The indeterminate form of Chagas' disease is characterized by positive serology for the disease in the absence of clinical findings and in the presence of both normal esophagogram and electrocardiogram. When more sensitive methods were used, abnormalities have been described either in the esophagus or in the heart. The authors have studied simultaneously the esophagus and the heart in the same subjects. In thirteen adults with diagnosis of indeterminate form and nine adult controls, the esophageal manometry both in basal conditions and after stimulus (bethanecol) and vectorcardiogram were performed. In the control group none of the subjects presented concomitant esophageal and cardiac alterations while in the chagasic group 92,3% of the patients presented results simultaneously altered. It is concluded that the studied patients showed indications of parasympathetic denervation manifested by simultaneously esophageal and heart alterations.

Serum samples were obtained from 154 individuals infected with Entamoeba histolytica (78 symptomatic and 76 asymptomatic). Twelve had trophozoites in the feces whereas 142 had only cysts. The sera were used to test the existence of antibodies anti-Entamoeba histolytica employing the Indirect Hemagglutination (IHA), Indirect Immunofluoresccnce (IFAT), Complement Fixation Reaction (CFR) and Counterimmunoelectrophoresis (CIEP). For those individuals with trophozoites in their feces, 75.0 were positive by IHA and IFAT, 83.0 by CFR and 41.7 by CIEP. In individuals who had only cysts, positive results by the same tests were respectively, 5.6%, 12.0%, 19.0% and 5.6%. The difference in relation to the tilers of antibodies detected through IHA, IFAT, CFR and CIEP and in relation to the presence of trophozoites or cysts in the feces was significative for four immunological reactions when X², was employed (P < 0.05).

The isoenzyme profiles (IP) of 33 strains of Entamoeba histolytica isolated from patients and carriers of two regions in Brazil (Amazonia and Southeast) were determined. The enzymes phosphoglucomutase, glucose-phosphate isomerase, hexokinase and malic enzyme were considered. IP of the strains was correlated with culture conditions, time of maintenance in laboratory and clinical history of patients. The strains were maintained under polyxenic, monoxenic and axenic culture conditions: 27 polyxenic, 1 polyxenic and monoxenic, 1 polyxenic, monoxenic and axenic and 4 axenic only. The patients were symptomatic and asymptomatic. The symptomatic patients presented either non dysenteric (NDC) or dysenteric colitis (DC), associated or not with hepatic abscess (HA). One patient presented anal amoeboma (AM). The analysis of IP for isolates maintained in polyxenic culture showed non pathogenic IP (I) for strains from carriers and patients with NDC, while the strains isolated from patients presenting DC, HA and AM resulted in isolates II or XIX pathogenic IP. This parameter was not able to differentiate strains from carriers from symptomatic patients when these strains were found in axenic or monoxenic culture. All these strains displayed pathogenic IP (II), demonstrating the inability of this parameter to classifying for virulence since it showed identical IP for strains isolated from carriers or symptomatic patients.

A two year-old female child was admitted at the Pediatric Intensive Care Unit in a septic shock associated with a lymphoproliferative syndrome, with history of fever, adynamia and weight loss during the last two months. On admission, the main clinical and laboratory manifestations were: pallor, jaundice, disseminated enlarged lymph nodes, hepatosplenomegaly, crusted warts on face, anemia, eosinophilia, thrombocytopenia, increased direct and indirect bilirubin, alkaline phosphatase, and gammaglutamyl transpeptidase. A parenteral administration of fluids, dobutamine and mechanical ventilation was started, without improvement of the clinical conditions. A direct examination of exsudate collected from cervical lymph node revealed numerous oval-to-around cells with multiple budding, like a "pilot wheel" cell, suggesting Paracoccidioides brasiliensis. Even though treatment with intravenous sulfamethoxazole-trimethoprine was soon started, the child died 36 hours after hospital admission. Disseminated paracoccidioidomycosis was confirmed in the autopsy. This is the youngest case of paracoccidioidomycosis in children reported in the literature.

Helicobacter pylori is a bacterium recognized as the major cause of peptic ulcer and chronic gastritis. Recently, a proteome-based approach was developed to investigate pathogenic factors related to H. pylori. In this preliminary study, H. pylori strains were isolated from gastric biopsies of patients with chronic gastritis and duodenal ulcers. A partial proteomic analysis of H. pylori strains was performed by bacterial lyses and proteins were separated by two-dimensional gel electrophoresis (2-DE). A comparative analysis was performed to verify a differential protein expression between these two 2-DE maps. These data should be useful to clarify the role of different proteins related to bacterial pathogenesis. This study will be completed using a larger number of samples and protein identification of H. pylori by MALDI-TOF mass spectrometry.

Two xenic isolates and cloned cultures of Entamoeba dispar were submitted to monoxenization using Crithidia fasciculata as the associated organism. Growth in monoxenic cultivation and ability of xenic and monoxenic trophozoites to destroy VERO cells and produce lesions in hamster livers were compared to those of a virulent E. histolytica. Parental and cloned E. dispar under monoxenic cultivation showed a remarkable lower growth than the monoxenic E. histolytica and were avirulent in both in vivo and in vitro tests. When xenically cultured, trophozoites of E. dispar showed a moderate lytic activity against VERO cells (1.5 to 41.8% of destruction) but caused severe hepatic lesions in hamsters as those caused by the virulent E. histolytica (29 to 100% in prevalence and 0.86 to 4.00 in lesion degree). Although E. dispar has not been associated with invasive disease in men, the ability of xenic trophozoites to produce prominent tissue damage in experimental conditions has indicated that some strains have a considerable pathogenic potential when in presence of bacteria.

The outbreaks of rabies in humans transmitted by Desmodus rotundus in 2004 and 2005, in the northeast of the Brazilian State of Para, eastern Amazon basin, made this a priority area for studies on this zoonosis. Given this, the present study provides data on this phenomenon in an urban context, in order to assess the possible circulation of the classic rabies virus (RABV) among bat species in Capanema, a town in the Amazon basin. Bats were collected, in 2011, with mist nets during the wet and dry seasons. Samples of brain tissue and blood were collected for virological and serological survey, respectively. None of the 153 brain tissue samples analyzed tested positive for RABV infection, but 50.34% (95% CI: 45.67-55.01%) of the serum samples analyzed were seropositive. Artibeus planirostris was the most common species, with a high percentage of seropositive individuals (52.46%, 95% CI: 52.31 52.60%). Statistically, equal proportions of seropositive results were obtained in the rainy and dry seasons (c2 = 0.057, d.f. = 1, p = 0.88). Significantly higher proportions of males (55.96%, 95% CI: 48.96-62.96%) and adults (52.37%, 95% CI: 47.35-57.39%) were seropositive. While none of the brain tissue samples tested positive for infection, the high proportion of seropositive specimens indicates that RABV may be widespread in this urban area.

O autor apresenta a comparação de dois extratos de Toxocara canis e um de Ascaris suum do ponto de vista antigênico. O método usado foi a imunodifusão em gel segundo Ouchtehlöny. O extrato de T. canis obtido pela técnica de Jeska mostrou 7 faixas de precipitação contra o anti-soro homólogo. O extrato de A. suum obtido pela mesma técnica apenas revelou 3 faixas, uma das quais com identidade parcial com o antígeno anterior. O extrato de T. canis segundo Woodruff e Thacker revelou 3 faixas de precipitação contra o anti-soro homólogo, todas elas com identidade total com o outro extrato do mesmo parasita.

O autor apresenta um estudo sobre a composição antigênica do Cysticercus cellulosae empregando as técnicas de gel-difusão e imunoeletroforese; através da gel-difusão evidenciaram-se 5 faixas de precipitação e, na imunoeletroforese, 9 sistemas de precipitação. Salienta, ainda, a importância de novos estudos a fim de melhor definir a composição antigênica do parasita.

To study the influence of host age, inoculum size, and route of infection on Trypanosoma (Herpetosoma) rangeli, 12 lots of 6.0 g albino mice (NMRI strain) were infected up. with from 25x10¹ to 25x10(6) trypomastigotes/gram body weight harvested from LIT medium. The lower inocula produced low but persistent parasitemias, while the higher inocula produced high levels of parasitemia that fell quickly, suggesting the mobilization of resistance mechanisms. In other experiments, i.p. inoculation produced higher parasitemias than s.c. inoculation, and 6.0 g mice had higher parasitemias than 16.0 or 26.0 g mice. Thus, a standard methodology would seem to be necessary in the study of the various strains and/or species that may make up the T. rangeli complex.

Caso fatal de balantidíase em mulher desnutrida de 63 anos, criadora de porcos, da zona rural de Uberaba. A doença evoluiu em oito dias com disenteria, náusea e vômitos, culminando em óbito por enterorragia. A necropsia constatou-se colite ulcerada causada por B. coli, facilmente identificado ao exame histológico do intestino grosso.

Rotavírus foram pesquisados em 607amostrasfecais de crianças de até 6 anos de idade com quadros de diarréia aguda, no período de maio de 1986 a abril de 1990. Foi utilizada a técnica de eletroforese em gel depoliacrilamida (PAGE), sendo os rotavírus detectados em 123 amostras (20,27%) das quais 107(87,00%) apresentaram perfil eletroforético longo, compatível com o subgrupo II. Os rotavírus não foram encontrados no grupo controle constituído de crianças sadias, sendo porém detectados em 7,80% das crianças internadas por outras causas que não diarréia aguda. A maioria das crianças positivas para rotavírus encontrava-se na faixa etária de 6a24 meses (73,98%). A média depositividade nos meses chuvosos (outubro a abril) foi igual a 9,60% e no período seco, 34,48% com picos que variaram entre 53,17 e 73,27% nos meses de junho e julho, os mais frios do ano.

Relata-se caso da associação paracoccidioidomicose - infecção pelo vírus da imunodeficiência humana (HIV) em homem de 43 anos, em que se descreve, pela primeira vez, de modo detalhado, o quadro anatomopatológico da referida associação. Discutem-se a baixa freqüência desta associação, o comportamento, evolução e tratamento da paracoccidioidomicose em pacientes HIV positivos e chama-se a atenção para a importância da associação da infecção pelo HIV com as doenças tropicais mais comuns em nosso meio.
