A metástase cervical é o fator de prognóstico mais relevante do carcinoma epidermóide de boca. Fatores clínicos e histológicos estão associados com o desenvolvimento da metástase cervical, porém a pesquisa de fatores moleculares está sendo amplamente realizada nos últimos anos. OBJETIVO: Observar a associação da expressão da ciclina D1 como fator de risco para a presença de metástase cervical. MATERIAL E MÉTODO: A expressão da ciclina D1 foi estudada e verificada sua associação com a metástase em 45 pacientes com câncer de boca. A leitura da expressão da ciclina D1 foi realizada pelo método estereológico. Características clínicas e histológicas foram pesquisadas e associadas com a presença de metástase. RESULTADOS: A expressão da ciclina D1 foi encontrada em 15 pacientes (33,4%) e não esteve associada a fatores clínicos, histológicos e com a presença de metástase cervical, sendo sua expressão independente. O estadiamento clínico e as embolizações vasculares foram os fatores preditivos de maior relevância para o desenvolvimento de metástase. CONCLUSÕES: A expressão da ciclina D1, embora seja independente, não está associada com a presença de metástase cervical, enquanto que o estadiamento clínico e as embolizações vasculares estão.

Tireoidectomia sob efeito de bloqueio do plexo cervical superficial (BPCS) tem sofrido resistência. OBJETIVO: Comparar variáveis cirúrgicas e anestésicas, custos do tratamento e grau de satisfação de pacientes submetidos à hemitireoidectomia sob efeito de anestesia geral e BPCS. CASUÍSTICA E MÉTODOS: Foram 21 pacientes submetidos à anestesia geral (AG) e outro tanto ao BPCS. Após sedação, no grupo com BPCS, usou-se marcaína com vasoconstritor, e quando necessário, lidocaína a 2% com vasoconstritor. Sedação intra-operatória com diazepam endovenoso e metoprolol para controle da PA e FC eram administradas quando necessário. Usou-se anestesia geral (AG) segundo padronização do serviço. RESULTADOS: Foram significantes (p<0,05, teste t de Student) para o tempo de cirurgia (ag111,4:bpcs125,5 min), tempo de anestesia (ag154,1:bpcs488,6 min), tempo de permanência na sala cirúrgica (ag15:bpcs1 min), custos do tratamento (ag203,2:bpcs87,4 R$), presença de bradicardia (ag0:bpcs23,8%) e lesão laringotraqueal (ag51:bpcs0%). Como resultados não significativos tiveram: tempo de internação (ag17,3:bpcs15,1 hora); volume de sangramento (ag41,9:bpcs47,6 gr), tamanho da peça operatória (ag52,1:bpcs93,69 cm3) e grau de satisfação dos pacientes (ag3,8:bpcs3,9). CONCLUSÃO: Embora com incidência maior de bradicardia (23,8%), o bloqueio permitiu ressecar tumorações de até 348 cm3 com menor custo e sem apresentar lesões laringotraqueais, presentes em 51% dos pacientes submetidos à AG.

A Doença de Rosai-Dorfman ou Histiocitose Sinusal com Linfadenopatia Maciça (SHML) é uma entidade clínica idiopática, rara e benigna, geralmente apresentando linfonodomegalia cervical. Usualmente acomete pacientes jovens. Manifestações extranodais na região da cabeça e pescoço são mais comuns em pacientes com anormalidades imunológicas. É uma doença autolimitada e, na maioria dos casos, não há necessidade de tratamento. Para os que requerem tratamento devido à persistência ou piora dos sintomas têm sido tentados tratamento cirúrgico, quimioterápico, radioterápico e corticoterapia. Os autores relatam um caso de um paciente do sexo masculino, 43 anos, com história de massas cervicais bilaterais, obstrução nasal, emagrecimento importante, febre e perda progressiva de acuidade visual há 6 meses. Trata-se de um paciente ex-tabagista e etilista crônico, onde a suspeita inicial foi de carcinoma espinocelular de rinofaringe. Realizada propedêutica, foi feito diagnóstico de doença de Rosai-Dorfman. Instituída a terapêutica, o seguimento de 6 meses revelou melhora do estado geral do paciente onde mantém as massas cervicais em regressão lenta. A apresentação clínica, as características histológicas, a patogênese e o tratamento são discutidos neste artigo.

Larvae of the genus Spodoptera spp. are highly polyphagous and can cause economical losses in several agricultural crops. Given their growing importance in the tomato crop, especially for industry, this work aimed to evaluate the feeding non-preference by larvae of Spodoptera frugiperda (J. E. Smith, 1797) and Spodoptera eridania (Cramer, 1782) on tomato genotypes and classify them by the levels of resistance. The commercial cultivar Santa Clara was set as the susceptible standard and line PI 134417 as the resistant standard to evaluate the lines PI 134418, PI 126931, LA 462 and LA 716. Feeding non-preference tests were performed under non-choice and free-choice conditions to evaluate the genotype attractiveness to larvae at predetermined times after their release, as well as the leaf area consumed. Overall, the genotypes LA 716 and PI 126931 were the least attractive to S. frugiperda, whereas Santa Clara was the most attractive and consumed. For S. eridania, the genotypes PI 126931, LA 462, LA 716 and PI 134418 were the least preferred for feeding, and Santa Clara and PI 134417 were the most attractive and consumed. The genotypes LA 716 and PI 126931 are moderately resistant to S. frugiperda and S. eridania; PI 134418 and LA 462 are moderately resistant to S. eridania; PI 134417 is susceptible to S. frugiperda and S. eridania; and Santa Clara is highly susceptible to both S. frugiperda and S. eridania.

Foi efetuada revisão dos aspectos epidemiológicos do câncer cervical, um dos mais freqüentes em mulheres de países em desenvolvimento. No Brasil a incidência varia de 23,7/100.000, em Porto Alegre, a 83,2/100.000, em Recife. Nos Estados Unidos a incidência em 1978 foi de 6,8/100.000 entre as mulheres brancas e de 14,7/100.000 entre as negras. Várias observações sugerem a hipótese de que o câncer cervical esteja relacionado com algum aspecto da atividade sexual, possivelmente algum agente transmitido por via venérea. As evidências têm implicado o papilomavirus humano (HPV) como o principal agente etiológico deste câncer. Vários trabalhos foram analisados quanto à validade desta hipótese etiológica, mostrando que há uma relação entre HPV e o câncer cervical. Foram analisados os fatores de risco mais conhecidos, tais como o comportamento sexual, o tabagismo e a contracepção, diante das várias possibilidades etiológicas existentes.

High mortality rates among those suffering from schizophrenia and related psychoses have been consistently described in developed societies. However, to date there is a lack of data on this matter in Brazil. In order to examine this issue, a prospective 2-year follow-up study was carried out in S. Paulo. The sample consisted of 120 consecutive admissions to psychiatric hospitals in a defined catchment area, aged 18 to 44 years old, with clinical diagnoses of non-affective functional psychoses according to the ICD-9. After 2 years, 116 (96.7%) subjects were traced. During the study period there were 7 deaths (6.0% of those traced), 5 (4.3%) due to suicide. All but one of the suicides occurred in the first year after discharge from hospital. Age and sex Standardised Mortality Ratios (relative to rates for the population of the city of Sao Paulo) were 8.4 for overall mortality (95% confidence interval: 4.0-15.9) and 317.9 for deaths due to suicide (95% confidence interval: 125.2-668.3). These results are in agreement with previous studies, and show that in Brazil non-affective functional psychoses are life-threatening illnesses, which need adequate care, particularly when patients go back to live in the community after hospital discharge.

INTRODUCTION: In Mexico, breast cancer (BC) is one of the main causes of cancer deaths in women, with increasing incidence and mortality in recent years. Therefore, the aim of the study is identify possible risk factors related to BC. METHODS: An epidemiological study of hospital cases of BC and controls with cervical uterine cancer (CUCA) was carried out at eight third level concentration hospitals in Mexico City. The total of 353 incident cases of BC and 630 controls with CUCA were identified among women younger than 75 years who had been residents of the metropolitan area of Mexico City for at least one year. Diagnosis was confirmed histologically in both groups. Variables were analyzed according to biological and statistical plausibility criteria. Univariate, bivariate and multivariate analyses were carried out. Cases and controls were stratified according to the menopausal hormonal status (pre and post menopause). RESULTS: The factors associated with BC were: higher socioeconomic level (OR= 2.77; 95%CI = 1.77 - 4.35); early menarche (OR= 1.32; 95%CI= 0.88 - 2.00); old age at first pregnancy (>31 years: OR= 5.49; 95%CI= 2.16 - 13.98) and a family history of BC (OR= 4.76; 95% CI= 2.10 - 10.79). In contrast, an increase in the duration of the breastfeeding period was a protective factor (>25 months: OR= 0.38; 95%CI= 0.20 - 0.70). CONCLUSIONS: This study contributes to the identification of risk factors for BC described in the international literature, in the population of Mexican women. Breastfeeding appears to play an important role in protecting women from BC. Because of changes in women`s lifestyles, lactation is decreasing in Mexico, and young women tend not to breastfeed or to shorten the duration of lactation.

OBJECTIVE: The aim of the study was to identify the variables that predict the revolving door phenomenon in psychiatric hospital at the moment of a second admission. METHODS: The sample consisted of 3,093 patients who have been followed during 5 to 24 years after their first hospital admission due to schizophrenia, and affective or psychotic disorders. Those who had had four or more admissions during the study period were considered as revolving door patients. Logistic regression analyses were used to assess the impact of gender, age, marital status, urban conditions, diagnosis, mean period of stay on the first admission, interval between the first and second admissions on the patterns of hospitalization. RESULTS: The variables with the highest predictive power for readmission were the interval between first and second admissions, and the length of stay in the first admission. CONCLUSIONS: These data may help public health planners in providing optimal care to a small group of patients with more effective utilization of the available services.

OBJETIVO: Identificar y evaluar los factores que predisponen a la utilización de la prueba de Papanicolaou en la población que usa el servicio del Programa Nacional de Detección Oportuna de Cáncer en México. MÉTODOS: Se realizó un estudio del tipo transversal de enero de 1997 a diciembre de 1998, en la Ciudad de México; se incluyeron a 2.107 mujeres en edad reproductiva que acudieron a un servicio de planificación familiar, las cuáles respondieron a un cuestionario con preguntas sobre características: sociodemográficas, factores de riesgo reproductivo asociados a cáncer cervical, historia de vida sexual, uso de métodos anticonceptivos, conocimiento de la utilidad y utilización del Papanicolaou. Para el análisis de los datos se construyeron modelos multivariados de regresión logística no condicional. RESULTADOS: Los predisponentes de utilización del programa de detección oportuna de cáncer en esta población fueron: el conocimiento de la utilidad del Papanicolaou incrementó en seis veces más la posibilidad de utilización (IC 95% 4,70-7,67); el antecedente de utilización de dos o más métodos de planificación familiar (OR=2,38; IC 95% 1,75-3,24); el antecedente de historia de infección vaginal (OR=2,18; IC 95% 1,73-2,75), y la aceptación del esposo para la realización de exploraciones ginecológicas (OR=1,56; IC 95% 1,07-2,29). CONCLUSIONES: La implementación de programas educativos en la prevención de cáncer, deberán incluir la utilidad de las pruebas de detección. En México, en mujeres en edad reproductiva utilización de la prueba de Papanicolaou se ofrece predominantemente en forma oportunista, por lo que el antecedente de utilización de los Servicios de Salud es un determinante para la utilización del Programa de Detección Oportuna de Cáncer Cervical. Estos resultados muestran la necesidad de ampliar la promoción a la salud de este programa a las mujeres de alto riesgo, incluyendo a sus parejas sexuales.

OBJECTIVE: To investigate the quality of life, life satisfaction, happiness and demands of work in workers with different work schedules. METHODS: The survey was carried out on professional workers in social care. Some were shiftworkers whose schedule included night shifts (N=311), some were shiftworkers without night shifts (N=207) and some were non-shiftworkers (N=1,210). Surveys were mailed and the response rate was 86%. For the purpose of this study several variables were selected from the Survey: The Quality of Life Profile, which measures importance, satisfaction, control and opportunities in nine domains of life plus measures of happiness, life satisfaction and demands of work. RESULTS: While both groups of shiftworkers, compared to non-shiftworkers, reported needing more physical effort to complete their work, and reported 'being' more physically tired, no differences were found in reports of overall happiness, life satisfaction or total quality of life. However, night-shiftworkers reported greater percentage of time unhappy than the other two groups of workers. In analyses of the quality of life, night-shiftworkers were less satisfied with domains of spiritual 'being' and physical and community 'belonging' than day-shiftworkers and non-shiftworkers. They also reported having fewer opportunities to improve their physical 'being', leisure, and personal growth than the other two groups. CONCLUSIONS: Quality of life in specific domains in night-shiftworkers was rated worse than in other groups of workers. Domain-based quality of life assessment gives more information regarding the particular needs of workers than overall or global measures of well-being.
