The aim of this study was to test if serological distinction between patients with active and inactive neurocysticercosis (NCC), could be accomplished by the recognition of immunodominant peptides in total saline antigenic extract of Taenia solium metacestodes by IgG antibody in cerebrospinal fluid (CSF) and serum paired samples. CSF and serum samples of 10 each, active NCC patients, inactive NCC, and individuals with other neurological disorders, were used to recognize the antigenic peptides by western blot (WB). In the active NCC the 28-32 and 39-42 kDa peptides were more frequently detected in CSF than in sera (p < 0.05). The 47-52, 64-68, and 70 kDa antigens showed high frequencies in both samples from patients with active NCC. All the CSF samples of inactive NCC and other neurological disorder (control) patients tested negative, while serum samples from these last two groups recognized mainly the 80, 86, 95, and 98 kDa bands. This finding eliminates the use of the high molecular weigh bands (> 80 kDa) for diagnosis of NCC. The final conclusions were that the difference between active and inactive NCC may be done with the detection of peptides only in the CSF samples and that the 47-52, 64-68, and 70 kDa bands may be included as specific markers for active NCC when detected in CSF samples by WB using total saline extract of T. solium metacestode.

Seven swine were experimentally infected with Taenia solium eggs and blood samples from each animal were periodically collected. At the end of the experiment (t140) the animals did not show clinical aspects of cysticercosis or parasites in tongue inspection. All animals were slaughtered and cut into thin slices in searching for cysts. The number of cysts found in each animal varied from 1 to 85. Enzyme-linked immunosorbent assay (ELISA) tests for antibody (Ab) detection and for antigen (Ag) detection were performed, which presented respectively 71 and 57% of positivity. By immunoblot (IB), using 18/14(T. crassiceps Ag) or lentil-lectin-purified glycoproteins from T. solium Ag (LLGP) as Ag, five (71%) and six (86%) animals were positive, respectively. The association between Ag-ELISA with any IB (18/14 or LLGP) allowed the detection of all animals at 140 days post-experimental infection (days p.e.i.). The use of IB 18/14 combined to the Ag-ELISA allowed the detection of all animals since 70 days p.e.i., and the association between IB LLGP and Ag-ELISA allowed the detection of all animals since 112 days p.e.i. While all animals could be considered healthy by conventional screening tests, the use of immunoassays for detecting Ab and Ag showed better accuracy; therefore it would be more useful than usual clinical examination for screening cysticercosis in slightly infected pigs.

The aim of the present study is to investigate genetic polymorphisms in Taenia solium metacestodes from different Brazilian geographical areas and to relate them to antibody recognition in serum samples of neurocysticercosis (NC) patients. Metacestodes were obtained from the Distrito Federal (DF), Bahia, Minas Gerais (MG) and São Paulo (SP) regions of Brazil. Samples of human sera from 49 individuals with NC, 68 individuals with other helminthiasis and 40 healthy volunteers were analysed (157 individuals in total). Antigens were prepared and used in enzyme-linked immunosorbent assay and western blotting assays to detect specific immunoglobulin G antibodies. Genetic distances between metacestode populations were analysed using random amplified polymorphic DNA (RAPD) analysis. Our results show that there was a higher frequency of reactivity in the DF region in the sera from NC patients (p < 0.05), while discrimination between active and inactive NC was seen only in extracts from the MG and SP regions (p < 0.05). Using RAPD, the sample from the DF region presented a greater increase compared to the other regions. A relationship between genetic polymorphisms among T. solium metacestodes from different areas in Brazil and the differences in antibody detection in patients with NC were established.

The aim of the present study was to analyse Taenia solium metacestode antigens that were derived from the unbound fraction of jacalin affinity chromatography and subsequent tert-octylphenoxy poly (oxyethylene) ethanol Triton X-114 (TX-114) partitioning in the diagnosis of human neurocysticercosis (NCC). Immunoassays were designed to detect T. solium-specific IgG antibodies by ELISA and immunoblot. Serum samples were collected from 132 individuals who were categorised as follows: 40 had NCC, 62 presented Taenia spp or other parasitic diseases and 30 were healthy individuals. The jacalin-unbound (J unbound ) fraction presented higher sensitivity and specificity rates than the jacalin-bound fraction and only this fraction was subjected to subsequent TX-114 partitioning, resulting in detergent (DJ unbound ) and aqueous (AJ unbound ) fractions. The ELISA sensitivity and specificity were 85% and 84.8% for J unbound , 92.5% and 93.5% for DJ unbound and 82.5% and 82.6% for AJ unbound . By immunoblot, the DJ unbound fraction showed 100% sensitivity and specificity and only serum samples from patients with NCC recognised the 50-70 kDa T. solium-specific components. We conclude that the DJ unbound fraction can serve as a useful tool for the differential immunodiagnosis of NCC by immunoblot.

Neurocysticercosis (NC) is a clinically and radiologically heterogeneous parasitic disease caused by the establishment of larval Taenia solium in the human central nervous system. Host and/or parasite variations may be related to this observed heterogeneity. Genetic differences between pig and human-derived T. solium cysticerci have been reported previously. In this study, 28 cysticerci were surgically removed from 12 human NC patients, the mitochondrial gene that encodes cytochrome b was amplified from the cysticerci and genetic variations that may be related to NC heterogeneity were characterised. Nine different haplotypes (Ht), which were clustered in four haplogroups (Hg), were identified. Hg 3 and 4 exhibited a tendency to associate with age and gender, respectively. However, no significant associations were found between NC heterogeneity and the different T. solium cysticerci Ht or Hg. Parasite variants obtained from patients with similar NC clinical or radiological features were genetically closer than those found in groups of patients with a different NC profile when using the Mantel test. Overall, this study establishes the presence of genetic differences in the Cytb gene of T. solium isolated from human cysticerci and suggests that parasite variation could contribute to NC heterogeneity.

Abstract: Taenia solium is a zoonotic tapeworm of great importance in developing countries, due to the occurrence of human taeniasis and cysticercosis. Pigs have an important role in the biological cycle of the parasite as intermediate hosts. The scientific literature has been describing risk factors associated with the occurrence of this disease that must be avoided in countries with poor sanitation, in order to reduce the exposure of swine to the parasite eggs. This research focused on testing pigs of non-technified rearing farms for serum antibodies against Taenia solium in the region of Jaboticabal municipality, in the state of São Paulo, Brazil. The found prevalence was 6.82% (CI 95% 4.18 - 9.45) at animal level and 28.87% (CI 95% 16.74 - 40.40) at herd level. These figures are probably associated with low technification adoption during animal rearing in the studied area, which increased the exposure of the animals to risk factors associated with the occurrence of Taenia solium complex. The results found based on serological evidences of swine cysticercosis in the studied region serves as a warning to public sanitary authorities to improve public health and control T. solium.

Estudo da prevalência de parasitas intestinais realizado em 370 indivíduos residentes na CECAP, distrito-sede de Botucatu, Estado de São Paulo, permitiu verificar que 41,62% encontravam-se infestados por uma ou mais espécies de parasitas intestinais. Foram encontrados os seguintes parasitas: Entamoeba histolytica 0,54%, E. coli 6,21%, I. butschlii 0,27%, Giardia lamblia 9,72%, Ancylostomidae 5,94%, Strongyloides stercoralis 6,75%, Trichuris trichiura 17,29%, Ascaris lumbricoides 7,56%, Enterobius vermicularis 3,78%, Hymenolepis nana 5,40% e Taenia sp. 1,62%. Apresentam-se dados sobre a distribuição dos parasitas em relação à idade e ao sexo dos indivíduos. O atributo cor não permite maiores considerações por serem os não brancos significantemente pouco numerosos. Dos indivíduos examinados, 25,67% apresentavam apenas uma espécie de parasita e entre as associações parasitárias mais freqüentes encontramos as de Ascaris lumbricoides - Trichuris trichiura e Trichuris trichiura - Giardia lamblia.

Procurou-se conhecer a prevalência de enteroparasitoses na população urbana do 2.° subdistrito de Botucatu, SP (Brasil) através de exames coprológicos realizados pelos métodos de FAUST, HOFFMAN e processo de tamização. A prevalência de enteroparasitoses foi relacionada com atributos da população, tais como sexo, idade, cor e com fatores ligados ao meio ambiente. O processo de amostragem empregado foi o casual simples em duplo estágio, sendo o quarteirão a unidade primária do primeiro estágio e o domicílio a unidade do segundo estágio. Os resultados mostraram que 53,76% das 895 pessoas amostradas apresentavam-se infestadas por uma ou mais espécies de parasitas intestinais. As prevalências foram as seguintes: Ancylostomidae, 17,54%; T. trichiurus, 13,63%; A. lumbricoides, 10,69%; S. stercoralis, 6,03%; E. vermicularis, 3,69%; H. nana, 1,79%; Taenia sp, 1,22%; S. mansoni, 0,22%; E. coli, 15,53%; G. lamblia, 14,07%; E. nana, 2,35%; I. bütschlii, 1,01% e E. histolytica, 0,22%.

Em virtude da elevada prevalência de parasitoses intestinais no Brasil e da possível participação de objetos na transmissão dessas afecções, foi desenvolvida pesquisa envolvendo dinheiro difundido na coletividade. Foram analisadas, quanto à presença de ovos de helmintos e de cistos de protozoários, 1.003 cédulas e 1.011 moedas, de valores e procedências diversos. Do estudo resultou o encontro de cistos de Entamoeba histolytica em duas eventualidades, de ovos de Ascaris lumbricoides larvado e infértil, de ovo de Taenia sp. morfologicamente íntegro e de outros elementos não patogênicos. Ficou salientada a importância do dinheiro circulante na epidemiologia das enteroparasitoses, tendo havido sugestão de novos trabalhos de natureza congênere, inclusive delineando diretrizes de ordem profilática.

Foi padronizado o teste de eritroimunoadsorção por captura (EIAC) para detecção de anticorpos específicos anti-cisticercos de Taenia solium, classe IgG, no líquido cefalorraquidiano (LCR) de pacientes com neurocisticercose. O reagente empregado para detecção de anticorpos específicos foi preparado com hemácias de carneiro em uma concentração de 0,25%, sensibilizadas com antígeno extrato salino bruto (ESB) obtido do Cysticercus cellulosae. A concentração ótima de ESB para sensibilização das hemácias de carneiro foi de 40ug/ml. O rendimento do ESB foi de 0,lug proteína/cavidade. A sensibilidade do teste foi de 84,5% (limite de confiança 95% de 75% a 94%), quando aplicado a 58 amostras de LCR de pacientes com neurocisticercose; e a especificidade foi de 95,3% (limite de confiança 95% de 90,7% a 99,9%) quando 85 amostras de LCR do grupo controle foram analisadas. O teste EIAC foi eficiente para o diagnóstico da neurocisticercose, e é importante para os laboratórios de saúde pública, tendo em vista a fácil execução, alto rendimento e baixo custo.

Twenty adult patients presenting dermal cysticercosis without cerebral or ocular involvement were treated with praziquantel. The first eleven cases received 60 mg/kg/day and the last nine cases 30 mg/kg/day. In both groups the daily dose was split into three oral intakes 4 to 6 hours apart and the drug administration lasted for 6 consecutive days. The latter group of patients also got dexamethasone, 3 mg daily, from one day before until four days after the treatment period with praziquantel. The drug proved to be 100% efficacious as demonstrated histopathologically by the death of the cysticerci of Taenia solium (Cysticercus cellulosae) in serial biopsies taken from the 2nd week on after the end of treatment, as well as clinically by the steady disappearence of the dermal nodules during the 6 months following the therapy. Tolerance of praziquantel was good as the incidence and severity of side-effects were not relevant. The drug safety was confirmed through laboratory tests which failed to detect any abnormal findings related to the hematopoietic, liver and kidney functions.

Se comunican resultados obtenidos empleando Medio de Hoyer para el montaje de huevos de helmintos enteroparásitos, destinado a preparaciones para colecciones docentes y/o de investigación. La utilización de esta técnica en muestras fecales conteniendo huevos de A. lumbricoides, T. trichiura, Uncinaria sp., Taenia sp., Diphyllobothrium sp., H. nana, H. diminuta y F. hepática, permitió la correcta observación de ellos en lecturas iniciadas a las 24 horas y mantenidas hasta 180 días después.

Se presenta un caso clínico de infección por cisticerco racemoso cerebral de localización parenquimatosa en un paciente de la ciudad de Valdivia (Chile) cuyo diagnóstico definitivo se efectuó a través del estudio morfológico del parásito. Se discute brevemente la escasa frecuencia de la localización parenquimatosa del cisticerco racemoso, así como su diagnóstico diferencial con otros estados larvarios de cestodos que desarrollan en el sistema nervioso.

Neurocysticercosis (NC), the presence of Taenia solium metacestodes in tissues, is the most frequent and severe parasitic infection of the central nervous system. We investigated the presence of total IgE by an automated chemiluminescence assay in 53 paired cerebrospinal fluid (CSF) and serum samples from patients with NC (P) and in 40 CSF samples from individuals with other neurological disorders as the control group (C). Total IgE concentration ranged from 1.2 to 6.6 IU/ml (mean = 1.4 IU/ml, standard deviation-sd = 1.1 IU/ml) in 28.3% of CSF samples from the P group, a value significantly higher than for the C group (£1.0 IU/ml). The serum samples from the P group showed concentrations ranging from 1.0 to 2330.0 IU/ml (mean = 224.1 IU/ml, sd = 452.1 IU/ml), which were higher than the normal value cited by the manufacturer (<100.0 IU/ml) in 32.1% of the samples. A significant difference was observed in CSF samples from the P and C groups (p = 0.005) and in serum samples from the P group compared to the normal value (p = 0.005), with sera showing more frequent abnormal results.

Simple and rapid latex-based diagnostic tests have been used for detecting specific antigens or antibodies in several diseases. In this article, we present the preliminary results obtained with a latex agglutination test (LAT) for diagnosing neurocysticercosis by detection of antibodies in CSF. A total of 43 CSF samples were assayed by the LAT: 19 CSF samples from patients with neurocysticercosis and 24 CSF samples from patients with other neurologic disorders (neurosyphilis, n = 8; neurotoxoplasmosis, n = 3; viral meningitis, n = 4, chronic headache, n = 9). The LAT exhibited 89.5% sensitivity and 75% specificity. The use of LAT seems to be an additional approach for the screening of neurocysticercosis with advantage of simplicity and rapidity. Further studies could be performed using purified antigens and serum samples.
