Cystic fibrosis is a genetic disease usually diagnosed by abnormal sweat testing. We report a case of an 18-year-old female with bronchiectasis, chronic P. aeruginosa infection, and normal sweat chloride concentrations who experienced rapid decrease of lung function and clinical deterioration despite treatment. Given the high suspicion ofcystic fibrosis, broad genotyping testing was performed, showing a compound heterozygous with deltaF508 and 3849+10kb C->T mutations, therefore confirming cystic fibrosis diagnosis. Although the sweat chloride test remains the gold standard for the diagnosis of cystic fibrosis, alternative diagnostic tests such as genotyping and electrophysiologic measurements must be performed if there is suspicion of cystic fibrosis, despite normal or borderline sweat chloride levels.

Conflicting opinions are recorded in the literature concerning the suitability of Amazon lands for sustainable agriculture following deforestation. This article has been written to shed light on this question by summarizing climate, landform, soil and vegetation features from the findings of a land resource study of the Brazilian state of Rondônia in south-west Amazonia. The work, which followed the World Soils and Terrain Digital Database (SOTER) methodology, was financed by the World Bank. During the course of the survey special emphasis was given to studying soils; 2914 profiles were analyzed and recorded. The study identified a complex pattern of land units with clear differences in climate, landform, soils and native vegetation. Forested areas mosaic with lesser areas of natural savannas. The latter occur on both poorly-drained and well-drained, albeit nutrient deficient sandy soils. The tallest and most vigorous forests or their remnants were seen growing on well-drained soils formed from nutrient-rich parent materials. Many of these soils could, or are being used for productive agriculture. Soils developed on nutrient-poor parent materials support forests that are significantly lower in height, and would require large lime and fertilizer inputs for agriculture. Low forests with high palm populations and minor areas of wet land savannas cover the poorly drained soils. It is evident that forest clearing in the past was indiscriminant; this cannot be condoned. The diversity of land conditions found throughout Rondônia would suggest that many past studies in the Amazon have simply been too broad to identify significant soil differences.

OBJECTIVE: To analyze clinical and histologic findings of 50 patients with primary neoplams of the heart in a tertiary referral center. METHODS: From 1980 to 1998, we retrospectively analyzed 50 patients, 32 of whom were females, whose ages ranged from 9 to 73 years (mean age = 44.16±18 years). RESULTS: Most tumors were located in the left side of the heart (72%), myxoma being the most common (84%) histologic type. The other histologic types found were as follows: fibroma (4%), lipoma (2%), rhabdomyosarcoma (2%), hemangioma (2%), sarcoma (2%), angiosarcoma (2%), and lymphoma (2%). Diagnosis was established by echocardiography in 94% of the cases. Clinical findings were as follows: dyspnea (36%), weight loss (20%), palpitations (18%), chest pain (16%), fever (8%), and arthralgia (6%). All patients with thromboembolic phenomena (10%) had left atrial myxoma. Approximately 20% of the patients were asymptomatic at the initial clinical assessment. CONCLUSION: Primary cardiac tumors are a rare entity with diverse clinical and histologic findings, requiring, therefore, a high level of clinical suspicion.

O Fibroelastoma Papilífero é um tumor cardíaco benigno, encontrado principalmente nas válvulas. A maioria é assintomática, mas, quando presentes, são inespecíficos ou relacionados a fenômenos embólicos. Em geral, são diagnosticados em exames de imagem de rotina ou em cirurgias valvares e autópsias. Por sua raridade, há controvérsia sobre sua terapêutica. Descrevemos sete casos diagnosticados e tratados em nossa instituição entre 1989 a 2010, o que constitui a maior casuística nacional dessa patologia até o momento.

In this document, the Inter-American Committee of Cardiovascular Prevention and Rehabilitation, together with the South American Society of Cardiology, aimed to formulate strategies, measures, and actions for cardiovascular disease prevention and rehabilitation (CVDPR). In the context of the implementation of a regional and national health policy in Latin American countries, the goal is to promote cardiovascular health and thereby decrease morbidity and mortality. The study group on Cardiopulmonary and Metabolic Rehabilitation from the Department of Exercise, Ergometry, and Cardiovascular Rehabilitation of the Brazilian Society of Cardiology has created a committee of experts to review the Portuguese version of the guideline and adapt it to the national reality. The mission of this document is to help health professionals to adopt effective measures of CVDPR in the routine clinical practice. The publication of this document and its broad implementation will contribute to the goal of the World Health Organization (WHO), which is the reduction of worldwide cardiovascular mortality by 25% until 2025. The study group's priorities are the following: • Emphasize the important role of CVDPR as an instrument of secondary prevention with significant impact on cardiovascular morbidity and mortality; • Join efforts for the knowledge on CVDPR, its dissemination, and adoption in most cardiovascular centers and institutes in South America, prioritizing the adoption of cardiovascular prevention methods that are comprehensive, practical, simple and which have a good cost/benefit ratio; • Improve the education of health professionals and patients with education programs on the importance of CVDPR services, which are directly targeted at the health system, clinical staff, patients, and community leaders, with the aim of decreasing the barriers to CVDPR implementation.

In order to study the action of herbicides - sodium salt, amine salt and ester of 2,4-D, TCA and 2,4,5-T a preliminary experiment for pre-emergence weed control was corried out, and the corresponding results are given in table I and II. The corn used in the experiments was of the flint type 1A 3531. The loam soil on which the experiment has been carried out is called "terra roxa". All treatments were highly significant when compared with the check plots, except the 2B one in the control of broad leaf weeds, and 4B in the control of grass weeds. Among these treatments there are no significant differences. But we note the following: (table I). a) treatments of higher concentrations were superior to lower ones. b) the treatments which gave the best control for broad leaf weeds were in the following decreasing order: 1A, 5A and 3A. For grass weeds, they were 5A, 1A and 3A. c) the amine 2,4-D (600 grs. per hectare) supplied very good control when we get into consideration that on the acid basis, it was in very low concentration. d) TCA in high concentration affected the germination, growth and yield, in the lower one it did not show good control of weeds, especially of grasses. It is not suitable for pre-emergence control in corn. e) 2,4,5-T was not better than the 2,4-D products. As it is much more expensive than the others, economically its use in pre-emergence weed control in corn is not praticable. f) all the products used controled grass weeds as well as broad leaf ones; this show the superiority of the pre-emergence treatment method over that of post-emergence. g) Even a dose as strong as the treatment 1A (3.400g. of 2,4-D acid per hectare) did not damage corn production (table II). h) the superiority noted in the production of all the treatments with the exception of 2A, which damaged the plants, we atribute to the lack of competion between corn and weeds; all chek-plots suffered this competition, because they were not Probably, there was, also, hormonial effect of 2,4-D on the corn plant. Not withstanding the fact that the present experiment has been successful, we think that new researches are necessary, especially with the purpose of studying factors as climate and soil which in other countries, interferred with the success of the pre-emergence weed control.

Seven species of the nematode genus Mononchus Bastian, 1865, were found in the State of São Paulo, Brazil. Such are: M. subsimilis Cobb, 1917; M. ibitiensis Carvalho, 1951; M. piracicabae Lordello, 1953; M. risoceiae Carvalho, 1955; M. soutoi Carvalho, 1956; M. coronatus Carvalho, 1956; and M. jairi n. sp. Outside the State, the following species have been found: M. papillatus (Bastian, 1865) Cobb, 1916; M. monhystera Cobb, 1917; M. muscorum (Dujardin, 1845) Cobb, 1916; M. gymnolaimus (Cobb, 1893) Cobb, 1916; M. consimilis Cobb, 1917; M. trichurus Cobb, 1917; M. reversus Cobb, 1917; and M. dentatus Cobb, 1917 (LORDELLO, 1953). This field of investigation has scarcely been touched and additional collections will doubtless add new forms to the number of species known as yet. M. jairi n. sp. most closely resembling species is M. monhystera Cobb, 1917, from which it differs in having: a) labial region more expanded, set off from neck by a broad shallow depression; b) anterior portion of the pharynx-wall rather thinner and directed outward; and, c) smaller dimensions (788.0-924.0: 1,100.0 micros).

The effects of growth retardants on infestation by Potyphagotarsonemus tatus (broad mite) on cotton (Gossypium hirsutum L. cv. IAC-17) plants was studied. Cotton plants were sprayed with (2-ch1oroethy1) trimethylammonium chloride (CCC) 250, 350 and 450 ppm, and with 1,1-dimethyl-piperidinium chloride (Pix) at concentrations of 84, 167 and 250 ppm. Growth retardants did not give effective control of Potyphagotarsone mus tatus but application of Pix 167 ppm showed a tendency to reduce mite attack.

A remarkable new species of pulmonate snail was recently collected in a small Atlantic Rainforest fragment near the city of Canavieiras, state of Bahia, Brazil, an area known for a high diversity of land snails. It is described herein as Leiostracus fetidus sp. nov. and can be easily identified by its color pattern of irregular brown to black axial stripes on a white to yellow background, a reddish axial band "separating" the white peristome from the rest of the shell and a broad brown spiral band surrounding the umbilical region. Other diagnostic features include a relatively small size, a proto columellar fold and two very weak folds delimiting the basal region of the aperture. This discovery is a reminder of how little this fauna is known and also an alarm for proper conservation of these forest fragments.

The lesser grison (Galictis cuja) is one of the least-known mustelids in the Neotropics, despite its broad range across South America. This study aimed to explore current knowledge of the distribution of the species to identify gaps in knowledge and anticipate its full geographic distribution. Eighty-nine articles have mentioned G. cuja since 1969, but only 13 focused on the species. We generated a detailed model of the species' potential distribution that validated previous maps, but with improved detail, supporting previous southernmost records, and providing a means of identifying priority sites for conservation and management of the species.

Apresentamos um caso de neurinoma retropharyngeo que offereceu consideravel difficuldade no diagnostico em vida do paciente. A existencia, em sua espessura, de cavidades que, punccionadas, davam sahida a sangue rutilante, constituio feição clinica peculiar. O diganostico só foi possivel post-mortem, o estudo histologico revelando um neurinoma com extensas áreas de necrose, inflammação purulenta e hemorrhagica. As cavidades encontradas são a consequencia desses processos. No momento da autopsia ellas apresentavam conteúdo francamente purulento, sendo a quantidade de coagulos sanguineos relativamente pequena, destacando-se os coagulos facilmente das paredes internas das cavidades. O diagnostico de aneurysma foi excluido pelo exame microscopico, o qual, tambem, não revelou, no tumor, a estructura de angioma ou de angiosarcoma. As hemorrhagias processadas nas áreas necrosadas reconhecem, provavelmente, como ponto de partida arterias ou arteriolas englobadas na massa tumoral. Estas se conservariam em communicação permanente com algumas cavidades originadas por necrose e desintegração dos tecidos do tumor. Assim seria explicada a sahida de sangue rutilante pela puncção das cavidades em vida do doente, a pressão do sangue em seu interior, e a rapida desapparição do meio de contraste na arteriographia. A origem do tumor não poude ser precisada. Não é fóra de proposito recordar o que dizem Wahl (1914) e Dawydow (1932). Segundo Wahl, embora os tumores compostos de elementos especificos do tecido nervoso possam derivar de qualquer ponto do systema nervoso, o maior numero delles tem sua origem, contudo, no systema sympathico. Estructuras cysticas são referidas em dois dentre quarenta e cinco casos de ganglioneuremas por elle colligidos na litteratura até aquella epocha. Em um delles, são: « cystic structures that are probably softened fibroma ». Dawydow (1932) refere um neurinoma retropharyngeo tendo origem no ganglío sympathico cervícal superior.

Hyla claresignata Lutz & Lutz, 1939, is a large species apparently not closely allied to the other known Brazilian hylas. It is characterized by the very small tympanum; the head is short and the snout rounded; the legs are long, the hands and feet unusually large, the latter extensively webbbed. The specific name is derived from the insular, irregular, or roughly triangular, dark spots, with a light halo, found mostly in the dorso-lateral region and on the legs. It belongs to the rain-forest fauna of the Marítime Range. The adult is a bromeliad-dweller and the tadpole rhyacophilous. DESCRIPTION. Vomerine teeth in two separate, oblique, groups, behind the large choanae, parallel to the posterior half of their inner border. Tongue entire, short, very broad and hardly free behind. Snout short, rounded, with distinct canthus rostralis and gradually sloping loreal region. Eye very large and prominent, its horizontal diameter almost equal to the distance between its anterior corner and the tip of the snout. Tympanum very small, less than one third of the diameter of the eye, but distinct, partly covered by a short, heavy ridge. Lateral fingers less than one third webbed; fourth finger slightly longer than the second, just reaching the base of the disk of the third; subarticular tubercles well developed; an angular pollex rudiment, more noticeable in the males. Toes almost completely webbed, the edge of the web inserted at the base of the disk on the third and the fifth; an inner metatarsal tubercle. Skin smooth above, granular beneath, on the throat minutely so. No dermal appendage on the hell. Habit robust, head broader than long, body rather heavy, slightly narrowed in the postaxillary region. Legs long, the tibiotarsal articulation reaching beyond the tip of the snout when adpressed. Type (female): 61 mm. (Fig. 1.) DIAGNOSIS of TADPOLE (by G. Orton). "A large specialized, mountain-stream tadpole, with wide head an elongated, flattened snout, greatly enlarged lips and high tooth formula. Eyes dorsal. Spiracle sinistral, projecting, situated far back on side. Anus dextral. Tooth formula 8/12 to 9/14 in fully grown larvae. Tail with a prominent, vertical dark band across musculature and fins; a second concentration of dark pigment near tip of tail, may or may not form a similar but narrower band. The author studies 5 species of Archytas Jaennicke, 1867, belonging to the "dissimilis group": A. seminigra (Wiedemann, 1830) and four species which are considered as new. The species of this group may be characterized as follow: Species of short body, exceptionally large ones. Abdomen yellowish, with a median blackish V-shapedspot. Second antennal segment with 2/3 length of third. Parafacialia with blackish hairs. Propleura pilose. Post alar wall with few hairs. The following key facilitates the identification of the species: 1. Third article of antennae, strongly convex in the anterior margin (fig. 10); posterior margin straight. Parafacialia with a facio-orbital bristle well differentiated . . . . A. arnaudi sp. n. — Third article of antennae not so convex in the anterior margin; facio-orbital bristle absent, if present not well differentiated [...] 2; 2. Parafrontalia with golden polen [...] 3; — Parafrontalia brownish to shining black with few polen . . . 4; 3. A method to purify trypanosomastigotes of some strains of Trypanosoma cruzi (Y, CL, FL, F, "Berenice", "Colombiana" and "São Felipe") from mouse blood by using DEAE-cellulose columns was standardized. This procedure is a modification of the Lanham & Godfrey methods and differs in some aspects from others described to purify T. cruzi bloodstream trypomastigotes, mainly by avoidance of prior purifications of parasites. By this method, the broad trypomastigotes were mainly isolated, accounting for higher recoveries obtained with strains having higher percentages of these forms: processing of infected blood from irradiated mice could be advantageous by increasing the recovery of parasites (percentage and/or total number) and elution of more slender trypomastigotes. Trypomastigotes purified by this method presented normal morphology and motility, remained infective to triatomine bugs and mice, showing in the latter prepatent periods and courses parasitemia similar to those of control parasites, and also reproducing the polymorphism pattern of each strain. Their virulence and pathogenicity also remained considerably preserved, the latter property being evaluated by LD 50 tests, mortality rates and mean survival time of inoculated mice. Moreover, these parasites presented positive, clear and peripheral immunofluorescence reaction at titres similar to those of control organisms, thus suggesting important preservation of their surface antigens.

Bloodstream trypomastigotes of some Trypanosoma cruzi strains were processed through DEAE-cellulose columns under standardized conditions. The results obtained suggest mainly that these strains present different surface charges, that there are subpopulations of bloodstream trypomastigotes as regards electrical charges and that the broad forms are less negative than the slender ones.
