Our objective was to evaluate the concentrations of serum 25-hydroxyvitamin D [25(OH)D], serum calcium, serum phosphorus, alkaline phosphatase, and parathormone (PTH) in patients with polyarticular juvenile idiopathic arthritis (JIA) and to associate them with disease duration and activity, bone mineral density and use of medications. In a cross-sectional and controlled study, 30 patients with polyarticular JIA were evaluated and compared to 30 healthy individuals matched for age and gender. Clinical status, anthropometry, laboratory markers in both patients and controls, and bone mineral density, only in the patients, were measured. Of the 30 patients included in the study, 23 (76.7%) were female and 16 (53.3%) non-Caucasian; mean age was 14 years (range = 4 to 20 years). Mean disease duration was 5 years (range = 1 to 12 years). The mean concentrations of serum albumin-corrected calcium (9.04 ± 0.41 mg/dL) and alkaline phosphatase (153.3 ± 100.1 IU) were significantly lower in patients with JIA than in controls (P < 0.0001 and P = 0.001, respectively). No differences in 25(OH)D, PTH or serum phosphorus were observed between JIA and control subjects. Regarding 25(OH)D concentration, 8 patients (26.7%) and 5 controls (16.7%) had 25(OH)D concentrations compatible with deficiency (lower than 20 ng/mL) and 14 patients (46.7%) and 18 controls (60%) had concentrations compatible with insufficiency (20-32 ng/mL). These values were not associated with disease activity, use of medications or bone mineral density. We observed a high frequency of 25(OH)D insufficiency and deficiency in the study sample. The compromised bone metabolism emphasizes the importance of follow-up of JIA patients.

Subjects with chronic liver disease are susceptible to hypovitaminosis A due to several factors. Therefore, identifying patients with vitamin deficiency and a requirement for vitamin supplementation is important. Most studies assessing vitamin A in the context of hepatic disorders are conducted using cirrhotic patients. A cross-sectional study was conducted in 43 non-cirrhotic patients with chronic hepatitis C to evaluate markers of vitamin A status represented by serum retinol, liver retinol, and serum retinol-binding protein levels. We also performed the relative dose-response test, which provides an indirect estimate of hepatic vitamin A reserves. These vitamin A indicators were assessed according to the stage of liver fibrosis using the METAVIR score and the body mass index. The sample study was predominantly composed of male subjects (63%) with mild liver fibrosis (F1). The relative dose-response test was <20% in all subjects, indicating vitamin A sufficiency. Overweight or obese patients had higher serum retinol levels than those with a normal body mass index (2.6 and 1.9 µmol/L, respectively; P<0.01). Subjects with moderate liver fibrosis (F2) showed lower levels of serum retinol (1.9 vs 2.5 µmol/L, P=0.01) and retinol-binding protein levels compared with those with mild fibrosis (F1) (46.3 vs 67.7 µg/mL, P<0.01). These results suggested an effect of being overweight on serum retinol levels. Furthermore, more advanced stages of liver fibrosis were related to a decrease in serum vitamin A levels.

ABSTRACT The analytical determination of nutrient levels in recently mature leaves in order to diagnose nutritional status is based on the fact that leaves are metabolically active and more sensitive to variation in nutrients of the soil. In most of cases, there is a direct well known between foliar content and the development and yield of the plant. However, for a more accurate interpretation, it is essential to establish the index leaf. There are few published studies about Jatropha with contrasting results. In order to establish the index leaf, in adult plants, the macronutrient levels were evaluated in samples collected in experimental plots, in which doses of nitrogen and phosphorus were applied, in two parts of the floral branches (in the top and in the middle thirds); and in three positions of leaves of the floral branch (between the 1st and 3rd, 6th and 8th, and 13th and 15th leaves below the inflorescence). The location of the leaf on the plant significantly affects nutrient contents. Nitrogen, phosphorus, potassium and sulfur tend to have higher concentration in young tissues. Calcium and magnesium showed higher levels in the basal leaves of floral branches. Samples collected in the top third of plants (between the 6th and 15th leaves of the floral branch) are more sensitive to variations of nitrogen and phosphorus fertilization. Therefore, we indicate the 6th to 15th leaves of the top third plants as index leaves estimate nutritional status of Jatropha.

Trata-se, neste artigo, de examinar o processo de legitimação que acompanhou a conquista do sertão baiano durante a segunda metade do século XVII, e de explorar as interações entre as dinâmicas locais e imperiais que levaram a uma situação de violência institucionalizada contra as populações indígenas do interior da Bahia.

A partir de uma modificação do esquema teórico de Max Weber, aplicado ao estudo do fenômeno da estratificação social, procurou-se a base conceptual para a elaboração de um índice de status sócio-econômico familiar. Selecionados os indicadores ocupação, renda e educação, consagrados pela literatura sociológica, os pesos respectivos foram obtidos através de um critério estatístico (correlação de cada variável com o "score" total). Cada status familiar, representado por um "score" total, encontra-se referido a um estrato no sistema de estratificação considerado.

De 3.200 "manchas" provenientes de insetos dos gêneros Triatoma e Panstrogylus, positivas para sangue humano, 12 deram resultados presuntivamente positivos, por imunodifusão, para HBsAg. Deste total, só 7 casos foram confirmados como positivos, por radioimunoensaio, correspondendo todos eles a "manchas" obtidas de ninfas de Triatoma infestans.

Considering that in most developing countries there are still no comprehensive lists of addresses for a given geographical area, there has always been a problem in drawing samples from the community, ensuring randomisation in the selection of the subjects. This article discusses the geographical stratification by socio-economic status used to draw a multistage random sample from a community-based elderly population living in a city like S. Paulo - Brazil. Particular attention is given to the fact that the proportion of elderly people in the total population of a certain area appeared to be a good discriminatory variable for such stratification. The validity of the stratification method is analysed in the light of the socio-economic results obtained in the survey.

Foram rastreados 1.033 primodoadores de sangue e 201 prisioneiros, para a infecção pelo vírus da hepatite B (VHB), durante o período de junho de 1988 a fevereiro de 1989, em Goiânia, GO (Brasil). Foi estimada a soroprevalência dos marcadores AgHBs e anti-HBs, e estudados os fatores de risco associados a soropositividade. Os testes sorológicos foram realizados pela técnica de ELISA e a presença de qualquer dos marcadores estudados foi interpretada como exposição ao vírus da hepatite B. Um questionário padronizado foi aplicado em ambos os grupos populacionais para avaliar: transfusão sangüínea prévia, número de parceiros sexuais, atividade homo/bissexual, história de outras doenças sexualmente transmissíveis, uso de droga injetável, uso de medicação injetável, acupuntura, tatuagem e soropositividade ao VDRL. Foram obtidas soroprevalências globais para a infecção pelo VHB de 26,4% e 12,8% para a população carcerária e de primodoadores respectivamente, diferença estatisticamente significante (p<0,05), observando-se tendência crescente da soropositividade com a idade (X² para tendência = 14,0, p<0,05). A população carcerária apresentou maiores percentuais de exposição a todos os fatores de risco quando comparada aos primodoadores, a exceção do número de parceiros sexuais. Grupo etário, encarcera-mento e presença de tatuagem foram os fatores de risco estatisticamente significantes associados a soropositividade, mesmo após análise multivariada controlada por idade e encarceramento. Foram discutidas as dificuldades metodológicas que poderiam ter influenciado nos resultados.

Levantamento sorológico realizado em 200 estudantes da Universidade de São Paulo, nos anos de 1984 e 1985, demonstrou ampla prevalência sorológica do vírus da influenza tipos A e B. Os anticorpos dos indivíduos foram detectados pela técnica de Hemólise Radial Simples (HRS), cujas médias aritméticas de títulos foram maiores entre as cepas dos subtipos (H1N1) e (H3N2) do vírus da influenza tipo A, mais recentemente isoladas da população. Porém, com relação ao tipo B, deste vírus, a situação foi inversa, pois apesar da cepa B/Engl./ 847/73 ser a mais antiga incidente, revelou melhor reatogenicidade sobre as demais cepas avaliadas e de acordo com a doutrina do "Pecado original antigênico", é suposto que tenha sido responsável pela primo infecção na maioria do grupo investigado. A avaliação sorológica dos subtipos do vírus influenza tipos A e B, desta população, revelou índices de anticorpos de baixos títulos HRS (2,5 a 3,5 mm) e de altos títulos (> 4,0 mm) que estão relacionadas ao menor e maior nível de proteção à infecção. Sendo que a capacidade individual da imunidade e da persistência de anticorpos contra o vírus, dependeram da atualidade e freqüência de exposição à influenza.

Results obtained with Anopheles albitarsis s.l. catches mainly performed through human bait at the Ribeira Valley region, SP (Brazil), are reported. Two species of the complex were recognized, namely An. albitarsis s.s. and species B. This latter predominated both in the rice fields and in the dwelling environments. The crepuscular rhythms showed an unimodal sunset pattern with most blood-seeking females caught during dusk. The absence of differences between indoor and outdoor behavior was confirmed for both species of the complex.

OBJECTIVE: To compare HIV seronegative (HIV-) and HIV seropositive (HIV+) males in terms of sexual behavior with female and male partners of different types. METHOD: Cross-sectional study. From August 1994 to February 1995, a sample of 236 respondents (150 HIV- and 86 HIV+) recruited from public health centers in the State of S. Paulo (Brazil), answered a questionnaire, including questions on demographic aspects, HIV and AIDS related knowledge, sexual orientation, use of alcohol and other drugs, sexual behavior with regular and casual female and male partners, and perceived risk of HIV infection. Sexual behavior with regular and casual female and male partners within the previous three months, was investigated. RESULTS: A lower proportion of HIV+ engaged in sexual contact with regular female partners (p < .01) and in vaginal intercourse with this type of partner (p < .01). A lower proportion of HIV+ engaged in overall sexual activity (p < .001) and reported lower frequency of penetrative sexual practices (p < .05). A high level of condom use with female and male partners was identified with no significant differences being found between the two serostatus groups. Some risky sexual behavior was identified, however, especially with regular partners, suggesting that some men were continuing to practice unsafe sex. CONCLUSIONS: The high level of condom use identified suggests that safer sex advice has been taken up. Condom use was not universal, however, and some men continue to place themselves at risk, especially with regular partners. Prevention programs should strive not only to encourage HIV- to practice safer sex, but also to encourage HIV+ to do so in order to prevent further transmission of the virus.

INTRODUCTION: Although obesity is well recognized as a current public health problem, its prevalence and impact among pregnant women have been less investigated in Brazil. The objective of the study was to evaluate the impact of pre-obesity and obesity among pregnant women, describing its prevalence and risk factors, and their association with adverse pregnancy outcomes. METHODS: A cohort of 5,564 pregnant women, aged 20 years or more, enrolled at aproximately 20 to 28 weeks of pregnancy, seen in prenatal public clinics of six state capitals in Brazil were followed up, between 1991 and 1995. Prepregnancy weight, age, educational level and parity were obtained from a standard questionnaire. Height was measured in duplicate and the interviewer assigned the skin color. Nutritional status was defined using body mass index (BMI), according to World Health Organization (WHO) criteria. Odds ratios and 95% confidence interval were calculated using logistic regression. RESULTS: Age-adjusted prevalences (and 95% CI) based on prepregnancy weight were: underweight 5.7% (5.1%-6.3%), overweight 19.2% (18.1%-20.3%), and obesity 5.5% (4.9%-6.2%). Obesity was more frequently observed in older black women, with a lower educational level and multiparous. Obese women had higher frequencies of gestational diabetes, macrosomia, hypertensive disorders, and lower risk of microsomia. CONCLUSIONS: Overweight nutritional status (obesity and pre-obesity) was seen in 25% of adult pregnant women and it was associated with increased risk for several adverse pregnancy outcomes, such as gestational diabetes and pre-eclampsia.

OBJETIVO: Estimar a prevalência das infecções pelo HIV, vírus das hepatites B e C, e da sífilis em moradores de rua. MÉTODOS: Estudo transversal com intervenção educativa, realizado no município de São Paulo, de 2002 a 2003. Selecionou-se amostra de conveniência de moradores de rua que utilizavam albergues noturnos, segundo os critérios: >18 anos e não apresentar distúrbios psiquiátricos. Em entrevistas, foram coletados dados sociodemográficos e de comportamento, e realizados exames laboratoriais para HIV, hepatite B e C e sífilis, e aconselhamento pós-teste. RESULTADOS: Participaram 330 usuários dos albergues, com 40,2 anos (média), 80,9% homens, nas ruas, em média, há um ano. Observaram-se prevalências de 1,8% de HIV, 8,5% de vírus de hepatite C, 30,6% de infecção pregressa por hepatite B, 3,3% de infecção aguda ou crônica pelo vírus hepatite B e 5,7% de sífilis. Uso consistente de preservativo foi referido por 21,3% e uso de droga injetável, por 3% dos entrevistados. A positividade para HIV foi de 10% e 50% para vírus da hepatite C entre usuários de drogas injetáveis, versus 1,5% para HIV e 7,3% para hepatite C nos demais, evidenciando associação entre esse vírus e uso de droga injetável. Prisão anterior foi referida por 7,9% das mulheres e 26,6% dos homens, com prevalência de 2,6% para HIV e 17,1% para vírus da hepatite C. CONCLUSÕES: As elevadas prevalências de HIV e vírus de hepatite B e C requerem programas de prevenção baseados na vacinação contra hepatite B, diagnóstico precoce dessas infecções e inserção dos moradores de rua em serviços de saúde.
