INTRODUÇÃO: Visando a estudar a relação entre câncer e industrialização analisou-se a evolução da mortalidade por câncer da região na Baixada Santista, SP (Brasil), importante complexo industrial-portuário cujos municípios se agrupam em duas diferentes áreas quanto ao processo de industrialização. MÉTODOS: Selecionaram-se 8.546 óbitos por câncer (CID-9), de indivíduos do sexo masculino acima de dez anos de idade, residentes nos municípios da Baixada Santista, no período de 1980 a 1993. Calcularam-se as taxas de mortalidade padronizada pela população mundial e as respectivas razões entre as taxas para a região e seus estratos: Estrato I (complexo industrial-portuário - Santos, São Vicente, Cubatão e Guarujá), e Estrato II (não industrializado - Praia Grande, Mongaguá, Itanhaém e Peruíbe). RESULTADOS: A taxa anual média de mortalidade da Baixada Santista mostrou ­ se alta (197,9/100.000). Houve diferença estatisticamente significante entre as taxas de mortalidade observadas para os Estratos I e II, respectivamente 209,2 e 146,7/100.000, com razão de 1,42 (IC 1,36 - 1,51). CONCLUSÕES: Supõe-se que a exposição ocupacional e ambiental a agentes químicos carcinogênicos relacionados ao processo produtivo do complexo industrial, vários deles já identificados, sejam fatores importantes na determinação da mortalidade por câncer. Nesse sentido outros estudos epidemiológicos são necessários para melhor caracterizar o excesso de mortalidade na área industrial da região estudada.

O artigo situa a questão da saúde no contexto do desenvolvimento nacional e da política industrial. Tomou-se a idéia de corte estruturalista, marxista e schumpeteriano, onde a indústria e as inovações constituem os elementos determinantes do dinamismo das economias capitalistas e de sua posição relativa na economia mundial. Todos os países que se desenvolveram e passaram a competir em melhores condições com os países avançados associaram uma indústria forte com uma base endógena de conhecimento, de aprendizado e de inovação. Todavia, na área da saúde essa visão é problemática, uma vez que os interesses empresariais se movem pela lógica econômica do lucro e não para o atendimento das necessidades da saúde. A noção de complexo industrial da saúde constitui uma tentativa e fornecer um referencial teórico que permita articular duas lógicas distintas: a sanitária e a do desenvolvimento econômico. O trabalho procurou mostrar, com base em dados de comércio exterior, como a desconsideração da lógica do desenvolvimento nas políticas de saúde levou a uma situação de vulnerabilidade econômica do setor que pode limitar os objetivos de universalidade, eqüidade e integralidade. Nesse contexto, propõe-se uma ruptura cognitiva e política com as visões antagônicas que colocam, de um lado, as necessidades da saúde e, de outro, da indústria. Um país que pretende chegar a uma condição de desenvolvimento e de independência requer, ao mesmo tempo, indústrias fortes e inovadoras, e um sistema de saúde inclusivo e universal.

Whilst their 'death' has often been certified, books remain highly important to most professions and academic disciplines. Analyses of citations received by epidemiologic texts may complement other views on epidemiology. The objective was to assess the number of citations received by some books of epidemiology and public health, as a first step towards studying the influence of epidemiological thought and thinking in academia. For this purpose, Institute for Scientific Information/ Thomson Scientific - Web of Science/ Web of Knowledgedatabase was consulted, in May 2006. The book by Rothman & Greenland appeared to have received the highest number of citations overall (over 8,000) and per year. The books by Kleinbaum et al, and by Breslow & Day received around 5,000 citations. In terms of citations per year the book by Sackett et al ranks 3rd, and the one by Rose, 4th of those included in this preliminary study. Other books which were influential in the classrooms collected comparatively less citations. Results offer a rich picture of the academic influences and trends of epidemiologic methods and reasoning on public health, clinical medicine and the other health, life and social sciences. They may contribute to assess epidemiologists' efforts to demarcate epidemiology and to assert epistemic authority, and to analyze some historical influences of economic, social and political forces on epidemiological research.

OBJETIVO: Avaliar efeitos respiratórios tardios da inalação ocupacional de talco contaminado por asbesto. MÉTODOS: Análise de série de casos de 29 ex-trabalhadores de mineração de talco contaminado com asbestos provenientes de Carandaí, MG, atendidos no Centro de Referência Estadual de Saúde do Trabalhador de Minas Gerais em 2004-2005. Os ex-trabalhadores foram submetidos a anamnese clínico-ocupacional, radiografia de tórax e espirometria. Foi criado um escore de exposição que, multiplicado pela duração, originou um índice de exposição cumulativa ao talco. Para verificar a associação do índice de exposição cumulativa com a presença de alterações radiológicas, sugestivas de pneumoconiose e/ou alterações pleurais, foi ajustado um modelo de regressão logística exata. RESULTADOS: Todos os ex-trabalhadores eram homens, com média de idade de 48,2 anos. Nas radiografias de tórax foram encontradas alterações pleurais em três deles, opacidades parenquimatosas compatíveis com pneumoconiose em um e suspeita em seis. Alterações espirométricas ocorreram em três. A regressão logística apontou odds ratio de 1,059 (IC 95%: 1,012;1,125) para o índice acumulado, ou seja, cada unidade no índice resulta em um aumento de 5,9% na chance de apresentar alteração radiológica compatível ou suspeita de pneumoconiose. Em relação à mediana do tempo estimado de latência entre o início da exposição até o diagnóstico das placas pleurais, ocorreu diferença significativa (p = 0,013) entre os casos (27,0 anos) e não casos (14,3 anos). CONCLUSÕES: Os achados apontam a necessidade de controle clínico nos trabalhadores que foram expostos a asbesto, particularmente devido aos efeitos tardios da exposição a esse mineral.

OBJECTIVE: To assess the impact of academic life on health status of university students. METHODS: Longitudinal study including 154 undergraduate students from the Universidade de Aveiro, Portugal, with at least two years of follow-up observations. Sociodemographic and behavioral characteristics were collected using questionnaires. Students' weight, height, blood pressure, serum glucose, serum lipids and serum homocysteine levels were measured. Regression analysis was performed using linear mixed-effect models, allowing for random effects at the participant level. RESULTS: A higher rate of dyslipidemia (44.0% vs. 28.6%), overweight (16.3% vs. 12.5%) and smoking (19.3% vs. 0.0%) was found among students exposed to the academic life when compared to freshmen. Physical inactivity was about 80%. Total cholesterol, high density lipoprotein-cholesterol (HDL-C), triglycerides, systolic blood pressure, and physical activity levels were significantly associated with gender (p<0.001). Academic exposure was associated with increased low density lipoprotein-cholesterol (LDL-C) levels (about 1.12 times), and marginally with total cholesterol levels (p=0.041). CONCLUSIONS: High education level does not seem to have a protective effect favoring a healthier lifestyle and being enrolled in health-related areas does not seem either to positively affect students' behaviors. Increased risk factors for non-transmissible diseases in university students raise concerns about their well-being. These results should support the implementation of health promotion and prevention programs at universities.

O papel estratégico da saúde na agenda de desenvolvimento nacional tem sido crescentemente reconhecido e institucionalizado. Além de sua importância como elemento estruturante do Estado de Bem-Estar Social, a saúde é protagonista na geração de inovação - elemento essencial para a competitividade na sociedade do conhecimento. Contudo, a base produtiva da saúde ainda é frágil, o que prejudica tanto a prestação universal de serviços em saúde quanto uma inserção competitiva nacional em ambiente globalizado. Essa situação sugere a necessidade de uma análise mais sistemática das complexas relações entre os interesses produtivos, tecnológicos e sociais no âmbito da saúde. Consequentemente, é necessário aprofundar o conhecimento sobre o Complexo Econômico-Industrial da Saúde devido ao seu potencial de contribuir para um modelo de desenvolvimento socialmente inclusivo. Isso significa reverter a hierarquia entre os interesses econômicos e os sociais no campo sanitário, e assim minimizar a vulnerabilidade da política de saúde brasileira.

Academic evaluation has been an essential component of modern science since its inception, as science has moved away from personalized patronage toward its contemporary role as an essential enterprise of contemporary, democratic societies. In recent years, Brazil has experienced sustained growth in its scientific output, which is nowadays fully compatible with its status as a high middle-income country striving to become a fully developed, more equitable country in the years to come. Growth usually takes place amidst challenges and dilemmas and, in Brazil as elsewhere, academic evaluation is not exempt from such difficulties. In a large, profoundly heterogeneous country with a national evaluation system and nationwide on-line platforms disseminating information on the most disparate fields of knowledge, the main challenges refer to how to pay attention to detail without losing sight of comprehensiveness and how to handle social and regional diversity while preserving academic excellence as the fundamental benchmark.

In this article, we evaluate electrocardiographic (ECG) alterations among urban workers from a mechanical-metallurgical industry in São Paulo, Brazil. In a cross-sectional survey carried out in 1980, we found 2.2% positive serological testing for Trypanosoma cruzi infection among 27,081 workers. A comparison between seropositive workers and a random sample of seronegative workers frequency-matched by age and occupation revealed that seropositive workers had a much lower educational level, and that a higher proportion of seropositive workers had ECG abnormalities (42.7%) when compared to those negative for T. cruzi infection (19.8%). The high frequency of ECG abnormalities suggests the need to provide medical assistance to these workers, without any kind of discrimination and to bring in a form of management that would decrease this dangerous risk to the workers and people around them.

Molecular characterization of Cryptosporidium spp.oocysts in clinical samples is useful for public health since it allows the study of sources of contamination as well as the transmission in different geographical regions. Although widely used in developed countries, in Brazil it is restricted to academic studies, mostly using commercial kits for the extraction of genomic DNA, or in collaboration with external reference centers, rendering the method expensive and limited. The study proposes the application of the modifications recently introduced in the method improving feasibility with lower cost. This method was efficient for clinical samples preserved at -20 °C for up to six years and the low number of oocysts may be overcomed by repetitions of extraction.

Chagas disease is a chronic, tropical, parasitic disease, endemic throughout Latin America. The large-scale migration of populations has increased the geographic distribution of the disease and cases have been observed in many other countries around the world. To strengthen the critical mass of knowledge generated in different countries, it is essential to promote cooperative and translational research initiatives. We analyzed authorship of scientific documents on Chagas disease indexed in the Medline database from 1940 to 2009. Bibliometrics was used to analyze the evolution of collaboration patterns. A Social Network Analysis was carried out to identify the main research groups in the area by applying clustering methods. We then analyzed 13,989 papers produced by 21,350 authors. Collaboration among authors dramatically increased over the study period, reaching an average of 6.2 authors per paper in the last five-year period. Applying a threshold of collaboration of five or more papers signed in co-authorship, we identified 148 consolidated research groups made up of 1,750 authors. The Chagas disease network identified constitutes a "small world," characterized by a high degree of clustering and a notably high number of Brazilian researchers.

Foodborne diseases represent operational risks in industrial restaurants. We described an outbreak of nine clustered cases of acute illness resembling acute toxoplasmosis in an industrial plant with 2300 employees. These patients and another 36 similar asymptomatic employees were diagnosed with anti-T. gondii IgG titer and avidity by ELISA. We excluded 14 patients based on high IgG avidity and chronic toxoplasmosis: 13 from controls and one from acute disease other than T. gondii infection. We also identified another three asymptomatic employees with T.gondii acute infection and also anti-T. gondii IgM positive as remaining acute cases. Case control study was conducted by interview in 11 acute infections and 20 negative controls. The ingestion of green vegetables, but not meat or water, was observed to be associated with the incidence of acute disease. These data reinforce the importance of sanitation control in industrial restaurants and also demonstrate the need for improvement in quality control regarding vegetables at risk for T. gondii oocyst contamination. We emphasized the accurate diagnosis of indexed cases and the detection of asymptomatic infections to determine the extent of the toxoplasmosis outbreak.

Objectives: Evaluate the production and the research collaborative network on Leishmaniasis in South America. Methods: A bibliometric research was carried out using SCOPUS database. The analysis unit was original research articles published from 2000 to 2011, that dealt with leishmaniasis and that included at least one South American author. The following items were obtained for each article: journal name, language, year of publication, number of authors, institutions, countries, and others variables. Results: 3,174 articles were published, 2,272 of them were original articles. 1,160 different institutional signatures, 58 different countries and 398 scientific journals were identified. Brazil was the country with more articles (60.7%) and Oswaldo Cruz Foundation (FIOCRUZ) had 18% of Brazilian production, which is the South American nucleus of the major scientific network in Leishmaniasis. Conclusions: South American scientific production on Leishmaniasis published in journals indexed in SCOPUS is focused on Brazilian research activity. It is necessary to strengthen the collaboration networks. The first step is to identify the institutions with higher production, in order to perform collaborative research according to the priorities of each country.

Publications are often used as a measure of research work success. Human T-lymphotropic virus (HTLV) type 1 and 2 are human retroviruses, which were discovered in the early 1980s, and it is estimated that 15-20 million people are infected worldwide. This article describes a bibliometric review and a coauthorship network analysis of literature on HTLV indexed in PubMed in a 24-year period. A total of 7,564 documents were retrieved, showing a decrease in the number of documents from 1996 to 2007. HTLV manuscripts were published in 1,074 journals. Japan and USA were the countries with the highest contribution in this field (61%) followed by France (8%). Production ranking changed when the number of publications was normalized by population (Dominican Republic and Japan), by gross domestic product (Guinea-Bissau and Gambia), and by gross national income per capita (Brazil and Japan). The present study has shed light on some of the defining features of scientific collaboration performed by HTLV research community, such as the existence of core researchers responsible for articulating the development of research in the area, facilitating wider collaborative relationships and the integration of new authors in the research groups.

OBJECTIVE: To evaluate clinical profiles, predictors of 30-day mortality, and the adherence to international recommendations for the treatment of myocardial infarction in an academic medical center hospital. METHODS: We retrospectively studied 172 patients with acute myocardial infarction, admitted in the intensive care unit from January 1992 to December 1997. RESULTS: Most patients were male (68%), white (97%), and over 60 years old (59%). The main risk factor for coronary atherosclerotic disease was systemic blood hypertension (63%). Among all the variables studied, reperfusion therapy, smoking, hypertension, cardiogenic shock, and age were the predictors of 30-day mortality. Most commonly used medications were: acetylsalicylic acid (71%), nitrates (61%), diuretics (51%), angiotensin-converting enzyme inhibitors (46%), thrombolytic therapy (39%), and beta-blockers (35%). CONCLUSION: The absence of reperfusion therapy, smoking status, hypertension, cardiogenic shock, and advanced age are predictors of 30-day mortality in patients with acute myocardial infarction. In addition, some medications that are undoubtedly beneficial have been under-used after acute myocardial infarction.

Reveste-se de grande importância o conhecimento da marcha de absorção de nutrientes pelas culturas, principalmente visando uma aplicação racional dos fertilizantes. Utilizou-se no presente experimento a cultivar Roma VF, de porte determinado, que tem tido grande aceitação tanto pelos tomaticultores como pelas industrias processadoras. As amostras para analise foram coletadas no período junho a outubro de 1977, em uma área experimental instalada num solo representativo da região oeste do Estado de São Paulo (Latossol Vermelho Amarelo), no município de Narandiba (DIRA de Presidente Prudente)· A cultura recebeu uma adubação de 85--300-100 kg/ha de NPK, sendo 1/3 do N aplicado juntamente com todo o P e K no plantio e os 2/3 restantes aplicados em cobertura aos 30 e 35 dias após a germinação. A área foi de 1,25 x 0,30 m (26,666 covas/ha), com culturais foram os normalmente recomendados para a cultura na região. As amostras (4 repetições) foram coletadas aos 15, 30, 45, 60, 75, 90 e 105 dias após a germinação. As plantas foram separadas em folhas cotiledonares, folhas, caule e frutos. Determinou-se o peso de matéria seca e analisou-se para N, P, K, Ca, Mg, S, B, Cu, Fe, Mo e Zn. Os resultados analíticos obtidos revelaram um crescimento lento ate aos 30 dias, após o que houve um crescimento acelerado, com o peso da materia seca praticamente dobrando a cada quinzena no período dos 45 aos 75 dias, atingindo o máximo aos 105 dias (5.706,61 kg/ha). Quanto ao crescimento dos frutos, expresso em peso de materia seca, houve um aumento de cerca de 20 vezes no intervalo dos 45 aos 75 dias, praticamente duplicando o peso no período dos 75 aos 90 dias e estabilizando-se aos 105 dias (2.708,6 kg/ha). Na época de floração, as folhas apresentavam, em função da materia seca, 3,7% N; 0,50% P; 4,44% K; 3,24% Ca; 0,99% Mg; 0,46% S; 72 ppm B; 15 ppm Cu; 434 ppm Fe; 375 ppm Mn; 0,18 ppm Mo; 148 ppm Zn. A produção efetiva de 65 ton/ha obtida na área experimental, contem as seguintes quantidades de nutrientes nos frutos: 67,8 g N; 8,9 g P; 112,2g K; 7,7 g Ca; 6,0 g Mg; 3,1 g S; 93 g B; 45 g Cu; 547 g Fe; 163 g Mn; 485 mg Mo.
