FUNDAMENTO: A detecção do limiar anaeróbico (LA) pela análise da variabilidade da frequência cardíaca (LiVFC) pode significar uma nova maneira de avaliação da capacidade funcional cardiorrespiratória (CFCR) em pré-adolescentes. OBJETIVO: Testar o método de LiVFC para detecção do LA em pré-adolescentes não obesos (NO), obesos (O) e obesos mórbidas (OM), a fim de determinar diferenças em sua CFCR. MÉTODOS: Foram estudados 30 pré-adolescentes, com idades entre 9 e 11 anos, divididos em três grupos de 10: a) grupo NO - índice de massa corpórea (IMC) com percentil do National Center for Chronic Disease Prevention and Health Promotion entre 5 e 85; b) grupo O - IMC de percentil entre 95 e 97 e c) grupo OM - IMC com percentil acima de 97. Todos foram submetidos a um protocolo incremental realizado em esteira rolante e registraram-se os batimentos cardíacos para detecção do LiVFC, que foi determinado pelo valor de 3,0 ms do índice do desvio-padrão 1 (SD1), extraído dos intervalos RR. RESULTADOS: Os valores médios no momento do LiVFC mostraram maiores valores para o grupo NO, destacando-se: a) VO2 (ml/kg/min) NO = 27,4 ± 9,2; O = 13,1 ± 7,6 e OM = 11,0 ± 1,7; b) FC (bpm): NO = 156,3 ± 18,0; O =141,7 ± 11,4 e OM = 137,7 ± 10,4; e c) distância percorrida (metros): NO = 1.194,9 ± 427,7; O = 503,2 ± 437,5 e OM = 399,9 ± 185,1. CONCLUSÃO: O LiVFC se mostrou efetivo para avaliação da CFCR e poderá vir a ser aplicado como método alternativo à ergoespirometria em determinadas situações.

FUNDAMENTO: Há poucos dados sobre análises da prevalência e da influência de características psicológicas adversas no prognóstico dos indivíduos submetidos a intervenções coronárias percutâneas. Nenhum estudo abordou essa questão no Brasil. OBJETIVO: Investigar a prevalência de depressão, ansiedade, estresse psicológico e personalidade tipo D, bem como sua associação com eventos cardiovasculares em pacientes submetidos à intervenção coronária percutânea (ICP). MÉTODOS: As características psicológicas foram avaliadas por escalas: inventário de depressão de Beck (IDB), inventário de ansiedade de Beck (IAB), inventário de sintomas de estresse de Lipp para adultos (ISSL) e escala de personalidade tipo D. A meta do estudo foi a ocorrência de eventos cardiovasculares maiores em um seguimento de um ano. RESULTADOS: Durante março e maio de 2006, 137 pacientes foram incluídos. A personalidade tipo D foi identificada em 34% dos casos; 29% apresentaram ansiedade, 25% apresentaram depressão, e 70% dos pacientes apresentaram estresse. Em relação à frequência de características psicológicas de acordo com a ocorrência de eventos cardiovasculares adversos maiores, não houve diferença estatística entre os dois grupos de pacientes no que se refere à depressão (29% vs. 26% p = 0,8), ansiedade (33% vs. 23% p = 0,3), estresse (76% vs. 65% p = 0,3) e personalidade tipo D (33% vs. 32% p = 0,9). No entanto, encontrou-se um escore de afetividade negativa significativamente maior no grupo de pacientes que apresentaram eventos (13,9 vs. 9,8 p = 0,01). CONCLUSÃO: Em pacientes submetidos à intervenção coronária percutânea, a prevalência de efeitos adversos psicológicos característicos foi alta. Eventos adversos cardiovasculares maiores em um ano foram associados à afetividade negativa basal, mas não a outras características psicológicas estudadas.

FUNDAMENTO: Diversos mecanismos podem estar envolvidos no desencadeamento da síncope em pacientes com cardiomiopatia hipertrófica (CMH), incluindo colapsos hemodinâmicos que podem estar relacionados a um desequilíbrio autonômico. OBJETIVO: Avaliar e comparar a função autonômica de pacientes que apresentam CMH com síncope inexplicada (SI) com os que não apresentam síncope. MÉTODOS: Trinta e sete pacientes foram incluídos, sendo 16 com SI e 21 sem síncope. Sua função autonômica foi avaliada por sensibilidade barorreflexa (SB) espontânea e induzida por fenilefrina, pela variabilidade da frequência cardíaca (VFC) no domínio do tempo durante o Holter de 24 horas e no domínio da frequência (análise espectral), ambos em decúbito dorsal e no teste de inclinação (TI) a 70º. RESULTADOS: A SB espontânea mostrou-se semelhante em ambos os grupos (16,46 ± 12,99 vs. 18,31 ± 9,88 ms/mmHg, p = 0,464), assim como a SB induzida por fenilefrina (18,33 ± 9,31 vs. 15,83 ± 15,48 ms/mmHg, p = 0,521). Não foram observadas diferenças no SDNN (137,69 ± 36,62 vs . 145,95 ± 38,07 ms, p = 0,389). O grupo com síncope apresentou um RMSSD significativamente menor (24,88 ± 10,03 vs. 35,58 ± 16,43 ms, p = 0,042) e tendência a menor pNN50 (4,51 ± 3,78 vs . 8,83 ± 7,98%, p =0,085) e a menores valores do componente de alta frequência da análise espectral da VFC em repouso (637,59 ± 1.295,53 vs. 782,65 ± 1.264,14 ms2 , p = 0,075). Nenhuma diferença significativa foi observada em resposta ao TI (p = 0,053). A sensibilidade, especificidade e acurácia do TI na identificação da etiologia da SI em pacientes com CMH foram 6%, 66% e 40%, respectivamente. CONCLUSÃO: Observou-se tônus parassimpático mais baixo em pacientes com CMH e SI, mas a relevância clínica deste achado ainda não está clara. O TI não é uma ferramenta vantajosa para avaliar a origem da síncope em tais doentes, principalmente por causa da sua baixa especificidade.

Abstract Background: Studies suggest that statins have pleiotropic effects, such as reduction in blood pressure, and improvement in endothelial function and vascular stiffness. Objective: To analyze if prior statin use influences the effect of renin-angiotensin-aldosterone system inhibitors on blood pressure, endothelial function, and vascular stiffness. Methods: Patients with diabetes and hypertension with office systolic blood pressure ≥ 130 mmHg and/or diastolic blood pressure ≥ 80 mmHg had their antihypertensive medications replaced by amlodipine during 6 weeks. They were then randomized to either benazepril or losartan for 12 additional weeks while continuing on amlodipine. Blood pressure (assessed with ambulatory blood pressure monitoring), endothelial function (brachial artery flow-mediated dilation), and vascular stiffness (pulse wave velocity) were evaluated before and after the combined treatment. In this study, a post hoc analysis was performed to compare patients who were or were not on statins (SU and NSU groups, respectively). Results: The SU group presented a greater reduction in the 24-hour systolic blood pressure (from 134 to 122 mmHg, p = 0.007), and in the brachial artery flow-mediated dilation (from 6.5 to 10.9%, p = 0.003) when compared with the NSU group (from 137 to 128 mmHg, p = 0.362, and from 7.5 to 8.3%, p = 0.820). There was no statistically significant difference in pulse wave velocity (SU group: from 9.95 to 9.90 m/s, p = 0.650; NSU group: from 10.65 to 11.05 m/s, p = 0.586). Conclusion: Combined use of statins, amlodipine, and renin-angiotensin-aldosterone system inhibitors improves the antihypertensive response and endothelial function in patients with hypertension and diabetes.

Abstract Background: Neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) are inflammatory markers used as prognostic factors in various diseases. The aims of this study were to compare the PLR and the NLR of heart failure (HF) patients with those of age-sex matched controls, to evaluate the predictive value of those markers in detecting HF, and to demonstrate the effect of NLR and PLR on mortality in HF patients during follow-up. Methods: This study included 56 HF patients and 40 controls without HF. All subjects underwent transthoracic echocardiography to evaluate cardiac functions. The NLR and the PLR were calculated as the ratio of neutrophil count to lymphocyte count and as the ratio of platelet count to lymphocyte count, respectively. All HF patients were followed after their discharge from the hospital to evaluate mortality, cerebrovascular events, and re-hospitalization. Results: The NLR and the PLR of HF patients were significantly higher compared to those of the controls (p < 0.01). There was an inverse correlation between the NLR and the left ventricular ejection fraction of the study population (r: -0.409, p < 0.001). The best cut-off value of NLR to predict HF was 3.0, with 86.3% sensitivity and 77.5% specificity, and the best cut-off value of PLR to predict HF was 137.3, with 70% sensitivity and 60% specificity. Only NLR was an independent predictor of mortality in HF patients. A cut-off value of 5.1 for NLR can predict death in HF patients with 75% sensitivity and 62% specificity during a 12.8-month follow-up period on average. Conclusion: NLR and PLR were higher in HF patients than in age-sex matched controls. However, NLR and PLR were not sufficient to establish a diagnosis of HF. NLR can be used to predict mortality during the follow-up of HF patients.

The oviposition behaviour of Gryon gallardoi (Brèthes, 1914) on eggs of Spartocera dentiventris Mendonça Jr. (Berg, 1884) of different ages (2, 3, 4, 5, 6, 7, 8 and 12 days) was investigated. Groups of 12 eggs of each age were exposed to single females of G. gallardoi, and the oviposition behaviour was recorded under a stereomicroscope for two hours. Ten replicates were used for each age. In order to identify the moment the parasitoid egg was released inside the host, 1-day old eggs of S. dentiventris were exposed to G. gallardoi females, and the oviposition was interrupted at intervals of 20, 40, 60, 80, 100, 120, 140 and 160s after ovipositor insertion had initiated. Five behavioural steps were recorded: drumming, ovipositor insertion, marking, walking and resting. The average drumming and ovipositor insertion times increased with the host age (P<0.01). Ovipositor insertion usually occurred next to the longitudinal extremities of the host eggs. Marking took on average 19.5 ± 0.7s, and as walking and resting, was not affected by host age. Self-parasitism behaviour was observed in only 13.8 ± 2.3% of the eggs, being more evident with increasing patch depletion (reduction in non-parasitized eggs in the egg group, P<0.01), again with no variation due to changes in host egg age. For all ages tested, self-parasitized host eggs were less frequently contacted and accepted than non-parasitized ones (P<0.01). The parasitoid egg was released 137.0 ± 3.7s after ovipositor insertion. Spartocera dentiventris egg condition can lead to parasitoid behavioural changes, especially during the process of host choice and discrimination.

This study presents original data on the reproduction and feeding ecology of two syntopic amphisbaenians, Amphisbaena munoai Klappenbach, 1969 and Anops kingi Bell, 1833, from southern Brazil. Sampling was carried out from April 2004 to April 2006 at a highland area located in São Jerônimo and Barão do Triunfo municipalities, Rio Grande do Sul, Brazil. A total of 647 amphisbaenians were collected, 510 specimens of A. munoai and 137 specimens of A. kingi, of which 130 and 93, respectively, had their gonads and gut content analyzed. Both species presented a unimodal pattern of seasonal distribution, sexually mature females with significantly larger snout vent length (SVL) than sexually mature males, and seasonal reproductive cycle. Adults of A. kingi had significantly larger (SVL) than those of Amphisbaena munoai. Both species had generalist diets, but termites (Isoptera) was a staple item in their diet. Greater predominance of insect larvae was observed in the diet of A. kingi, which may be due to its larger body in comparison to A. munoai. Insect larvae ingested by A. kingi were significantly larger than those ingested by A. munoai. Diets of adult males and females of both species were not significantly different. The ontogenetic diet comparison in both species revealed significant numerical differences, with adults consuming higher numbers of prey.

Em 137 amostras de alimentos de diferentes origens (animal e vegetal) foi investigada a ocorrência de sorotipos de Escherichia coli mais comumente descritos como produtores de enterotoxinas. A análise sorológica dos antígenos somáticos, de envoltórios e flagelares nas 265 culturas isoladas, resultou na identificação de 34 amostras distribuídas em doze sorotipos e oriundas de 24 produtos de origem animal. Outros aspectos foram analisados, visando associá-los como possíveis marcadores epidemiológicos. Assim, na biotipificação, verificou-se o perfil das 34 amostras diante da melibiose, rafinose, sacarose, salicina e sorbitol, obtendo-se a caracterização de 11 biotipos. Todavia, a acentuada heterogeneidade de biotipos distribuídos pelos sorotipos, não permitiu um relacionamento de tipos soro-fermentativos com as fontes de isolamento. Os demais testes, representados pela atividade hemolítica e a capacidade hemaglutinante, pouco acrescentaram para o problema da diferenciação de fenótipos ou na caracterização de marcadores epidemiológicos.

Forty-tree (31.4%) out of 137 serum samples obtained from two Indian communities living in the Amazon region were found to be positive for HTLV-I antibody, as tested by enzyme-linked immunosorbent assay (Elisa). Eighty-two sera were collected from Mekranoiti Indians, yielding 39% of positivity, whereas 11 (20.0%) or the 55 Tiriyo serum samples had antibody to HTLV-I. In addition, positive results occurred in 10 (23.2%) out of 43 sera obtained from patients living in the Belem area, who were suffering from cancer affecting different organs. Five (16.7%) out of 30 Elisa positive specimens were also shown to be positive by either Western blot analysis (WB) or indirect immunogold electron microscopy (IIG-EM).

From March 1996 to August 1997, a study was carried out in a malaria endemic area of the Brazilian Amazon region. In vivo sensitivity evaluation to antimalarial drugs was performed in 129 patients. Blood samples (0.5 ml) were drawn from each patient and cryopreserved to proceed to in vitro studies. In vitro sensitivity evaluation performed using a radioisotope method was carried out with the cryopreserved samples from September to December 1997. Thirty-one samples were tested for chloroquine, mefloquine, halofantrine, quinine, arteether and atovaquone. Resistance was evidenced in 96.6% (29/30) of the samples tested for chloroquine, 3.3% (1/30) for quinine, none (0/30) for mefloquine and none for halofantrine (0/30). Overall low sensitivity was evidenced in 10% of the samples tested for quinine, 22.5% tested for halofantrine and in 20% tested for mefloquine. Means of IC 50 values were 132.2 (SD: 46.5) ng/ml for chloroquine, 130.6 (SD: 49.6) ng/ml for quinine, 3.4 (SD: 1.3) ng/ml for mefloquine, 0.7 (SD: 0.3) ng/ml for halofantrine, 1 (SD: 0.6) ng/ml for arteether and 0.4 (SD: 0.2) ng/ml for atovaquone. Means of chloroquine IC 50 of the tested samples were comparable to that of the chloroquine-resistant strain W2 (137.57 ng/ml) and nearly nine times higher than that of the chloroquine-sensitive strain D6 (15.09 ng/ml). Means of quinine IC 50 of the tested samples were 1.7 times higher than that of the low sensitivity strain W2 (74.84 ng/ml) and nearly five times higher than that of the quinine-sensitive strain D6 (27.53 ng/ml). These results disclose in vitro high resistance levels to chloroquine, low sensitivity to quinine and evidence of decreasing sensitivity to mefloquine and halofantrine in the area under evaluation.

Between 1984 and 1993 the prevalence of the Trypanosoma cruzi infection in opossums (Didelphis marsupialis) was studied in Santa Catarina and Arvoredo Islands, State of Santa Catarina, Brazil. The association of the triatomine bug Panstrongylus megistus with opossums nests and the infection rate of these triatomines by T. cruzi was also studied. Thirteen different locations were studied in Santa Catarina Island (SCI), in which 137 D. marsupialis were collected. Sixty two opossums were collected at the Arvoredo Island (AI), located 12 miles north from SCI. All captured animals were submitted to parasitological examinations that revealed the presence of T. cruzi in 21.9% of the opossums captured in SCI and 45.2% among opossums captured in the AI. The presence of P. megistus was detected in most of the D. marsupialis nests collected in the SCI, however, in the non-inhabited AI only eight triatomines were collected during the whole study. The presence of T. cruzi-infected D. marsupialis associated with P. megistus in human dwellings in the SCI, and the high infection rate of D. marsupilais by T. cruzi in the absence of a high vector density are discussed.

The subfamily Triatominae is actually represented by 137 species distributed among 6 tribes and 19 genera. Within this subfamily, the genus Panstrongylus, Berg 1879, is composed by 13 species widespread in sylvatic, peridomestic, and domestic habitats of Neotropical regions. These species are vectors of Chagas disease and consequently are found associated with its main hosts, such as birds and mammals. Interest in species of this genus has been increasing in the last few years. Reports of several authors indicate these Triatominae to invade and colonize houses, increasing their epidemiological significance. Morphometry was used in this study to investigate correlations among possible closely related species. We measured 224 specimens among 13 species through a set of metric variables of the head. The results indicated that the genus Panstrongylus seems to be homogeneous since 10 of the 14 species were shown to be closely related.

Anopheles (Nyssorhynchus) benarrochi, An. (N.) oswaldoi, and An. (N.) rangeli are the most common anthropophilic mosquitoes in the southern Colombian state of Putumayo. Adult females are most commonly collected in epidemiological studies, and this stage poses significant problems for correct identification, due to overlapping inter-specific morphological characters. Although An. rangeli is easy to identify, the morphological variant of An. benarrochi found in the region and An. oswaldoi are not always easy to separate. Herein we provide a rapid molecular method to distinguish these two species in Southern Colombia. Sequence data for the second internal transcribed spacer (ITS2) region of rDNA was generated for link-reared progeny of An. benarrochi and An. oswaldoi, that had been identified using all life stages. ITS2 sequences were 540 bp in length in An. benarrochi (n = 9) and 531 bp in An. oswaldoi (n = 7). Sequences showed no intra-specific variation and ungapped inter-specific sequence divergence was 6.4%. Species diagnostic banding patterns were recovered following digestion of the ITS2 amplicons with the enzyme Hae III as follows: An. benarrochi (365, 137, and 38 bp) and An. oswaldoi (493 and 38 bp). This polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) assay provides rapid, accurate, and inexpensive species diagnosis of adult females. This will benefit future epidemiological studies and, as PCR amplification can be achieved using a single mosquito leg, the remaining specimen can be either retained as a morphological voucher or further used in vector incrimination studies. That An. benarrochi comprises a complex of at least two species across Latin America is discussed.

To elucidate the Anopheles nuneztovari s.l. taxonomic status at a microgeographic level in four malaria endemic localities from Antioquia and Córdoba, Colombia, fragments of the cytochrome oxidase subunit I (COI) and the white gene were used. The COI analysis showed low genetic differentiation with fixation index (F ST) levels between -0.02-0.137 and Nm values between 3-∞, indicating the presence of high gene flow among An. nuneztovari s.l. populations from the four localities. The COI network showed a single most common haplotype, type 1 (n = 55), present in all localities, as the likely ancestral haplotype. Analysis of the white gene showed that An. nuneztovari s.l. populations from both departments grouped with haplotypes 19 and 20, which are part of lineage 3 reported previously. The results of the present study suggest that An. nuneztovari s.l. is a single taxon in the area of the present study.

The serology of human Trypanosoma cruziinfection in the Rio Negro microregion is very complex because of the large numbers of false-positive cases that result from low antibody titres and cross-reactions with other infections. In the present study, we collected 4,880 blood samples on filter paper; of these, indirect immunofluorescence (IIF) was strongly reactive in 221 (4.5%), which were considered to be positive (IIF strongly reactive; high intensity of fluorescence) and weakly reactive in 302 (6.2%), which were considered to be doubtful (IIF weakly reactive; low intensity of fluorescence). The confirmatory test on the serum using at least two of three techniques (IIF, conventional ELISA and recombinant ELISA) on 137 samples that were positive in the screening test only confirmed 33 cases (24.1%). Of the 178 samples that were considered doubtful in the screening test, only 10 (5.6%) were considered to be positive in the confirmatory test. Finally, we recommend that the serological diagnosis of T. cruziinfection in the Amazon region be made using at least two different techniques, for example immunofluorescence and ELISA and confirmed by Western blot analysis when possible.
