Healthcare-Associated Infective Endocarditis: a Case Series in a Referral Hospital from 2006 to 2011

Background: Healthcare-associated infective endocarditis (HCA-IE), a severe complication of medical care, shows a growing incidence in literature. Objective: To evaluate epidemiology, etiology, risk factors for acquisition, complications, surgical treatment, and outcome of HCA-IE. Methods: Observational prospective case series study (2006-2011) in a public hospital in Rio de Janeiro. Results: Fifty-three patients with HCA-IE from a total of 151 cases of infective endocarditis (IE) were included. There were 26 (49%) males (mean age of 47 ± 18.7 years), 27 (51%) females (mean age of 42 ± 20.1 years). IE was acute in 37 (70%) cases and subacute in 16 (30%) cases. The mitral valve was affected in 19 (36%) patients and the aortic valve in 12 (36%); prosthetic valves were affected in 23 (43%) patients and native valves in 30 (57%). Deep intravenous access was used in 43 (81%) cases. Negative blood cultures were observed in 11 (21%) patients, Enterococcus faecalis in 10 (19%), Staphylococcus aureus in 9 (17%), and Candida sp. in 7 (13%). Fever was present in 49 (92%) patients, splenomegaly in 12 (23%), new regurgitation murmur in 31 (58%), and elevated C-reactive protein in 44/53 (83%). Echocardiograms showed major criteria in 46 (87%) patients, and 34 (64%) patients were submitted to cardiac surgery. Overall mortality was 17/53 (32%). Conclusion: In Brazil HCA-IE affected young subjects. Patients with prosthetic and native valves were affected in a similar proportion, and non-cardiac surgery was an infrequent predisposing factor, whereas intravenous access was a common one. S. aureus was significantly frequent in native valve HCA-IE, and overall mortality was high.

Background:Primary graft dysfunction is the main cause of early mortality after heart transplantation. Mechanical circulatory support has been used to treat this syndrome.Objective:Describe the experience with extracorporeal membrane oxygenation to treat post-transplant primary cardiac graft dysfunction.Methods:Between January 2007 and December 2013, a total of 71 orthotopic heart transplantations were performed in patients with advanced heart failure. Eleven (15.5%) of these patients who presented primary graft dysfunction constituted the population of this study. Primary graft dysfunction manifested in our population as failure to wean from cardiopulmonary bypass in six (54.5%) patients, severe hemodynamic instability in the immediate postoperative period with severe cardiac dysfunction in three (27.3%), and cardiac arrest (18.2%). The average ischemia time was 151 ± 82 minutes. Once the diagnosis of primary graft dysfunction was established, we installed a mechanical circulatory support to stabilize the severe hemodynamic condition of the patients and followed their progression longitudinally.Results:The average duration of extracorporeal membrane oxygenation support was 76 ± 47.4 hours (range 32 to 144 hours). Weaning with cardiac recovery was successful in nine (81.8%) patients. However, two patients who presented cardiac recovery did not survive to hospital discharge.Conclusion:Mechanical circulatory support with central extracorporeal membrane oxygenation promoted cardiac recovery within a few days in most patients.

Abstract Background: More than 50% of the patients with heart failure have normal ejection fraction (HFNEF). Iodine-123 metaiodobenzylguanidine (123I-MIBG) scintigraphy and cardiopulmonary exercise test (CPET) are prognostic markers in HFNEF. Nebivolol is a beta-blocker with vasodilating properties. Objectives: To evaluate the impact of nebivolol therapy on CPET and123I-MIBG scintigraphic parameters in patients with HFNEF. Methods: Twenty-five patients underwent 123I-MIBG scintigraphy to determine the washout rate and early and late heart-to-mediastinum ratios. During the CPET, we analyzed the systolic blood pressure (SBP) response, heart rate (HR) during effort and recovery (HRR), and oxygen uptake (VO2). After the initial evaluation, we divided our cohort into control and intervention groups. We then started nebivolol and repeated the tests after 3 months. Results: After treatment, the intervention group showed improvement in rest SBP (149 mmHg [143.5-171 mmHg] versus 135 mmHg [125-151 mmHg, p = 0.016]), rest HR (78 bpm [65.5-84 bpm] versus 64.5 bpm [57.5-75.5 bpm, p = 0.028]), peak SBP (235 mmHg [216.5-249 mmHg] versus 198 mmHg [191-220.5 mmHg], p = 0.001), peak HR (124.5 bpm [115-142 bpm] versus 115 bpm [103.7-124 bpm], p= 0.043), HRR on the 1st minute (6.5 bpm [4.75-12.75 bpm] versus 14.5 bpm [6.7-22 bpm], p = 0.025) and HRR on the 2nd minute (15.5 bpm [13-21.75 bpm] versus 23.5 bpm [16-31.7 bpm], p = 0.005), but no change in peak VO2 and 123I-MIBG scintigraphic parameters. Conclusion: Despite a better control in SBP, HR during rest and exercise, and improvement in HRR, nebivolol failed to show a positive effect on peak VO2 and 123I-MIBG scintigraphic parameters. The lack of effect on adrenergic activity may be the cause of the lack of effect on functional capacity.

Abstract Background: Studies have questioned the downward trend in mortality from cardiovascular diseases (CVD) in Brazil in recent years. Objective: to analyze recent trends in mortality from ischemic heart disease (IHD) and stroke in the Brazilian population. Methods: Mortality and population data were obtained from the Brazilian Institute of Geography and Statistics and the Ministry of Health. Risk of death was adjusted by the direct method, using as reference the world population of 2000. We analyzed trends in mortality from CVD, IHD and stroke in women and men in the periods of 1980-2006 and 2007-2012. Results: there was a decrease in CVD mortality and stroke in women and men for both periods (p < 0.001). Annual mortality variations for periods 1980-2006 and 2007-2012 were, respectively: CVD (total): -1.5% and -0.8%; CVD men: -1.4% and -0.6%; CVD women: -1.7% and -1.0%; DIC (men): -1.1% and 0.1%; stroke (men): -1.7% and -1.4%; DIC (women): -1.5% and 0.4%; stroke (women): -2.0% and -1.9%. From 1980 to 2006, there was a decrease in IHD mortality in men and women (p < 0.001), but from 2007 to 2012, changes in IHD mortality were not significant in men [y = 151 + 0.04 (R2 = 0.02; p = 0.779)] and women [y = 88-0.54 (R2 = 0.24; p = 0.320). Conclusion: Trend in mortality from IHD stopped falling in Brazil from 2007 to 2012.

O bicho do cesto, Oiketicus kirbyi (Lands.-Guilding, 1827), inseto extremamente polífago, é praga de várias culturas de importância econômica, principalmente na região sul do Brasil. Sua ocorrência em áreas reflorestadas com eucalipto é freqüente, embora não tenha causado, ainda, danos econômicos de monta. Este trabalho trata da biologia deste inseto em folhas de Eucalyptus spp., com a finalidade de trazer subsídios para o caso de eventuais surtos de O. kirbyi em florestas implantadas. O bicho do cesto foi criado em laboratório em temperatura de 25±3°C, umidade relativa de 70± 10% e fotofase de 13 h. Os seguintes valores médios foram obtidos: período embrionário de 43,1 dias, período larval de 140 dias (machos) e 151 dias (fêmeas), período pupal de 38,2 dias (machos), longevidade dos adultos de 3,0 dias (machos) e 3,9 dias (fêmeas) e período de oviposição 2,1 dias. Foram determinadas, também, as dimensões de ovos, pupas, adultos e das cápsulas cefálicas, o número de ínstares larvais, período de cópula, além da estimativa do dano foliar.

Um estudo sobre a flutuação anual da abundância, composição de tamanho dos indivíduos, proporção de sexos, período reprodutivo e de recrutamento dos juvenis de uma população de Uca maracoani (Latreille, 1802-1803) foi realizado no Baixio Mirim, Baía de Guaratuba, Paraná (48º36'W e 25º52'S). Os animais foram coletados mensalmente, de fevereiro/2005 a janeiro/2006, durante as marés baixas de sizígia, e a sua largura da carapaça (LC) medida. A temperatura pontual do ar variou de 17 a 29ºC, a luminosidade de 8.740 a 151.300 lux, a salinidade de 8 a 25 e a temperatura do solo (superfície, 5 cm, 10 cm, 15 cm e 20 cm de profundidade) de 18,3 a 28,9ºC. Foram analisados 7.120 indivíduos, dos quais, 2.578 juvenis sexualmente indefinidos, 2.377 machos e 2.165 fêmeas. A abundância da população variou de 341 (abril) a 994 indivíduos (janeiro), mas a sua flutuação anual não esteve relacionada com a das variáveis abióticas estudadas. A proporção de sexos foi de 1:1 e a reprodução da espécie é do tipo contínuo, com dois picos de intensidade: um em abril e outro em novembro. O recrutamento de juvenis, também, é contínuo com dois períodos mais intensos no ano, um em julho e outro em dezembro-janeiro. A LC da população variou de 1,14 a 2,62 mm para juvenis sexualmente indefinidos, 2,58 a 17,83 mm para machos juvenis, 2,60 a 11,72 mm para fêmeas juvenis, 17,85 a 35,81 mm para machos maduros e 11,75 a 31,76 mm para fêmeas maduras. Os machos atingem tamanhos maiores do que as fêmeas.

Realizou-se levantamento malacológico na praia de Carne de Vaca, município de Goiana, litoral norte de Pernambuco, entre novembro de 2006 e outubro de 2007, com o objetivo de conhecer a fauna malacológica dessa localidade e verificar as condições naturais, pouco ou bastante alteradas das áreas de estudo através da aplicação de um protocolo de avaliação de diversidade de hábitats. Foram coletados 5.912 moluscos, representados por sete espécies e quatro famílias, dos quais, 5.209 exemplares de Biomphalaria glabrata (Say, 1818), 113 de Drepanotrema lucidum (Pfeiffer, 1839), 55 de Drepanotrema cimex (Moricand, 1837), 13 de Drepanotrema anatinum (Pfeiffer, 1839), 222 de Melanoides tuberculatus (Muller, 1774), 263 de Pomacea sp. e 37 de Physa marmorata Guilding, 1828. Entre os exemplares de B. glabrata coletados, 44 mostraram-se positivos para Schistosoma mansoni Sambon, 1907 e 91 mostraram-se positivos para outras larvas de trematódeos. Um exemplar de Pomacea sp. mostrou-se positivo para larva de trematódeo. Os dados obtidos, georreferenciados espacialmente, serão utilizados para a determinação das áreas de risco para a transmissão da esquistossomose na praia de Carne de Vaca, além de simulações computacionais para estudos de previsibilidade e comportamento do processo de expansão da esquistossomose no estado de Pernambuco.

1. In a series of 21 normal cases we found for fatty acids per 100 cc. of plasma an average of 332 mgm., being 314 mgm. for the male sex and 350 mgm. for the female sex. 2.- For lecithin, in four normal cases we found per 100 cc. of plasma 182 mgm. estimated by the contents is phosphorus which ranged from 6.12 to 9.0 mgrs. 3.- Cholesterol in 20 normal cases showed 172 mgm. per 100 cc. of plasma. The averages were 151 mgm. for men and 194 mgm. for women. 4.- The readings of the fractions were 2.01 for the ratio fatty acids divided by lecithins 0.90 for lecithin divided by cholesterol and 1.93 for fatty acids divided by cholesterol. 5.- On comparing the results obtained by us with those reported in foreign literature an absolute conformity is noted chiefly with the values supplied by Bloor and Horiuchi for the ratios among the various lipoid fractions. The average for Cholesterol is comparable with that obtained by Myers but is slightly under that of Bloor's. The lecithin contents found by us did not reach such high values as those supplied by foreign authors.

Relatam-se, no presente trabalho, experiências feitas em hamsters visando observar a transmissão congênita do vírus da gripe. inocularam-se 151 hamsters prenhes ou acasaladas, usando-se a via parenteral ou a nasal. o vírus foi isolado em percentagens variáveis, conforme o caso, sempre acima de 50%, quer das hamsters mães, quer dos filhotes, fetos ou embriões. Observaram-se 15,2% de perturbações embrionárias ou fetais. Processou-se, infecção latente ou inaparente nos animais e os órgãos dos que foram sacrificados não revelaram alterações comuns, macroscópicas ou microscópicas.

A preliminary baseline epidemiological malaria survey was conducted in the village of Punta Soldado, Colombia. Parasite prevalence and density as well as serological data were obtained from 151 asymptomatic children and adults. Fifty individuals were infected with Plasmodium falciparum. The mean parasite density was 184 parasites/mm3. Greater than 90 of the sample population were P. falciparum antibody positive as detected by the indirect immunofluorescent antibody test (IFAT). The enzyme-linked immunosorbent assay (ELISA) was used to detect antibodies against the major merozoite surface protein (MSP-1) of P. falciparum. In this population, anti-MSP-1 antibody concentration is acquired in an age dependent manner with equal immunogenicity to both the N- and C-terminal regions of the molecule. Infection at the time of sampling was associated with a higher anti-MSP-1 antibody concentration than that found in non-infected individuals. Further studies are planned to assess the role of immune and non-immune factors in limiting the number of cases of severe malaria seen in this population.

We studied the stool samples of 151 school children in a district of the city of Portoviejo (Ecuador) in order to determine the prevalence and intensity of soil-transmitted helminthiasis (STH) and their relationships with anthropometric indices. The samples were analyzed with the semiquantitative Kato-Katz technique and the intensity of infections was categorized as light, moderate or high according to the thresholds set by the World Health Organization. Prevalence of soil transmitted helmintiasis was 65% (92 out of 141 collected samples), Ascaris lumbricoides was the most common STH (63%) followed by Trichuris trichiura (10%) and hookworm (1.4%). Heavy intensity infections were found in 8.5% of the stool samples, with T. trichiura showing higher worm burdens than A. lumbricoides. Sixteen percent of the children were below the third percentile for weight (wasted), while 27% were below the third percentile for height (stunted). A significant relationship was found between the worm burden and the degree of stunting. This study suggests that the periodic administration of an antihelminthic drug should be targeted to preschool and school children to allow a normal growth spurt and prevent stunting.

We describe a reverse transcription-polymerase chain reaction (RT-PCR) and a nested-PCR for diagnosis of Piry, Carajás, Cocal, and Alagoas vesiculoviruses from Brazil. The RNA extracts of viral and clinical samples were submitted to a RT-PCR using Vesiculovirus G primers that amplify part of the glycoprotein gene. The RT-PCR produced amplicons of expected size, 290 base pair, for the four studied viruses. The RT-PCR showed a high sensitivity being 151.3 times (2.18 log) more sensitive for the detection of Piry virus than the classical procedure for virus detection in tissue culture based on the viral cytophatic effect. Amplicons had nucleotides sequenced and were aligned in order to select internal primers for a nested-PCR to confirm the origin of Piry, Carajás, Cocal, and Alagoas Vesiculovirus. Ten blood and tarsal pad epithelial samples of infected Guinea-pigs had Vesiculovirus genome amplified by RT-nested-PCR.
