OBJECTIVE Collection of triatomines in domestic, peridomestic and sylvatic environments in states of Bahia and Rio Grande do Sul, Northeastern and Southern Brazil respectively, and isolation of Trypanosoma cruzi strains. METHODS First, the captured triatomines were identified using insect identification keys, then their intestinal content was examined by abdominal compression, and the samples containing trypanosomatid forms were inoculated in LIT medium and Swiss mice. RESULTS Six triatomine species were collected in cities in Bahia, namely Panstrongylus geniculatus (01), Triatoma melanocephala (11), T. lenti (94), T. pseudomaculata (02), T. sherlocki (26) and T. sordida (460), and two in cities in Rio Grande do Sul, namely T. circummaculata (11) and T. rubrovaria (115). Out of the specimens examined, T. cruzi was isolated from 28 triatomine divided into four different species: T. melanocephala (one), T. lenti (one), T. rubrovaria (16) and T. sordida (10). Their index of natural infection by T. cruzi was 6.4%. CONCLUSIONS The isolation of T. cruzi strains from triatomines found in domestic and peridomestic areas shows the potential risk of transmission of Chagas disease in the studied cities. The maintenance of those T. cruzi strains in laboratory is intended to promote studies that facilitate the understanding of the parasite-vector-host relationship.

Os valores de complemento hemolítico total, C3 total (nativo + produtos de degradação) e o grau de conversão de C3 nativo foram estudados em dois subgrupos de pacientes chagásicos, nas formas cardíaca e indeterminada, e em um subgrupo de indivíduos não chagásicos, clinicamente sadios. Os níveis de C3 total e as taxas de conversão de C3 em seus produtos de degradação foram semelhantes nos três subgrupos. Os valores de complemento hemolítico total foram estatisticamente diferentes nos três subgrupos (nível de significância descritivo p = 0,0757), tendo sido observada média aritmética mais baixa no subgrupo de cardíacos e mais elevada no subgrupo de controles. Maior amplitude de variação dos níveis de complemento hemolítico total foi notada no subgrupo de cardíacos, no qual se encontraram os valores extremos (máximo e mínimo), considerando-se todos os subgrupos.

A doença de Chagas se tornou conhecida do mundo científico a partir de 1909. Dadas suas características clínicas, evidências de sua ocorrência até 1909 são fragmentárias. No entanto, uma análise cuidadosa de escritos médicos e leigos, particularmente do século 19, permitiu tirar algumas conclusões acerca da epidemiologia da doença no Brasil, particularmente no Estado de São Paulo. Possivelmente o acesso a obras raras nos permitirá lançar novas luzes sobre o assunto.

Right ventricular endomyocardial biopsies were studied in 30 patients, 15 with myocardiopathy from chronic Chagas'disease and 15 with idiopathic congestive myocardiopathy; five other myocardial samples were taken at necropsies of patients with chronic Chagas' disease. The authors tried to establish by means of direct immunofluorescence techniques whether there were immunoglobulins G, A and M, fibrinogen and C3 complement deposition in the myocardium; only one of these 30 patients exhibited a positive reaction to IgG, it was a patient with idiopathic congestive myocardiopathy. All fragments from patients with Chagas' disease showed no response to any of the fluorescent conjugates. These findings do not support the idea that anti-myoeardial antibodies have pathogenic importance in the evolution of dilated or chagasic myocardiopathies.

This report describes two patients with chronic Chagas' Heart Disease who developed clinical and laboratorial signs of myocardial infarction. Both patients presented sudden oppressive chest pain, without precipitating factor. In the first case, the highest MB-CK value was 65 IU, 22 hours after the beginning of the pain. On the second case, it was 77 IU at 18 hours after the beginning of the pain. In both cases ECG changes suggesting non-transmural infarction were present. The 99mTc PYP myocardial scintigram of the first case was positive. Coronary angiograms performed on the 18th and 9th day, respectively, after the acute infarction did not display obstructive lesions. Possible mechanisms causing myocardial infarction with normal coronary arteries in Chagas' Disease may include: embolic event's, particularly when there is associated congestive heart failure; coronary thrombosis and coronary spasms.

Uma amostra de soros de pacientes e controles de uma área endêmica da Doença de Chagas do País foi titulada contra hemácias de grupo O tratadas pela neuraminidase (células OT ativadas) e hemacias normais de grupo A e B (sistema ABO). Também foram feitos alguns testes de aglutinação de hemácias de pacientes com cardiomiopatia com as lectinas de amendoim (anti-T + anti-Tk) e com anticorpo humano anti-T. O escore médio da aglutinina anti-T dos pacientes foi similar ao do grupo controle. En- tretanto, o escore médio dos indivíduos com a forma cardíaca íoi significantemente maior que o dos pacientes assintomáticos e também maior que o do grupo controle. A comparação entre indivíduos com sorologia diagnóstica positiva versus negativa foi não significante. Nem a sorologia para Chagas nem as variáveis raça, sexo, idade, idade ao quadrado, in-fecção intestinal parasitária, teor de proteínas do soro, volume globular eritrocitário (hema-tócrito), grau de palidez da mucosa bucal, cardíomiopatia e os termos de interação sexo x idade, sexo x idade ao quadrado, estão associados significativamente com o logaritmo do título ou com o logaritmo do escore de aglutinação. Os escores de aglutinação das aglutininas anti-A e anti-B do sistema de grupo sangüíneo ABO estão dentro da faixa de variação normal. As hemácias de pacientes com a forma cardíaca não são aglutinadas nem pelas lectinas de amendoim (antiT + anti-Tk) e da soja (anti-T) nem pela aglutinina anti-T de origem humana. A prova de Coombs direta foi negativa em todos os pacientes, demonstrando a aparente ausência de imunoglobulinas IgG e IgM na superfície eritrocitária. O fato de que o aumento dos níveis de anti-T só foi detectado em uma das formas clínicas da doença em pacientes da mesma área endêmica constitui um argumento contra a possibilidade de que a variação observada seja produzida por uma infecção concomitante.

Com o intuito de estudar a denervação brônquiea na fase crônica da doença de Chagas investigaram-se as propriedades mecânicas e a motricidade brônquiea de 6 coelhos chagásicos crônicos e 4 controles. Utilizou-se o método da pletismografia de corpo inteiro e todas as medidas foram feitas antes e depois da administração endovenosa de cloridrato de histamina (0,11 mg/kg). No grupo chagásico crônico obteve-se um menor acréscimo da capacidade residual funcional e uma quase inalteração do pico de fluxo na expiração passiva após a administração de histamina. Este grupo apresentou também um desvio para a direita na curva estática de pressão transpulmonar/volume. Estes resultados sugerem uma alteração na árvore brônquiea, que consiste principalmente numa broncoreatividade diminuída.

The presence of circulating immune complexes formed by IgM and IgG (CIC-IgM and CIC-IgG) was investigated, using antigen-specific enzyme-immunoassays (ELISA), in 30 patients with acute Chagas' disease who showed parasitemia and inoculation chagoma. Control population consisted of patients with chronic T. cruzi infection (30), acute toxoplasmosis 10), leishmaniasis (8), rheumatoid arthritis (3) and healthy individuals with negative serology for Chagas* disease (30). Acute chagasic patients were 100% CIC-IgG and 96.66% CIC-IgM positive whereas immunofluorescence tests yielded 90% and 86.66% of positivity for specific IgG and IgM antibodies, respectively. Chronic patients were 68% CIC-IgG and 0% CIC-IgM positive. The 30 negative and the 21 cross-reaction controls proved negative for ELISA (CIC-IgM and CIC-IgG). The high sensitivity of ELISA assays would allow early immunologic diagnosis, as well as prompt treatment, of acute T. cruzi infection, thus eliminating the problem of the false-positive and false-negative results which affects traditional methods for detection of circulating antibodies.

Germfree (GF) and conventional (CV) CFW (LOB) mice and Wistar and Sprague-Dawley rats were infected with Trypanosoma cruzi. The disease was more severe in the GF than in the CV animals as revealed by: (1) an earlier and more intense parasitemia; (2) a more precocious mortality; (3) a twice enlarged spleen: (4) a more intense cell and tissue parasitism; (5) visceral signs of cardiac failure.

The indeterminate form of Chagas' disease is characterized by positive serology for the disease in the absence of clinical findings and in the presence of both normal esophagogram and electrocardiogram. When more sensitive methods were used, abnormalities have been described either in the esophagus or in the heart. The authors have studied simultaneously the esophagus and the heart in the same subjects. In thirteen adults with diagnosis of indeterminate form and nine adult controls, the esophageal manometry both in basal conditions and after stimulus (bethanecol) and vectorcardiogram were performed. In the control group none of the subjects presented concomitant esophageal and cardiac alterations while in the chagasic group 92,3% of the patients presented results simultaneously altered. It is concluded that the studied patients showed indications of parasympathetic denervation manifested by simultaneously esophageal and heart alterations.

Report of a case of acute transfusional Chagas'disease in a four-year-old child with a previous diagnosis of acute lymphocytic leukemia, transmitted in São Paulo, the Capital of São Paulo State, Brazil. Epidemiological investigation disclosed the donor's serological positivity and his previous residence in an area where Chagas' disease is endemic. The importance of adequate sorological screening in blood donors is evident. It should be stressed that this is the first case notified to the Superintendência de Controle de Endemias (SUCEN) (Superintendency for the Endemy Control) of the State Secretariat of Health, São Paulo, for the last five years.

The effects of infection with Trypanosoma cruzi on the electrocardiographic tracings of mice were studied in 4.groups of animals: (1) normal; (2) infected with a pathogenic T. cruzi strain (TS COB); (3) immunized with 3 intraperitoneal inocula of 10(6) attenuated T. cruzi epimastigotes (TCC) and (4) immunized-infected, which sequentially received the treatments of groups 3 and 2. Infection and protection were confirmed by xenodiagnosis and histopathology. Isolated alterations such as extrasystolia, 1st degree atrioventricular block, arrhythmia and ST elevation were observed in normal as well as infected mice. However, tracings taken repeatedly on each mouse over a 293 day period revealed a set of alterations which were more frequently seen in infected (14/22) than in normal (4/27) animals (p = 0.00048). These alterations consisted of supraventricular tachycardia, sinus bradycardia and persisting, first degree AV blocks, often associated to pacemaker changes. Inoculation of attenuated T. cruzi (group 3) did not increase these alterations (2/27 mice) but significantly prevented their development after challenge with the pathogenic strain (1/19 versus 14/22 mice, p = 0.000095). Thus, preimmunization reduced not only parasitemia but also a pathogenic consequence of T. cruzi infection. This evidence is relevant for immunoprevention studies against Chagas' disease.

Eosinophil dynamics, in bone marrow, blood and peritoneal exudate, of resistant C57B1/6 (C57) and susceptible A/Snell (A/Sn) mice was comparatively studied during the acute phase of infection by Trypanosoma cruzi Y strain. A decline was observed in bone marrow eosinophil levels in A/Sn, but not in C57 mice, soon after infection, those of the former remaining significantly below those of the latter up to the 4th day of infection. Bone marrow eosinophil levels of C57 mice declined subsequently to levels comparable to those of A/Sn mice, the number of these cells in this compartment remaining 50% those of non infected controls, in both strains, up to the end of the experiment on the 14th day of infection. The fluctuations in eosinophil levels in blood and peritoneal space were similar in both mice strains studied. Concomitantly with depletion of eosinophils in the marrow, depletion in blood and a marked rise of these cells in the peritoneal space, initial site of infection, occurred in both strains. The difference in eosinophil bone marrow levels, between C57 and A/Sn mice, observed in the first four days of infection, suggests a higher eosinopoiesis capacity of the former in this period, which might contribute to their higher resistance to T. cruzi infection.
