Background:Recent studies have suggested that B-type Natriuretic Peptide (BNP) is an important predictor of ischemia and death in patients with suspected acute coronary syndrome. Increased levels of BNP are seen after episodes of myocardial ischemia and may be related to future adverse events.Objectives:To determine the prognostic value of BNP for major cardiac events and to evaluate its association with ischemic myocardial perfusion scintigraphy (MPS).Methods:This study included retrospectively 125 patients admitted to the chest pain unit between 2002 and 2006, who had their BNP levels measured on admission and underwent CPM for risk stratification. BNP values were compared with the results of the MPS. The chi-square test was used for qualitative variables and the Student t test, for quantitative variables. Survival curves were adjusted using the Kaplan-Meier method and analyzed by using Cox regression. The significance level was 5%.Results:The mean age was 63.9 ± 13.8 years, and the male sex represented 51.2% of the sample. Ischemia was found in 44% of the MPS. The mean BNP level was higher in patients with ischemia compared to patients with non-ischemic MPS (188.3 ± 208.7 versus 131.8 ± 88.6; p = 0.003). A BNP level greater than 80 pg/mL was the strongest predictor of ischemia on MPS (sensitivity = 60%, specificity = 70%, accuracy = 66%, PPV = 61%, NPV = 70%), and could predict medium-term mortality (RR = 7.29, 95% CI: 0.90-58.6; p = 0.045) independently of the presence of ischemia.Conclusions:BNP levels are associated with ischemic MPS findings and adverse prognosis in patients presenting with acute chest pain to the emergency room, thus, providing important prognostic information for an unfavorable clinical outcome.

Background: The maternal cardiovascular system undergoes progressive adaptations throughout pregnancy, causing blood pressure fluctuations. However, no consensus has been established on its normal variation in uncomplicated pregnancies. Objective: To describe the variation in systolic blood pressure (SBP) and diastolic blood pressure (DBP) levels during pregnancy according to early pregnancy body mass index (BMI). Methods: SBP and DBP were measured during the first, second and third trimesters and at 30-45 days postpartum in a prospective cohort of 189 women aged 20-40 years. BMI (kg/m2) was measured up to the 13th gestational week and classified as normal-weight (<25.0) or excessive weight (≥25.0). Longitudinal linear mixed-effects models were used for statistical analysis. Results: A decrease in SBP and DBP was observed from the first to the second trimester (βSBP=-0.394; 95%CI: -0.600- -0.188 and βDBP=-0.617; 95%CI: -0.780- -0.454), as was an increase in SBP and DBP up to 30-45 postpartum days (βSBP=0.010; 95%CI: 0.006-0.014 and βDBP=0.015; 95%CI: 0.012-0.018). Women with excessive weight at early pregnancy showed higher mean SBP in all gestational trimesters, and higher mean DBP in the first and third trimesters. Excessive early pregnancy BMI was positively associated with prospective changes in SBP (βSBP=7.055; 95%CI: 4.499-9.610) and in DBP (βDBP=3.201; 95%CI: 1.136-5.266). Conclusion: SBP and DBP decreased from the first to the second trimester and then increased up to the postpartum period. Women with excessive early pregnancy BMI had higher SBP and DBP than their normal-weight counterparts throughout pregnancy, but not in the postpartum period.

Foi estudado o comportamento de forrageamento de Suiriri affinis (suiriri-do-cerrado) e S. islerorum (suiriri-da-chapada), duas espécies de Tyrannidae que ocorrem em sintopia nos cerrados do Brasil central. Durante o ano de 2003 foram registradas 188 observações de forrageamento para S. affinis e 150 para S. islerorum. Foram encontradas diferenças significativas entre as duas espécies em relação à altura e substrato de forrageamento, fitofisionomia utilizada e densidade da folhagem no local do ataque. O comportamento e a direção do ataque, a espécie de planta e a distância de vôo não diferiram entre as espécies. A análise de 26 conteúdos estomacais de membros do gênero revelou o consumo exclusivo de artrópodes. A baixa porcentagem de observações de forrageamento direcionadas a frutos (apenas 7% e 9%, respectivamente) confirmou o caráter insetívoro do gênero.

ABSTRACT Atlantirivulus riograndensis (Costa & Lanés, 2009) is a fish registered to the basin of Patos lagoon and the adjacent coastal plains in southern states of Rio Grande do Sul and Santa Catarina, Brazil, found in shallow water courses with that have large quantities of aquatic vegetation and forest edges. The objective of this study was to investigate the reproductive strategy and tactics of this species including the sex ratio, the length at first maturity, spawning type, fecundity and the possible associations among reproduction and abiotic factors. Sampling of specimens occurred in perennial wetlands within the Banhado dos Pachecos wildlife refuge, in the city of Viamão, state of Rio Grande do Sul, Brazil, which is a conservation unit that belongs to an area of environmental protection in the Pampa Biome. The capture of 30 A. riograndensis specimens per month occurred from January to December of 2012. A total of 188 females and 172 males were captured and the total sex ratio was 1:1 in the sampled population. Sexual maturity of the species occurs after 13.59 and 11.92 mm (SL) for females and males, respectively. Both a multiple spawning and a long reproductive period (since August to March) were confirmed by the presence of post-ovulatory follicles that were observed through histological analysis and the values of the gonadosomatic index in females considered spawning capable. The average absolute fecundity of the species is of 19.33 (± 6.18) vitellogenic oocytes in mature ovaries. No significant relationship was found between mean GSI and the abiotic data. Reproductive tactics presented by A. riograndensis indicate a species with an opportunistic reproductive strategy, following the pattern of other species of the Rivulidae family.

Um estudo longitudinal clínico, radiológico e eletrocardiográfico do tipo caso-controle realizado no município de Virgem da Lapa, Minas Gerais, Brasil, com o acompanhamento de 124 chagásicos crônicos durante seis anos, revelou que 62,1% dos pacientes permaneceram com o quadro inicial inalterado, a maioria deles na forma indeterminada, 32,3% evoluíram com progressão da doença e 5,6% tiveram normalização do eletrocardiograma. Os resultados mencionados, quando comparados aos obtidos no grupo controle composto de pares não chagásicos da mesma idade e sexo, demonstraram uma progressão de 27,4% maior entre os pacientes com sorologia positiva, o que representa o excesso de risco ou componente exclusivamente chagásico na evolução da doença. Não houve diferença de progressão da doença em relação ao sexo, porém ela foi mais precoce e sete vezes mais freqüente em relação à cardiopatia do que ao megaesôfago, ambas ocorrendo na maioria das vezes em grau leve ou moderada. Em 192 chagásicos e 188 não chagásicos observados na área, no referido período, houve uma mortalidade 3,6 vezes maior entre os chagásicos, com uma letalidade pela cardiopatia de 8,9%, sem diferença entre os sexos, porém mais precoce no sexo masculino. A morte súbita foi mais freqüente do que a morte por insuficiência cardíaca. O prognóstico foi bom para os pacientes da forma indeterminada e digestiva e reservado para os casos de cardiopatia, principalmente os de graus mais elevados.

A schedule of repeated chemotherapy with oxamniquine, consisting of biannual treatment of school-aged (7-13 years) children and annual treatment of all other age groups, was used in a representative rural village from a highly endemic area of schistosomiasis in Pernambuco. Significant reductions in infection were obtained only after two cycles of treatment, as the overall prevalence decreased from 72.6% to 41.7% and the geometric mean egg counts per gram of faeces among positives fell from 188.4 to 76. In a school-aged cohort (n=29) three treatments at six-month intervals were necessary to significantly reduce the proportion of positives (from 75.9% to 51.7%). In a cohort of children under 7 years of age (n=20) the proportion of positives actually increased (from 30% to 45%) despite two annual treatments. Water contact was intense and host snail density was relatively high. As there is no short-term perspective of improved sanitation, auxiliary measures such as focal mollusciciding are needed for an adequate control of schistosomiasis in this and alike areas.

The use of in situ techniques to detect DNA and RNA sequences has proven to be an invaluable technique with paraffin-embedded tissue. Advances in non-radioactive detection systems have further made these procedures shorter and safer. We report the detection of Trypanosoma cruzi, the causative agent of Chagas disease, via indirect and direct in situ polymerace chain reaction within paraffin-embedded murine cardiac tissue sections. The presence of three T. cruzi specific DNA sequences were evaluated: a 122 base pair (bp) sequence localized within the minicircle network, a 188 bp satellite nuclear repetitive sequence and a 177 bp sequence that codes for a flagellar protein. In situ hybridization alone was sensitive enough to detect all three T. cruzi specific DNA sequences.

Single-stranded DNA (ssDNA) is a prerequisite for electrochemical sensor-based detection of parasite DNA and other diagnostic applications. To achieve this detection, an asymmetric polymerase chain reaction method was optimised. This method facilitates amplification of ssDNA from the human lymphatic filarial parasite Wuchereria bancrofti. This procedure produced ssDNA fragments of 188 bp in a single step when primer pairs (forward and reverse) were used at a 100:1 molar ratio in the presence of double-stranded template DNA. The ssDNA thus produced was suitable for immobilisation as probe onto the surface of an Indium tin oxide electrode and hybridisation in a system for sequence-specific electrochemical detection of W. bancrofti. The hybridisation of the ssDNA probe and target ssDNA led to considerable decreases in both the anodic and the cathodic currents of the system's redox couple compared with the unhybridised DNA and could be detected via cyclic voltammetry. This method is reproducible and avoids many of the difficulties encountered by conventional methods of filarial parasite DNA detection; thus, it has potential in xenomonitoring.

A presente pesquisa teve por objetivo mapear o processo de treinamento compartilhado em ressuscitação cardiopulmonar para técnicos e auxiliares de enfermagem da UTI e Semi-Intensiva de Adulto do HU-USP, e aferir os custos diretos das principais atividades do processo. Trata-se de um estudo exploratório, retrospectivo, de levantamento documental, nos moldes de estudo de caso. Seus resultados mostraram que o custo direto total do programa de treinamento em ressuscitação cardio-pulmonar foi de R$ 9.081,44. O custo direto com pessoal representou 96,74% e com material 3,26%. No subprocesso planejamento do treinamento, o enfermeiro instrutor-assistencial teve o maior custo direto do pessoal com R$ 5.451,60 (62,04%). Os custos diretos relacionados com o material utilizado foram maiores, também, no subprocesso planejamento do treinamento, R$ 188,80 (63,73%). O custo total por treinando foi de R$ 206,40.

Objetivou-se descrever o perfil sócio-demográfico e clínico-epidemiológico de portadores de Tuberculose Multirresistente (TBMR), matriculados em Centro de Referência para Tuberculose (TB) do Estado de São Paulo. Estudo quantitativo, descritivo, retrospectivo e seccional. Levantaram-se dados de fichas de notificação e de prontuários de sujeitos matriculados de agosto de 2002 a dezembro de 2009. Os dados foram coletados de fevereiro a junho de 2010, com instrumento estruturado, e sistematizados no Excel e Epi Info. Identificaram-se 188 sujeitos, 74,2% do Município de São Paulo; 93,1% na faixa etária produtiva; 61,6% desempregados; 64,4% homens; 34,6% com 4 a 7 anos de escolaridade; 98,9% com tratamentos anteriores para TB; 98,4% com TBMR pulmonar; 71,4% com lesão bilateral cavitária e todos realizaram teste de sensibilidade aos antimicrobianos. Os achados evidenciam pistas de que os sujeitos desenvolveram TBMR principalmente devido às condições de vida e acesso aos serviços de saúde. A limitação dos dados de fichas de notificação e prontuários limitou abordagem ampla como requer a TB, enfermidade consagradamente socialmente determinada.

Estudo randômico e controlado que objetivou verificar se a ultrassonografia vascular (USV) aumenta a assertividade na utilização do cateter intravenoso periférico e o tempo de permanência do cateter quando comparado ao método tradicional de punção. A coleta de dados ocorreu após aprovação do mérito ético, incluindo-se no estudo crianças e adolescentes submetidos a punção intravenosa periférica guiada pela USV, constituindo o grupo USV (GUSV), ou após avaliação clínica da rede venosa, denominado grupo controle (GC). Os valores de p<0,05 foram considerados significativos. A amostra foi constituída por 382 punções, 188 (49,2%) no GUSV e 194 (50,8%) no GC, realizadas em 335 crianças. Identificou-se assertividade em 73 (71,6%) cateteres do GUSV e em 84 (71,8%) do GC (p=0,970). O tempo de permanência do cateter apresentou mediana inferior a um dia nos dois grupos (p=0,121), não havendo diferença estatisticamente significativa. Concluindo-se que a USV não influenciou os resultados das variáveis dependentes investigadas. ClinicalTrials.govNCT00930254.

OBJECTIVE To identify the prevalence of Burnout Syndrome in medical professionals, nurses and nursing technicians working in an Onco-Hematological Pediatric Hospital in São Paulo. METHOD An exploratory, descriptive study with cross-sectional design and quantitative approach, with a sample of 188 health professionals. Data were collected using two self-report instruments: the Maslach Burnout Inventory (MBI-HSS) which is a biosocial data form, and a non-participant observation guide. RESULTS High depersonalization for nurses (29.8%), low job performance for physicians (27.8%), and of nursing technicians (25.5%). High scores were identified in at least two domains of Burnout in 19.2% of nurses, 16.8% of nursing technicians, and 16.6% of doctors. CONCLUSION Health professionals are highly vulnerable to each of the dimensions of Burnout syndrome - namely emotional exhaustion, alienation, and low job performance/satisfaction- in the hospital work.

Six individuals of the palm A. phalerata, in Poconé floodplains of Mato Grosso, were sprayed with a synthetic pyrethroid (0.25% concentration) in order to study the biomass, diversity, and richness of the canopy arthropods. A total of 17,188 (238.7±80.6 ind./m²) arthropods belonging to 22 Orders, was collected in a 72 m² funnel area. Two hours after spraying, 58.9% of the total number fell into the funnels, 37.6% was obtained by shaking the trees, and finally, 3.5% after cutting and washing all the palm leaves. The Coleoptera (27.4%), Hymenoptera-Formicidae (19.0%), Collembola (13.6%), Psocoptera (10.7%), Diptera (9.0%) and Araneae (6.4%) were the predominant. The total biomass was 15.1 g dry weight (0.4mg/m²; 0.13+0.04/tree). A total of 4,715 beetles representing 48 families and 326 morphospecies were obtained. Tenebrionidae (22.9%), Curculionidae (22.0%), Carabidae (10.9%) and Staphylinidae (7.9%) were the most abundant, while Curculionidae (44 spp.), Staphylinidae (40 spp.) and Chrysomelidae (34 spp.) presented the largest number of morphospecies. Herbivores (37.5%) were the dominant in the trophic guilds of adult Coleoptera, followed by predators (35.4%), fungivores (14.6%), and saprophages (12.5%). Although most arthropod Orders were represented in all the palms sampled, analysis of variance showed no significant differences in their composition, however there was a significant difference in their frequency of occurrence.

Host plants of Cerambycidae and Vesperidae (Coleoptera, Chrysomeloidea) from South America. This paper offers a contribution to the knowledge on host plants of Cerambycidae and Vesperidae (Coleoptera, Chrysomeloidea) from South America. A total of 211 species of longhorn beetles belonging to 111 genera, 49 tribes, five subfamilies and two families are presented with records of host plants. Data on 259 species of host plants distributed among 188 genera and 69 families are listed with the beetle species.

Thermal hygrometric requirements for the rearing and release of Tamarixia radiata (Waterston) (Hymenoptera, Eulophidae). Tamarixia radiata is the main agent for the biological control of Diaphorina citri in Brazil with a parasitism rate ranging from 20 to 80%. This study investigated the influence of temperature on the development, fecundity and longevity of adults of T. radiata and the effect of relative humidity (RH) on their parasitism capacity and survival rate in the pre-imaginal period. The effect of temperature was assessed in the range between 15 and 35 ± 1ºC, 70 ± 10% RH, and a 14-h photophase. The RH effect was evaluated in the range from 30 to 90 ± 10%, temperature at 25 ± 1ºC, and photophase of 14-h. At 25ºC, circa 166.7 nymphs were parasitized, the highest parasitism capacity observed compared to other treatments. The longest longevity of females was observed at 25ºC, although the rate did not differ in the 20-30ºC temperature range. The threshold temperature (TT) was 7.2ºC, and 188.7 degrees-day were required for the development (egg-to-adult period). The parasitism rate and longevity were higher at 50 and 70% of RH. This shows that temperature and RH may affect the parasitism capacity of T. radiata on nymphs of D. citri, which can explain the great parasitism variation for D. citri observed in citrus groves in São Paulo State, Brazil.
