The importance of hands in the transmission of nosocomial infection has been world wide admitted. However, it is difficult to induce this behavior in health-care workers. The aim of the present work was to point out the importance of hand bacteria colonization, the influence of hand washing and of patient physical examination. One hundred health-care workers were randomly divided in two groups: Group A without hand washing previous to patient physical examination or handling (PPE); group B with hand washing previous to PPE. Direct fingerprint samples in Columbia agar before and after PPE were obtained. The colonies were counted and identified by conventional techniques, and antibiograms according to NCCLS were performed. Before PPE group A participants showed a high number of bacteria regarding group B participants (73.9 Vs 20.7; p < 0.001); 44 out of 50 participants were carriers of potentially pathogen bacteria. No group B participants were carriers of potential pathogen bacteria before PPE. The latter group showed an increase in number of bacteria after PPE (20.7 CFU (before) Vs 115.9 CFU (after); p < 0.001). Sixteen group B participants were contaminated after PPE with potential pathogens such as S. aureus (50% of them meticillin resistant); Escherichia coli, Pseudomonas aeruginosa and Enterococcus faecalis, half of them multiresistant. We can conclude on the importance of these results to implement educational programs and to provide the health-care workers with the proper commodities to fulfill this practice.

Hand, foot and mouth disease (HFMD) is a contagious enteroviral infection occurring primarily in children and characterized by vesicular palmoplantar eruptions and erosive stomatitis. Echovirus 4 (EV-4) has been commonly associated with aseptic meningitis. The association of HFMD with EV-4 has not been reported previously. Two samples of a 14-month child who presented mild fever, sores in the mouth, rash with blisters on the palm of hands and soles of feet were sent to Enteric Viruses Laboratory of Adolfo Lutz Institute. Clinical samples were inoculated in three different cell lines, and those which presented cytopathic effect (CPE), were submitted to Indirect Immunofluorescence Assay (IFA) and "one step" RT-PCR. Agarose gel electrophoresis from RT-PCR product, showed a product with 437 bp, which is characteristic of Enterovirus group. Echovirus 4 was identified by IFA. Although HFMD is a viral infection associated mainly with Enterovirus 71 (HEV-71) and Coxsackievirus A16 (CV-A16), our results demonstrate a diversity of serotype related to HFMD and stress the importance of epidemiological surveillance to this disease and its complications.

To compare two yeast identification methods, i. e, the manual and the VITEK mechanical methods, 62 clinical samples from hemocultures and animal sources were analyzed. After identification as Candida yeasts by the VITEK method, the strains were recharacterized using manual assimilation methods and sugar fermentation tests. Our findings reveal 58% concurrent identification between the two methods for animal strains, and 51% for human hemoculture strains.

This cross-sectional study assessed the grade of physical impairments in 61 individuals with leprosy receiving multidrug therapy (MDT) under the Brazilian Unified Health System (SUS), and residing in Campina Grande, Paraíba State, Brazil. Impairments were assessed using the disability grade (DG) standardized by the WHO, and the EHF score (Eye-Hand-Foot sum of impairment scores). Impairments were detected in 25 (41%) of the subjects. A total of 14 (23%) patients scored DG 1, while 11 (18%) were assigned DG 2. The EHF score ranged from 1 to 10 points in the group of patients with physical impairments, with a mean score of 3.6 points. The majority of individuals with impairments were affected in at least two sites. We conclude that the EHF score showed overlapping impairments in the segments examined and may be more appropriate than the DG classification system for describing the degree of physical impairment of leprosy patients.

Hand-foot-and-mouth disease (HFMD) is becoming one of the extremely common airborne and contact transmission diseases in Guangzhou, southern China, leading public health authorities to be concerned about its increased incidence. In this study, it was used an ecological study plus the negative binomial regression to identify the epidemic status of HFMD and its relationship with meteorological variables. During 2008-2012, a total of 173,524 HFMD confirmed cases were reported, 12 cases of death, yielding a fatality rate of 0.69 per 10,000. The annual incidence rates from 2008 to 2012 were 60.56, 132.44, 311.40, 402.76, and 468.59 (per 100,000), respectively, showing a rapid increasing trend. Each 1 °C rise in temperature corresponded to an increase of 9.47% (95% CI 9.36% to 9.58%) in the weekly number of HFMD cases, while a one hPa rise in atmospheric pressure corresponded to a decrease in the number of cases by 7.53% (95% CI -7.60% to -7.45%). Similarly, each one percent rise in relative humidity corresponded to an increase of 1.48% or 3.3%, and a one meter per hour rise in wind speed corresponded to an increase of 2.18% or 4.57%, in the weekly number of HFMD cases, depending on the variables considered in the model. These findings revealed that epidemic status of HFMD in Guangzhou is characterized by high morbidity but low fatality. Weather factors had a significant influence on the incidence of HFMD.

O estudo dos hábitos alimentares e do conteúdo intestinal dos flebotomíneos permite a identificação dos hospedeiros, indicando os potenciais reservatórios das leishmanias. Este trabalho objetivou determinar a preferência alimentar de Lutzomyia longipalpis e sua relação com a transmissão da leishmaniose visceral. As capturas mensais foram realizadas em área de transmissão de leishmaniose visceral, município de Várzea Grande, Estado de Mato Grosso, no período de janeiro de 2004 a junho de 2006, utilizando-se armadilhas de luz CDC. Foram capturadas 2.376 fêmeas de Lutzomyia longipalpis, das quais 104 (4,4%) estavam ingurgitadas, sendo 32 (30,8%) capturadas no intradomicílio e 72 (69,2%) no peridomicílio. Após reação de precipitina, observou-se que as fêmeas de Lutzomyia longipalpis alimentaram-se preferencialmente em aves (30,8%) e roedores (21,2%), mas também foram encontradas fêmeas alimentadas de sangue de humanos, gambás, bois, cavalos e cães, demonstrando o caráter oportunista da espécie.

O objetivo deste estudo foi determinar as fontes de alimento sanguíneo de fêmeas de Lutzomyia whitmani, espécie de flebotomíneo incriminada no Maranhão como principal vetor da leishmaniose cutânea americana. Para isso, 70 fêmeas desta espécie coletadas no município de Axixá, área com um dos maiores números de casos de leishmaniose cutânea americana em humanos no Maranhão, foram analisadas utilizando a técnica da precipitina. Dos indivíduos analisados, 90% apresentaram reação a algum tipo de antissoro e dentre estes, 73% apresentaram reações do tipo simples com predominância para sangue de galinha (22,2%), roedor (14,3) e humano (12,7%). Nas reações duplas predominaram as combinações galinha/humano (6,3%), galinha/gambá (4,8%), boi/humano e gambá/humano (3,2%). Assim, concluímos que seres humanos, animais domésticos e sinantrópicos constituem fonte alimentar sanguínea para Lutzomyia whitmani podendo desempenhar um papel importante no ciclo de transmissão da leishmaniose cutânea americana explicando os casos da doença em Axixá.

The authors analysed a series of 22 patients undergoing surgical correction of congenital hand syndactyly by the rectangular flap technique. Using our evaluation method, we found that good functional and aesthetic results were obtained in 77.3% of the patients, with a complication rate of 13.6%. We concluded that the rectangular flap technique has a simple design, is easily reproducible by in-training staff, has good results, and can be applied on the majority of the syndactyly cases.

Historically, the dorsal arterial system of the hand received less attention than the palmar system. The studies concerning dorsal arterial anatomy present some controversies regarding the origin and presence of the dorsal metacarpal artery branches. Knowledge of the anatomy of dorsal metacarpal arteries is especially applied in the surgical planning for flaps taken from the dorsum of the hand. The purpose of this study is to analyze the arterial anatomy of the dorsum of the hand, compare our observations with those of previous studies from the literature, and therefore to define parameters for surgical planning for flaps supplied by the dorsal metacarpal arteries. METHOD: Twenty-six dissections were performed at the dorsum of the right hand of 26 cadavers by making a distal-based U-shaped incision. After catheterization of the radial artery at the wrist level, a plastic dye solution with low viscosity and quick solidification was injected to allow adequate exposure of even small vessels. The radial artery and its branches, the dorsal arterial arch, the dorsal metacarpal arteries, the distal and proximal communicating branches of the palmar system, and the distal cutaneous branches were carefully dissected and identified. RESULTS: The distal cutaneous branches originating from the dorsal metacarpal arteries were observed in all cases; these were located an average of 1.2 cm proximal from the metacarpophalangeal joint. The first dorsal metacarpal artery presented in 3 different patterns regarding its course: fascial, subfascial, and mixed. The branching pattern of the radial artery at the first intermetacarpal space was its division into 3 branches. We observed the presence of the dorsal arterial arch arising from the radial artery in 100% of the cases. The distance between the dorsal arterial arch and the branching point of the radial artery was an average of 2 cm. The first and second dorsal metacarpal arteries were visualized in all cases. The third and fourth dorsal metacarpal arteries were visualized in 96.2% and 92.3% of cases, respectively. There was proximal and distal communication between the dorsal arterial arch and the palmar system through the communicating branches contributing to the dorsal metacarpal artery formation. CONCLUSION: At the dorsum of the hand there is a rich arterial net that anastomoses with the palmar arterial system. This anatomical characteristic allows the utilization of the dorsal aspect of the hand as potential donor site for cutaneous flaps.

O cupuaçu (Theobroma grandiflorum,Sterculiaceae) é uma frutífera da Amazônia usada na fabricação de sucos, sorvetes, doces e chocolate. A sua popularidade vem aumentando e atualmente é considerada uma das culturas mais lucrativas da região Amazônica. O cupuaçu é uma espécie predominantemente alógama e muitas flores não chegam a dar frutos mesmo quando polinizadas com póíen compatível. A taxa de polinização natural é baixa (estimado em 1,6% para pístílos polínízados com mais de 60 grãos de pólen). No presente trabalho, foram estudados alguns aspectos da sua polinização. Polinizações manuais foram feitas próximo à antese (entre 17-18:00 h) e na manhã seguinte após a antese, da seguinte maneira: retiraram-se três estaminódios da flor que iria ser polinizada para facilitar o acesso ao pistilo; da flor doadora de pólen retirou-se um estame, cujas anteras foram passadas nos braços estigmáticos. O maior êxito foi obtido nas polinizações feitas próximas à antese, atribuído ao maior número de grãos depositados durante as polinizações. Polinizações controladas entre plantas compatíveis mostraram que as flores que receberam 60 grãos de pólen tiveram 20% de probabilidade de formar fruto; as flores que receberam mais de 400 grãos de pólen, sempre formaram mitos. O uso de polinização manual aumentou o número de frutos maduros colhidos. O número de sementes nos frutos derivados de polinização natural e manual foi similar. A polinização manual poderá ser utilizada tanto no melhoramento como para aumentar a produtividade dos cupuaçuzeiros que estejam com problemas de polinização.

Species distribution modeling has relevant implications for the studies of biodiversity, decision making about conservation and knowledge about ecological requirements of the species. The aim of this study was to evaluate if the use of forest inventories can improve the estimation of occurrence probability, identify the limits of the potential distribution and habitat preference of a group of timber tree species. The environmental predictor variables were: elevation, slope, aspect, normalized difference vegetation index (NDVI) and height above the nearest drainage (HAND). To estimate the distribution of species we used the maximum entropy method (Maxent). In comparison with a random distribution, using topographic variables and vegetation index as features, the Maxent method predicted with an average accuracy of 86% the geographical distribution of studied species. The altitude and NDVI were the most important variables. There were limitations to the interpolation of the models for non-sampled locations and that are outside of the elevation gradient associated with the occurrence data in approximately 7% of the basin area. Ceiba pentandra (samaúma), Castilla ulei (caucho) and Hura crepitans (assacu) is more likely to occur in nearby water course areas. Clarisia racemosa (guariúba), Amburana acreana (cerejeira), Aspidosperma macrocarpon (pereiro), Apuleia leiocarpa (cumaru cetim), Aspidosperma parvifolium (amarelão) and Astronium lecointei (aroeira) can also occur in upland forest and well drained soils. This modeling approach has potential for application on other tropical species still less studied, especially those that are under pressure from logging.
