A case of meningitis due to Staphylococcus warneri in a patient with a hyperinfection with Strongyloides stercoralis possibly associated with rituximab treatment for mantel cell lymphoma is reported for the first time in the literature. The patient was a 59-year-old woman, with a 3-year history of an apparently well controlled lymphoma after treatment with chemotherapy-immunotherapy and then immunotherapy alone, and diagnosis of strongyloidiasis. Meningitis was diagnosed by cerebrospinal fluid culture and tested with an automated plate system. The patient was successfully treated with vancomycin; although fever and productive cough persisted. Severe gastrointestinal symptoms and pneumonia developed three weeks later. Hyperinfection syndrome by S. stercoralis was diagnosed, with abundant larvae in feces and expectoration.

The first report to our knowledge, of hyperinfection by Strongyloides stercoralis (HS) and hypereosinophilia, associated to immune suppression by Rituximab (the only drug received for the last one year and 10 months), in a patient with mantle-cell lymphoma (MCL), is presented. The patient has a 3-year history of MCL, and developed two accesses of HS during 2008, including meningitis, pneumonia and presence of larvae of S. stercoralis in the lungs. We had a unique chance to look at cytotoxicity of filariform larvae in the expectoration after Ivermectin treatment, showing immobilization and death of larvae, associated with eosinophils attached to the cuticle of the parasite.

The present study aimed at measuring seropositivities for infection by Ascaris suum and Toxocara canis using the excretory/secretory (E/S) antigens from Ascaris suum (AES) and Toxocara canis (TES) within an indigenous population. In addition, quantification of cytokine expressions in peripheral blood cells was determined. A total of 50 Warao indigenous were included; of which 43 were adults and seven children. In adults, 44.1% were seropositive for both parasites; whereas children had only seropositivity to one or the other helminth. For ascariosis, the percentage of AES seropositivity in adults and children was high; 23.3% and 57.1%, respectively. While that for toxocariosis, the percentage of TES seropositivity in adults and children was low; 9.3% and 14.3%, respectively. The percentage of seronegativity was comparable for AES and TES antigens in adults (27.9%) and children (28.6%). When positive sera were analyzed by Western blotting technique using AES antigens; three bands of 97.2, 193.6 and 200.2 kDas were mostly recognized. When the TES antigens were used, nine major bands were mostly identified; 47.4, 52.2, 84.9, 98.2, 119.1, 131.3, 175.6, 184.4 and 193.6 kDas. Stool examinations showed that Blastocystis hominis, Hymenolepis nana and Entamoeba coli were the most commonly observed intestinal parasites. Quantification of cytokines IFN-γ, IL-2, IL-6, TGF-β, TNF-α, IL-10 and IL-4 expressions showed that there was only a significant increased expression of IL-4 in indigenous with TES seropositivity (p < 0.002). Ascaris and Toxocara seropositivity was prevalent among Warao indigenous.

SUMMARYResearch on Paracoccidioides brasiliensis has centered in the yeast cell probably because of the lack of distinctive features in the mycelium. In 1942 and for the first time, lateral conidia were noticed in the fungus' hyphae. Later on, Brazilian, Venezuelan and Argentinean researchers described "aleurias" when the fungus was grown in natural substrates. In 1970 authors became interested in the conidia and were able to obtain them in large numbers and treat them as individual units. Their shape and size were defined and the presence of all the elements of a competent eukaryotic cell were demonstrated. Conidia exhibited thermal dimorphism and, additionally, when given intranasally to BALB/c male mice, they converted into yeasts in the lungs and produce progressive pulmonary lesions with further dissemination to other organs. Studies on the phagocyte-conidia interaction were revealing and showed that these versatile structures allow a better understanding of the host- P. brasiliensisinteractions.

Dos trinta e dois macacos capturados no interior do Estado de São Paulo e mantidos em laboratório em gaiolas individuais (24 a 25 C° e 70% de umidificação) após vários xenodiagnósticos negativos, 12 foram infectados por via intraperitoneal com diferentes cepas de Trypanosoma cruzi, cujas formas tripomastigotas injetadas variaram de 1.10(5) a 5.10(6) Os 20 macacos restantes foram mantidos como controle. No período de 1 a 6 anos tanto os animais inoculados como os não inoculados, foram submetidos axenodiagnóstico e teste soro lógico de aglutinação direta, exame clínico e o eletrocardiograma. Posteriormente os macacos foram necropsiados e todos os órgãos submetidos a exame macro e microscópico. O exame clínico e o eletrocardiograma não revelaram alterações. Dos 12 macacos infectados, 4 apresentaram evidências de infecção ao exame histopatológico: utn com formas amastigotas nos tecidos e 3 com miocardite crônica de grau leve. A parasitemiafoi comprovada em 66,66% dos animais na fase aguda e a sorologia em 91,66% na fase crônica. Concluiu-se que os macacos Cebus não expressaram susceptibilidade ao desenvolvimento das lesões que caracterizam a fase . crônica da doença de Chagas mas poderiam ser usados para manter as cepas de T. cruzi e estudos de pesquisa sorológico a longo termo.

Con el propósito de identificar a reservorios del Trypanosoma cruzi se investigaron 60 mamíferos en los Departamentos Capital y San Luis del Palmar. Se examinaron: primates, roedores, marsupiales, carnívoros y edentados; 40 vivían en cautiverio y 20 fueron capturados mediante trampas en una comunidad rural forestal. Los mamíferos fueron analizados por xenodiagnóstico, empleándose ninfas de 3o o 4o estadío de Triatoma infestans ayunadas durante 2 semanas. Las heces de los triatominos fueron observadas al microscopio (400x) a los 30, 60 y 90 días post-alimentación. En 2 Saimiri sciureus y en 1 Cebus apella se constató infección por tripanosomas cruziformes. Se concluye que la parasitemia detectada fue baja. La presencia de Didelphis albiventris, reservorio potencial del Trypanosoma cruzi , en una zona de transmisión activa del parásito representa un factor de riesgo, por lo que son necesarias futuras investigaciones epidemiológicas para determinar la real diagnosis de esta parasitosis en la provincia de Corrientes, Argentina.

Com o objetivo de avaliar a apresentação clínica da doença de Chagas em idosos foi realizado estudo retrospectivo utilizando-se os prontuários de doentes atendidos em ambulatório de referência. A casuística foi dividida em idosos (> 60 anos) e não idosos. Avaliou-se: sexo, co-morbidades, forma clínica, eletrocardiograma e títulos das sorologias. Idosos (61 casos): média de idade de 66,0 ± 5 anos, 67,2% do sexo feminino; comorbidades em 59%, mais freqüente a hipertensão arterial sistêmica (HAS)= 39,3%; forma indeterminada= 1,6%, forma cardíaca= 88,5%, forma digestiva= 36,1%; alterações freqüentes no eletrocardiograma: bloqueio divisional ântero-superior esquerdo (BDASE)= 41%, bloqueio completo de ramo direito (BCRD)= 32,8%, extra-sístole ventricular (EV)=22,9%. Não idosos (61 casos): média de idade: 39,30±8,36 anos, 54,1% do sexo feminino; comorbidades em 50,8%, mais freqüente a HAS (26,2%); forma indeterminada= 18% (p<0,05), forma cardíaca= 78,7%, forma digestiva= 32,8%; alterações freqüentes no eletrocardiograma: BDASE= 24,6%, BCRD= 21,3%, EV =18%. Concluindo, não houve diferenças clínicas entre indivíduos idosos e não idosos e a forma indeterminada predominou nos indivíduos abaixo dos 60 anos.

Este estudo avaliou as características de 125 chagásicos, > 25 anos, atendidos no Hospital de Clínicas da Universidade Estadual de Campinas, SP, considerando-se: pressão arterial, idade, gênero, cor, cardiopatia, índice de massa corporal, perfil lipídico, glicemia, etilismo, tabagismo, dislipidemia, diabetes, distúrbio de ansiedade e obesidade. Apresentavam hipertensão arterial 69 (55,2%) pacientes. Verificou-se que os chagásicos hipertensos eram mais idosos que os não hipertensos (p = 0,028). Entre os hipertensos havia: mais mulheres (p = 0,015); níveis mais elevados de glicemia, LDL-colesterol e colesterol total (p = 0,005; p = 0,024; p = 0,017); mais diabéticos (p = 0,006), dano cardíaco (p = 0,04) e sobrecarga ventricular esquerda (p = 0,003). Apenas a idade mostrou-se mais elevada nos pacientes com dano cardíaco (p = 0,003). Os chagásicos hipertensos apresentaram características clínico-laboratoriais semelhantes à população hipertensa, em geral. Dessa associação pode haver somatória de efeitos deletérios para o aparelho cardiovascular.

A doença de Chagas é uma parasitose causada pelo protozoário Trypanosoma cruzi, transmitido por insetos triatomíneos. A doença ocorre desde o sul dos Estados Unidos da América do Norte até a Argentina, sendo que, aproximadamente, 14 milhões de pessoas devam estar infectados na América Latina, predominantemente na forma crônica da doença. A reagudização da doença de Chagas pode ocorrer em imunossuprimidos, como tem sido observado em pacientes com aids. Verificou-se descompensação cardíaca em um destes casos, com grave disfunção ventricular e arritmias sendo considerada a possibilidade de reagudização da doença de Chagas no miocárdio, uma vez que o xenodiagnóstico foi positivo. Face a gravidade foi tratado especificamente para o Trypanosoma cruzi com benznidazol, porém sem completar o tempo estipulado para este fim, vindo a falecer em conseqüência de complicações da cardiopatia. A necropsia apresentou os estigmas habituais da cardiopatia chagásica crônica como miocardite fibrosante e redução do número de neurônios no tubo digestório, não sendo encontradas formas amastigotas do Trypanosoma cruzi em nenhum dos tecidos examinados. Assim, não ficou demonstrada a reagudização da doença de Chagas, mas sim evolução natural da cardiopatia chagásica crônica.

This study aimed to identify the main comorbidities in elderly chagasic patients treated in a reference service and identify possible associations between the clinical form of Chagas' disease and chronic diseases. Ninety patients aged 60 years-old or over were interviewed and their clinical diagnoses recorded. The study population profile was: women (55.6%); median age (67 years); married (51.1%); retired (73.3%); up to four years' education (64.4%); and earning less than two minimum wages (67.8%). The predominant forms of Chagas' disease were the cardiac (46.7%) and mixed forms (30%). There was a greater proportion of mild cardiac dysfunction (84.1%), frequently in association with megaesophagus. The mean number of concurrent diseases was 2.856 ± 1.845, and 33% of the patients had four or more comorbidities. The most frequent were systemic arterial hypertension (56.7%), osteoporosis (23.3%), osteoarthritis (21.2%) and dyslipidemia (20%). Positive correlations were verified between sex and comorbidities and between age group and comorbidities.

INTRODUCTION: Leishmaniasis is an infectious and parasitic zoonotic, non-contagious, vector-borne disease caused by protozoa of the genus Leishmania. In Brazil, the major vector of Leishmania (Leishmania) infantum chagasi (Cunha & Chagas, 1934) is Lutzomyia longipalpis. Barra do Garças, State of Mato Grosso, was designated as a priority area by the Brazilian Ministry of Health for american visceral leishmaniasis, and it is important to identify the vector species present in this municipality. Our objective was to raise sandflies and study the influence of environmental variables on the vector density of Lutzomyia longipalpis. METHODS: We performed entomological monitoring in 3 districts using Centers for Disease Control and Prevention (CDC) light traps and recorded human cases of american visceral leishmaniasis in the city. We calculated the relative frequency and richness of sandflies and applied a transfer function model to the vector density correlate with relative humidity. RESULTS: The sandfly population was composed of 2 genera and 27 species, totaling 8,097 individuals. Monitoring identified Lutzomyia longipalpis (44%), followed by Lutzomyia lenti (18.9%), Lutzomyia whitmani (13.9%), Lutzomyia carmelinoi (9.1%), Lutzomyia evandroi (5.1%), Lutzomyia termitophila (3.3%), Lutzomyia sordellii (1.9%), and 20 other species (<4%). The male:female ratio was 3.5:1. We observed high species diversity (Dα = 6.65). Lutzomyia longipalpis showed occurrence peaks during the rainy season; there was a temporal correlation with humidity, but not with frequency or temperature. CONCLUSIONS: The presence of Lutzomyia longipalpis in the urban area of Barra do Garças underscores the changing disease profile, which was previously restricted to the wild environment.

In order to compare the development of strata in the early stages of secondary forest succession with vessel parameters of the tree species, a forest inventory was carried out in 4-year (Q1: 48 m2), 11-year (Q2: 400 m2) and 20-year (Q3: 400 m2) forests and vessel parameters were investigated from stem cross sections of 18 species obtained in Q2. Thirty three species (21 families), 77 species (35 families), 39 species (20 families) were found in Ql, Q2, Q3, respectively. The percentage of dead individuals, dead stems and the percentage of individuals with multiple stems increased with time after clear cutting. Also, the total D2H of Q3 was 26.1 times that of Q1, and the development of strata started in Q2 and Q3. The image analysis of vessel size, area and number of vessels revealed that species which reach the forest canopy had a large D2H value, vessel diameter and area, while species which remain near the forest floor had smaller ones. Poecilanthe effusa (Huber) Ducke is an example of the latter case, with a large number of individuals and abundant sprouting of new stems from stumps, but with high mortality.
