OBJETIVO: Avaliar o impacto das necessidades ortodônticas tecnicamente definidas (critérios normativos) sobre a satisfação com a aparência e a mastigação e compará-las com as autopercebidas (critérios subjetivos) em um grupo de adolescentes. MÉTODOS: Foi realizado um estudo transversal com a totalidade dos alunos entre 14 e 18 anos de idade (n=315) de um colégio em Florianópolis, SC, Brasil, em 1999. Uma cirurgiã-dentista realizou os exames clínicos para diagnóstico das principais oclusopatias (Dental Aesthetic Index) e aplicou um questionário para conhecer a satisfação dos indivíduos quanto a aparência, mastigação e percepção das necessidades de tratamento ortodôntico. Foi utilizada análise de regressão logística múltipla para conhecer o impacto de cada oclusopatia sobre a percepção dos indivíduos a respeito dos problemas oclusais. RESULTADOS: Obtiveram-se alta taxa de resposta (95%) e alta concordância intra-examinadora (Kappa 0,6 a 1,0). A prevalência de pelo menos um tipo de oclusopatia foi de 71,3%. Presença de apinhamento incisal (OR=2,8 [1,6-4,9]) e overjet (trespasse horizontal) (OR=2,4[1,4-4,3]) foram fatores de risco para insatisfação com a aparência. Adolescentes que apresentaram irregularidade anterior da mandíbula (OR=3,3 [1,6-6,9]), overjet (OR=1,7 [1,1-3,0]) e diastema anterior (OR=3,1 [1,4-6,9]) apresentaram maior percepção para a necessidade de tratamento ortodôntico. CONCLUSÕES: Os resultados sugerem que existem graus de problemas oclusais tecnicamente definidos que são aceitáveis pela população e que devem influenciar na decisão de tratamento, interferindo diretamente na demanda para esse tipo de atendimento. Medidas subjetivas poderiam ser incorporadas aos critérios clínicos atualmente utilizados.

O estudo avaliou desdobramentos de um treinamento para abordagem de tabagistas para 40 profissionais da rede pública, de novembro de 2004 a março de 2007. Os profissionais realizaram abordagem mínima individual (3.419 tabagistas) e grupal (982). Dos pacientes fumantes que participaram dessas atividades, 9,2% conseguiram parar de fumar. As dificuldades relatadas pelos profissionais foram baixa adesão dos pacientes (11%), falta de medicação (38%) e de apoio/estrutura da unidade (29%). O treinamento aumentou a população abordada pelas unidades de saúde nas estratégias de prevenção do tabagismo.

OBJECTIVE To analyze whether the level of institutional and matrix support is associated with better certification of primary healthcare teams.METHODS In this cross-sectional study, we evaluated two kinds of primary healthcare support – 14,489 teams received institutional support and 14,306 teams received matrix support. Logistic regression models were applied. In the institutional support model, the independent variable was “level of support” (as calculated by the sum of supporting activities for both modalities). In the matrix support model, in turn, the independent variables were the supporting activities. The multivariate analysis has considered variables with p < 0.20. The model was adjusted by the Hosmer-Lemeshow test.RESULTS The teams had institutional and matrix supporting activities (84.0% and 85.0%), respectively, with 55.0% of them performing between six and eight activities. For the institutional support, we have observed 1.96 and 3.77 chances for teams who had medium and high levels of support to have very good or good certification, respectively. For the matrix support, the chances of their having very good or good certification were 1.79 and 3.29, respectively. Regarding to the association between institutional support activities and the certification, the very good or good certification was positively associated with self-assessment (OR = 1.95), permanent education (OR = 1.43), shared evaluation (OR = 1.40), and supervision and evaluation of indicators (OR = 1.37). In regards to the matrix support, the very good or good certification was positively associated with permanent education (OR = 1.50), interventions in the territory (OR = 1.30), and discussion in the work processes (OR = 1.23).CONCLUSIONS In Brazil, supporting activities are being incorporated in primary healthcare, and there is an association between the level of support, both matrix and institutional, and the certification result.

ABSTRACT OBJECTIVE To determine the distribution of total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides in Brazilian adolescents, as well as the prevalence of altered levels of such parameters. METHODS Data from the Study of Cardiovascular Risks in Adolescents (ERICA) were used. This is a country-wide, school-based cross-sectional study that evaluated 12 to 17-year old adolescents living in cities with over 100,000 inhabitants. The average and distribution of plasma levels of total cholesterol, LDL cholesterol, HDL cholesterol, and triglycerides were evaluated. Dyslipidemia was determined by levels of total cholesterol ≥ 170 mg/dl, LDL cholesterol ≥ 130 mg/dl, HDL cholesterol < 45 mg/dL, or triglycerides ≥ 130 mg/dl. The data were analyzed by gender, age, and regions in Brazil. RESULTS We evaluated 38,069 adolescents – 59.9% of females, and 54.2% between 15 and 17 years. The average values found were: total cholesterol = 148.1 mg/dl (95%CI 147.1-149.1), HDL cholesterol = 47.3 mg/dl (95%CI 46.7-47.9), LDL cholesterol = 85.3 mg/dl (95%CI 84.5-86.1), and triglycerides = 77.8 mg/dl (95%CI 76.5-79.2). The female adolescents had higher average levels of total cholesterol, LDL cholesterol, and HDL cholesterol, without differences in the levels of triglycerides. We did not observe any significant differences between the average values among 12 to 14 and 15- to 17-year old adolescents. The most prevalent lipid alterations were low HDL cholesterol (46.8% [95%CI 44.8-48.9]), hypercholesterolemia (20.1% [95%CI 19.0-21.3]), and hypertriglyceridemia (7.8% [95%CI 7.1-8.6]). High LDL cholesterol was found in 3.5% (95%CI 3.2-4.0) of the adolescents. Prevalence of low HDL cholesterol was higher in Brazil’s North and Northeast regions. CONCLUSIONS A significant proportion of Brazilian adolescents has alterations in their plasma lipids. The high prevalence of low HDL cholesterol and hypertriglyceridemia, especially in Brazil’s North and Northeast regions, must be analyzed in future studies, to support the creation of strategies for efficient interventions.

Intravenous injection of scorpion toxin (Tityus serrulatus) in normal and Trypanosoma cruzi infected rats did not cause ultrastructural morphologic changes on enterochromaffin-like (ECL) cells of the stomach, although it induced a significant increase of the gastric secretion. Our data seem to indicate that gastric ECL cells structure is not affected by stimulation with scorpion toxin or by acute infection with T. cruzi in the rat.

É descrito um caso de entomoftoromicose rinofacial por Conidiobolus coronatus, em paciente procedente de São Paulo (Capital), tratado com sucesso pelo fluconazol, por via oral, através terapêutica prolongada. Dados histopatológicos e micológicos confirmaram o diagnóstico.

The aim of this research was to evaluate the protein polymorphism degree among seventy-five C. albicans strains from healthy children oral cavities of five socioeconomic categories from eight schools (private and public) in Piracicaba city, São Paulo State, in order to identify C. albicans subspecies and their similarities in infantile population groups and to establish their possible dissemination route. Cell cultures were grown in YEPD medium, collected by centrifugation, and washed with cold saline solution. The whole-cell proteins were extracted by cell disruption, using glass beads and submitted to SDS-PAGE technique. After electrophoresis, the protein bands were stained with Coomassie-blue and analyzed by statistics package NTSYS-pc version 1.70 software. Similarity matrix and dendrogram were generated by using the Dice similarity coefficient and UPGMA algorithm, respectively, which made it possible to evaluate the similarity or intra-specific polymorphism degrees, based on whole-cell protein fingerprinting of C. albicans oral isolates. A total of 13 major phenons (clusters) were analyzed, according to their homogeneous (socioeconomic category and/or same school) and heterogeneous (distinct socioeconomic categories and/or schools) characteristics. Regarding to the social epidemiological aspect, the cluster composition showed higher similarities (0.788 < S D < 1.0) among C. albicans strains isolated from healthy children independent of their socioeconomic bases (high, medium, or low). Isolates of high similarity were not found in oral cavities from healthy children of social stratum A and D, B and D, or C and E. This may be explained by an absence of a dissemination route among these children. Geographically, some healthy children among identical and different schools (private and public) also are carriers of similar strains but such similarity was not found among other isolates from children from certain schools. These data may reflect a restricted dissemination route of these microorganisms in some groups of healthy scholars, which may be dependent of either socioeconomic categories or geographic site of each child. In contrast to the higher similarity, the lower similarity or higher polymorphism degree (0.499 < S D < 0.788) of protein profiles was shown in 23 (30.6%) C. albicans oral isolates. Considering the social epidemiological aspect, 42.1%, 41.7%, 26.6%, 23.5%, and 16.7% were isolates from children concerning to socioeconomic categories A, D, C, B, and E, respectively, and geographically, 63.6%, 50%, 33.3%, 33.3%, 30%, 25%, and 14.3% were isolates from children from schools LAE (Liceu Colégio Albert Einstein), MA (E.E.P.S.G. "Prof. Elias de Melo Ayres"), CS (E.E.P.G. "Prof. Carlos Sodero"), AV (Alphaville), HF (E.E.P.S.G. "Honorato Faustino), FMC (E.E.P.G. "Prof. Francisco Mariano da Costa"), and MEP (E.E.P.S.G. "Prof. Manasses Ephraim Pereira), respectively. Such results suggest a higher protein polymorphism degree among some strains isolated from healthy children independent of their socioeconomic strata or geographic sites. Complementary studies, involving healthy students and their families, teachers, servants, hygiene and nutritional habits must be done in order to establish the sources of such colonization patterns in population groups of healthy children. The whole-cell protein profile obtained by SDS-PAGE associated with computer-assisted numerical analysis may provide additional criteria for the taxonomic and epidemiological studies of C. albicans.

SUMMARY Parasitic infections are responsible for substantial mortality and morbidity worldwide. In most healthy individuals, little overt pathology is observed during infection with S. stercoralis. However, the symptoms in advanced cases may include gastrointestinal bleeding. Anemia is most commonly associated with hookworm infection, especially when several hundred worms are present. Our study evaluates the relationship between the hookworm or S. stercoralis infection status and the hemoglobin concentration of individuals examined by a private network of laboratories in Salvador, Bahia, Brazil. We examined 374,120 samples from middle-class individuals living in Salvador City from January 2004 to April 2008. The stool samples were analyzed by the Lutz and Baermann-Moraes methods, and the blood samples were analyzed for hemoglobin concentration and eosinophil counting. The prevalence of hookworm and S. stercoralis were 0.27% (1,027) and 0.34% (1,286), respectively. The prevalence of hookworm and S. stercoralis infection was significantly higher in males than in females and increased with age. Eosinophilia was a common laboratorial finding in individuals infected with hookworm and S. stercoralis. The hemoglobin concentration was lower in the hookworm-infected individuals than in non-infected ones, but none of the examined patients were anemic. Lack of anemia could be a consequence of the socioeconomic status of these patients.

Foram feitos o isolamento e a identificação dos agentes etiológicos da cromomicose, através do estudo macroscópico e microscópico das colônias. O fungo Fonsecaea pedrosoi foi isolado do material dos quatro casos estudados.

Foram realizados testes de susceptibilidade "in vitro" com várias amostras de agentes, isoladas de pacientes com cromomicose frente à 5-fluorocitosina e ao butil-simpatol (Vasculat), nas concentrações de 0,1, 1,0, 5,0 10,0 mcg/ml. Em alguns casos, em que se verificou resistência à 5-fluorocitosina, a concentração desta chegou a 100,0 mcg/ml.

Relata-se um caso de hidatidosepolicistica, em homem de 22 anos, clinicamente manifestada por dor no hipocôndrio direito, icterícia obstrutiva, hepatoesplenomegalia, perda de peso e, em estágio final, ascite. O diagnóstico foi após laparotomia exploradora, com biópsia hepática e peritoneal. O paciente evoluiu para o óbito, a despeito de ter sido tratado com mebendazol na dose de 1200 mg/dia, durante um ano. A necrópsia, constatou-se hidatidose do fígado, omento maior e peritônio diafragmático. Não se conseguiu identificar, à microscopia óptica, a espécie de Echinococcus envolvida; com base nos dados epidemiológicos e morfológicos disponíveis, o parasita em apreço poderia ser o Echinococcus vogeli ou o E. oligarthrus. Este parece ser o quarto caso de hidatidose policistica na literatura nacional e é, com muita probabilidade, autóctone de Minas Gerais.

The phlebotomine sandfly fauna of a primary forest reserve at Morretes (eastern Paraná State) was studied, using CDC-like light traps, one night per month, at canopy and ground level, between April 1995 and March 1996. A total of 3,106 insects were collected, identified as belonging to nine species. Lutzomyia ayrozai and Lu.geniculata were predominant, seven other species also being present. Monthly mean temperature, rainfall and the temperature of the collection night significantly influenced the numbers of Lu. ayrozai while the two first factors influenced the numbers of Lu. geniculata, besides the collected quantities of females of the two species. The influence of the factors on Lu. ayrozai numbers was more immediate than in those of Lu. geniculata. Numbers of both species and of the females of Lu. geniculata collected in different seasons, but not at the different heights, varied significantly. Differences between the behaviour of Lu. ayrozai in Morretes and in other regions could be attributed to environmental differences and/or to regional variations in the species, which could constitute species complexes. Hourly variations of collections were different in the species and seasons.

Em exame parasitológico de fezes de 227 alunos de escola pública de São Paulo (SP), encontramos 87 (38,3%) positivas para Blastocystis hominis. A blastocistose ainda suscita controvérsias e indefinições, merecedoras de esclarecimentos sobretudo para evitar contratempos no âmbito da saúde pública e das atenções médico-assistenciais.

No Brasil, índices elevados da esquistossomose mansônica correspondem, na grande maioria dos casos, à presença da espécie Biomphalaria glabrata, principal transmissora do Schistosoma mansoni, nas localidades endêmicas. Foi realizado estudo em 40 municípios endêmicos do Estado de Alagoas, com o objetivo de verificar a existência da espécie e sua importância na manutenção da esquistossomose nesse Estado. Desses municípios, 28 são pertencentes à mesorregião do leste Alagoano e 12 à mesorregião do Agreste Alagoano. Os moluscos procederam de diversos tipos de criadouros: riachos, córregos, valas, açudes, brejos e poços. As coletas foram realizadas no período de fevereiro de 1996 a dezembro de 1998. Para a identificação de Biomphalaria glabrata, foi efetuado o exame anatômico das partes moles, após remoção das conchas. A detecção de cercárias do Schistosoma mansoni foi realizada através da técnica de esmagamento, calculando-se o percentual de infecção. Em 32 (80%) dos municípios estudados foi encontrada Biomphalaria glabrata, seis deles apresentando moluscos infectados com cercárias do parasita. Penedo apresentou a maior (6,6%) taxa de infecção, seguindo-se Ibateguara (5,6%). Taxas menores foram observadas em Chã Preta (2,7%), em Murici (2,5%), Porto Real do Colégio (0,1%) e Igreja Nova (0,1%). O inquérito copro-parasitológico efetuado pela Fundação Nacional de Saúde em 1997, 1998/1999 e 2000, confirmou a importância da endemia nessas regiões do Estado.
