleven patients, between 17 and 46 years old, were studied: 54 underwent treatment (nifurtimox 27, benznidazole 27) and 57 remained untreated (control group). Xenodiagnosis was performed on 65% of them: 36/38 of the treated and 9/34 of the untreated patients had previous positive xenodiagnosis. Post-treatment, 133 xenodiagnoses were performed on 41 patients, all resulting negative. In the control group, 29 xenodiagnoses were performed on 14 patients; 2 resulted positive. Sera stored during the follow-up were simultaneously analyzed through conventional serology tests (IHA; DA-2ME; IIF). The serological evolution in the treated group was: a) 37% underwent negative seroconversion (nifurtimox 11, benznidazole 9); b) 27.8% decreased titers (nifurtimox 9, benznidazole 6), 9 showed inconclusive final serology (nifurtimox 7, benznidazole 2); c) 35.2% remained positive with constant titers (nifurtimox 7; benznidazole 12). The control group conserved the initial antibody levels during the follow-up. In the clinical evolution, 2/54 (3.7%) of the treated and 9/57 (15.8%) of the untreated patients showed electrocardiographic disturbances attributable to Chagas myocardiopathy, with a statistically relevant difference (p<0.05). Treatment caused deparasitation in at least 37% of the chronically infected adults and a protective effect on their clinical evolution.

All adults (n = 334) living in Brejo do Mutambal, an endemic area for cutaneous leishmaniasis, were included in this study. Contrary to our initial hypothesis, it was observed that men (23.7 ± 3.2 vs. 22.1 ± 2.6 kg/m²) and women (24.1 ± 4.7 vs. 22.5 ± 3.4 kg/m²) with cutaneous leishmaniasis presented higher body mass index than the controls.

INTRODUCTION: Human T cell lymphotropic virus types 1 and 2 (HTLV-1/2) are endemic in Brazil and are screened for in transfusion services since 1993. This study evaluated the evolution of the prevalence of HTLV-1 and 2 in blood donors of the Hemominas Foundation from 1993 to 2007, and its geographical distribution in State of Minas Gerais, Brazil. METHODS: The Hemominas Foundation is a centralized blood center in Minas Gerais, Brazil. The sources of data were the Hemominas Foundation Technical Bulletin and files from the centralized serological laboratory. Donors were tested in the period using enzyme linked immuno sorbent assays (ELISA), followed by Western blot, when repeatedly reactive. The data were analyzed by EPIINFO 6.2 and TABWIN 3.5 softwares. RESULTS: The average seroprevalence in the period 1993-2007 was 0.1%. A steady decline occurred from 0.4% in 1993 to below 0.1% in 2002 and later, with a transient peak of 0.5% in 1994. HTLV reactivity distribution was asymmetrical in the state, with regions of higher prevalence, interspersed with low prevalence areas. Comparison of positive and negative donors verified that increasing age was proportional to virus positivity. Odds ratio for age ranged from 1.43 (30 to 39 years-old) to 3.09 (50 to 65 years-old). Women had a greater chance of being positive (OR-1.64), as previously described. CONCLUSIONS: Possible explanations for HTLV-1/2 prevalence decline are the exclusion of positive donors from the donor pool, an increase in repeat donors and ELISA test improvement, with reduction in the number of false positive results.

INTRODUCTION: Leptospirosis is often mistaken for other acute febrile illnesses because of its nonspecific presentation. Bacteriologic, serologic, and molecular methods have several limitations for early diagnosis: technical complexity, low availability, low sensitivity in early disease, or high cost. This study aimed to validate a case definition, based on simple clinical and laboratory tests, that is intended for bedside diagnosis of leptospirosis among hospitalized patients. METHODS: Adult patients, admitted to two reference hospitals in Recife, Brazil, with a febrile illness of less than 21 days and with a clinical suspicion of leptospirosis, were included to test a case definition comprising ten clinical and laboratory criteria. Leptospirosis was confirmed or excluded by a composite reference standard (microscopic agglutination test, ELISA, and blood culture). Test properties were determined for each cutoff number of the criteria from the case definition. RESULTS: Ninety seven patients were included; 75 had confirmed leptospirosis and 22 did not. Mean number of criteria from the case definition that were fulfilled was 7.8±1.2 for confirmed leptospirosis and 5.9±1.5 for non-leptospirosis patients (p<0.0001). Best sensitivity (85.3%) and specificity (68.2%) combination was found with a cutoff of 7 or more criteria, reaching positive and negative predictive values of 90.1% and 57.7%, respectively; accuracy was 81.4%. CONCLUSIONS: The case definition, for a cutoff of at least 7 criteria, reached average sensitivity and specificity, but with a high positive predictive value. Its simplicity and low cost make it useful for rapid bedside leptospirosis diagnosis in Brazilian hospitalized patients with acute severe febrile disease.

INTRODUCTION: Reductions in the prevalence of hepatitis B virus (HBV) infection and carriage, decreases in liver cancer incidence, and changes in patterns of liver dysfunctions are described after hepatitis B vaccination. METHODS: We conducted a population-based seroprevalence study aimed at estimating the HBV prevalence and risk of infection in the rural area of Lábrea following nineteen years of HBV vaccination. RESULTS: Half of the subjects showed total anti-HBc of 52.1% (95% CI 49.6-54.7). The HBsAg prevalence was 6.2% (95% CI 5.1-7.6). Multivariate analysis showed an inverse association between HBV infection and vaccination (OR 0.62; 95% CI 0.44-0.87). HBsAg remained independently associated with past hepatitis (OR 2.44; 95% CI 1.52-3.89) and inversely to vaccination (OR 0.43; 95% CI 0.27-0.69). The prevalence of HBeAg among HBsAg-positive individuals was 20.4% (95% CI 12.8-30.1), with the positive subjects having a median age of 11 years (1-46) p=0.0003. CONCLUSIONS: We demonstrate that HBV infection is still an important public health issue and that HBV vaccination could have had better impact on HBV epidemiology. If we extrapolate these findings to other rural areas in the Brazilian Amazon, we can predict that the sources of chronic infected patients remain a challenge. Future studies are needed regarding clinical aspects, molecular epidemiology, surveillance of acute cases, and risk groups.

INTRODUCTION: Some viruses of the Herpesviridae family are frequently the etiologic agents of oral lesions associated with HIV. The aim of this study was to identify the presence of herpes simplex virus types 1 and 2 (HSV-1, HSV-2), Varicella Zoster virus (VZV), Epstein-Barr virus (EBV), human cytomegalovirus (HCMV), human herpesvirus type 6, type 7 and type 8 (HHV-6, HHV-7 and HHV-8) in the oral cavity of HIV-infected children/adolescents and verify the association between viral subtypes and clinical factors. METHODS: The cells of oral mucosa were collected from 50 HIV infected children/adolescents, 3-13 years old (mean age 8.66). The majority (66%) of selected were girls, and they were all outpatients at the pediatric AIDS clinic of a public hospital in Rio de Janeiro. Nested-PCR was used to identify the viral types. RESULTS: Absence of immunosuppression was observed in 66% of the children. Highly active antiretroviral therapy (HAART) was used by 72.1% of selected and moderate viral load was observed in 56% of the children/adolescents. Viral types were found in 86% of the children and the subtypes were: HSV-1 (4%), HSV-2 (2%), VZV (4%), EBV (0%), HCMV (24%), HHV6 (18%), HHV-7 (68%), HHV8 (0%). CONCLUSIONS: The use of HAART has helped to reduce oral lesions, especially with herpes virus infections. The health professionals who work with these patients should be aware of such lesions because of their predictive value and the herpes virus can be found circulating in the oral cavity without causing lesions.

Abstract: INTRODUCTION : This study evaluated the clinical forms and manifestation severities of Chagas disease among serologically reactive individuals from Western Rio Grande do Norte (Northeastern Brazil). METHODS : This cross-sectional study included 186 adults who were evaluated using electrocardiography, echocardiography, chest radiography, and contrast radiography of the esophagus and colon. A clinical-epidemiological questionnaire was also used. RESULTS : The indeterminate, cardiac, digestive, and cardiodigestive clinical forms of Chagas disease were diagnosed in 51.6% (96/186), 32.2% (60/186), 8.1% (15/186) and 8.1% (15/186) of the participants, respectively. Heart failure (functional classes I-IV) was detected in 7.5% (14/186) of the participants, and 36.4% (24/66), 30.3% (20/66), 15.2% (10/66), 13.6% (9/66), and 4.5% (3/66) of the patients were at stage A, B1, B2, C, and D, respectively. Dilated cardiomyopathy and electrocardiographic changes were detected in 10.2% (19/186) and 48.1% (91/186) of the participants, respectively. Apical aneurysm was diagnosed in 10.8% (20/186) of the participants, and other changes in the segmental myocardial contractility of the left ventricle were diagnosed in 33.9% (63/186) of the participants. Megaesophagus (groups I-IV) was observed in 7% (13/186) of the participants, megacolon (grades 1-3) was detected in12.9% (24/186) of the participants, and both organs were affected in 29.2% (7/24) of the megacolon cases. CONCLUSIONS : We detected various clinical forms of Chagas disease (including the digestive form). Our findings indicate that clinical symptoms alone may not be sufficient to exclude or confirm cardiac and/or digestive damage, and the number of patients with symptomatic clinical forms may be underestimated.

Pyranojacareubin; 1,5-dihydroxy-6',6'-dimethyl-2H-pyran(2',3':6,7) -6",6"-dim-ethyI-2H,4H-pyran(2",3":2,3)xanthone and a new xanthone l,6-dihydroxy-5-methoxy-6',6'-dim-ethyl-2H-pyran(2',3':3,2)-7-(3,3-dimethylprop-2-enyl)xanthone were isolated from the ether extract of the root bark of Rheedia acuminata together with friedelin and friedelanol.

Relaciona-se áreas de interesse biológico na Amazônia visando planejamento em Conservação, a saber: 1. Pará-leste, 2. Ilha Mexiana, 3. Amapá-sul, 4. Monte Tipac, 5. Óbidos/ Alenquer/Santarém, 6. Parintins, 7. Serra de Tepequem, 8. Amazonas-central, 9. Alto Rio Negro, 10. Solimões-oeste, 11. Acre/Amazonas, 12. Rondônia/Amazonas. Estas áreas foram selecionadas com base na variação taxonômica e na distribuição geográfica de 33 espécies de lianas do género Bauhinia(secções Schnellae Caulotretus)ocorrentes no Brasil. Após consulta a coleções herborizadas, as localidades de ocorrência de cada espécie foram mapeadas e analizadas, revelando-se os seguintes padrões de distribuição: extra-amazônico (9 espécies), ampla distribuição intra e extra-amazônica (3 espécies), e exclusivamente intra-amazônico (21 espécies). As ocorrências intra-amazônicas foram analizadas em detalhe, apontando-se endemismos e disjunções importantes. Em seguida é dada a ocorrência das 24 espécies da Amazónia por setor fitogeográfico. São apresentados mapas de distribuição de 23 espécies da seção Caulotretus.Apresenta-se um mapa com as 12 áreas de interesse biológico apontando-se justificativas para cada área indicada.
