We present a case of a 4.5-month-old boy from Turkey with hemophagocytic lymphohistiocytosis (HLH) associated with H1N1 virus and Leishmania spp. coinfection. Because visceral leishmaniasis can mimic hematologic disorders like HLH, it is important to rule out this clinical condition before starting immunosuppressive therapy. In our case, treatment with liposomal amphotericin B resulted in a dramatic resolution of clinical and laboratory abnormalities.

Introduction. The genera Enterococcus, Staphylococcus and Streptococcus are recognized as important Gram-positive human pathogens. The aim of this study was to evaluate the performance of Vitek 2 in identifying Gram-positive cocci and their antimicrobial susceptibilities. Methods. One hundred four isolates were analyzed to determine the accuracy of the automated system for identifying the bacteria and their susceptibility to oxacillin and vancomycin. Results. The system correctly identified 77.9% and 97.1% of the isolates at the species and genus levels, respectively. Additionally, 81.8% of the Vitek 2 results agreed with the known antimicrobial susceptibility profiles. Conclusion. Vitek 2 correctly identified the commonly isolated strains; however, the limitations of the method may lead to ambiguous findings.

IntroductionAntiretroviral therapy (ART) has been used to treat large numbers of patients living with human immunodeficiency virus (HIV) infection. Lipid disorders are often observed in these patients, and include elevations in total cholesterol (TC) and triglycerides (TG).MethodsA cross-sectional study was performed using 333 patient records from the Regional Hospital of São José Doutor Homero de Miranda Gomes (HRSJHMG). The study population consisted of patients with HIV who were under medical follow up, either on or off drug treatment. The data were entered into Excel and exported to SPSS 16.0 for analysis using chi-square testing. We used prevalence ratios as the measure of association.ResultsLipid abnormalities were observed in 78.9% of individuals who received ART. Of the 308 subjects on ART, 59.1%, 41.9%, and 33.1% had TG, TC and low-density lipoprotein (LDL) abnormalities, respectively. The prevalence of LDL changes was 2.57-fold higher in individuals who had been using ART for more than 12 months, compared to those using ART for 6 to 12 months.ConclusionsHIV patients showed a significant increase in the association between TC and TG levels and the use of ART. In particular, changes in TC, LDL and TG were greater in individuals who had received ART for over more than 12 months.

Foreign body aspiration (FBA) is one of leading causes of death in children, especially among those younger than 3 years of age. The inhalation of a foreign body may cause a wide variety of symptoms, and early diagnosis is highly associated with the successful removal of the inhaled foreign material. Despite the great advances in endoscopic procedures and anesthesia, a large number of difficulties and complications still result from foreign body aspiration. We describe 5 cases of serious acute complications following aspiration of foreign bodies that became lodged in the tracheobronchial tree, including pneumomediastinum, pneumothorax, total atelectasis, foreign body dislodgment, and need for thoracotomy in children admitted into our intensive care unit in 1999 and 2000; these were all situations that could have been prevented with early recognition and prompt therapeutic intervention.

The aim of this report is to describe the anatomic-pathologic findings from necropsies of 5 drug addicts with titanium pigment in several organs after chronic intravenous injection of crushed propoxyphene hydrochloride tablets. Samples from liver, spleen, lungs, lymph nodes, and bone marrow were obtained, and after being grossly studied, they were submitted to evaluation using common light and polarized microscopy. In all 5 cases, a pigment with characteristics of titanium dioxide was found within tissue samples of the organs studied. Our findings suggest that research concerning titanium pigment within body tissues should be enhanced, considering the potential contribution of this morphologic data to forensic pathology.

OBJETIVO: Avaliar o efeito sublingual do mononitrato-5 de isossorbida (MN5IS) e nitroglicerina (NTG) sobre o diâmetro luminal de artérias coronárias epicárdicas, pressão arterial média e efeitos colaterais. MÉTODOS: Cinqüenta pacientes foram submetidos a cateterismo cardíaco e cinecoronariografia, na condição inicial e 5min após administração sublingual de MN5IS grupo A (GA) ou NTG grupo B (GB). RESULTADOS: O diâmetro coronário de referência aumentou em ambos os grupos, sem significância estatística entre os mesmos. Nos GA e GB foram demonstrados uma diminuição (1,66mmHg) e um aumento (0,79mmHg) na pressão arterial média, respectivamente (p=0,123). Não foram observados efeitos colaterais com o uso destas drogas. CONCLUSÃO: MN5IS sublingual é uma alternativa à administração de NTG durante cinecoronariografia e representa uma alternativa terapêutica para o tratamento de doença cardíaca isquêmica.

OBJETIVO: Avaliar os efeitos da redução de peso superior a 5% nos perfis hemodinâmico, metabólico e neuroendócrino de obesos grau I. MÉTODOS: Estudo observacional com 47 obesos grau I, média de idade de 33 anos, submetidos a orientação mensal quanto a dieta, exercício físico e comportamento alimentar, durante quatro meses. A pressão arterial, pelo método auscultatório, e a freqüência cardíaca, pelo método palpatório, foram avaliadas mensalmente, enquanto as seguintes variáveis (e respectivos métodos) foram medidas no início e final do estudo: colesterol total, triglicerídeos, HDL-colesterol (enzimático), LDL-colesterol (fórmula de Friedwald), glicemia (enzimático hexoquinase), leptina, adiponectina, renina, aldosterona, insulina (radioimunoensaio) e índice de resistência à insulina (HOMA). RESULTADOS: Observamos, após ajuste para outras variáveis, reduções significativas de 6 mmHg na pressão arterial diastólica, 7 pg/ml na renina, 13 mg/dl no colesterol total e 12 mg/dl no LDL-colesterol, no grupo com redução de peso superior a 5%. Notamos, também nesse grupo, tendência ao aumento de maior magnitude da adiponectina ao final do estudo, bem como diminuição três vezes maior dos níveis de glicemia, insulina e HOMA, e seis vezes maior da leptina. CONCLUSÃO: Medidas não-farmacológicas capazes de promover redução de peso superior a 5% produzem efeitos hemodinâmicos, metabólicos e neuroendócrinos que melhoram o risco cardiovascular de obesos.
