Porphyria cutanea tarda (PCT) is caused by inherited or acquired partial deficiency of the uroporphyrinogen-decarboxylase (Uro-D) enzyme activity. It is the most common form of porphyria. The main triggering factors to the development of porphyria cutanea tarda are alcohol, hepatitis C virus and human immunodeficiency virus. There are several reports of PCT associated with drugs, among them, antiretroviral therapy. We describe three HIV-positive patients, which showed photosensitivity as well as the emergence of tense blisters on sun-exposed areas during the use of highly active antiretroviral therapy (HAART) and discuss the possibility of PCT after the use of these drugs by those patients.

Introduction This study compares virulence markers of Helicobacter pylori isolated from patients in 2 cities in the Brazilian Amazon. Methods The study analyzed 168 patients with chronic gastritis from Belém and 151 from Bragança, State of Pará, Brazil. Levels of bacterial DNA associated with cagA and vacA alleles were checked by PCR, and hematoxylin-eosin staining was used for histologic diagnosis. Results In Bragança 87% of patients were genotype s1m1 cagA-positive (s1m1 cagA+), compared with 76% in Belém. In samples from patients in both cities, there was an association between s1m1 cagA+ strains and gastric mucosal damage. Conclusions Both cities have a high frequency of s1m1 cagA+ strains of H. pylori.

Introduction Dogs play a primary role in the zoonotic cycle of visceral leishmaniasis (VL). Therefore, the accurate diagnosis of infected dogs, primarily asymptomatic dogs, is crucial to the efficiency of VL control programs. Methods We investigated the agreement of four diagnostic tests for canine visceral leishmaniasis (CVL): parasite detection, either after myeloculture or by direct microscopic examination of tissue imprints; kinetoplast-deoxyribonucleic acid-polymerase chain reaction (kDNA-PCR); and an immunochromatographic test (ICT). An enzyme-linked immunosorbent assay (ELISA) and an indirect immunofluorescence test (IFAT), both of which were adopted as part of the screening-culling program in Brazil, were used as reference tests. Our sample set consisted of 44 seropositive dogs, 25 of which were clinically asymptomatic and 19 were symptomatic for CVL according to ELISA-IFAT. Results The highest and lowest test co-positivities were observed for ICT (77.3%) and myeloculture (58.1%), respectively. When analyzed together, the overall percentage of co-positive tests was significantly higher for the symptomatic group compared to the asymptomatic group. However, only ICT was significantly different based on the results of a separate analysis per test for each group of dogs. The majority (93.8%) of animals exhibited at least one positive test result, with an average of 2.66 positive tests per dog. Half of the symptomatic dogs tested positive for all four tests administered. Conclusions The variability between test results reinforces the need for more efficient and reliable methods to accurately diagnose canine VL, particularly in asymptomatic animals.

OBJECTIVES: To determine the presence of immunoglobulin E-rheumatoid factor in patients with juvenile rheumatoid arthritis and to correlate it with clinical and laboratory parameters. METHODS: A multicenter prospective study was carried out from January 1993 to January 1999 with the enrollment of 3 centers of pediatric rheumatology. Ninety-one children with juvenile rheumatoid arthritis diagnosed according to the American College of Rheumatology criteria were studied: 38 (42%) with systemic, 28 (31%) with pauciarticular, and 25 (27%) with polyarticular onset. Ages ranged from 2.1 years to 22.6 years (mean 10.5 ± 4.7), with 59 (65%) girls. The control group consisted of 45 healthy children. The detection of immunoglobulin E-rheumatoid factor was carried out utilizing an enzyme-linked immunosorbent assay. Associations of immunoglobulin E-rheumatoid factor with immunoglobulin M-rheumatoid factor (latex agglutination test), total serum immunoglobulin E, erythrocyte sedimentation rate, antinuclear antibody, and functional and radiological classes III or IV were analyzed. RESULTS: Positive immunoglobulin E-rheumatoid factor was found in 15 (16.5%) of the 91 children with juvenile rheumatoid arthritis: 7 (18.5%) with systemic, 5 (18%) with pauciarticular, and 3 (12%) with polyarticular onset. A significant correlation was observed between immunoglobulin E-rheumatoid factor and total serum immunoglobulin E in the juvenile rheumatoid arthritis patients. No correlation was found between immunoglobulin E-rheumatoid factor and positive latex agglutination slide test, erythrocyte sedimentation rate, antinuclear antibody, or the functional and radiological classes III or IV in any disease onset group. In 4 out of 45 control children (8.9%), immunoglobulin E-rheumatoid factor was positive but with no correlation with total serum immunoglobulin E levels. CONCLUSIONS: Immunoglobulin E-rheumatoid factor could be detected in 16.5% of juvenile rheumatoid arthritis patients, particularly in those with high levels of total serum immunoglobulin E, and immunoglobulin E-rheumatoid factor appears not to be associated with disease activity or severity.

Algumas características de farinha de mandioca dependem da variedade da raiz utilizada para o processamento. Objetivou-se avaliar as características físico-químicas da farinha de mandioca oriundas de variedades utilizadas no estado do Acre. Os tratamentos foram compostos pelas variedades: T1= Paxiubão, T2= Im221, T3 = Caboquinha, T4 = Araçá, T5 = Colonial, T6 = Branquinha, T7 = Panati e T8 = Mansa e Brava. Foram coletadas amostras das oito variedades de mandioca em casas-de-farinha no município de Cruzeiro do Sul, Acre. As raízes foram transportadas via aérea para o Laboratório de Tecnologia de Alimentos da Embrapa-AC, na cidade de Rio Branco, Acre. Foram avaliados: teor de umidade, cinzas, lipídios, proteína, fibra bruta, carboidratos, acidez, pH e atividade de água. Todas as amostras se apresentaram de acordo com os padrões estabelecidos pela Legislação Brasileira para farinha de mandioca quanto ao teor de umidade, cinzas e carboidratos. As farinhas analisadas apresentam teores baixos de fibras e baixa acidez. A atividade de água das farinhas analisadas esteve abaixo do limite mínimo capaz de permitir o desenvolvimento de microrganismos. As variedades Araçá, Colonial e Branquinha se mostraram adequadas para a fabricação de farinha de mandioca devido, principalmente, ao elevado teor de proteína e carboidratos presente nas farinhas produzidas.

Este trabalho teve como objetivo avaliar a qualidade de castanhas-do-brasil beneficiadas e comercializadas em Rio Branco, Acre. Foram analisadas amostras das três marcas de castanha encontradas no mercado local quanto às variáveis: atividade de água, teor de umidade, contagem total de fungos filamentosos, quantificação de Aspergillus flavus e de A. parasiticus, bem como quantificação de aflatoxinas B1, B2, G1 e G2. As castanhas do comércio se encontravam com um teor de umidade e atividade de água adequados, o que pode ter sido responsável pela baixa contaminação por fungos e por aflatoxinas. Quanto a estas micotoxinas, as amostras estão de acordo com o recomendado pela Anvisa, podendo ser esta uma consequência da grande divulgação no Estado do uso de Boas Práticas no manejo da castanha.

OBJETIVO: Analisar a experiência inicial no fechamento percutâneo da comunicação interatrial ostium secundum (CIA OS) com a prótese de Amplatzer. MÉTODOS: Sete pacientes foram submetidos ao procedimento através da via venosa anterógrada, orientados pela ecocardiografia transesofágica (ETE) e sob anestesia geral. Uma criança era portadora de 2 CIA e de canal arterial (CA). As CIA medidas pelo ETE variaram de 8,7 a 20mm. Um ecocardiograma transtorácico foi realizado na manhã seguinte do procedimento. RESULTADOS: Oito próteses foram implantadas nos 7 pacientes com sucesso. Em um paciente, o CA foi ocluído na mesma sessão com mola de Gianturco, tendo surgido taquicardia supraventricular durante a oclusão de uma das CIA, controlada com adenosina. Todos receberam alta hospitalar na manhã seguinte, com oclusão total dos defeitos. CONCLUSÃO: O procedimento mostrou-se seguro, eficaz e versátil, podendo ser considerado como uma alternativa terapêutica inicial em pacientes selecionados com CIA OS.

OBJETIVO: Avaliar a experiência global da nossa instituição com o fechamento percutâneo do canal arterial. MÉTODOS: Desde dezembro/92, 150 pacientes foram submetidos a 178 procedimentos (85 - técnica de Rashkind; 87 - coils de Gianturco; 6 - coils Duct Occlud). A mediana de idade foi de 6,5 anos (1 a 57). A média do diâmetro mínimo do canal foi de 3,05±1,24mm (1 a 8). Avaliações clínicas e ecocardiográficas foram realizadas seriadamente. RESULTADOS: Implantes adequados ocorreram em 143 (95,3%) pacientes. A prevalência de shunt residual imediato foi de 52,1%, decrescendo para 15,9% no seguimento. Esta taxa caiu para 5,2% após a realização de procedimentos adicionais. Embolização de coils ocorreu em 12 procedimentos e de umbrella em 1. Um paciente apresentou hemólise e outro estenose discreta da artéria pulmonar esquerda. Não houve mortalidade. CONCLUSÃO: Esta modalidade terapêutica é segura e eficaz, proporcionando bons resultados a longo prazo.

We report new percutaneous techniques for perforating the pulmonary valve in pulmonary atresia with intact ventricular septum, in 3 newborns who had this birth defect. There was mild to moderate hypoplastic right ventricle, a patent infundibulum, and no coronary-cavitary communications. We succeeded in all cases, and no complications related to the procedure occurred. The new coaxial radiofrequency system was easy to handle, which simplified the procedure. Two patients required an additional source of pulmonary flow (Blalock-Taussig shunt) in the first week after catheterization. All patients had a satisfactory short-term clinical evolution and will undergo recatheterization within 1 year to define the next therapeutic strategy. We conclude that this technique may be safely and efficiently performed, especially when the new coaxial radiofrequency system is used, and it may become the initial treatment of choice in select neonates with pulmonary atresia and intact ventricular septum.

This report concerns a patient with articular and cardiac manifestations of Whipple's disease. The disease was diagnosed only when gastrointestinal symptoms had appeared, because all cardiac symptoms were attributed exclusively to myocardial bridging. After 18 months of treatment with trimethoprim-sulfamethoxazole, the patient is fully asymptomatic with a normal echocardiogram.

OBJECTIVE: To report short and midtem follow-up results of balloon aortic valvuloplasty to treat congenital aortic stenosis. METHODS: Seventy-five patients (median age: 8 years) underwent the procedure through the retrograde femoral or carotid route. RESULTS: The procedure was completed in 74 patients (98.6%). The peak-to-peak systolic gradient dropped from 79.6±27.7 to 22.3±17.8 mmHg (P<0.001), the left ventricular systolic pressure dropped from 164±39.1 to 110±24.8 mmHg (P<0.001), and the left ventricular end diastolic pressure dropped from 13.3±5.5 to 8.5±8.3 mmHg (P< 0.01). Four patients (5.3%) died due to the procedure. Aortic regurgitation (AoR) appeared or worsened in 27/71 (38%) patients, and no immediate surgical intervention was required. A mean follow-up of 50±38 months was obtained in 37 patients. Restenosis and significant AoR were observed in 16.6% of the patients. The estimates for being restenosis-free and for having significant AoR in 90 months were 60% and 50%, respectively. CONCLUSION: Aortic valvuloplasty was considered the initial palliative method of choice in managing congenital aortic stenosis, with satisfactory short- and midterm results.

OBJETIVO: Determinar a incidência de fístulas arteriovenosas pulmonares (FAVPs) pós-operação de Glenn bidirecional; as possíveis variáveis independentes que poderiam influenciar no seu aparecimento; comprovar o uso do ecocardiograma contrastado com microbolhas na sua detecção; e testar a sensibilidade e especificidade da angiografia pulmonar. MÉTODOS: Operados 59 pacientes, entre março 1990 e dezembro 1995, com idades entre 2 e 132 meses (média 32,7±33,6). Todos submetidos a exames clínico, laboratorial, ecocardiograma contrastado com microbolhas e cateterismo cardíaco. RESULTADOS: Nos 54 pacientes sobreviventes as FAVPs ocorreram em 20 (37,0%). A idade desses pacientes variou de 2 a 132 meses (média 29,6± 29,7). A atresia tricúspide em 10 (50,0%) e o coração univentricular em 8 (40,0%), foram as cardiopatias prevalentes. Em 13 (65%) a operação de Glenn bidirecional foi realizada à direita, em 2 (10,%), à esquerda e em 5 (25%) foi bicaval. O tempo de seguimento dos pacientes FAVPs, variou de 4 a 84 meses (média 32,4±21,65), e nos sem fístulas de 1 a 77 meses (média de 23,4±18,8), com valor de p=0,04, com significância estatística. O diagnóstico de FAVPs foi feito pelo ecocardiograma contrastado com microbolhas nos 20 casos, sendo considerado positivo, ao se detectar retorno de microbolhas pelas veias pulmonares. A angiografia pulmonar mostrou alterações compatíveis com FAVPs em 16, demonstrando sensibilidade de 80%. CONCLUSÃO: A incidência de FAVPs pós-operação de Glenn bidirecional foi alta (37%), e o intervalo de tempo decorrido após a operação de Glenn bidirecional, foi a única variável independente que se correlacionou de forma significativa com o aparecimento das FAVPs (p=0,04); o ecocardiograma com microbolhas foi o método padrão de diagnóstico; a angiografia pulmonar se revelou um método com sensibilidade de 80,0%.
