As formas severas de esquistossomose mansônica, particularmente a hépato-esplênica, se acompanham de glomerulopatia geralmente manifesta por síndrome nefrótica. Nos últimos 10 anos reduziu-se muito o número de casos observados desta glomerulopatia no Hospital Universitário Prof. Edgard Santos, um hospital geral num estado onde esta parasitose é endêmica. O objetivo deste estudo foi verificar se estava havendo, de fato, desaparecimento desta patologia ou se este número decrescente refletia apenas diminuição marcante de casos graves de esquistossomose mansônica acompanhados neste Hospital. Para isso foram comparadas as prevalências de glomerulopatia em hepatosplênicos esquistossomóticos autopsiados no Hospital Universitário Prof. Edgard Santos em duas décadas: de 1960-70, antes da intervenção terapêutica nas áreas endêmicas de esquistossomose com oxamniquine, e de 1980-1990, após a adoção desta intervenção. Houve grande redução no número de doentes com hepatosplenomegalia esquistossomótica autopsiados no Hospital Universitário Prof. Edgard Santos quando comparadas a década antes (140 autópsias) e depois (31 autópsias) do tratamento com oxamniquine. A prevalência de glomerulopatia, entretanto, persistiu a mesma (11,4% comparada com 12,9%, respectivamente). A conclusão é que a redução do achado de glomerulopatia esquistossomótica em nossos dias reflete apenas a redução das formas graves da parasitose após a intervenção terapêutica nas áreas endêmicas.

Com o objetivo de investigar a relação entre os níveis de anticorpos (IgG) anti-T. cruzi e a evolução da cardiopatia chagásica crônica, no período de 10 anos, foi realizado um estudo envolvendo soros de 140 pacientes não submetidos ao tratamento específico de Virgem da Lapa, Minas Gerais. Entre os pacientes, 92 são mulheres e 48 homens, com idades ao exame inicial de 10 a 70 anos (média = 38 ±13,7 anos). Os níveis de anticorpos foram estimados pelas médias dos títulos registrados através dos testes de imunofluorescência indireta e hemaglutinação indireta, e pelo índice de reatividade (D.O. da amostra /cut-off) indicado nos testes ELISA convencional e recombinante CRA+FRA. No período, 49 pacientes se mantiveram sem cardiopatia, 29 desenvolveram cardiopatia, 33 mantiveram o grau inicial da cardiopatia, 25 evoluíram com agravamento da cardiopatia inicial e 4 normalizaram o eletrocardiograma. A análise dos resultados de todos os testes sorológicos mostrou aumento estatisticamente significativo dos níveis de IgG anti-T. cruzi no grupo de pacientes com evolução progressiva da cardiopatia principalmente na faixa etária de 20 a 59 anos, independentemente do sexo. Estes resultados indicaram uma associação direta entre os níveis séricos desses anticorpos e a evolução progressiva da cardiopatia chagásica crônica.

Amostras de fezes de 140 cães adultos da Grande São Paulo (Estado de São Paulo, Brasil) foram examinadas com a finalidade de procurar oocistos de Cyclospora sp.. Nenhuma infecção por esse coccídeo ficou detectada.

O estudo utilizou parâmetros ambientais associados aos coeficientes padronizados de leishmaniose tegumentar americana (LTA), no período de 1986 a 1995. Nos 140 municípios com transmissão da doença a pesquisa entomológica realizada mostrou que as espécies mais freqüentes capturadas no ambiente domiciliar, foram Lutzomyia intermedia em 87,1% dos municípios, 53,6% L. whitmani, 49,7% L. migonei, 28,5% L. pessoai e 53,6% L. fischeri. Verificou-se que as variáveis tipos de relevo e de cobertura vegetal natural influíram, significativamente (p < 0,001), nas médias dos coeficientes padronizados de incidência média acumulada da LTA no estado. A análise de regressão linear múltipla mostrou que a incidência da doença esteve associada, significativamente (p = 0,029), à presença de L. migonei nos municípios situados na região geomorfológica do Planalto Atlântico (p = 0,005) e, naqueles cuja cobertura vegetal predominante foi Tipo V - mata (p = 0,000). Esta análise resgata a discussão sobre o papel vetorial de L. migonei no Estado de São Paulo.

In order to determine the prevalence of ocular lesions in HTLV-1 infected patients in Salvador Bahia, a transversal study was conducted on 140 HTLV-1 infected patients (90 asymptomatic and 50 tropical spastic paraparesis/HTLV-1-associated myelopathy) between June 2004 and November 2005. The ophthalmological examination included visual acuity measurement, ocular motility, biomicroscopy of the anterior and posterior chambers, intraocular pressure and evaluation of lachrymal secretion. Observation verified 4 (2.8%) out of 140 patients with uveitis (two patients had intermediate uveitis and two had pan-uveitis) and 39 (36.4%) out of 107 patients with keratoconjunctivitis sicca. The prevalence of Keratoconjunctivitis sicca was significantly higher among the TSP/HAM patients (OR age adjusted=3.64; 95%CI 1.59-8.32). Uveitis and corneal opacities were also important findings, indicating the strong need for periodic ophthalmological follow-up in all HTLV-1 subjects.

INTRODUCTION: HIV-infected children and adolescents treated with highly active antiretroviral therapy (HAART) regimens that include a protease inhibitor (PI) can show significant improvements in clinical outcomes, nutritional status and quality of life. The study aimed to report nutritional and metabolic alterations for pediatric patients continuously exposed to HAART and for healthy controls for up to 1 year. METHODS: Clinical, anthropometric, lipid profile and food intake data were collected prospectively over approximately 12-months for each patient. RESULTS: Fifty-one individuals were studied, of these, 16 were healthy. After 12 months follow-up, HIV-positive individuals remained below the healthy control group parameters. No change was observed concerning food intake. Triglyceride serum levels were higher in patients using protease inhibitor at the onset of the study [PI groups: 114 (43 - 336), and 136 (63 - 271) versus control group: 54.5 (20 - 162); p = 0.003], but after twelve months follow-up, only the group using protease inhibitor for up to two months presented higher values [140 (73 - 273) versus 67.5 (33 - 117); p = 0.004]. HDL-cholesterol was lower in HIV-positive individuals [HIV-positive groups: 36 (27 - 58) and 36 (23 - 43); control 49.5 (34 - 69); p = 0.004]. CONCLUSIONS: HIV-infected children and adolescents treated with highly active antiretroviral therapy showed compromised nutritional parameters compared to a paired healthy control group. Individuals using protease inhibitor presented worse triglyceride serum levels compared to their healthy counterparts.

INTRODUCTION: The correlation between the immunological assay and the antibody titer can offer a tool for the experimental analysis of different phases of the disease. METHODS: Two simple immunological assays for Schistosoma mansoni in mice sera samples based on specific IgG detection for worms soluble antigens and eggs soluble antigens were standardized and evaluated in our laboratory. Fifty mice were used in negative and positive groups and the results obtained by enzyme-linked immunosorbent assays (ELISA) assays were compared with the number of worms counted and the IgG titers at different times of infection. RESULTS: Data showed that ELISA using adult worm antigens (ELISA-SWAP) presented a satisfactory correlation between the absorbance value of IgG titers and the individual number of worms counted after perfusion technique (R²=0.62). In addition, ELISA-SWAP differentially detected positive samples with 30 and 60 days post infection (p=0.011 and 0.003, respectively), whereas ELISA using egg antigens (ELISA-SEA) detected samples after 140 days (p=0.03). CONCLUSIONS: These data show that the use of different antigens in immunological methods can be used as potential tools for the analysis of the chronological evolution of S. mansoni infection in murine schistosomiasis. Correlations with human schistosomiasis are discussed.

INTRODUCTION: Methicillin-resistant Staphylococcus aureus (MRSA) can be difficult to detect at the clinical practice. METHODS: We analyzed 140 MRSA isolates from inpatients to correlate the antimicrobial susceptibility with the SCCmec types. RESULTS: Type III (n = 63) isolates were more resistant to ciprofloxacin, clindamycin, cloramphenicol, erythromycin, gentamicin, and rifampin than type IV (n = 65) ones (p < 0.05). Moreover, type IV isolates were susceptible to tetracycline (100%) and trimethoprim/sulfamethoxazole (98%), while type III isolates presented resistance to them. CONCLUSIONS: In regions where these SCCmec types are prevalent, the detection of specific resistant phenotypes could help to predict them, mainly when there are no technical conditions to SCCmec typing.

IntroductionInfections caused by methicillin-resistant Staphylococcus aureus (MRSA) have become common in hospitals and the community environment, and this wide resistance has limited patient treatment. Clindamycin (CL) represents an important alternative therapy for infections caused by S. aureus. Antimicrobial susceptibility testing using standard methods may not detect inducible CL resistance. This study was performed to detect the phenotypes of resistance to macrolides-lincosamides-streptogramin B (MLSB) antibiotics, including CL, in clinical samples of S. aureusfrom patients at a tertiary hospital in Santa Maria, State of Rio Grande do Sul, Brazil.MethodsOne hundred and forty clinical isolates were submitted to the disk diffusion induction test (D-test) with an erythromycin (ER) disk positioned at a distance of 20mm from a CL disk. The results were interpreted according to the recommendations of the Clinical and Laboratory Standards Institute (CLSI).ResultsIn this study, 29 (20.7%) of the 140 S. aureus samples were resistant to methicillin (MRSA), and 111 (79.3%) were susceptible to methicillin (MSSA). The constitutive resistance phenotype (cMLSB) was observed in 20 (14.3%) MRSA samples and in 5 (3.6%) MSSA samples, whereas the inducible resistance phenotype (iMLSB) was observed in 3 (2.1%) MRSA samples and in 8 (5.8%) MSSA samples.ConclusionsThe D-test is essential for detecting the iMLSBphenotype because the early identification of this phenotype allows clinicians to choose an appropriate treatment for patients. Furthermore, this test is simple, easy to perform and inexpensive.

INTRODUCTION: Visceral leishmaniasis (VL) is a zoonosis of great importance to public health and is considered a neglected disease by the World Health Organization. The disease has expanded and become more prevalent in urban areas in Brazil. METHODS: Geospatial analyses were performed and thematic maps of the triad of the disease were produced for the study period (2003-2012) in the urban area of the municipality of Rondonópolis in the midwestern State of Mato Grosso (MT), Brazil, TerraView 4.2.2 software was used for the analyses. RESULTS: A total of 87.9% of the 186 confirmed human cases of VL were cured. Children between the ages of 1 and 4 were the most affected. Registered deaths were predominant among adults aged 60 years or older. The urban area of the municipality consists of eight strata and 12 census districts include 237 neighborhoods. All sectors had confirmed cases of VL. During the study period, human cases of the disease were recorded in 90 neighborhoods. The 23 deaths from the disease were distributed in 21 neighborhoods. Sandflies carrying the parasite were captured in 192 out of 200 neighborhoods evaluated for the presence of the VL vector. The presence of dogs carrying the parasite was confirmed in, 140 out of 154 surveyed neighborhoods. CONCLUSIONS: The data demonstrated the endemic nature of VL, with a high percentage of infected children, a high distribution of canine infection, and a wide adaptation and dispersal of the vectors in the urban environment. These results, illustrate the process of urbanization of VL in the municipality of Rondonópolis, MT, Brazil.

The treatment of malignant or benign colorectal pathologies that require more complex management are priorities in tertiary hospitals such as "Hospital das Clínicas" University of São Paulo Medical Center (HCFMUSP). Therefore, benign, uncomplicated orifice conditions are relegated to second place. The number of patients with hemorrhoids, perianal fistulas, fissures, condylomas and pilonidal cysts who seek treatment at the HFMUSP is very great, resulting in over-crowding in the outpatient clinics and a long waiting list for recommended surgical treatment (at times over 18 months). The authors describe the experience of the HCFMUSP over an eight-day period with day-hospital surgery in which 140 patients underwent surgery. Data was prospectively taken on the patients undergoing surgery for benign orifice pathologies including age, sex, diagnosis, surgery performed, immediate and late postoperative complications, and follow-up. 140 patients operated on over eight days were studied. 68 were males (48.75%) with ages ranging from 25 to 62 (mean 35.2 yrs.). Hemorrhoids was the most frequent condition encountered (82 hemorrhoidectomies, 58.6%), followed by perineal fistula (28 fistula repairs, 20.0%). The most common complication was headache secondary to rachianesthesia occurring in 9 patients (6.4%). One patient (0.7%) developed bleeding immediately PO that required reoperation. Mean follow-up was 104 days. Day-surgery characterized by quality care and low morbidity is feasible in tertiary public hospitals, permitting surgery for benign orifice pathologies on many patients within a short period of time.

PURPOSE: To find out the prevalence of hypertension in employees of the Hospital and relate it to social demographic variables. METHODS: Blood pressure measurement was performed with a mercury sphygmomanometer, using an appropriate cuff size for arm circumference, weight, and height in a population sample of 864 individuals out of the 9,905 employees of a University General Hospital stratified by gender, age, and job position. RESULTS: Hypertension prevalence was 26% (62% of these reported being aware of their hypertension and 38% were unaware but had systolic/diastolic blood pressures of >140 and/or >90 mm Hg at the moment of the measurement). Of those who were aware of having hypertension, 51% were found to be hypertensive at the moment of the measurement. The prevalence was found to be 17%, 23%, and 29% (P <.05) in physicians, nursing staff, and "others", respectively. The univariate analysis showed a significant odds ratio for the male gender, age >50 years, work unit being the Institute of Radiology and the Administration Building, educational level 10 years, and body mass index >30 kg/m². The multivariate logistic regression model revealed a statistically significant association of hypertension with the following variables: gender, age, skin color, family income, and body mass index. CONCLUSIONS: Hypertension prevalence was high, mainly in those who were not physicians or members of the nursing staff. High-risk groups (obese, non-white, men, low family income) should be better advised of prevention and early diagnosis of hypertension by means of special programs.

Estudaram-se de agosto de 1977 a junho de 1979 16 espécies de Psitacídeos na região do Núcleo Pioneiro Humboldt (10019' S, 590 12' W), alto rio Aripuanã, MT, Brasil. Verificou-se que diferentes tamanhos de corpo e peso dão acesso a espectros alimentares diferentes. Com respeito ao peso, as 16 espécies podem ser divididas em quatro grupos:55 até 110g: Tuit huetii, Pyrrhura picta, Brotogeris chrysopterus, Pyrrhura rhodogaster, Aratinga weddellii.140 até 300g: Aratinga leucophthalmus, Pionopsitta barrabandi, Pionus menstruus, Deroptyus accipitrinus.360 até 600g: Ara severa, Ara manilata, Amazona ochrocephala, Amazona farinosa.950 até 1350g: Ara ararauna, Ara macao, Ara chloroptera.As estratégias alimentares variam dentro de cada grupo de peso: Ara manilata é especialista puro, Tuit huetti, Brotogeris chrysopterus, Pionopsitta barrabandi, Ara severa e Ara ararauna são especialistas parciais. Os especialistas parciais têm bicos relativamente compridos e estreitos. Com respeito às proporções do bico, Brotogerischrysopterus é mais especializado do que Tuit huetii no mesmo grupo de peso.As espécies muito próximas morfologicamente, como Ara chloroptera e Ara macao ou Pyrruhura rhodogaster e Pyrrhura picta, podem ser reconhecidas principalmente pelo modo com que exploram o habitat. Ara chloroptera se encontra nas estratos superiores da floresta, junto às copas de árvores muito altas, com maior freqüência do que Ara macao. Pyrrhura rhodogaster visita mais freqüentemente matas densas e vegetação secundária do que Pyrrhura picta. Diferenças sazonais de abundância e épocas diferentes de reprodução separam as duas espécies de Amazona. Amazona ochrocephala é mais comum e inclusive cria os seus filhotes na época seca, enquanto que Amazona farinosa reproduz e se torna mais comum na época chuvosa.

Este estudo teve como objetivo averiguar o conhecimento dos alunos da 5a série das escolas públicas estaduais de Manaus sobre a floresta amazônica e seus recursos. As entrevistas foram realizadas em 14 escolas, com o auxílio de um questionário padrão contendo perguntas abertas. De cada escola foram entrevistados 10 alunos, perfazendo um total de 140 no final da pesquisa. Os resultados mostraram a necessidade de enfatizar nos currículos escolares, assuntos sobre a região amazônica.

Objetivou-se com o presente trabalho estudar a cinética da secagem de quatro clones de café da espécie Coffea canephora submetidos à secagem em terreiro de chão batido, bem como ajustar diferentes modelos matemáticos aos valores experimentais selecionando aquele que melhor representa o fenômeno em estudo. Foram utilizados frutos de café dos clones: Cpafro 194, Cpafro 193, Cpafro 167 e Cpafro180, colhidos com os teores de água iniciais de 1,20; 1,32; 1,51 e 1,46 (decimal base seca (b.s.)), respectivamente. A secagem prosseguiu em terreiro de chão batido até que o produto atingisse o teor de água de 0,137; 0,133; 0,142 e 0,140 (decimal b.s.) respectivamente para os clones Cpafro 194, Cpafro 193, Cpafro 167 e Cpafro 180. Aos dados experimentais foram ajustados dez modelos matemáticos citados na literatura específica e utilizados para representação do processo de secagem de produtos agrícolas. Baseando-se em parâmetros estatísticos, conclui-se que os modelos Verma, Dois Termos e Aproximação da Difusão foram adequados para representação da secagem dos quatro clones de café analisados, e além destes, para o clone Cpafro 167, os modelos Thompson, Page, Newton, Logarítmico, Henderson e Pabis e Exponencial de Dois Termos também se mostraram satisfatórios na descrição do fenômeno; já o tempo necessário para a secagem em terreiro de chão batido dos clones de café Cpafro 194, Cpafro 193, Cpafro 167 e Cpafro 180 foi de 189,5 h.
