The aim of the present study was to investigate the detection percentage of tuberculosis among patients that are respiratory symptomatic (TB suspects). In this work, we present the preliminary results of research carried out at "Hospital das Clínicas de Teresópolis Costantino Ottaviano da Fundação Educacional Serra dos Órgãos (FESO)" from November 2003 to April 2004. Among the 40 respiratory symptomatic individuals identified and referred to the Tuberculosis Control Program in Teresópolis, two (5.0%) were characterized as smear-positive. These results confirm reports in the literature and underscore the need for and importance of this strategy.

A retrospective study was conducted in nine patients with rabies admitted to a hospital of Fortaleza, Brazil. Autopsy was performed in all cases. The ages ranged from three to 81 years and six were males. They all were bitten by dogs. The time between the accident and the hospital admission ranged from 20 to 120 days (mean 45 ± 34 days). The time until death ranged from one to nine days (mean 3.3 ± 5.5 days). The signs and symptoms presented were fever, hydrophobia, aerophobia, agitation, disorientation, dyspnea, sialorrhea, vomiting, oliguria, sore throat, pain and hypoesthesia in the site of the bite, headache, syncope, cough, hematemesis, mydriasis, hematuria, constipation, cervical pain and priapism. In three out of six patients, there was evidence of acute renal failure, defined as serum creatinine > 1.4 mg/dL. The post-mortem findings in the kidneys were mild to moderate glomerular congestion and mild to intense peritubular capillary congestion. Acute tubular necrosis was seen in only two cases. This study shows some evidence of renal involvement in rabies. Histopathologic findings are nonspecific, so hemodynamic instability, caused by autonomic dysfunction, hydrophobia and dehydration must be responsible for acute renal failure in rabies.

Criança de sete anos recebeu ceftriaxona para o tratamento de meningite, evoluindo com dor em hipocôndrio direito associada a cálculo na vesícula biliar. Após três meses, a ultrassonografia abdominal foi normal. O conhecimento de que a ceftriaxona pode levar ao surgimento de colelitíase pode evitar intervenções cirúrgicas desnecessárias.

Com o objetivo de avaliar a situação epidemiológica da infecção chagásica no Estado do Piauí e sua relação com idade, gênero, transfusão de sangue e aborto espontâneo, foi realizado, de agosto a dezembro de 2002, um inquérito sorológico com o exame de uma amostra aleatória simples de 36.399 moradores da área rural. A infecção chagásica foi definida pelo teste de imunofluorescência indireta para anticorpos antiTrypanosoma cruzi em amostra de sangue coletada em papel de filtro. A soroprevalência total foi de 1,9%, variando de 0,1% em menores de 5 anos a 6,6% em maiores de 79 anos; foi significativamente maior nas mulheres (2,1%), analfabetos (4,1%), receptores de sangue (3,3%) e nas mulheres com abortamento espontâneo (5,4%). Esses dados ao serem comparados com os obtidos durante o inquérito sorológico nacional (1975-1980) mostraram significativa queda da soroprevalência da infecção chagásica no Estado do Piauí (4% para 1,9%), indicando a eficácia das medidas de controle vetorial implementadas no período 1975-2002.

A Adultrap é uma nova armadilha feita para capturar fêmeas de Aedes aegypti. Foram realizados testes para avaliar sua especificidade tendo como referência a técnica da aspiração da espécie em abrigos artificiais. A Adultrap ficou exposta por 24 horas no intradomicílio e peridomicílio de 120 casas sorteadas em dois bairros da Cidade de Foz do Iguaçu, Estado do Paraná. O teste estatístico foi o modelo log-linear de Poisson. O resultado foi a captura de 726 mosquitos Culicidae, dos quais 80 eram Aedes aegypti. A Adultrap capturou apenas fêmeas desta espécie, enquanto o aspirador os dois sexos de Aedes aegypti e mais cinco outras espécies. A Adultrap capturou Aedes aegypti dentro e fora das casas, mas a análise indicou que no peridomicílio a armadilha capturou significantemente mais fêmeas do que a aspiração. Também, ficou evidenciada a sensibilidade da Adultrap para detectar Aedes aegypti em situação de baixa freqüência.

Este estudo avaliou a ocorrência de genes para metalo β-lactamases em isolados clínicos de Pseudomonas aeruginosa do Hospital São Vicente de Paulo, RS. Os genes foram pesquisados por PCR e o perfil de susceptibilidade foi avaliado por disco-difusão. Foram analisadas 46 cepas, sendo que cinco apresentaram o gene blaSPM-1.

O objetivo deste estudo foi comparar amostras de efluente do Hospital São Vicente de Paulo com amostras de água do Rio Passo Fundo, quanto ao perfil de susceptibilidade de isolados de Pseudomonas aeruginosa, para inferir sobre a presença de isolados de origem hospitalar em amostras de água superficial. A significância estatística entre os perfis de susceptibilidade das amostras foi testada por análise de variância e a comparação das amostras foi feita por contrastes de interesse. Foram identificados 198 isolados de Pseudomonas aeruginosa a partir das amostras analisadas. O fenótipo de multirresistência não foi observado nas amostras do Rio Passo Fundo, embora alguns isolados resistentes a carbapenêmicos tenham sido identificados, indicando a presença de contaminação com bactérias provenientes de um ambiente sob forte pressão seletiva. Diferenças significativas entre as amostras de água e efluente hospitalar foram observadas a partir da análise de variância por contrastes de interesse.

Apresentamos neste trabalho os primeiros relatos de acidente escorpiônico causado pela espécie Tityus pusillus (Buthidae). Os acidentes ocorreram no ambiente doméstico, na área rural situada nas cidades de Paudalho e São Lourenço da Mata, Pernambuco, Brasil. Os dois casos descritos, uma criança e uma mulher grávida, foram classificados como leve e moderado, respectivamente. Os sintomas clínicos apresentados foram distúrbios locais (dor e parestesia) e distúrbios sistêmicos (calafrios, tontura, cefaléia e vômito). Esses registros permitem incluir Tityus pusillus como uma espécie de importância médica no Brasil.

Anticorpos antiTrypanosoma cruzi no cordão umbilical de 351 parturientes da Cidade de Pelotas, RS foram pesquisados a fim de investigar a prevalência da doença de Chagas em gestantes. Um (0,3%) caso foi identificado, não sendo detectada transmissão congênita. Salienta-se a importância da investigação da doença de Chagas em gestantes de zonas endêmicas ou provenientes destas.

INTRODUCTION: Malaria during pregnancy remains a serious public health problem. The aim of this study was to establish the prevalence and possible risk factors for malaria in pregnant women attending antenatal care at Augusto Ngangula Specialized General Hospital in Luanda, Angola. METHODS: Pregnant women (679 total) who attended antenatal care from April to September 2008 were included in the study after signing informed consent. For each participant, the social-demographic profile and malaria and obstetric histories were investigated via a questionnaire. Diagnosis was made by optic microscopy, and hemoglobin concentration measured. The associations between age, parity, gestational age, residence, schooling, malaria during gravity, anemia and treatment with incidence of Plasmodium falciparum infection were analyzed through logistic regression. RESULTS: During the period of study, 74 (10.9%) out of 679 women were infected by P. falciparum. The average concentration of hemoglobin was 11.1 ± 0.07g/dL, and there were significant associations between the history of malaria during pregnancy, P. falciparum infection (p<0.01) and anemia at the time of observation (p<0.001). CONCLUSIONS: Previous history of malaria during pregnancy represents a risk factor for current infection and anemia was an important complication associated with malaria, even in women who were treated with sulfadoxine-pyrimethamine during pregnancy.

INTRODUCTION: Leprosy in Brazil is a public health issue, and there are many regions in the State of Espírito Santo with high endemic incidence levels of leprosy, characterizing this state as a priority for leprosy programs. The aim of this study was to determine the spatial distribution of coefficients of new cases of leprosy in the State of Espírito Santo, Brazil. METHODS: We conducted a descriptive and ecologic study based on the spatial distribution of leprosy in the State of Espírito Santo between 2004 and 2009. Data were gathered from the available records of the Espírito Santo State Health Secretary. The global and local Bayesian empirical methods were used to produce an estimate of leprosy risk, smoothing the fluctuation effects of the detection coefficients. RESULTS: The study resulted in a coefficient adjustment of new cases in 10 towns that changed their classification, among which, 2 went from low to medium, 4 from medium to high, 3 from high to very high, and 1 from very high to hyper-endemic. An average variation of 1.02, fluctuating between 0 and 12.39 cases/100,000 inhabitants, was found in a comparative calculation between the Local Ebest value and the average coefficient of new leprosy cases in the State of Espírito Santo. CONCLUSIONS: The spatial analysis of leprosy favors the establishment of control strategies with a better cost-benefit relationship since it reveals specific and priority regions, thereby enabling the development of actions that can interfere in the transmission chain.

INTRODUCTION: We report the first find of Lutzomyia longipalpis in the State of Paraná, Brazil. METHODS: The specimens were captured in the urban area of the municipality of Foz do Iguaçu, with Falcão light traps, in domiciliary and peridomiciliary areas of 61 properties, on two consecutive nights from 18:00 to 06:00hs in March 2012. RESULTS: We captured 40 specimens of Lu. longipalpis and 54 specimens of other sandfly species. CONCLUSIONS: This find expands knowledge of the geographical distribution of this sandfly in Brazil.

INTRODUCTION: Human immunodeficiency virus type 1 (HIV-1) has spread worldwide, with several subtypes and circulating recombinant forms. Brazil has an incidence of 20.5 HIV-1/acquired immunodeficiency syndrome (AIDS) patients per 100,000 inhabitants; however, the Southernmost State of Rio Grande do Sul (RS) has more than twice the number of HIV-1-infected people (41.3/100,000 inhabitants) and a different pattern of subtype frequencies, as previously reported in studies conducted in the capital (Porto Alegre) and its metropolitan region. This study examined HIV-1/AIDS epidemiological and molecular aspects in the countryside of Rio Grande do Sul. METHODS: Socio-demographic, clinical and risk behavioral characteristics were obtained from HIV-1-positive adult patients using a structured questionnaire. HIV-1 subtypes were determined by nested-polymerase chain reaction (PCR) and sequencing of the pol and env genes. RESULTS: The study sample included 149 (55% women) patients with a mean age of 41.8 ± 11.9 years. Most (73.8%) patients had a low education level and reported heterosexual practices as the most (91.9%) probable transmission route. HIV-1 subtypes were detected in 26 patients: 18 (69.2%) infected with subtype C, six (23.1%) infected with subtype B and two (7.7%) infected with BC recombinant forms. CONCLUSIONS: These data highlight the increasing number of HIV-1 subtype C infections in the countryside of South Brazil.

AbstractDespite its infrequent occurrence, testicular schistosomiasis forming pseudo-tumors can be considered in the differential diagnosis of testicular tumors, especially in areas where the parasitic disease is endemic. In this report, we present a case of testicular schistosomiasis caused by Schistosoma mansoni and mimicking a testicular neoplasm. We describe the patterns of a testicular nodule on ultrasonography and magnetic resonance images in a 46-year-old man. The nodule was removed after a pre-operative diagnosis of a non-malignant lesion. Histology demonstrated granulomas with epithelioid macrophages and eosinophils around S. mansoni eggs within a fibrous tissue that formed a nodular structure.
